OBJECTIVE: This study aimed to investigate the clinical characteristics and outcomes of coronavirus disease-19 (COVID-19) long-term nucleic acid positive patients (hereinafter referred to as CLTAPs). METHODS: Patients were recruited from the Xiaogan Central Hospital between 16 January 2020 and 28 March 2020. Among the 562 cases of patients with laboratory-identified COVID-19 infection by real-time polymerase chain reaction (qtPCR), 19 cases of COVID-19 patients with more than 41 days from the first to the last time of nucleic acid test were selected as the study group, and 76 cases of age- and gender-matched COVID-19 patients were selected as the control group (hereinafter referred to as C-CLTAPs). Demographic characteristics, clinical symptoms, laboratory examination and computed tomography (CT) imaging characteristics were retrospectively analyzed. RESULTS: On admission, among the 562 cases of patients with COVID-19, there were 398 cases of ordinary COVID-19 patients, 99 cases of severe COVID-19 patients and 99 cases of critical COVID-19 patients. CLTAPs had milder clinical symptoms and longer viral shedding time in comparison to C-CLTAPs. Compared to C-CLTAPs, CLTAPs had a lower infection index at admission. CLTAPs used less oxygen therapy and a higher proportion of hydroxychloroquine treatment in comparison to C-CLTAPs. In comparison to C-CLTAPs, CLTAPs showed slower pulmonary CT progression and faster pulmonary CT absorption. CONCLUSION: In this study, out of the 562 cases, we found 19 CLTAPs. The clinical differences between CLTAPs and C-CLTAPs were compared and analyzed. We hope that these finding can provide a theoretical basis for the treatment of CLTAPs.

The hypothesis of a low self-esteem in depressive patients was tested using the Rosenberg Self-Esteem Scale in 24 recovered unipolar and 27 recovered bipolar patients, compared with a normal control group of 26 subjects matched for age and sex. The hypothesis was confirmed only for unipolars; bipolar patients presented a self-esteem score not significantly different from normal scores. Self-esteem was not related to clinical characteristics of the affective disorder, suggesting that low self-esteem may be a basic component of a depression-prone personality. The investigation of the relationship between self-esteem and social adjustment confirmed the presence of social conformism in bipolar patients and rigidly set low self-esteem in unipolar patients. These results should stimulate the evaluation of different psychotherapeutic treatments in the long-term psychosocial management of affectively ill patients.

SummaryWe prospectively studied the long-term course of individuals with acute and transient psychotic disorders and a control group with positive schizophrenia matched for age and gender. Follow-up investigations using standardised instruments were performed at three time-points covering 7 years after the index episode or 12 years after the first episode. During follow-up, those with positive schizophrenia experienced a deterioration in their general functioning whereas those with acute and transient psychotic disorders retained their high level of functioning. At the end of the observation period, 12 out of 39 (31%) of those with acute and transient psychotic disorders were functioning well without medication compared with 0 out of 38 with positive schizophrenia.

Abstract In order to identify the clinical characteristics of patients with Corona Virus Disease 2019 (COVID-19) and find out the characteristic effects of 2019 New Coronavirus (SARS-CoV-2) infection on changes in clinical and laboratory data, we analyzed the medical records of 80 suspected cases who admitted in the national designated hospital due to the relevant clinical manifestations of SARS-CoV-2 infection from January 22 to February 13, 2020. 62 (77.5%) confirmed cases and 18 (22.5%) negative cases were confirmed by SARS-CoV-2 nucleic acid test. Epidemiological investigation and statistical analysis were carried out on the clinical and laboratory data of all suspected cases of COVID-19, the specific indicators were found, and the clinical characteristics of COVID-19 were described. Compared with the patients with negative nucleic acid test, the patients with positive nucleic acid test showed shorter time of onset of symptoms, higher plasma CO2 level, lower eosinophil ratio, lower platelet count and hematocrit, lower serum sodium level, higher serum creatinine, higher blood urea and plasma albumin levels (all P<0.05). Our results might provide some suggestions in diagnosis, clinical treatment and prevention for COVID-19.

Background: Coronavirus disease-19 (COVID-19) has become a world wellbeing compromise. The danger factors for unfavorable occasions of coronavirus disease-19 (COVID-19) have not been very much depicted. This study aimed to explore clinical characteristics, laboratory results, and CT imaging characteristics of COVID-19 patients in Midnapore medical college and hospital and provide evidence for the prevention and treatment of COVID-19.Methods: In this retrospective, single-center study, data of all confirmed patients with COVID-19 admitted at SARI HDU of Midnapore medical college and hospital from 1st July to 21July 2021 were collected and analyzed. Data including clinical presentations, basic laboratory investigations, and CT severity scores were analyzed and compared between survival and death patients.Results: In this study total of 81 patients of COVID-19 admitted at SARI HDU were included (male=50, female=31). The patients were divided into 2 groups according to the outcome: survival group (n=38) and death group (n=43). Information on the overall clinical characteristics of the patients was collected, including age, sexual orientation, past medical history, clinical symptoms, and so on. Compared with the patients in the Survival group, the patients in the death group, the proportion of patients presented with Dyspnoea and suffering from Diabetes and hypertension were higher (p<0.05). In patients of the death group, TLC, CRP, D-dimer, urea, and creatinine had higher values, while the levels of albumin were significantly reduced, and the differences were statistically significant (p<0.05). Between the Survival group and death group, there was no significant difference in other indexes such as NLR, platelet count, and liver enzymes (SGOT, SGPT) (p>0.05). Conclusions: The risk factors of comorbidities, like diabetes, hypertension and others like leucocytosis, high CRP, high d-dimer, altered renal function, low serum albumin, and higher CT severity score could help clinicians identify patients with potential adverse events.

Abstract Background: To describe the clinical characteristics and the time of nucleic acid turning negative in patients with common type Corona Virus Disease 2019(COVID-19) pneumonia and analyze the treatment time and prognostic factors.Methods: It’s a retrospective analysis of 72 cases of common type COVID-19 pneumonia diagnosed by nucleic acid test and Computed tomography (CT).Results: The average time of nucleic acid test turning negative is 17.37±9.29 days, and the average treatment time was 23.74±8.06 days. There was no influence of Blood leukocyte count, Neutrophil count, lymphocyte count, C-reactive protein (CRP), glutamic-pyruvic transaminase (ALT), glutamic oxalacetic transaminase (AST), creatinine, D-dimer on the prognosis of the patients, whether univariate or multivariate logistic regression analysis. By χ2 test of the drugs and prognosis, we found that oseltamivir may have an impact on the prognosis of patients (P<0.05), while other drugs have no significant impact on the prognosis of patients. Through the correlation analysis, we found that age, the admission temperature, leukocyte count, neutrophil count, CRP were positively correlated with the treatment time of the patients (both P<0.05). Leukocyte count and neutrophil count were positively correlated with the time of nucleic acid test turning negative (both P<0.05).Conclusions: Age, the admission temperature, leukocyte count, neutrophil count, CRP were positively correlated with the treatment time of the patients. Leukocyte count and neutrophil count were positively correlated with the time of nucleic acid test turning negative. Oseltamivir may have an impact on the prognosis of patients, but the detailed role needs further study.

ObjectiveMost patients who have been treated for craniopharyngioma (CP) are GH deficient (GHD). GH replacement therapy (GHRT) may stimulate tumour regrowth; and one of the concerns with long-term GHRT is the risk of tumour progression. Therefore, the objective was to study tumour progression in CP patients on long-term GHRT.DesignCase–control study.Patients and methodsThe criteria for inclusion of cases were: i) GHD caused by CP; ii) GHRT >3 years; and iii) regular imaging. This resulted in 56 patients (mean age at diagnosis 25±16 years) with a mean duration of GHRT of 13.6±5.0 years. As controls, 70 CP patients who had not received GHRT were sampled with regard to follow-up, gender, age at diagnosis and initial radiation therapy (RT).ResultsThe 10-year tumour progression-free survival rate (PFSR) for the entire population was 72%. There was an association (hazard ratio, P value) between PFSR and initial RT (0.13, <0.001) and residual tumour (3.2, <0.001). The 10-year PFSR was 88% for the GHRT group and 57% for the control group. Substitution with GHRT resulted in the following associations to PFSR: GHRT (0.57, 0.17), initial RT (0.16, <0.001), residual tumour (2.6, <0.01) and gender (0.57, 0.10). Adjusted for these factors, the 10-year PFSR was 85% for the GHRT group and 65% for the control group.ConclusionsIn patients with CP, the most important prognostic factors for the PFSR were initial RT and residual tumour after initial treatment. Long-term GHRT did not affect the PFSR in patients with CP.

Background.Encapsulating peritoneal sclerosis (EPS) is a rare but serious clinical complication of long-term peritoneal dialysis (PD) patients with high mortality. The purpose of this study was to assess the clinical characteristics of patients with EPS and to search for possible factors useful for EPS prevention and early diagnosis.Method.This retrospective study was performed in a single dialysis center in Taiwan between August 1990 and April 2014. Overall, a total of 565 patients were included and the medical records of those patients who had developed EPS (EPS group) and those who had not developed EPS (control group) were collected. We compared several factors between these two groups.Result.In the univariate analysis, EPS was significantly associated with a change of transport state (Delta 2) (p= 0.007), duration of PD (p< 0.001), duration of peritonitis treatment (p= 0.001), number of peritonitis episodes (p= 0.002), and fungus related peritonitis (p= 0.031). After multivariate logistic model analysis, we found that only the duration of PD was independently significantly associated with EPS (p= 0.034). In addition, we used the ROC curve and found that a duration of peritoneal dialysis of about 8.4 years is the best cut-off point to predict EPS occurrence.Conclusion.In this study, long-termed PD duration is the only strong independent risk factor for EPS development. Total peritonitis times, total peritonitis treatment duration, and marked increased peritoneal D/Pcrratio were also significantly associated with the duration of PD.

Much has been reported on the clinical course of severe COVID-19, but less is known about the natural history and sequalae of mildly symptomatic cases and the prospects of reinfection or recurrence of symptoms. We report a case of a patient with mildly symptomatic PCR-confirmed COVID-19 who, after being symptom-free for 2 weeks, redeveloped symptoms and was found to be PCR-positive again >4 weeks from original testing. Surprisingly, IgG and IgM antibody testing was negative 2 months after reinfection. Although no negative testing was performed between the two symptomatic bouts, this case raises the possibility of reinfection after controlling the virus and highlights the long period with which a patient can shed virus and experience symptoms after initial infection. Characterising variations in clinical symptoms and length of viral shedding after improvement is essential for informing recommendations on patients safely resuming contact with others.

This research aimed to study the histopathological heart rat of sub chronic toxicity 70% ethanol extract Citrus nobilis Lourr. Peel. This study used 40 male white Galur Wistar rats in sub-chronic toxicity and each divided into 4 groups (control group giving CMC Na 0,5%, dose group 40 mg/kg BB, dose group 200 mg/kg BB, and dose group 1000 mg/kb BB). Each group consists of 10 males. Each cage contains 5 rats of the same age and gender, fed and drunk in ad libitum method. Furthermore, the rats are being weighing in. 70% extraction of Citrus nobilis Lour peel is orally given to the rats, every day for 90 days accordance with the group. After rats were given dose treatment for 90 days then observed histopathology of heart. Each rat organ preparation was observed in five different fields of view then scored and processed with rank ratings then analyzed using non parametric statistical test using Kruskal Wallis test. 70% extraction of Citrus nobilis Lour peel for 90 days with a dose of 1000mg / kg of rats based on statistical tests showed damaged heart muscle cell on histopathological observation of the heart. It is then expected to continue on the chronic toxicity test of 70% extraction of Citrus nobilis Lour peel for 12 months to predict the safety of clinical use in humans in the long term. Objective: In the early stages of the outbreak, Covid patients were followed-up in isolated Covid Wards, and the examinations of other pulmonary diseases continued in Non-Covid wards. Differential diagnosis between COVID-19 and infections caused by other pathogens is not adequately recognized. For this reason, it is even more difficult to identify patients who are infected with SARS-CoV2 or other pathogens. In the present study, the clinical characteristics and transmission routes of the Covid-19 cases in Non-Covid Wards within approximately 2 months’ time after the onset of the pandemic were analyzed speculatively. Method: In the early periods of the pandemic, quarantine wards were created for Covid patients in Chest Diseases Hospital, but there were also changes in Non-Covid patients and new Non-Covid hospitalizations. The clinical characteristics and transmission routes of the Covid cases in Non-Covid Ward were examined retrospectively and observationally between 10.03.2020 and 30.04.2020. Results: During this period, a total of 35 Covid cases were detected as a patient, companion and healthcare employees. The median age of the patients was 50 years (min-max 25-85). There were 17 patients (48.6%) who were not severe, and 18 severe patients (51.4%), and 10 (28.6%) patients died. A total of 25 (71,4%) of infected cases were verified with the SARS-CoV-2 Nucleic Acid Test, and 10 (28,6%) were diagnosed clinically and radiologically as possible COVID-19 cases. Being 65 years old or over, having comorbid diseases, especially COPD, the presence of dyspnea as a symptom and involvement on chest radiography were found to be significantly associated with survival (p0.027, 0.009, 0.038, 0.000 and 0.033, respectively). Lymphopenia, increase in neutrophil count, CRP and NLR value were found to be statistically related to survival (p 0.005, 0.001, 0.014 and 0.000, respectively). We found 4 super spreaders, one of whom was a companion, and 3 patients. Conclusion: Potential “super spreaders” can be the source of infection before the quarantine conditions are applied and comprehensive protection is implemented. For this reason, quarantine, use of personal protective equipment, application of social distancing, and the implementation of comprehensive preventive measures, such as disinfection, are crucial in controlling nosocomial infection
