Congenital heart disease (CHD) is a congenital condition caused by a lack of heart development during the fetal stage. There is a notion that children with CHD have low nutritional status due to their condition. Nutritional input, energy requirements, dietary components, and prenatal circumstances impact a person's nutritional status. An analytical observational design with a cross-sectional approach was used in this research. Data were obtained via medical records and questionnaires posed to the patients' guardians who had provided informed consent. The Chi-square test and multivariate logistic regression analysis were used to analyze the data. The study was conducted on 62 subjects. There were 74.19% of subjects with acyanotic CHD and 25.81% with cyanotic CHD. 61.29 % of the subjects were exclusively breastfed for the first six months. The Chi-Square test revealed a relationship between breastfeeding and the nutritional status of children with CHD, with p = 0.0001 (p<0.05). The multivariate logistic regression analysis showed the significance of the type of CHD suffered by the subjects was 0.003 (p<0.05). There is a significant association between breastfeeding for the first six months and the type of CHD suffered by children with their nutritional status at Dr. Soetomo General Hospital.

Abstract: Congenital heart disease (CHD) is a congenital disorder that is common, with the incidence of 30% of all congenital abnormalities. The incidence of congenital heart disease in developed countries and developing countries ranges from 6-10 cases per 1000 live births, with an average of 8 per 1,000 live births. Nutritional status of a person is basically the person's state of health as a reflection of food consumption and use by the body. Many factors influence the nutritional status of infants and children with congenital heart disease. Nutritional status of patients with CHD is influenced nutrient inputs, energy requirements, dietary components. Objective: Knowing the relationship between congenital heart disease with nutritional status in children.  Methods:  This study is a retrospective analytic approach. The subjects were all children with congenital heart disease who are hospitalized in the Section of Child Health, Prof. Dr.  R. D. Kandou Manado in 2009-2013. The data taken in the form of data gender of the child, the child's age, weight and height as well as nutritional status. Data analysis was performed by Chi-Square test. Result: The number of respondents were 53 children, 34 boys and 19 girls. Types of congenital heart disease is the most common type of Atrial Septal Defect. Most people with experience malnutrition (54.7%), followed by poor nutrition (37.8%) and good nutrition (7.5%). With Chi-Square test of the hypothesis, obtained p-value = 0.045 which suggests a link between congenital heart disease with nutritional status in children. Conclusion: Based on the results of congenital heart disease associated with poor nutritional status in children or less. Keyword: congenital heart disease, nutritional status.   Abstrak: Penyakit jantung bawaan (PJB) merupakan kelainan bawaan yang sering dijumpai, dengan angka kejadian 30% dari seluruh kelainan bawaan.1 Insiden PJB dinegara maju maupun negara berkembang berkisar 6 – 10 kasus per 1000 kelahiran hidup, dengan rata-rata 8 per 1000 kelahiran hidup. Status gizi seseorang pada dasarnya merupakan keadaan kesehatan orang tersebut sebagai refleksi dari konsumsi pangan serta penggunaannya oleh tubuh.1 Banyak faktor ikut mempengaruhi status gizi pada bayi dan anak dengan PJB.2 Status gizi penderita PJB dipengaruhi masukan nutrien, kebutuhan energi, komponen diet.1 Tujuan: Mengetahui adanya hubungan antara penyakit jantung bawaan dengan status gizi pada anak. Metode: Penelitian ini bersifat analitik dengan pendekatan retrospektif. Subjek penelitian adalah semua anak dengan penyakit jantung bawaan yang dirawat inap di Bagian Ilmu Kesehatan Anak RSUP Prof. Dr. R. D. Kandou Manado tahun 2009-2013. Data yang diambil berupa data jenis kelamin anak, umur anak, berat badan dan tinggi badan anak serta status gizi. Analisis data dilakukan dengan uji Chi-Square. Hasil: Jumlah responden sebanyak 53 anak, 34 anak laki-laki dan 19 anak perempuan. Jenis PJB yang paling banyak diderita adalah jenis Atrial Septal Defect. Kebanyakan penderita mengalami gizi kurang (54.7%), diikuti dengan gizi buruk (37.8%) dan gizi baik (7.5%). Dengan uji hipotesis Chi-Square, didapatkan p-value = 0.045 yang menunjukkan adanya hubungan antara penyakit jantung bawaan dengan status gizi pada anak. Simpulan: Berdasarkan hasil penelitian didapatkan penyakit jantung bawaan berhubungan dengan status gizi buruk atau kurang pada anak. Kata Kunci: penyakit jantung bawaan, status gizi.

Abstract: Congenital Heart Disease (CHD) is a disorder that is commonly found. The incidence of CHD in the world is 8-10 among 1,000 births. Although children with mild or moderate CHD usually have normal growth and development, the presence of CHD can result in physical growth retardation. This study aimed to obtain the difference of nutritional status among children with cyanotic and non-cyanotic CHD. This study was a retrospective analytical study with a cross sectional design. This study was conducted at the Child Health Department Prof. Dr. R. D. Kandou Manado. Data of children treated with CHD from 2009-2014 included age, gender, weight, height, and nutritional status. Data were analyzed with Chi-Square test. Of 55 samples, there were 34 boys and 21 girls. The results showed that the most suffered CHD was ventricular septal defect. There were 54.5% of patients with good enough nutrition, 25.5% with malnutrition, and 16.4% with severe malnutrition. The Chi-Square test obtained a P-value of 0.464 (> 0.050) which indicated that there was no difference in nutritional status among children with cyanotic and non-cyanotic CHD. Conclusion: According to the study there was no difference in nutritional status between cyanotic and non-cyanotic congenital heart disease.Keywords:nutritional status, congenital heart disease, cyanotic, non-cyanoticAbstrak: Penyakit Jantung Bawaan (PJB) merupakan kelainan yang cukup banyak ditemukan. Insidensi PJB di dunia 8-10 di antara 1.000 kelahiran. Anak dengan PJB yang tidak begitu parah biasanya memiliki pertumbuhan dan perkembangan yang normal, tetapi adanya PJB dapat mengakibatkan hambatan pertumbuhan jasmani penderita. Penelitian ini bertujuan untuk mengetahui perbedaan status gizi pada anak dengan PJB sianotik dan non sianotik. Penelitian ini menggunakan metode retrospektif analitik dengan desain potong lintang dan dilaksanakan di Bagian Ilmu Kesehatan Anak BLU RSUP Prof. Dr. R. D. Kandou Manado. Data diambil dari semua anak yang dirawat dengan PJB tahun 2009-2014 meliputi umur, jenis kelamin, berat badan, tinggi badan, dan status gizi anak. Analisis data dilakukan dengan uji Chi-Square. Jumlah sampel 55 anak terdiri dari 34 lelaki dan 21 perempuan. Jenis PJB terbanyak yang diderita ialah ventricular septal defect. Penderita dengan gizi cukup 54,5%; gizi kurang 25,5%; dan gizi buruk 16,4%. Dengan uji Chi-Square, didapatkan nilai P 0,464 (< 0,050) yang menunjukkan tidak terdapat perbedaan status gizi antara anak dengan PJB sianotik dan non sianotik. Simpulan: Tidak terdapat perbedaan status gizi antara penyakit jantung bawaan sianotik dan non sianotik.Kata kunci: status gizi, penyakit jantung bawaan, sianotik, nonsianotik

Objective To assess the prevalence and survival rate of newborns with a delayed diagnosis of critical congenital heart defects (CCHD) in Beijing. Methods This retrospective study analysed data from births between 2010 and 2017 from the Birth Defects Monitoring Network in Beijing. Newborns with CCHD were analysed according to seven categories. Statistical analyses were used to calculate the mortality rate within the first week (days 0–6) after live birth. Multivariate logistic regression analysis of survival was performed to analyse the potential risk factors for newborn mortality. Results A total of 1 773 935 perinatal newborns were screened in Beijing and 1851 newborns were diagnosed with CCHD, showing a prevalence of 10.43 per 10 000. Among the total 1851 CCHD patients, the majority (1692 of 1851; 91.41%) were identified through prenatal diagnosis, 104 of 1851 (5.62%) were diagnosed before obstetric discharge/transfer and 55 of 1851 (2.97%) were identified through delayed diagnosis. The prevalence of CCHD in newborns was 1.96 per 10 000 births. Multivariate logistic regression analysis of survival demonstrated that gestational age at delivery was the only risk factor for death within the first week after birth. Conclusions Within the first week after birth, gestational age was the only risk factor for death in newborns with CCHD.

Abstract Objective: To identify the predictors of prolonged mechanical ventilation (PMV) after cardiopulmonary bypass (CPB) in infants with congenital heart disease (CHD) less than 3 months. Methods: From June 2017 to May 2020, a total of 165 infants less than 3 months old with CHD admitted to the Children's Hospital of Nanjing Medical University for CPB were enrolled in this study. The following data were collected including gender, age, weight, Risk Adjustment in Congenital Heart Surgery-1 (RACHS-1) score, preoperative levels of thyroid hormones, CPB time, aortic cross-clamping (ACC) time, mechanical ventilation time, ICU mortality and infection. The PMV was defined as the ventilator assisted time > 72h. PMV prediction was assessed by multivariate binary logistic regression analysis. Results: Compared with non-PMV group, PMV group was younger (44.74 ± 25.27 days vs. 35.44 ± 26.91 days, P= 0.001), and most were newborns (41/93 vs. 10/57, P=0.000), with a higher proportion of RACHS-1 (29/93 vs.6/57, P=0.000) and more cases of infection (47/93 vs. 17/57,P= 0.004).PMV group had significantly lower weight than non-PMV group (3.79 ± 0.83Kg vs. 4.28 ± 1.01Kg, P=0.001). In PMV group, CPB (133.74 ± 89.65 vs. 72.30 ± 44.82, P =0.000) and ACC time (52.02 ± 24.80 vs. 36.98 ± 16.63, P =0.000) were both longer. FT4 and TT4 were higher while FT3, TT3 and TSH were lower in PMV group, but only FT3 (4.99 ± 1.67 vs. 5.29 ± 1.23, P =0.017) and TT3 (1.91 ± 0.59 vs. 1.96 ± 0.49, P =0.050) showed significant differences between PMV group and non-PMV group. Conclusion: Multiple logistic regression analysis showed that weight, infection, FT3 and CPB time were independent predictors of PMV after CPB in infants with CHD.

Abstract Background: Inflammatory cytokines are involved in the development of venous thromboembolism (VTE) and may influence the mortality. But the association between their levels and severity of pulmonary embolism (PE) is unclear. This study aimed to investigate the association of inflammatory cytokines and PE severity. Methods: We retrospectively analyzed patients who were admitted to Xinhua Hospital with a confirmed diagnosis of PE between Jan 2019 and Apr 2021. Inflammatory cytokines were compared among different severities of PE groups. Spearman rank-order correlation analysis and multivariate Logistic regression analysis were used to analyze the relationship between cytokine level and PE severity. In addition, we estimated the association between comorbidities and PE severity by using multivariate Logistic regression analysis. Results: A negative correlation between IL-8 level and PE severity was found (r=-0.466, P=0.018). Comorbidities of coronary heart disease [3.76 (1.99-7.10) (P=0.000)], deep venous thrombosis [2.40 (1.15-5.01) (P=0.019)], respiratory failure [2.92 (1.33-6.4) (P=0.007)] were correlated with the severity of PE. Conclusions: IL-8 level is negative correlated with PE severity. Patients with comorbidities regarding coronary heart disease, deep venous thrombosis, and respiratory failure may have higher risk to surfer a more sever PE.

Scabies is a skin disease caused by mite infestation and sensitization Sarcoptes scabiei variety hominis. Every student who live in dormitories boarding school has the same opportunities for the occurrence of scabies. Students and environmental characteristics of the rooms thought to contribute to the incidence of scabies. Tis study aims to determine what factors most influence on the incidence of scabies disease at boarding Qotrun Nada Cipayung Depok academic year 2015/2016. Tis research was observational analytic with cross sectional design. The subjects were 258 students of MTs and MA Boarding school of Qotrun Nada academic year 2015/2016. Data analysis was based on the incidence of disease scabies using Chi-square test (p < 0,05) and followed by multivariate logistic regression. Chi-Square test results showed that there was influence among knowledge, attitudes, student behavior, density of occupant, humidity, lighting, temperature, and room ventilation students with the incidence of scabies disease at boarding Qotrun Nada Cipayung, Depok. Results of multivariate logistic regression analysis showed the most influences characteristics were student attitude and density of occupant.

Abstract Background Postanesthetic shivering is an unpleasant adverse event in surgical patients. A nonsteroidal anti-inflammatory drug has been reported to be useful in preventing postanesthetic shivering in several previous studies. The aim of this study was to evaluate the efficacy of flurbiprofen axetil being a prodrug of a nonsteroidal anti-inflammatory drug for preventing postanesthetic shivering in patients undergoing gynecologic laparotomy surgeries. Method This study is a retrospective observational study. I collected data from patients undergoing gynecologic laparotomy surgeries performed between October 1, 2019, and September 30, 2020, at Kumamoto City Hospital. All the patients were managed with general anesthesia with or without epidural analgesia. The administration of intravenous 50 mg flurbiprofen axetil for postoperative pain control at the end of the surgery was left to the individual anesthesiologist. The patients were divided into two groups: those who had received intravenous flurbiprofen axetil (flurbiprofen group) and those who had not received intravenous flurbiprofen axetil (non-flurbiprofen group), and I compared the frequency of postanesthetic shivering between the two groups. Additionally, the factors presumably associated with postanesthetic shivering were collected from the medical charts. Intergroup differences were assessed with the χ2 test with Yates’ correlation for continuity category variables. The Student’s t test was used to test for differences in continuous variables. Furthermore, a multivariate logistic regression analysis was performed to elucidate the relationship between the administration of flurbiprofen axetil and the incidence of PAS. Results I retrospectively examined the cases of 141 patients aged 49 ± 13 (range 21-84) years old. The overall postanesthetic shivering rate was 21.3% (30 of the 141 patients). The frequency of postanesthetic shivering in the flurbiprofen group (n = 31) was 6.5%, which was significantly lower than that in the non-flurbiprofen group (n = 110), 25.5% (p value = 0.022). A multivariate logistic regression analysis showed that administration of flurbiprofen axetil was independently associated with a reduced incidence of postanesthetic shivering (odds ratio 0.12; 95% confidence interval, 0.02-0.66, p value = 0.015). Conclusions My result suggests that intraoperative 50 mg flurbiprofen axetil administration for postoperative pain control is useful to prevent postanesthetic shivering in patients undergoing gynecologic laparotomy surgeries.

Abstract Background This study aimed to explore predictive factors of time below target glucose range (TBR) ≥ 1% among patients’ characteristics and glycemic variability (GV) indices using continuous glucose monitoring data in elderly patients with type 2 diabetes. Methods We conducted a prospective observational study on 179 (71 female) Japanese outpatients with type 2 diabetes aged ≥ 65 years. The characteristics of the participants with TBR ≥ 1% were evaluated by multivariate logistic regression analysis. Receiver-operating characteristic (ROC) curve analyses of GV indices, comprising coefficient of variation (CV), standard deviation, and mean amplitude of glycemic excursions, were performed to identify the optimal index for the identification of patients with TBR ≥ 1%. Results In the multivariate logistic regression analysis, none of the clinical characteristics, including HbA1c and C-peptide index, were independent markers for TBR ≥ 1%, while all three GV indices showed significant associations with TBR ≥ 1%. Among the three GV indices, CV showed the best performance based on the area under the curve in the ROC curve analyses. Conclusions Among elderly patients with type 2 diabetes, CV reflected TBR ≥ 1% most appropriately among the GV indices examined. Trial registration UMIN-CTR: UMIN000029993. Abstract Background Tissue expansion is used for scar reconstruction owing to its excellent clinical outcomes; however, the complications that emerge from tissue expansion hinder repair. Infection is considered a major complication of tissue expansion. This study aimed to analyze the perioperative risk factors for expander infection. Methods A large, retrospective, single-institution observational study was carried out over a 10-year period. The study enrolled consecutive patients who had undergone tissue expansion for scar reconstruction. Demographics, etiological data, expander-related characteristics and postoperative infection were assessed. Univariate and multivariate logistic regression analysis were performed to identify risk factors for expander infection. In addition, we conducted a sensitivity analysis for treatment failure caused by infection as an outcome. Results A total of 2374 expanders and 148 cases of expander infection were assessed. Treatment failure caused by infection occurred in 14 expanders. Multivariate logistic regression analysis identified that disease duration of ≤1 year (odds ratio (OR), 2.07; p &lt; 0.001), larger volume of expander (200–400 ml vs &lt;200 ml; OR, 1.74; p = 0.032; &gt;400 ml vs &lt;200 ml; OR, 1.76; p = 0.049), limb location (OR, 2.22; p = 0.023) and hematoma evacuation (OR, 2.17; p = 0.049) were associated with a high likelihood of expander infection. Disease duration of ≤1 year (OR, 3.88; p = 0.015) and hematoma evacuation (OR, 10.35; p = 0.001) were so related to high risk of treatment failure. Conclusions The rate of expander infection in patients undergoing scar reconstruction was 6.2%. Disease duration of &lt;1 year, expander volume of &gt;200 ml, limb location and postoperative hematoma evacuation were independent risk factors for expander infection.

Background: Vascular factors and mitochondria dysfunction contribute to the pathogenesis of Alzheimer’s disease (AD). DL-3-n-butylphthalide (NBP) has an effect in protecting mitochondria and improving microcirculation. Objective: The aim was to investigate the effect of donepezil combined NBP therapy in patients with mild-moderate AD. Methods: It was a prospective cohort study. 92 mild-moderate AD patients were classified into the donepezil alone group (n = 43) or the donepezil combined NBP group (n = 49) for 48 weeks. All patients were evaluated with Alzheimer’s Disease Assessment Scale-Cognitive subscale (ADAS-cog), Clinician’s Interview-Based Impression of Change plus caregiver input (CIBIC-plus), Alzheimer’s Disease Cooperative Study-Activities of Daily Living (ADCS-ADL), and Neuropsychiatric Inventory (NPI) every 12 weeks. All patients were monitored for adverse events (AEs). The efficacy was analyzed using multivariate logistic regression analysis. Results: The multivariate logistic regression analysis showed that the changes of ADAS-cog score (OR = 2.778, 95% CI: [1.087, 7. 100], p = 0.033) and ADCS-ADL score (OR = 2.733, 95% CI: [1.002, 7.459], p = 0.049) had significant difference between donepezil alone group and donepezil combined NBP group, while the changes of NPI (OR = 1.145, 95% CI: [0.463, 2.829], p = 0.769), MMSE (OR = 1.563, 95% CI: [0.615, 3.971], p = 0.348) and CIBIC-plus (OR = 2.593, 95% CI: [0.696, 9.685], p = 0.156) had no significant difference. The occurrence of AEs was similar in the two groups. Conclusion: Over the 48-week treatment period, donepezil combined NBP group had slower cognitive decline and better activities of daily living in patients with mild to moderate AD. These indicated that the multi-target therapeutic effect of NBP may be a new choice for AD treatment.
