Introduction: The objective of this study was to evaluate the practice of hand hygiene in the maternities of the public hospitals of Lubumbashi. Method and techniques: We carried out a cross-sectional descriptive study in which the population consisted on the one hand of the technical rooms of the maternity hospitals and the operating rooms for cesarean sections and on the other hand the nurses responsible for these maternities. The observation technique using an observation guide of the WHO model allowed us to collect the data. Results: Hand hygiene was not properly carried out in the maternity wards of Lubumbashi's public hospitals. Most of the prerequisites before hand washing and hydro-alcoholic hand rubbing were not respected, the prerequisites for surgical hand washing were observed only at 21.4%:e wearing of short-sleeved clothing (43.0%),absence of jewelry (78.6%), short nails without varnish and false nails (50.0%). In the absence of hydroalcoholic solution (SHA) (71.4%) for the friction, they used denatured alcohol (28.6%). The duration of soaping and friction was in an interval between 11 to 20 seconds or 42.9% and that of hygienic hand washing was observed at 28.6%. The duration of surgical and antiseptic hand washing was 21 to 40 seconds to 35.7%. Only 21.4% of the departments surveyed had used the broad-spectrum antiseptic foaming solution, no structure had a sterile disposable brush for hygienic and surgical hand washing, 2/14 departments surveyed had sterile disposable towels, the water used had not undergone bacteriological control and only one service, ie 7.1%, which had non-manually operated bins. Conclusion: Hand hygiene is not properly carried out in maternity hospitals in Lubumbashi's public hospitals, due to a lack of equipment and knowledge on this practice, equipment must be provided and staff trained in hand hygiene.

Recent research calls for distinguishing whether the failure to comply with World Health Organisation hand hygiene guidelines is driven by omitting to rub/wash hands, or subsequently recontamination of clean hands or gloves prior to a procedure. This study examined the determinants of these two behaviours. Across the 10 highest-volume labour wards in Zanzibar, we observed 103 birth attendants across 779 hand hygiene opportunities before aseptic procedures (time-and-motion methods). They were then interviewed using a structured cross-sectional survey. We used mixed-effect multivariable logistic regressions to investigate the independent association of candidate determinants with hand rubbing/washing and avoiding glove recontamination. After controlling for confounders, we found that availability of single-use material to dry hands (OR:2.9; CI:1.58–5.14), a higher workload (OR:29.4; CI:12.9–67.0), more knowledge about hand hygiene (OR:1.89; CI:1.02–3.49), and an environment with more reminders from colleagues (OR:1.20; CI:0.98–1.46) were associated with more hand rubbing/washing. Only the length of time elapsed since donning gloves (OR:4.5; CI:2.5–8.0) was associated with avoiding glove recontamination. We identified multiple determinants of hand washing/rubbing. Only time elapsed since washing/rubbing was reliably associated with avoiding glove recontamination. In this setting, these two behaviours require different interventions. Future studies should measure them separately.

ABSTRACT Both antiseptic hand rubbing (AHR) using ethanol-based disinfectants (EBDs) and antiseptic hand washing (AHW) are important means of infection control to prevent seasonal influenza A virus (IAV) outbreaks. However, previous reports suggest a reduced efficacy of ethanol disinfection against pathogens in mucus. We aimed to elucidate the situations and mechanisms underlying the reduced efficacy of EBDs against IAV in infectious mucus. We evaluated IAV inactivation and ethanol concentration change using IAV-infected patients’ mucus (sputum). Additionally, AHR and AHW effectiveness against infectious mucus adhering to the hands and fingers was evaluated in 10 volunteers. Our clinical study showed that EBD effectiveness against IAV in mucus was extremely reduced compared to IAV in saline. IAV in mucus remained active despite 120 s of AHR; however, IAV in saline was completely inactivated within 30 s. Due to the low rate of diffusion/convection because of the physical properties of mucus as a hydrogel, the time required for the ethanol concentration to reach an IAV inactivation level and thus for EBDs to completely inactivate IAV was approximately eight times longer in mucus than in saline. On the other hand, AHR inactivated IAV in mucus within 30 s when the mucus dried completely because the hydrogel characteristics were lost. Additionally, AHW rapidly inactivated IAV. Until infectious mucus has completely dried, infectious IAV can remain on the hands and fingers, even after appropriate AHR using EBD, thereby increasing the risk of IAV transmission. We clarified the ineffectiveness of EBD use against IAV in infectious mucus. IMPORTANCE Antiseptic hand rubbing (AHR) and antiseptic hand washing (AHW) are important to prevent the spread of influenza A virus (IAV). This study elucidated the situations/mechanisms underlying the reduced efficacy of AHR against infectious mucus derived from IAV-infected individuals and indicated the weaknesses of the current hand hygiene regimens. Due to the low rate of diffusion/convection because of the physical properties of mucus as a hydrogel, the efficacy of AHR using ethanol-based disinfectant against mucus is greatly reduced until infectious mucus adhering to the hands/fingers has completely dried. If there is insufficient time before treating the next patient (i.e., if the infectious mucus is not completely dry), medical staff should be aware that effectiveness of AHR is reduced. Since AHW is effective against both dry and nondry infectious mucus, AHW should be adopted to compensate for these weaknesses of AHR.

Joint Commission International ( JCI ) required the achievement of 100% five moment hand hygiene for the nurses who have provided care to patients. The adherence of five moments hand hygiene has been done to reduce the incidence of nosocomial infections. The adherence of five moments hand hygiene has been determined by inside and outside factors, and one of the inside factors is the motivation. The research is a qualitative research with cross sectional approach. The number of sample is 153 nurses with the proportionate random sampling technique at the Public Hospital of Loekmono Hadi Kudus. The research instruments used the questionaire and observation sheet. The research showed the result of Rank Spearman test p value = 0,000 with r value = 0.296, positive correlation means that the higher the nurse motivation, the higher the nurse aderence of five moment hand hygiene.

Background: Hand hygiene is one of the most effective ways to control health care related infection. Every year millions of patients around the world are affected by infections that are transmitted by the health-care professionals(HCPs).(2)(3). Rationale: Nurses and physicians are the main health care workers contacting with patients, representing the vector in the chain of infection. Thus, assessing their knowledge, attitude and practice regarding hand hygiene is very important to decrease the incidence of health care related infection and to improve quality of care. Objective: The objective of our study was to assess the knowledge, attitude and practices of handwashing among healthcare professionals of Kashmir Division. Methods: This study was an institutional based cross sectional study, conducted in various healthcare institutions of Kashmir Division which were selected randomly. A predesigned questionnaire was used to collect the data from the participants. The questionnaires contained questions about four different parts which included sociodemographic characteristics, knowledge of hand washing, attitude and practice of hand washing among healthcare professionals. The questionnaires were distributed to various healthcare professionals working in wards, emergency department, laboratories, outpatient departments, injection and dressing rooms, EPI unit and others. The distributed questionnaires were then collected back. Results: A total of 110 participants were included. Out of total 110 participants,53% were male ,36% were nurses 36% of health professionals were working in IPD (In Patient Department). 89% of the participants agreed that, direct or indirect contacts are the most important routes for transmission of hospital-acquired infections,97% agreed that
 proper and consistent hand washing prevents infections in health facilities,100% agreed that health professionals should always wash their hands immediately when they arrive at health institutions,98% said that they knew steps of handwashing(WHO)/rules of hand hygiene,91% agreed that hand washing is the single most effective mechanism to prevent spread of infection and 95% agreed that wearing jewellery, artificial fingernails, damaged skin and regular use of hand cream are associated with increased likelihood of colonisation of hands with harmful germs. 83% of the participants said that they are committed to the proper rules of hand hygiene all the time,78% said they comply with rules of hand hygiene even in emergencies,80% said that they think when they are wearing gloves it is not necessary to wash hands,92% feel irritated when others don’t follow hand hygiene rules,95% advice others to follow the rules of hand hygiene and 97% said it is easy for you to follow rules of hand hygiene. 40%(each) of the participants said that they always and usually wash hands before touching a patient,100% of the participants said that they always wash hands before performing aseptic and clean procedures, 100% of the participants said that they always wash hands after being at risk of exposure to body fluids and 95% of patients said that they wash hands after coming to and before leaving the hospital. DISCUSSION In this study we had a total of 110 participants. Health professionals had a satisfactory knowledge of hand hygiene but some lacunae are still there as is evident in the result part. Thus from this study we conclude adherence to handwashing is lacking among health professionals, so we need to have regular handwashing sessions for health professionals which will regularly sensitize them, also we need to address the reasons of this poor adherence both at the administrative and personal level. We also recommend to conduct more studies in this field so as to highlight the shortcomings in hand hygiene and then to improve upon them.

Abstract Background: Many studies show that working mothers are unable to exclusively breastfeed successfully. Expression and storage of breastmilk is a strategy that ensures continued breastmilk consumption in the event of temporary separation of an infant from the mother. The 2017 Kenyan Health Act mandates employers to provide lactation rooms with guidance for appropriate utilization.Methodology: The study objective was to assess knowledge attitude and practice of breastmilk expression and storage among working women with infants below six months of age in Public Well Baby Clinics. This was a cross sectional study among 395 working women attending two large public hospitals in Nairobi Kenya. A structured questionnaire with open and closed ended questions was used to establish the knowledge and practice while a Likert scale was used to explore attitudes of the mothers towards expression and storage of breast milk. Results: Only 161(41%) were expressing breast milk. Those with tertiary education and those working in the public sector had significantly higher odds of having satisfactory knowledge OR4.47(95%CI 2.01-11.07) and OR2.26(95%CI 1.33-3.85) respectively. Attaining tertiary education was significantly associated with a possibility of expressing and storing breastmilk OR3.6(95% CI 1.81-7.95). The workplace did not provide adequate equipment to facilitate breastmilk expression. Challenges experienced were breast pain and cumbersome nature of expressing milk.Conclusion: The study revealed knowledge gaps in expression and storage of breastmilk that need to be addressed to enable the mothers fully utilize the lactation rooms when they become available at the workplace.

Introductions: Transmission of healthcare associated infections through contaminated hands of healthcare workers are common. This study was designed to explore the existing compliance of hand hygiene among the healthcare workers workings in different level of health care centers of Makwanpur district of Nepal. Methods: This was a cross sectional observational study conducted in Makwanpur district, Nepal, during 2015. Healthcare workers from nine healthcare centers were selected randomly for the study. Standard observation checklists and World Health Organization guidelines on hand hygiene were used to assess the compliance of hand hygiene during patient care. Results: There were 74 participants. Overall compliance for hand washing was 24.25% (range 19.63 to 45.56). Complete steps of hand washing were performed by 38.3% of health care workers. The factors associated for noncompliance were lack of time (29.3%), example set by seniors (20%), absence or inadequate institution protocol (20%) and unfavourable health care setting (> 20%). Conclusions: Overall hand washing compliance rate amongst the healthcare workers in rural health facilities of Nepal were low (24.25%).

This cross-sectional type of descriptive study was undertaken to assess the awareness and practice of hand-washing among children in three villages of Puthia Upazila, Rajshahi District, Bangladesh. A total of 1152 children aged 5 – 18 years old were purposively selected from the study area. Majority (96%) of them was students of primary or secondary grade. A few were involved in business, farming, service and others. Face to face interview was conducted with the help of a semi structured questionnaire on the variables of interest. Nearly half (49.5%) of the respondents was 11-15 years and the rest >15 years old. Male-female distribution was almost equal. Fathers of the children were service-holder (18.7%), businessman (28.6%), farmer (26.1%) and day-laborer/rickshaw puller (26.5%). Nearly 60% of respondents belonged to poor family having monthly income Taka < 10000. Majority (97.1%) of the respondents was aware of the importance of hand washing and 75% perceived that diseases could be prevented through hand washing. More than 90% of the respondents were used to using soap and water for washing hands and over 80% washed hands regularly. Over three-quarters (77.8%) were accustomed to washing hands in school before tiffin. Most (81.8%) of the respondents used sanitary latrine for defecation. The use of soap-water for washing hands after defecation was significantly higher in older children (> 12 years) than that in younger children (≤ 12 years) (p = 0.020). The use of soap water for washing hands after defecation tends to be associated with children’s grade of education with higher tendency being observed in children of secondary and higher secondary school (p < 0.001). Children belonging to higher family income were more cautious in using soap/antiseptics (p < 0.001). The study concluded that perception and practice of hand-washing is appreciably high among rural children. However, older children, children studying in secondary grade and belonged to families with higher monthly income were more cautious about hand-washing.TAJ 2013; 26: 82-91
