IntroductionTo assess the feasibility of a home-based aerobic exercise and nutrition counseling intervention and effect on cardiorespiratory fitness, cardiovascular disease risk profile, and immune response in obese endometrial cancer survivors.MethodsA longitudinal pilot study assessed a 12-week home-based aerobic exercise and nutrition counseling intervention in obese endometrial cancer survivors. The primary outcome was feasibility defined as 80% adherence to weekly walking sessions calculated among individuals that completed the intervention. Secondary outcomes comprised pre- and post-intervention differences in cardiorespiratory fitness, cardiovascular risk factors, and T-cell function. Descriptive statistics summarized data. Wilcoxon sign tests identified differences between and pre and post-intervention variables.ResultsNineteen women with stage 1 endometrial cancer consented; 9 withdrew and one was a screen failure. Median adherence to weekly walking sessions was 83.3%. Body composition was significantly altered with a reduction in median fat mass from 52.5 kg to 46.9 kg (p=0.04), and BMI from 37.5 kg/m2 to 36.2 kg/m2 (p = 0.004). There was no significant difference in cardiorespiratory fitness or cardiovascular parameters. The percentage of CD4+ and CD8+ T-cells producing IFNγ towards MAGE-A4 significantly increased from and 5.9% to 7.2% (p=0.043) and 13.9% to 14.8% (p=0.046), respectively. There were 3 related adverse events: hip pain, back sprain, and abdominal pain.DiscussionOur home-based exercise and nutrition counseling program was feasible based on 80% adherence to walking sessions and favored altered body composition. However, the discontinuation rate was high and further research is needed to overcome barriers to implementation. Improvement in cardiovascular parameters will most likely require longer and more intensive programs.

We assessed the effects of resistance circuit-based training (CT) on strength, cardiorespiratory fitness, and body composition. A systematic review with meta-analysis was conducted in three databases, ending on March, 2020. Meta-analysis and subgroup analysis were used to analyze the effects of pre–post-intervention CT and differences from control groups (CG). Of the 830 studies found, 45 were included in the meta-analysis (58 experimental groups (n = 897) and 34 CG (n = 474)). The CT interventions led to increases in muscle mass (1.9%; p < 0.001) and decreases in fat mass (4.3%; p < 0.001). With regard to cardiorespiratory fitness, CT had a favorable effect on VO2max (6.3%; p < 0.001), maximum aerobic speed or power (0.3%; p = 0.04), and aerobic performance (2.6%; p = 0.006) after training. Concerning strength outcome, the CT increased the strength of the upper and lower extremities. Only the magnitude of strength performance appears to be influenced by the training (number of sessions and frequency) and the training status. Moreover, low and moderate intensities and short rest time between exercise increase the magnitude of change in fat mass loss. Therefore, CT has been shown to be an effective method for improving body composition, cardiorespiratory fitness, and strength of the lower and upper limbs.

Nutrient consumption and body mass index (BMI) are closely related to the gut microbiota, and exercise effects on gut bacteria composition may be related to those variables. Thus, we aimed to investigate the effect of 10-week moderate aerobic exercise on the cardiorespiratory fitness and gut bacteria composition of non-obese men with the same nutritional profile. Twenty-four previously sedentary men (age 25.18 [SD 4.66] years, BMI 24.5 [SD 3.72] kg/m2) were randomly assigned into Control (CG; n = 12) or Exercise Groups (EG; n = 12). Body composition, cardiorespiratory parameters, blood markers, dietary habits and gut bacteria composition were evaluated. EG performed 150 min per week of supervised moderate (60–65% of VO2peak) aerobic exercise, while CG maintained their daily routine. The V4 16S rRNA gene was sequenced and treated using QIIME software. Only EG demonstrated marked improvements in cardiorespiratory fitness (VO2peak, p < 0.05; Effect Size = 0.971) without changes in other gut bacteria-affecting variables. Exercise did not promote clustering based on diversity indices (p > 0.05), although significant variations in an unclassified genus from Clostridiales order and in Streptococcus genus were observed (p < 0.05). Moreover, α-diversity was correlated with VO2peak (Pearson’s R: 0.47; R2 0.23: 95%CI: 0.09 to 0.74, p = 0.02) and BMI (Pearson’s R: −0.50; R2 0.25: 95%CI: −0.75 to −0.12, p = 0.01). Roseburia, Sutterella and Odoribacter genera were associated with VO2peak, while Desulfovibrio and Faecalibacterium genera were associated with body composition (p < 0.05). Our study indicates that aerobic exercise at moderate intensity improved VO2peak and affected gut bacteria composition of non-obese men who maintained a balanced consumption of nutrients.

Abstract Background There is a high prevalence of women in South Africa with overweight and obesity which is associated with an increased risk of cardiometabolic disorders. Perceived barriers such as lack of time and motivation reduce engagement in beneficial activity behaviours for health. High-intensity interval training (HIIT) is a time-efficient and effective way to improve cardiometabolic risk profile regardless of a loss in body mass or change in body composition. This randomized controlled trial aims to determine the effects on cardiorespiratory fitness, body composition and cardiometabolic health and feasibility of a home-based 14-week HIIT program in women with overweight/obesity or normal body mass. Methods One hundred and twenty women (18–40 years old) with a body mass index between 20 and 35 kg/m2, will be stratified according to their BMI (normal, BMI 20–24.9 kg/m2; or high BMI ≥25 kg/m2) and randomized into a HIIT exercising group (HIIT) or a non-exercising control group (CON). HIIT participants will perform exercises for 11 min/session six times per week for a period of 14 weeks. The 2 × 4 HIIT protocol will require a work phase of own-body weight exercise lasting 2 minutes (85% VO2peak), repeated four times and separated by a one-minute active rest phase (65% VO2peak). CON participants will be asked to maintain their normal habitual lifestyle. Outcomes of cardiorespiratory fitness, body composition, echocardiography, central blood pressure, arterial stiffness and biomarkers of cardiometabolic health will be measured before and after the 14-week intervention. Every 4 weeks during the intervention, an objective estimation of compliance to the study protocol will be assessed by measuring participant physical activity over 7 days using an Actigraph GT3X accelerometer. Discussion Supervised laboratory-based HIIT interventions are effective in improving cardiometabolic health. More pragmatic exercise protocols may however show to be successful for mitigating barriers to the engagement in physical activity and exercise resulting in positive benefits to health. Investigation into home-based HIIT regimens are important in women, where globally the rising trend of overweight and obesity overshadows that of men. The results from this study may therefore inform future research on effective exercise prescription for women’s health. AbstractPurpose. Hypercholesterolaemia is a highly prevalent condition that has major health- and cost-related implications for the society. Aerobic-type exercise improves lipoprotein-lipid profiles, cardiorespiratory fitness and body composition in healthy young women. Thus, the aim of the study was to assess the impact of 9 weeks of low-high aerobic-type exercise on the lipid profile among young women. Methods. On the basis of the lipid profile, 64 women (median age, 21.8 years; range, 19.0-24.7 years) were divided into two groups: with low (LRH) and intermediate (IRH) risk of developing hypercholesterolaemia. The participants completed a 9-week-long low-high aerobic exercise programme. Before and after the training programme, we determined the lipid profile: triglycerides (TG), total cholesterol (TC), lipoprotein cholesterol: HDL-C and LDL-C, and glucose levels. Selected cardiorespiratory fitness variables and body composition were also determined. Results. It was found that aerobic-type fitness exercise in the IRH group caused statistically significant decreases in TC and TG levels in comparison with baseline values. Significant increase in maximum oxygen uptake and decrease in HDL-C in the LRH group were also observed. Conclusions. Aerobic fitness exercises, a combination of two alternating styles, could influence the blood lipid profile by decreasing plasma TC and TG levels. In non-athlete women, physical activity may be a good tool to prevent cardiovascular diseases.

Background: Exercise has been shown to reduce adverse outcomes related to breast cancer. However, the rate of adherence to physical exercise is very low among breast cancer survivors (BCS). This study investigated the effects of high supervision ratio resistance training (RT), once a week for 8 weeks, on changes in body composition and muscular strength in BCS. Methods: Twenty-five female BCS undergoing hormone therapy were randomized into resistance training group (TG, n = 12) or control (CG, n = 13) group. The TG performed 8 weeks of supervised RT, with 1 trainer per volunteer, once a week. Body composition was evaluated by dual-energy X-ray absorptiometry, and muscle strength was evaluated by 10 repetition maximum (10 RM) for leg press (45°) and bench press exercises. A 1-way analysis of variance was used to compare within-group effects at pre- and post-intervention. An analysis of covariance test was used to compare post-intervention values, using pre-intervention measures as covariates. The effect size (ES) was calculated by Cohen’s d. Results: The TG improved muscle strength in 10 RM leg press (45°; Δ 33.75 ± 11.51 kg, P = .02; ES = 0.96) and bench press (Δ 4.08 ± 1.83 kg, P = .01; ES = 1.15). Adherence to training was more than 99%. Changes in body composition were not detected. There were no changes in the CG for any assessment. Conclusion: Once-weekly supervised RT could be an alternative to increase the adherence to exercise and improve muscular strength in BCS.

170 Background: Endometrial cancer affects 50,000 women a year. Obesity plays a significant role in the pathogenesis of endometrial cancer. Obese endometrial cancer survivors (ECS) are at significant lifetime risk of diabetes, cardiovascular disease (CVD), recurrence and death. There are no prospective studies examining the role of diet only in attempting to achieve weight loss in obese ECS. Protein sparing modified fast (PSMF), an approach to rapid weight loss, has been used to treat obesity in a safe manner. A pilot study is underway to investigate the feasibility of a PSMF for weight loss in this high risk population. Methods: To date, seven obese (BMI > 30kg/m^2) ECS have been placed on a PSMF under the supervision of a physician and dietitian. Patients provided demographic information and Obesity Quality of Life (OQOL) questionnaire. Comprehensive metabolic panel with lipid panel and biomarkers of inflammation were drawn. Patients were instructed to eliminate carbohydrate containing foods and to augment with 1.2g/kg of protein per obesity-adjusted ideal body weight. Primary objectives are: total weight loss, subject retention, compliance, side effects, QOL and alterations in markers of obesity and inflammation. Results: The median age of the patient group was 56 years. Median baseline weight was 292.4 pounds (185.9-369). Median BMI was 44.5 kg/m2 (37.5-61.4). Mean baseline leptin level was 54.3 ng/ml (normal: 2.5-21.8). Mean baseline C-reactive protein level, a strong marker for CVD, was 4.574 (high risk for CVD > 3). At 4 weeks median percent body weight lost was -6.48% (5.19%-7.00%). At 3 months the median loss nearly doubled to -13% (8.31%-14.11%). Significant reduction in CRP and leptin occurred in 2 patients: mean decrease was 3.9 and 28 points respectively. Conclusions: Our early data demonstrate that significant weight loss in obese endometrial cancer survivors is achievable in a standard outpatient gynecologic oncology practice. While long term follow up data and elucidation of the true significance of improvement in serum inflammatory markers are needed, we do know that even a 5-10% loss of body weight can lead to substantial improvement in CVD and diabetes risk.
