scholarly journals An Unusual Case of Ruptured Uterus Associated With Placenta Abruption: Case Report

Association of abruption of placenta and ruptured uterus is extremely rare, there were only a few cases described in the past. A 24 year-old woman, G2P1 L0 with history of previous Cesarean Section for obstructed labour and now at 7 months of pregnancy, presented with features of acute abdomen with severe anemia. Her pulse rate was 136 per minute, temperature 36.1o C., respirations 28 per minute, blood pressure 96/50 mm. of Hg. Height of uterus 28 weeks, had tenderness and rigidity over the upper abdomen. She was suspected to have Haemoperitoneum due to ruptured uterus, and laparotomy was performed under IV Ketamine as provision of general anaesthesia was unavailable. Intra-operative findings were Haemoperitoneum, recovery of huge amount of clots from peritoneal cavity, Posterior fundal rupture with intact amniotic sac. A fresh stillborn fetus was delivered from the intact amniotic cavity with clear liquor through the rupture site followed by repair of the rent. She received blood transfusion and recovered without any complication. Laparotomy allowed us to discover this unusual presentation in this patient.

ABSTRACT Uterus didelphys is rare and sometimes not even diagnosed. We report a case of didelphys uterus in a 26-year-old pregnant lady who previously had three successful pregnancies with previous one C-section and remained undiagnosed till she presented to us as ruptured uterus. This case report aimed to summarize the clinical characteristics and perinatal outcome of VBAC in pregnancy with didelphys uterus and history of previous cesarean section.

Unusual presentation of multiple myeloma (MM) poses a diagnostic dilemma. The objective of this case report is to document the unique presentation of MM and to highlight the importance of backache which could be the only indicator for the diagnosis of MM. Although a history of trauma in the past is present in this case, but there is no molecular basis/evidence to declare trauma as a risk factor for MM. It can be concluded that the patients with backache should be thoroughly investigated to rule out any possibility of malignant disease process.Bangladesh Journal of Medical Science Vol.17(2) 2018 p.311-315

To evaluate the safety and integrity of uterine scar at repeat cesarean section in patients with previous one cesarean section (C/S). A prospective study was Carried out in a tertiary care, obstetric unit over a period of one year, 2010. All pregnant mothers who underwent cesarean section either emergency or elective with history of previous one cesarean sections were included in this study. The variables noted were age, parity, socioeconomic status, residential area, location of previous cesarean section, previous wound infection and associated complaints. Data was analyzed on SPSS 11 Operative findings during cesarean sections were recorded in terms of thinning of scar, dehiscence or rupture. One hundred and twenty cesarean section patients were included in the study. Out of all patients, extreme thinning of scar was noted in 28 (23.33%) patients. Four patients (3.33%) had scar dehiscence,only 3( 2.5%) patients with scar dehiscence had associated complaint of scar tenderness, while 17 (14.16%) of 120 cases of scar thinning were having scar tenderness. All 4 cases of scar dehiscence had their previous c/s at peripheral hospitals. No patient underwent hysterectomy and all patients with scar dehiscence had successful repair. Our findings shows relatively inadequate scar thickness rate but at the same time relatively acceptable scar dehiscence rate. This study is aimed at describing a noninvasive conservative strategy to the treatment of cervical pregnancy and highlighting the success of ultrasound-guided therapeutic techniques. A 43-year-old woman with a history of one previous cesarean section presented in our unit with vaginal spotting and a positive urine pregnancy test. She was diagnosed with a cervical pregnancy, and she was successfully treated conservatively with the administration of intragestational sac methotrexate under ultrasound guidance. Cervical pregnancy is a rare form of ectopic pregnancy that results from conceptus implantation in the cervical canal. The main concern is the associated life-threatening hemorrhage and subsequent need for urgent hysterectomy. The evolution of ultrasound over the past decades has enabled early diagnosis and has shifted the management from a radical surgical approach towards a stepwise conservative therapeutic approach, when possible.

It is perhaps one of the most important characteristics of the past decade in astronomy that the evolution of some major classes of astronomical objects has become accessible to detailed research. The theory of the evolution of individual stars has developed into a substantial body of quantitative investigations. The evolution of galaxies, particularly of our own, has clearly become a subject for serious research. Even the history of the solar system, this close-by intriguing puzzle, may soon make the transition from being a subject of speculation to being a subject of detailed study in view of the fast flow of new data obtained with new techniques, including space-craft.

Purpose The current “specific language impairment” and “developmental language disorder” discussion might lead to important changes in how we refer to children with language disorders of unknown origin. The field has seen other changes in terminology. This article reviews many of these changes. Method A literature review of previous clinical labels was conducted, and possible reasons for the changes in labels were identified. Results References to children with significant yet unexplained deficits in language ability have been part of the scientific literature since, at least, the early 1800s. Terms have changed from those with a neurological emphasis to those that do not imply a cause for the language disorder. Diagnostic criteria have become more explicit but have become, at certain points, too narrow to represent the wider range of children with language disorders of unknown origin. Conclusions The field was not well served by the many changes in terminology that have transpired in the past. A new label at this point must be accompanied by strong efforts to recruit its adoption by clinical speech-language pathologists and the general public.

Abstract. Background: Medical students are at high risk of suicidal ideation. Aim: We aimed to obtain information on suicidal ideation among medical students in Dammam located in the Eastern Province of Saudi Arabia. Method: This cross-sectional study was conducted at the College of Medicine affiliated with Imam Abdulrahman Bin Faisal University in the Eastern Province of Saudi Arabia. Suicidal ideation in the past 12 months was assessed based on responses to four questions in the depression subscale of the General Health Questionnaire 28 (GHQ-28). In addition, data were collected to examine the association of suicidal ideation with various factors. Results: We found that 1 in 3 medical students in the study had suicidal ideation in the past 12 months, while around 40% had lifetime suicidal ideation. Suicidal ideation was associated with feelings of parental neglect, history of physical abuse, and dissatisfaction with academic performance. Limitations: The cross-sectional nature of this study limits its ability to determine causality regarding suicidal ideation. Conclusion: These rates are considerably high when compared with rates from studies in other countries around the world. This study provides a reference in the field of suicidology for this region of Saudi Arabia.

The present situation is analyzed, four possible scenarios are put forward for Russia: “inertia”, “mobilization”, “decisive leap”, “gradual democratic development”. More than 30 experts were questioned in the process of working out the scenarios.

The article is devoted to the study of inscriptions on the gravestones of Haji Ibrahim al-Uradi, his father, brothers and other relatives. The information revealed during the translation of these inscriptions allows one to date important events from the history of Highland Dagestan. Also we can reconsider the look at some important events from the past of Hidatl. Epitaphs are interesting in and of themselves, as historical and cultural monuments that needed to be studied and attributed. Research of epigraphy data monuments clarifies periodization medieval epitaphs mountain Dagestan using record templates and features of the Arabic script. We see the study of medieval epigraphy as one of the important tasks of contemporary Caucasian studies facing Dagestani researchers. Given the relatively weak illumination of the picture of events of that period in historical sources, comprehensive work in this direction can fill gaps in our knowledge of the medieval history of Dagestan. In addition, these epigraphs are of great importance for researchers of onomastics, linguistics, the history of culture and religion of Dagestan. The authors managed to clarify the date of death of Ibrahim-Haji al-Uradi, as well as his two sons. These data, the attraction of written sources and legends allowed the reconstruction of the events of the second half of the 18th century. For example, because of the epidemic of plague and the death of most of the population of Hidatl, this society noticeably weakened and could no longer maintain its influence on Akhvakh. The attraction of memorable records allowed us to specify the dates of the Ibrahim-Haji pilgrimage to Mecca and Medina, as well as the route through which he traveled to these cities.
