Determination of factors affecting cognitive function in patients with chronic kidney disease stage 5

Objective. Patients with stage 5 chronic kidney disease.Study objective. Determination of factors affecting cognitive function in patients with CKD-5.Design and methods. The study included young and middle-aged patients (18 to 60 years old) (n = 40), there is a terminal stage of chronic kidney disease (CKD). Exclusion criteria: history of diseases of the central nervous system, brain injury, episodes of cerebrovascular accident with severity, coronary artery disease, chronic heart failure, pregnancy, abuse of alcohol, refusal to participate in the study. In a previous work, patient examination data were obtained on the Status PF hardware complex. Neurodynamic indicators were estimated, including the determination of the time of a simple visual-motor reaction (MTCM) and a complex visual-motor reaction (MPSM), reaction to a moving object (RDO), memory and attention. Using Spearman’s correlation analysis, we evaluated the impact on cognitive functions of a number of factors, presumably related to neurodynamics. The following factors were selected: age, creatinine level, NSE and S100 proteins, hemoglobin level, Beck’s depression points, indicators of personal and situational anxiety.Results. All patients examined on the Status PF software package had mild cognitive impairment in terms of neurodynamics, attention, and memory. The average positive relationship between the level of depression and various indicators of RDO was established: with RDO_average (ρ = 0.405; p = 0.018), with RDO_total delay (ρ = 0.540; p < 0.001), with RDO_average delay (ρ = 0.421; p = 0.007), as well as weak positive – with CEMR average exposure (ρ = 0.358; p = 0.023). An average positive relationship of creatinine level with RDO_O (ρ = 0.438; p = 0.005) is noted. A weak positive relationship between the level of personal anxiety and RDO_total delay (ρ = 0.334; p = 0.035) was revealed. The average negative relationship between hemoglobin level and RDO_total delay (ρ = –0.535; p < 0.001) was revealed. A weak positive relationship between situational anxiety and SEMR is determined by the number of errors (ρ = 0.364; p = 0.021), as well as a weak negative one with the attention volume (ρ = –0.357; p = 0.024). The results of a number of studies have proved the relationship between the functional state of the pineal gland and renal function. However, violations of the melatonin-forming function of the epiphysis (MFE) in patients with chronic kidney disease (CKD) undergoing hemodialysis (HD) and its relationship with dyslipidemia in this patient population is a poorly understood issue. The objective: to analyze disorders of MFE and blood lipid spectrum in patients with CKD of 5 stage treated with HD and to determine the relationship of epiphysis dysfunction with dyslipidemia. Materials and methods. 130 people (50% of men) aged 58.5 were surveyed [43; 66] which are on permanent hemodialysis treatment. Control passed 20 healthy individuals. The determination of day and night level of melatonin (MT) in saliva was conducted, based on the level of which patients (treated with HD) were divided into two groups: group I – 110 patients with impaired MFE, group II – 20 patients with normal MFE. Clinical and laboratory researches were carried out for all patients: general and biochemical analyzes of blood with determination of cholesterol level and its fractions, measurements of office blood pressure (BP) were made. Results. Significant prevalence of MFE disorders in patients with CKD of 5 stage treated with hemodialysis and its relationship with blood lipid spectrum were found. The level of total cholesterol (TC), triglycerides (TG) and low density lipoproteins (LDL) in patients with impaired MFE was higher by 26.4 % (p<0.05), 16.7 % (p<0.05) and 22,6 % (p= 0.03) according to the outcome of the comparison group patients. The level of high-density lipoprotein (HDL) of the main group is lower by 11.8 % compared to the group with preserved MFE. The data obtained indicate the relationship of MFE disorders with the duration of RRT treatment, the duration of arterial hypertension, the age of patients, and their effect on the lipid spectrum of patients with CKD of 5 stage treated with hemodialysis. Night feedback correlation of MT with TC level was established (r=–0.256; p<0.05). Correlation analysis confirms that a decrease in MT at night is combined with an increase of TG level (r=–0.272; p<0.05) in the blood of patients. The feedback correlation of night (r=–0.347; p=0.03) and daytime level (r=–0.198; p<0.05) of MT with LDL level and positive relationships between MT in daytime (r=0.27; p=0.03) and the night period (r=0.331; p=0.02) with HDL levels. Conclusion. For patients with CKD of 5 stage undergoing hemodialysis, there is a frequent violation of MFE (84.6%) and significant disorders of lipid metabolism (58%). Analysis of the lipid metabolism study revealed more profound abnormalities in the form of an increased concentration of TC and all its fractions in patients with impaired MFE, which may indicate a connection between epiphysis dysfunction and lipid metabolism in patients with RRT. In patients with hemodialysis, melatonin-forming dysfunction and disorders of lipid metabolism are age-dependent and are determined by the duration of RRT, the duration of hypertension, the level of hemoglobin. We have identified a relationship between the deterioration of lipid metabolism on the background of deeper disturbance of MFE by daytime and nighttime MT.

Abstract Objectives The contribution of the clinical laboratory to diagnostics is increasingly important since a great deal of clinical decisions rely on laboratory test results. Content Parathyroid hormone (PTH) measurement presents a considerable analytical variability due to the heterogeneity of its circulating forms and the antigenic configuration of the different assays commercially available. Such variability may have an impact on pathological conditions associated with significant increases in circulating PTH, as it is the case of chronic kidney disease (CKD). Summary Despite the recent identification of new molecules involved in bone and mineral disorders associated with CKD, such as klotho or the fibroblastic factor 23 (FGF23), nephrologists still base their clinical decisions on PTH concentrations. The problem is that unawareness of these analytical considerations may cause errors in the clinical interpretation of test results. Outlook This systematic review addresses these issues from the clinical laboratory perspective and proposes new approaches related to PTH method selection and result expression. These new strategies will help laboratory medicine specialists and nephrologist better determine the status of CKD patients.

Abstract Background Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) to ALT ratio (AST/ALT ratio) have been shown to be related to non-alcoholic fatty liver disease or insulin resistance, which was associated with chronic kidney disease (CKD). However, it is unclear whether ALT and AST/ALT ratio are associated with CKD. In this study, we examined the relationship of ALT and AST/ALT ratio to CKD among middle-aged females in Japan. Methods The present study included 29,133 women aged 40 to 64 years who had an annual health checkup in Japan during April 2013 to March 2014. Venous blood samples were collected to measure ALT, AST, gamma-glutamyltransferase (GGT), and creatinine levels. In accordance with previous studies, ALT > 40 U/L and GGT > 50 U/L were determined as elevated, AST/ALT ratio < 1 was regarded as low, and CKD was defined as estimated glomerular filtration rate < 60 mL/min/1.73 m2 and/or proteinuria. Logistic regression model was used to calculate the odds ratio (OR) and 95% confidence interval (CI) for CKD. Results “Elevated ALT and elevated GGT” and “elevated ALT and non-elevated GGT” significantly increased the OR for CKD when compared with “non-elevated ALT and non-elevated GGT” (OR: 2.56, 95% CI: 2.10–3.12 and OR: 2.24, 95% CI: 1.81–2.77). Compared with “AST/ALT ratio ≥ 1 and non-elevated GGT”, “AST/ALT ratio < 1 and elevated GGT” and “AST/ALT ratio < 1 and non-elevated GGT” significantly increased the OR for CKD (OR: 2.73, 95% CI: 2.36–3.15 and OR: 1.68, 95% CI: 1.52–1.87). These findings still remained after adjustment for confounders. Conclusions Elevated ALT was associated with CKD regardless of GGT elevation. Moreover, low AST/ALT ratio was also associated with CKD independent of GGT elevation.

The objectives of this research are to evaluate acoustic environments and to forecast STI values from spatial component variables in the large classrooms of the Thai public university that were specially controlled the same room finishing materials including the floor, walls, and ceiling. Whereas the five spatial component factors included (1) Room Volume (RV), (2) Ceiling Height (CH), (3) the Ratio of Depth to Width (Rdw), (4) Total Room Surface (TS), and (5) Percentage of Absorbing Surface areas (PAS). The research tools were the smartphones that used the applications for acoustical evaluation and speech intelligibility analysis. The Speech Transmission Index (STI), Reverberation Time (RT), and Background Noise Level (BNL) were collected by the calibrated microphone in the nine points distributed across the entire room. And also, the sounds for testing were simulated such as balloon burst, and STIPA signal via a sound generator. The Thailand Speech Intelligibility (T-SI) model was developed by the multiple regression analysis with a statistical at a confidence level of 95%.The results showed that this T-SI model depended on the strongly positive relationship of PAS and the slightly positive relationship of CH, TS while the RV, Rdw were slightly the negative relationship and which predicted STI values. Moreover, the highest affecting variable of T-SI model was CH and the lowest was PAS. However, this research implies that the improving room acoustic quality would be adjusting the sound absorbing surface areas i.e., increase the cloth curtain or appropriate methods.

This study aims to analyze the green CSR  of innovation performance  with firms approval variables  and public visibility   can support moderating the relationship of green CSR  and innovation. The research sample was 33 manufacturing companies. The results showed that the  green CSR has a positive and significant effect on innovation . Also, the company approval variable has been proven to moderate the direction of a positive relationship between green CSR and innovation . The results also prove that public visibility is proven to moderate the direction of the negative relationship between green CSR and performance. This study provide information that shows great concern for the environment; it will increase the company in making changes through innovation activities. Also, the higher the company's approval and public visibility, the company will get support from various stakeholders to run the firms. The level of company concern for CSR activities will be a misjudgment for investors.

Abstract To determine effects of cow BCS and calving season on calf vigor and size and the relationship of calf size with vigor, 310 multiparous and primiparous beef cows (age: 4.6 ± 2.2 yr; BW: 663 ± 86 kg; BCS: 5.3 ± 0.6) from 3 spring (2015, 2016, 2017) and 4 fall (2015, 2017, 2018, 2019) calving seasons were observed during parturition. Time to stand (n = 191) was determined as minutes from birth to standing successfully for 5 sec. Within 24 h, calf BW (n = 187) and size [shoulder to tailhead length (STL), heart girth (HG), and abdominal girth (AG; n = 154)] were recorded. Data were analyzed using a mixed model containing effects of calving season (spring and fall), BCS category (&lt; 5, =5, ≥6), and their interaction. Correlations were determined between time to stand and calf measurements separately for spring (n = 79) versus fall (n = 111). Cows with BCS &lt; 5 gave birth to lighter (P = 0.02) calves that tended to have a faster (P ≤ 0.13) time to stand than =5 and ≥6. Calves from cows with BCS ≥6 tended to have greater (P ≤ 0.09) STL than =5 and &lt; 5. Calves from cows with BCS &lt; 5 had smaller (P = 0.13) HG than =5. Fall-born calves tended to have smaller (P ≤ 0.06) BW, HG, and AG than spring-born calves. Within fall calves, there tended to be a positive relationship (r = 0.16; P = 0.09) between time to stand and calf BW. Despite this, spring calves had a negative relationship (r = -0.26; P = 0.02) between time to stand and calf BW. In conclusion, cow BCS and calving season affect calf size and may affect calf vigor. These data suggest that calving season may dictate the relationship between calf vigor and size.

The action of hemodialysis in patients with chronic kidney disease can trigger anxiety due to situational crisis, death threat and not know the final result of hemodialysis action. The purpose of this systematic review is to gain an understanding of the anxiety of patients with chronic kidney disease with hemodialysis action. The results of a review of 15 journals that have been selected suggest that patients with chronic kidney disease with hemodialysis actions mostly experience anxiety. The most widely used instruments are (HADS) Hospital Anxiety And Depression Scale (n = 5). Factors affecting anxiety of chronic kidney disease patients with hemodialysis action are social demographic factors such as sex, age, occupation, duration of hemodialysis and education. Other factors are psychological, social perception, non pharmacological action (progressive muscular), (aroma therapy) and spiritual intelligence. The conclusion that anxiety disorder is very important, and appropriate Instrument will affect patient objective anxiety results. Indonesia is a country that adheres to a dual banking system, namely conventional and Islamic Banking. The growth rate of Islamic banking in the last three years is higher than conventional banking. However, in total assets, Islamic banking is still far behind conventional banking. Therefore, it is necessary to study further the performance of Islamic banking reflected in its profitability. So, it becomes an alternative input in determining Islamic banking policies. This study aims to know the factors affecting the profitability (ROA) of Islamic Banking in Indonesia. The data used are the 2014-2020 monthly data in the amount of 79 data. The method used in this study is a Vector Error Correction Model (VECM) to determine the effect of long-run and short-run relationships. The results of the study showed that the long-run relationship of the NPF variable affected and was significant positive toward ROA, CAR affected and was significant negative toward ROA, while the inflation variable had a negative relationship and not significant toward ROA. The results of the short-run relationships showed that the NPF and CAR variables positively affected ROA, while the inflation variable did not significantly affect the ROA.
