BackgroundOveruse and inappropriate prescribing of antibiotics is driving antibiotic resistance. GPs often prescribe antibiotics for upper respiratory tract infections (URTIs) in young children despite their marginal beneficial effects.AimTo assess the quality of antibiotic prescribing for common infections in young children attending primary care and to investigate influencing factors.Design and settingAn observational, descriptive analysis, including children attending primary care sites in England and Wales.MethodThe Diagnosis of Urinary Tract infection in Young children study collected data on 7163 children aged <5 years, presenting to UK primary care with an acute illness (<28 days). Data were compared with the European Surveillance of Antimicrobial Consumption Network (ESAC-Net) disease-specific quality indicators to assess prescribing for URTIs, tonsillitis, and otitis media, against ESAC-Net proposed standards. Non-parametric trend tests and χ2tests assessed trends and differences in prescribing by level of deprivation, site type, and demographics.ResultsPrescribing rates fell within the recommendations for URTIs but exceeded the recommended limits for tonsillitis and otitis media. The proportion of children receiving the recommended antibiotic was below standards for URTIs and tonsillitis, but within the recommended limits for otitis media. Prescribing rates increased as the level of deprivation decreased for all infections (P<0.05), and increased as the age of the child increased for URTIs and tonsillitis (P<0.05). There were no other significant trends or differences.ConclusionThe quality of antibiotic prescribing in this study was mixed and highlights the scope for future improvements. There is a need to assess further the quality of disease-specific antibiotic prescribing in UK primary care settings using data representative of routine clinical practice.

ABSTRACTYoung children learning English are biased to attend to the shape of solid rigid objects when learning novel names. This study seeks further understanding of the processes that support this behavior by examining a previous finding that three-year-old children are also biased to generalize novel names for objects made from deformable materials by shape, even after the materials are made salient. In two experiments, we examined the noun generalizations of 72 two-, three- and four-year-old children with rigid and deformable stimuli. Data reveal that three-year-old, but not two- or four-year-old, children generalize names for deformable things by shape, and that this behavior is not due to the syntactic context of the task. We suggest this behavior is an overgeneralization of three-year-old children's knowledge of how rigid things are named and discuss the implications of this finding for a developmental account of the origins of the shape bias.

This study aims to analyze the epistemology of zakat in the perspective of fiqh. The reality of Muslims shows an imbalance between the awareness of tithe and the understanding of zakat fiqh. so the vision of zakat changing mustahik into muzakki cannot be optimized. The method used is library research, with qualitative descriptive analysis techniques. The results showed that zakat was intended as "settlement". Zakat is required by Allah to be given to those who are entitled. The texts of the commands of zakat are analyzed through the linguistic approach of the Qur’an displaying them in three language styles (uslứb), namely: Using uslub insyâi, which is mandatory. Using the uslub tarhib (warning) aimed at people who like to accumulate wealth and do not want to pay zakat. Using uslub madh (praise/adulation) is God's praise for those who pay zakat. Philosophically, humanity is positioned into two groups, namely rich and poor, as a framework for God's plan to create a harmonious balance in realizing true justice and educating people to behave and behave fairly (ummatan wasatan).
