Irregular dietary intakes impairs estimations from food records. Biomarkers and method combinations can be used to improve estimates. Our aim was to examine reproducibility from two assessment methods, compare them, and validate intakes against objective biomarkers. We used the Malmö Offspring Study (55% women, 18–71 y) with data from a 4-day food record (4DFR) and a short food frequency questionnaire (SFFQ) to compare (1) repeated intakes (n = 180), (2) intakes from 4DFR and SFFQ (n = 1601), and (3) intakes of fatty fish, fruits and vegetables, and citrus with plasma biomarkers (n = 1433) (3-carboxy-4-methyl-5-propyl-2-furanpropanoic acid [CMPF], β-carotene and proline betaine). We also combined 4DFR and SFFQ estimates using principal component analysis (PCA). Moderate correlations were seen between repeated intakes (4DFR median ρ = 0.41, SFFQ median ρ = 0.59) although lower for specific 4DFR-items, especially fatty/lean fish (ρ ≤ 0.08). Between-method correlations (median ρ = 0.33) were higher for intakes of overall food groups compared to specific foods. PCA scores for citrus (proline betaine ρ = 0.53) and fruits and vegetables (β-carotene: ρ = 0.39) showed the highest biomarker correlations, whereas fatty fish intake from the SFFQ per se showed the highest correlation with CMPF (ρ = 0.46). To conclude, the reproducibility of SFFQ data was superior to 4DFR data regarding irregularly consumed foods. Method combination could slightly improve fruit and vegetable estimates, whereas SFFQ data gave most valid fatty fish intake.

AbstractObjectiveFew biomarkers for dietary intake of various food groups have been established. The aim of the present study was to explore whether selenium (Se), iodine, mercury (Hg) or arsenic may serve as a biomarker for total fish and seafood intake in addition to the traditionally usedn-3 fatty acids EPA and DHA.DesignIntake of fish and seafood estimated by an FFQ was compared with intake assessed by a 4 d weighed food diary and with biomarkers in blood and urine.SettingValidation study in the Norwegian Mother and Child Cohort Study (MoBa).SubjectsOne hundred and nineteen women.ResultsTotal fish/seafood intake (median 39 g/d) calculated with the MoBa FFQ was comparable to intake calculated by the food diary (median 30 g/d,rS= 0·37,P< 0·001). Erythrocyte DHA and blood Hg, Se and arsenic concentrations were positively correlated with intake of fish and seafood, but the association for DHA was weakened by the widespread use of supplements. The main finding was the consistent positive association between the intake of fish/seafood and blood arsenic concentration. In multivariate analyses, blood arsenic was associated with blood Hg and fish and seafood intake. In these models, arsenic turned out to be the best indicator of intake of fish and seafood, both totally and in subgroups of fish/seafood intake.ConclusionsWhile DHA reflected the intake of fatty fish andn-3 PUFA supplements, blood arsenic concentration also reflected the intake of lean fish and seafood. Blood arsenic appears to be a useful biomarker for total fish and seafood intake.

Abstract BackgroundHigh adherence to healthy Nordic diets may enhance longevity. However, optimal intake levels of healthy Nordic foods are not known. Hence, in a large prospective cohort of women in Norway we examined all-cause mortality in relation to intake of five food groups that are part of a healthy Nordic diet: Nordic fruits and vegetables, fatty fish, lean fish, wholegrain products, and low-fat dairy products. MethodsA total of 87 899 women who completed a food frequency questionnaire between 1996 and 2004 were followed for mortality until the end of 2018. Cox proportional hazards regression models were used to examine the associations between consumption of the Nordic food groups and all-cause mortality. The food groups were examined as categorical exposures, and all but wholegrain products also as continuous exposures in restricted cubic spline models.Results A total of 9 168 women died during the 20-year follow-up. Nordic fruits and vegetables, fatty fish and low-fat dairy products were not linearly associated with mortality (p < 0.05). The optimal intake levels and hazard ratios (HR) and 95% confidence intervals (CI) associated with these intakes were approximately 200 grams/day of Nordic fruits and vegetables (HR 0.84 (95% CI: 0.77–0.90)), 10–20 grams/day of fatty fish (HR 0.98 (95% CI: 0.92–1.03)) and 200 grams/day of low-fat dairy products (HR 0.94 (95% CI: 0.89–0.99)) compared to no consumption. High consumption of fatty fish (≥ 70 grams/day) was associated with increased mortality. Intake of wholegrain products of >120 grams/day was associated with lower mortality (HR 0.92 (95% CI: 0.85–0.99)) compared to < 60 grams per day. Lean fish consumption was not associated with mortality. After stratification by smoking status, the observed association for Nordic fruits and vegetables was only significant in ever smokers with the optimal intake level at 250 grams/day (HR 0.78 (95% CI: 0.71-0.86)).Conclusion Moderate intake of healthy Nordic foods seems to be better than low or high intake, and high consumption either does not add any beneficial effects or can compromise longevity. Stronger inverse associations for Nordic fruits and vegetables were observed in former and current smokers.

Abstract Objectives To systematically identify, compile, and harmonize national and sub-national individual-level dietary data around the world using a common food classification and description system (FCDS); promote collaboration with data owners worldwide; and publicly disseminate methods and datasets to the global nutrition community. Methods The Global Dietary Database (GDD) and the FAO/WHO Global Individual Food consumption data Tool (FAO/WHO GIFT) initiative, in collaboration with the European Food Safety Authority (EFSA), developed a framework to standardize the characterization of 24-hour recall and food record survey data at their finest level, using FoodEx2 (Figure). FoodEx2 is a comprehensive FCDS developed by EFSA, adapted for international high-quality food characterization and methodologically superior to prior international FCDS. Single food items are characterized through a classification system (hierarchical nested food groups) and 32 description elements (facets), such as cooking process. Standardized criteria, procedures, and templates were developed to: (1) identify and (2) prioritize eligible surveys for FoodEx2 mapping; (3) efficiently contact data owners and obtain approval for public dissemination; (4) allocate financial assistance as appropriate; (5) obtain survey information and assess survey and diet tool quality; (6) train data owners on dataset preparation and FoodEx2 mapping; (7) collect harmonized microdata, including structure, sociodemographics, diet metrics, units of measure, and food characterization; and (8) publicly disseminate methods and datasets via the GDD and FAO/WHO GIFT platforms. Results A total of 151 surveys have been identified as high priority. Harmonization has been completed for 24 European surveys by EFSA, which will be incorporated in the GDD and FAO/WHO GIFT platforms upon approval of data owners; 8 surveys by FAO/WHO GIFT; and is ongoing for another 4. Thus far, the data owners of 53 surveys from 41 countries, including Brazil, India, and Iran, confirmed interest in joining; all surveys either used none or a local FCDS. Conclusions These new methods developed will provide the most comprehensive and standardized databases of dietary intakes worldwide. Abstract Background Although several studies have examined the link between different food groups intake and stunting among children, no study, to our knowledge, was done on exceptional children. The aim of this study was assessed the association of dietary intake and stunting in Iranian exceptional children.Methods This cross-sectional study was conducted on 470 exceptional children (226 mentally retarded, 182 deaf and 62 blind children), aged 5–15 years. Height was measured using standard tool. Stunting was defined as height-for-age z-score of <-1. A validated dietary habit questionnaire was applied to assess dietary intakes.Results Mean age of children was 10.02 ± 2.04 years. Stunting was prevalent among 50.6% of children. Compared with children in the lowest category of dairy consumption, those in the highest category had lower odds of stunting. This association remained significant even after adjusting for covariates (OR: 0.50, 95% CI: 0.29–0.87). In addition, moderate consumption of egg (1–3 time/wk) was inversely associated with stunting either before or after controlling for potential confounders (OR: 0.36, 95% CI: 0.21–0.64). Such finding was also seen among mentally retarded children (OR: 0.38, 95% CI: 0.16–0.89). No other significant association was seen between intakes of other food groups (including meat, fruits and vegetables) and stunting.Conclusion We found that higher intake of dairy products and egg was associated with lower risk of stunting. However, intakes of other food groups including meat, fruits and vegetables were not significantly related to stunting.

AbstractA standardised, national, 160-item FFQ, the FFQ-NL 1.0, was recently developed for Dutch epidemiological studies. The objective was to validate the FFQ-NL 1.0 against multiple 24-h recalls (24hR) and recovery and concentration biomarkers. The FFQ-NL 1.0 was filled out by 383 participants (25–69 years) from the Nutrition Questionnaires plus study. For each participant, one to two urinary and blood samples and one to five (mean 2·7) telephone-based 24hR were available. Group-level bias, correlation coefficients, attenuation factors, de-attenuated correlation coefficients and ranking agreement were assessed. Compared with the 24hR, the FFQ-NL 1.0 estimated the intake of energy and macronutrients well. However, it underestimated intakes of SFA andtrans-fatty acids and alcohol and overestimated intakes of most vitamins by >5 %. The median correlation coefficient was 0·39 for energy and macronutrients, 0·30 for micronutrients and 0·30 for food groups. The FFQ underestimated protein intake by an average of 16 % and K by 5 %, relative to their urinary recovery biomarkers. Attenuation factors were 0·44 and 0·46 for protein and K, respectively. Correlation coefficients were 0·43–0·47 between (fatty) fish intake and plasma EPA and DHA and 0·24–0·43 between fruit and vegetable intakes and plasma carotenoids. In conclusion, the overall validity of the newly developed FFQ-NL 1.0 was acceptable to good. The FFQ-NL 1.0 is well suited for future use within Dutch cohort studies among adults.

A number of studies have examined dietary patterns in various populations. However, to study to what extent such patterns capture meaningful differences in consumption of foods is of interest. In the present study, we identified important dietary patterns in Norwegian postmenopausal women (age 50–69 years,n361), and evaluated these patterns by examining their associations with plasma carotenoids. Diet was assessed by a 253-item FFQ. These 253 food items were categorised into forty-six food groups, and dietary patterns were identified using principal component analysis. We used the partial correlation coefficient (radj) and multiple linear regression analysis to examine the associations between the dietary patterns and the plasma carotenoids α-carotene, β-carotene, β-cryptoxanthin, lutein, lycopene and zeaxanthin. Overall, four dietary patterns were identified: the ‘Western’; ‘Vegetarian’; ‘Continental’; ‘High-protein’. The ‘Western’ dietary pattern scores were significantly inversely correlated with plasma lutein, zeaxanthin, lycopene and total carotenoids ( − 0·25 ≤ radj≤ − 0·13). The ‘Vegetarian’ dietary pattern scores were significantly positively correlated with all the plasma carotenoids (0·15 ≤ radj≤ 0·24). The ‘Continental’ dietary pattern scores were significantly inversely correlated with plasma lutein and α-carotene (radj= − 0·13). No significant association between the ‘High-protein’ dietary pattern scores and the plasma carotenoids was found. In conclusion, the healthy dietary pattern, the ‘Vegetarian’ pattern, is associated with a more favourable profile of the plasma carotenoids than our unhealthy dietary patterns, the ‘Western’ and ‘Continental’ patterns.

β-Carotene (BC), β-cryptoxanthin (CX) and α-carotene (AC) are common carotenoids that form retinol. The amount of retinol (vitamin A) formed from carotenoid-rich foods should depend chiefly on the bioavailability (absorption and circulation time in the body) of carotenoids from their major food sources and the selectivity and reactivity of carotene cleavage enzymes towards them. The objective of the present study was to estimate the apparent bioavailability of the major sources of provitamin A (AC, BC and CX) from the diet by comparing the concentrations of these carotenoids in blood to their dietary intakes. Dietary intakes were estimated by FFQ (three studies in this laboratory, n 86; apparent bioavailability calculated for six other studies, n 5738) or by food record (two studies in our laboratory, n 59; apparent bioavailability calculated for two other studies, n 54). Carotenoid concentrations were measured by reversed-phase HPLC. Apparent bioavailability was calculated as the ratio of concentration in the blood to carotenoid intake. Then apparent bioavailabilities for AC and CX were compared to BC. Eating comparable amounts of AC-, CX- and BC-rich foods resulted in 53 % greater AC (99 % CI 23, 83) and 725 % greater CX (99 % CI 535, 915) concentrations in the blood. This suggests that the apparent bioavailability of CX from typical diets is greater than that of BC. Thus, CX-rich foods might be better sources of vitamin A than expected.

AbstractObjectiveTo investigate associations of maternal periconceptional shellfish, lean fish and fatty fish intake with risk of pregnancy complications.DesignIn this prospective cohort study, we collected information on intake of seafood subtypes using FFQ. We categorized seafood intake into frequencies of <0·2 servings/month, 0·2 servings/month–<0·5 servings/week, 0·5–1·0 servings/week and >1 servings/week. We ascertained gestational hypertension, pre-eclampsia, gestational diabetes and preterm birth diagnoses from medical records. Using generalized linear models with a log link, the Poisson family and robust standard errors, we estimated risk ratios and 95 % confidence intervals across seafood intake categories.SettingThe Omega study, a study of risk factors for pregnancy complications among women recruited from prenatal clinics in Washington State, USA, 1996–2008.SubjectsThe current study included 3279 participants from the Omega study.ResultsMedian (interquartile range) shellfish, lean fish and fatty fish intake was 0·3 (0–0·9), 0·5 (0–1·0) and 0·5 (0·1–1·0) servings/week, respectively. Lean fish intake of >1 servings/week (v. <0·2 servings/month) was associated with a 1·55-fold higher risk of preterm birth (95 % CI 1·04, 2·30) and was not associated with the other pregnancy complications. Higher intake of seafood (total or other subtypes) was not associated with pregnancy complications (separately or combined).ConclusionsHigher intake of lean fish, but not fatty fish or shellfish, was associated with a higher risk of preterm birth; these findings may have significance for preterm birth prevention. Studies of mechanisms and potential contributing factors (including seafood preparation and nutrient/contaminant content) are warranted.

Abstract We aimed to assess the dietary composition of lunch meal using a posteriori derived dietary patterns and to determine the association of lunch composition with obesity in a sample of Iranian adults. This cross-sectional study was conducted on 850 men and women in Tehran (aged 20-59 y). Dietary intakes were assessed using three 24-h dietary recalls, and dietary patterns were identified via principal component factor analysis. For each identified pattern, scores were calculated for each participant and then classified into tertiles. Central obesity was defined WHO criteria. General obesity was defined as a body mass index of more than 30 kg/m2. Three major dietary patterns were identified at lunch meal using 12 food groups: “Bread, grains and fat”, “Western”, and “Potato and eggs”. After adjustment for potential confounders, participants at the top tertile of the “Bread, grains and fat” dietary pattern had greater odds for a higher waist to hip ratio, compared with those in the lowest tertile (OR, 1.44; 95% CI: 1.01 to 2.07). However, we found no association between ‘Western or ‘“potato and eggs” patterns and waist to hip ratio (OR 0.89, 95% CI: 0.62 to 1.28 and OR 1.16, 95% CI: 0.69 to 1.42, respectively). None of the identified dietary patterns was associated when defining obesity with waist circumference or body mass index. In conclusion, participants had a greater chance of central obesity defined based on waist to hip ratio following a lunchtime pattern with a higher and positive loading factor for “Bread, grains and fat”.

AbstractThe prevalence of type 2 diabetes (T2D) is low in populations with a high fish intake; however prospective studies with fish intake have shown positive, negative or no association between fish intake and the risk for T2D. The aim of this study was to investigate the effects of high intake of lean or fatty fish on glucose tolerance, leucocyte membrane fatty acid composition and leucocyte function in overweight/obese adults. In this randomised clinical trial, sixty-eight healthy overweight/obese participants consumed 750 g/week of either lean or fatty fish as dinners, or were instructed to continue their normal eating habits but to avoid fish intake (control group), for 8 weeks. Energy and macronutrient intake and physical activity were not changed within the groups during the study period. High intake of fatty fish, but not of lean fish, significantly improved glucose regulation 120 min postprandially (P=0·012), but did not affect fasting glucose concentration. A smaller increase in fasting to 120 min postprandial insulin C-peptide concentration was seen after fatty fish intake (P=0·012). Lean fish increased the DHA content in leucocyte membranes (P=0·010), and fatty fish increased the total content of n-3 PUFA (P=0·00016) and reduced the content of n-6 PUFA (P=0·00057) in leucocyte membranes. Lean and fatty fish intake did not affect phagocytosis of bacteria ex vivo. The findings suggest that high intake of fatty fish, but not of lean fish, beneficially affected postprandial glucose regulation in overweight/obese adults, and may therefore prevent or delay the development of T2D in this population.
