EXPERIMENTAL ESTIMATION OF THE EFFICIENCY OF COMPLEX TREATMENT OF PERIODONTAL DISEASES USING AN ADAPTOGEN

Заболевания пародонта представляют собой серьезное воспалительное заболевание, которое с прогрессированием постепенно разрушает опорный аппарат зуба. Чаще всего, в практической стоматологии встречаются такие воспалительные заболевания как гингивит и пародонтит, тяжесть которых обусловлена степенью выраженности воспалительных процессов. Профилактика, диагностика и лечение данных состояний являются актуальной проблемой современной стоматологии. Научные данные подтверждают, что современные лекарственные средства оказывают влияние на все звенья этиологии и патогенеза, но тем не менее переход данных заболеваний в хроническую форму, а также неполноценный терапевтический эффект обуславливает актуальность поиска новых препаратов и комбинаций для профилактики и лечения воспалительных заболеваний пародонта. Анализ научной литературы показал, что в медицине широко используется комплексное лечение различных заболеваний органов и систем с применением адаптогенов, которые обладают не только богатым аминокислотным и витаминным составом, но и оказывают противовоспалительное, иммуномодулирующее, вяжущее и седативное действие. В данной статье представлены результаты гистологического исследования биоматериала слизистой губы крыс-самцов линии Вистар при экспериментальном моделировании воспалительного процесса в нем. Было проведено дифференцированное исследование влияния комплексного лечения с применением геля «Асепта с прополисом», модифицированного пантогематогеном на течение воспалительного процесса в сравнительном аспекте Periodontal diseases are a serious inflammatory disease that gradually destroys the supporting apparatus of the tooth as it progresses. Most often, in practical dentistry, there are such inflammatory diseases as gingivitis and periodontitis, the severity of which is due to the degree of severity of inflammatory processes. Prevention, diagnosis and treatment of these conditions are an urgent problem of modern dentistry. Scientific data confirm that modern medicines have an impact on all parts of the etiology and pathogenesis, but nevertheless, the transition of these diseases to a chronic form, as well as an incomplete therapeutic effect, determines the urgency of the search for new drugs and combinations for the prevention and treatment of inflammatory periodontal diseases. The analysis of scientific literature has shown that in medicine, complex treatment of various diseases of organs and systems is widely used with the use of adaptogens, which have not only a rich amino acid and vitamin composition, but also have anti-inflammatory, immunomodulatory, astringent and sedative effects. This article presents the results of a histological study of the biomaterial of the lip mucosa of male Wistar rats with experimental modeling of the inflammatory process in it. A differentiated study of the effect of complex treatment with the use of "Asepta with propolis" gel modified with pantohematogen on the course of the inflammatory process in a comparative aspect was conducted

It has been stated that changes of lipid exchange and increasing of lipid peroxidation are accompanied by the inflammatory process, which show the increase of proatherogenic blood characteristics. In patients with chronic non-specific inflammatory diseases of genitals disorders of phagocytosis mechanisms, increasing of autointoxication and reaction of hypersensitivity of immediate type are revealed after full blood count index. On the basis of analyses results one can conclude that the increase of atherogenic properties of blood plasma in patients with chronic non-specific inflammatory diseases of genitals took place in accordance with vascular endothelium damage. The article is the first to show the effectiveness of complex treatment of different atherosclerotic pathogenesis links by means of correction of proatherogenic lipid fractions and antioxidative complexes.

In this article the review of foreign and domestic literary sources, which are devoted to the actual problem of modern dentistry – the treatment of inflammatory diseases of periodontal tissues: gingivitis and periodontitis are presented. The complex approach to their treatment involves the appointment of a significant amount of pharmacotherapeutic drugs. Therapeutic failures and iatrogenic complications have led to the fact that today the interests of doctors and population to medicinal products significantly increased. The purpose of the study is to analyze the data of scientific literature on the use of plant-based medicinal products for the treatment of periodontal inflammatory diseases over the past 10 years. Materials and methods. Comprehensive and systematic analysis of literature. Review and discussion. The analysis of information sources on the use of plant-based medicinal products in dentistry both independently and in the composition of medical and prophylactic means has established that the modern assortment of plant-based preparations in the pharmaceutical market of Ukraine to a certain extent is limited. The emergence of new plant-based species that have been tested in conditions of experimental pathology and require an evidence-based clinical base is noted. The composition of plant-based preparations used for the treatment of inflammatory periodontal diseases include vitamins, biologically active substances, glycosides, alkaloids, in connection with a wide range of action: antiseptic, anti-inflammatory, regenerating, hemostatic, antioxidative. The data on plant-based preparations that are most often used such as chamomile extracts, calendula, hypericum, plantain, kalanchoe, aloe, eucalyptus, milfoil, nettle, calamus and plant-based species are summarized. The medicinal agents considered are mainly recommended for local treatment of periodontal diseases in the form of dental care means, mouth rinse, gel, chewing gum, herbal liquer. It is known that the complex treatment of periodontal diseases includes a general influence on the body. The properties of green tea with its wide range of actions are investigated. With antioxidant properties, it can be a healthy alternative for controlling destructive changes in periodontal diseases. Attention is drawn to the proposed unique natural complex “Resverazin” due to a wide range of pharmacological action, low toxicity and relative safety. The drug produces antioxidant, anti-inflammatory, immune stimulating, vasodilative, neuroprotective action. Conclusion. Based on the literature analysis, it can be concluded that the accumulated experimental and clinical data on the therapeutic properties of plants prove perspective of their use in the complex treatment of inflammatory periodontal diseases. Future studies are mandatory for further confirmation of the effectiveness of these medicinal plants

The article discusses the use of various dosage forms of anti-inflammatory periodontal drugs used in patients with inflammatory periodontal diseases. The rapid anti-inflammatory efficacy of toothpaste, gel, aerosol, and rinse aid has been proven. The combination of several active components in one product actively affects all parts of the pathogenesis of the inflammatory process from antimicrobial effects to the elimination of halitosis.

An analysis of the results of treatment of 48 patients with acute purulent and inflammatory disease of the small pelvis organs with endolymphatic introduction of the Selenase is presented. The age of the patients was from 20 to 45 years. Course of treatment was 5-10 days. The use of the Selenase in the scheme of intensive therapy facilitated substantial positive dynamics of clinical status. A distinct reduction of paresis of the gastrointestinal tract and decrease phenomena endotoxemia were observed clinically in most cases (94,2±2,1%). In laboratory indicators after 5-10 daily courses of intensive therapy with use of the Selenase, a tendency to normalization of the blood formula and reduce the number of leukocytes, the increase in 2 times the number of lymphocytes were noted. The use of CRT was neutralized antioxidant and immunological imbalance, increased phagocytic activity. Endolymphatic introduction of preparation contributed cleansing of wounds and cavities, regress of purulent and inflammatory processes. Application of the Selenase is significantly important factor in improving the prediction of treatment patients with purulent and inflammatory disease of the organs of small pelvis. Results of this research can be the basis for further research to develop new approaches in the field of endolymphatic therapy. Complications and cases of mortality from endolymphatic therapy by the Selenase didn´t revealed.

Local inflammatory diseases caused by various infections are one of the most common pathologies in medical practice. For example, tonsillopharyngitis. This disease is extremely frequent for a physician’s practice. There is a fair amount of drugs, which supposed to be helpful against tonsillopharyngitis, but different drugs are also not the same in their pharmacological effects. In gynecological practice, frequent diseases are specific and non-specific vulvovaginitis, which have inflammatory and infectious components. For administrating local pharmacotherapy in gynecology against inflammation, a large number of drugs with different mechanisms of action are proposed. In this study, we focused on key pathological mechanisms associated with acute and chronic inflammation, for which these drugs should be exposed by their pharmacokinetic and pharmacodynamic properties. The best combination of these properties is available for benzydamine hydrochloride. Benzydamine hydrochloride has a wide antimicrobic activity against bacteria and Candida spices both albicans and non-albicans strains and allows to influence on etiologic cause of the disease. Also benzydamine hydrochloride associated with «cytokine» mechanism of anti-inflammatory action, which means that he does not affect COX enzymes and it allows to avoid gastrotoxic adverse events. Also, in this work showed and discussed aspects of the interaction of benzydamine with local immune system and justification of useful usage the benzydamine for local therapy of acute and chronic inflammatory processes caused by various infections.

Analysis of current inflammatory of uterus diseases and uterine appendages diseases was conducted at 1769 patients - age from 18-48 years. It is revealed, that the introduction IUC is responsible gynecological operation, requiring careful selection of woman with account of contraindicated, with observance of the rules aseptic and antiseptic subsequent strict regular medical check-up not less twice per year with vaginal bacteriological researching. The development of inflammatory process internal genital organs on a background IUC requires timely hospitalization, removal IUC and complex treatment as there is tendency, to spreading of infection for area of uterus and uterine appendages, that determines volume of surgical operation.

For the first time on the basis of clinical and laboratory researches were revealed the correlation of indicators of lipid metabolism, platelet level of hemostasis and general reactivity in patients with chronic inflammatory processes of the genital system. The new pathogenetic aspects of the course of inflammatory diseases have been established. It has been shown that chronic inflammatory process is accompanied by disorders of lipid metabolism. The degree of severity of these changes depends on the severity of the inflammatory process, the reduction of phagocytic activity of polymorphonuclear leukocytes and the activation of macrophage and specific cellular mechanisms, accompanied by auto-intoxication. It is established that the frequency of atherogenic dyslipoproteinemias, as well as the intensity of lipid peroxidation increases in accordance with the increase age of the examined patients. Most atherogenic dyslipoproteinemias are found in patients with viral and chlamydial infections. The summarized data of the correlation analysis allow us to establish that the growth of atherogenic properties of blood plasma of patients with chronic inflammatory processes of the reproductive system occurs in accordance with the increase of adhesive-aggregation properties of platelets and damage of the vascular endothelium. These shifts occur in parallel with the predominance of the monocyte-macrophage linkage of phagocytosis against the background of reduced activity of neutrophils and cellular mechanisms of development of immunopathological reactions.

The purpose of our study was to increase the level of treatment of periodontal diseases in children with rheumatic diseases through the use of injections of homeopathic drugs in complex treatment. To do this, they conducted a dental examination of children with rheumatological diseases, determined the degree of damage to periodontal tissues using the indices PMA, GI, CPITN and compared with the control group of children. The relationship between inflammatory processes in in periodontal tissues and rheumatic diseases in children was revealed and the effectiveness of homeopathic drugs in the complex treatment of periodontal tissues was assessed.

The search for new ways of early detection and improving the effectiveness of treatinginflammatory periodontal diseases is one of the urgent tasks in modern dentistry. Inflammatory diseases in the periodontal tissues lead to the loss of teeth, the appearance offoci of chronic infection in the oral cavity, a decrease in the body's reactivity, microbial sensitization, and the development of allergic conditions. This pathological process is not a strictly limited pathology but, as a rule, is just one of the manifestations of more serious systemic diseases. A smile is an important part of the attractiveness and beauty of a modern person [1]. Today, more than ever, it’s important to be a well-groomed and healthy person, in whom “...everything should be fine...” as the classic said. The vast majority of patients tend to lead a healthy lifestyle, which includes dental health as well [3]. Health of periodontal tissues directly depends not only on the proper oral hygiene carried out by patients at home, but also on an individually selected set of therapeutic measures performed by the dentist [4, 5]. Thus, dentists and related specialists should provide highly professional medical care in the treatment of inflammatory diseases of periodontal tissues to achieve maximum results, stop the inflammatory process and restore the periodontium as a whole [6, 7].
