TO STUDY THE CPITN INDEX IN THE TREATMENT OF CHRONIC CATARRHAL GINGIVITIS AND CHRONIC GENERALIZED PERIODONTITIS OF MILD SEVERITY USING AN ADAPTOGEN-MODIFIED GEL

Актуальность постоянных научных исследований в области пародонтологии обусловлена большой частотой воспалительно-деструктивных заболеваний пародонта среди всех возрастных групп населения во всем мире. Лечение и профилактика данной патологии является одной из наиболее сложных и трудоемких в терапевтической стоматологии. На сегодняшний день существует множество лекарственных препаратов отечественного и зарубежного производства, предназначенных для лечения воспалительных заболеваний пародонта. Чаще всего в их состав входят антисептики, противовоспалительные компоненты, ферментные и витаминные препараты. Нехватка в составе данных лекарственных средств иммунокорригирующих препаратов и адаптогенов, в дальнейшем приводят к развитию частых рецидивов. Отсутствие комплексного подхода к лечению данных заболеваний не только приводит к длительному его течению, но и не редко приводит к хронизации процесса, что явилось предпосылкой для исследования влияния стоматологического геля, модифицированного адаптогеном на ткани пародонта при лечении хронического катарального гингивита и хронического генерализованного пародонтита легкой степени. Изучение научной литературы подтверждает, что применение в медицине адаптогенов, при лечении различных заболеваний, оказывает выраженное противовоспалительное, иммуномодулирующее, вяжущее и седативное действие. Данная статья предусматривает изучение комплексного пародонтального индекса у пациентов при лечении хронического катарального гингивита и хронического генерализованного пародонтита легкой степени тяжести в сравнительном аспекте The relevance of ongoing scientific research in the field of periodontology is due to the high frequency of inflammatory and destructive periodontal diseases among all age groups of the population around the world. Treatment and prevention of this pathology is one of the most difficult and time-consuming in therapeutic dentistry. To date, there are many medicines of domestic and foreign production intended for the treatment of inflammatory periodontal diseases. Most often, they include antiseptics, anti-inflammatory components, enzyme and vitamin preparations. The lack of immunocorrecting drugs and adaptogens in the composition of these drugs, further lead to the development of frequent relapses. The lack of an integrated approach to the treatment of these diseases not only leads to its prolonged course, but also not infrequently leads to the chronization of the process, which was a prerequisite for studying the effect of a dental gel modified with an adaptogen on periodontal tissues in the treatment of chronic catarrhal gingivitis and chronic generalized periodontitis of mild degree. The study of scientific literature confirms that the use of adaptogens in medicine, in the treatment of various diseases, has a pronounced anti-inflammatory, immunomodulatory, astringent and sedative effect. This article provides for the study of the complex periodontal index in patients in the treatment of chronic catarrhal gingivitis and chronic generalized periodontitis of mild severity in a comparative aspect

The aim of the study. Study of the effectiveness of our proposed scheme of treatment of periodontal tissues in patients with pathology of the urinary system comparing to a traditional scheme. Research methods. The study involved 72 patients with pathologies of the urinary system: glomerulonephritis, pyelonephritis and urolithiasis. They were divided into 2 groups: basic and comparison. The first group received our treatment, which consisted of professional hygiene and subsequent application of a combination of ointments of Thiotriazoline and Zinc Oxide, and rinsing with 0.05% solution of Chlorhexidine bigluconate for 5 days. The traditional treatment consisted of professional oral hygiene and rinsing with 0.05 % chlorhexidine bigluconate solution twice a day for 5 days. Patients before and after treatment were examined and indexed. In patients with a complex of antioxidant drugs, the results were better than in patients who underwent traditional treatment of periodontal tissues. Scientific novelty. Periodontal tissue pathology is one of the predominant groups of diseases of the oral cavity, so the problem of their treatment or prevention is quite relevant. Periodontal diseases in patients with pathology of the urinary system to date are studied insufficiently. There are not enough studies about the causes, features of the course, treatment and prevention of periodontal pathologies in this group of patients. Therefore, the development of new or improvement of existing treatment regimens in patients with diseases of the urinary system is of considerable interest to both scientists and practitioners. Conclusions. Our results showed that our proposed scheme of treatment of periodontal tissues in patients with pathologies of the urinary system is more effective than traditional treatment. In this article the review of foreign and domestic literary sources, which are devoted to the actual problem of modern dentistry – the treatment of inflammatory diseases of periodontal tissues: gingivitis and periodontitis are presented. The complex approach to their treatment involves the appointment of a significant amount of pharmacotherapeutic drugs. Therapeutic failures and iatrogenic complications have led to the fact that today the interests of doctors and population to medicinal products significantly increased. The purpose of the study is to analyze the data of scientific literature on the use of plant-based medicinal products for the treatment of periodontal inflammatory diseases over the past 10 years. Materials and methods. Comprehensive and systematic analysis of literature. Review and discussion. The analysis of information sources on the use of plant-based medicinal products in dentistry both independently and in the composition of medical and prophylactic means has established that the modern assortment of plant-based preparations in the pharmaceutical market of Ukraine to a certain extent is limited. The emergence of new plant-based species that have been tested in conditions of experimental pathology and require an evidence-based clinical base is noted. The composition of plant-based preparations used for the treatment of inflammatory periodontal diseases include vitamins, biologically active substances, glycosides, alkaloids, in connection with a wide range of action: antiseptic, anti-inflammatory, regenerating, hemostatic, antioxidative. The data on plant-based preparations that are most often used such as chamomile extracts, calendula, hypericum, plantain, kalanchoe, aloe, eucalyptus, milfoil, nettle, calamus and plant-based species are summarized. The medicinal agents considered are mainly recommended for local treatment of periodontal diseases in the form of dental care means, mouth rinse, gel, chewing gum, herbal liquer. It is known that the complex treatment of periodontal diseases includes a general influence on the body. The properties of green tea with its wide range of actions are investigated. With antioxidant properties, it can be a healthy alternative for controlling destructive changes in periodontal diseases. Attention is drawn to the proposed unique natural complex “Resverazin” due to a wide range of pharmacological action, low toxicity and relative safety. The drug produces antioxidant, anti-inflammatory, immune stimulating, vasodilative, neuroprotective action. Conclusion. Based on the literature analysis, it can be concluded that the accumulated experimental and clinical data on the therapeutic properties of plants prove perspective of their use in the complex treatment of inflammatory periodontal diseases. Future studies are mandatory for further confirmation of the effectiveness of these medicinal plants

Obesity prevalence has significantly increased especially in young adults, which is caused by a particular lifestyle, food quality and dietary behavior. Obesity leads to development of huge array of comorbid conditions such as arterial hypertonia, heart stroke, arthritis and other diseases. We conducted standard clinical examination of oral cavity of 154 young patients (18-21 years old) – all of them were students of Ukrainian Medical Stomatological Academy. It has been determined that the majority of patients with BMI >30 kg/cm2 have aggravated family heredity, in 66,2% cases one parent of obese students had obesity and 32,43% patients have both parents with obesity that is significantly higher compared with persons with normal BMI and overweight. There was a high prevalence of periodontal diseases about 74% and carious lesions of teeth - 97.4% among young people despite the age and contingent of examined patients - medical students. In patients with BMI >30 kg/cm2 prevalence of generalized forms of gingivitis and periodontitis was by three times higher compared with individuals with normal BMI. The values of oral hygienic indexes were poor in all examined groups, but their values were slightly lower in patients with normal BMI than in those with overweight and obesity. The prevalence of inflammatory changes in gums was higher in persons with obesity: all of them had a mild degree of lesions in periodontal tissues. Inflammatory processes in the gums were the most intense in patients with the second degree of obesity. According to the results of the study, the presence of the first and the second degree of obesity should be considered as a risk factor triggering periodontal tissues diseases. For persons with BMI >30 kg/cm2 with periodontal disease measures for the secondary prevention of inflammatory and inflammatory dystrophic periodontal diseases should be carried out and in persons without periodontal disease on the background of obesity measures primary prevention should be done.

Background. Periodontal disease is one of the most common and complex pathologies in dentistry. It is known frequently damage to the tissues of the periodontal complex with dermatoses. The most relevant among the dermatoses of the mucous membrane of the mouth and the red border of the lips is oral lichen planus. Among the six clinical forms of red flat oral lichen planus and the red border of the lips, the exudative-hyperemic and erosive-ulcerous forms occur most often. Atypical form is much less common than other forms and is often diagnosed by dentists as an inflammatory periodontal disease. At the same time, the pathogenetic mechanisms of inflammatory processes in the gums, which are different in oral lichen planus, are not taken into account, which, accordingly, complicates adequate treatment. Objectives. The aim of the study was to analyze the periodontal status in patients with exudative-hyperemic, erosive-ulcerative and atypical forms of oral lichen planus. Methods. Under our supervision there were 181 patients with oral lichen planus, in whom a simplified Green-Vermillion hygiene index was determined. To assess the state of periodontal used periodontal index according to Russell. Results. When assessing the hygienic condition of the oral cavity, high values were observed in individuals with severe forms of the disease: erosive-ulcerative, exudative-hyperemic. The highest values of the periodontal index were found in patients with atypical, erosive and ulcerative forms of oral lichen planus. Conclusions. The results of the clinical examination of patients with oral lichen planus dictate, first of all, to include in the scheme of complex treatment of it the sanitization of oral cavity and complex treatment of inflammatory periodontal diseases as well.

In recent decades, both in the world and in our country, thyroid gland diseases are not only widespread, but also have a tendency to constant growth. Currently, more attention is paid to diseases of the thyroid gland that are associated with endemic foci of iodine defi ciency in soil and water.The aim of the study – to investigate the state of periodontal tissues in individuals with primary hypothyroidism.Materials and Methods. We conducted a survey of 86 people with primary hypothyroidism, 16 of them with manifestation and 70 with subclinical forms. A comparison group was 45 people without thyroid gland diseases. Index assessment of periodontal tissues included the defi nition of a simplifi ed oral hygiene index of the oral cavity of the Green-Vermillion (Oral Hygiene Index-Simlifi ed, Green-Vermillion, 1964), the degree of gingival infl ammation was determined by Parma (C. Parma, 1960, Papillary-Marginal Alveolar Index)), the state of periodontal tissues in the complex periodontal index (KPI) (1987), destructive changes in the bone tissue of the alveolar appendix were determined by the Fux index.Results and Discussion. The article presents the results of the index estimation of periodontal tissues in individuals with primary hypothyroidism. An analysis of the conducted index evaluation found that they had a poor level of hygienic state of the oral cavity. This was manifested by the development of infl ammatory and, to a lesser extent, infl ammatory and dystrophic lesions of periodontal tissues, as evidenced by papillarymarginalalveolar (PMA) and complex periodontal indices (KPI).Conclusions. Even more signifi cant changes in these indicators were found in individuals with a manifestation of hypothyroidism. So, we can conclude that in persons with primary hypothyroidism metabolic processes ofthe organism are violated, which leads through a number of pathophysiological changes to the activity of parodontopathogenic microfl ora and the development of infl ammatory and infl ammatory and dystrophic periodontal diseases.

Relevance. Inflammatory periodontal diseases (IPD) on the background of microangiopathy among patients with diabetes mellitus (DM) are considered one of the main complications. Reduced resistance of periodontal tissues and trophic disorders due to changes in capillary walls leads to development of inflammatory processes with the destruction of gingival joint. The presence of diabetes mellitus in patients with chronic generalized periodontitis requires not only conservative periodontal therapy by a dentist, but also constant maintenance therapy, which consists in the ability of patient to maintain satisfactory oral hygiene and the use of therapeutic and prophylactic toothpastes.Purpose. To evaluate the effectiveness of using toothpastes with anti-inflammatory components among pa-tients with chronic generalized periodontitis and type 1 diabetes after professional oral hygiene complex.Materials and methods. The examination of 60 patients with diabetes mellitus type 1 and chronic generalized periodontitis at the age of 20 to 50 years was conducted. All patients underwent a questionnaire on the presence of complaints about state of the oral cavity and a comprehensive dental examination with definition of basic dental indices. Then, a complex of professional oral hygiene with individual hygiene training was carried out. Patients were divided into groups depending on the prescribed paste with anti-inflammatory components. Repeated examination was carried out after 1 month.Results. In 1 month after conservative therapy was carried out, improvement of all indexes in all groups, improvement of the acid-base state of the oral fluid was found. Patients themselves noted a better way change in state of oral cavity tissues. The best dynamics after treatment complex was observed in the 1st group.Conclusion. Hygienic and periodontal condition of the oral cavity among patients with diabetes mellitus type 1 was initially at a rather low level. It has been revealed that complex of professional oral hygiene and prescription of therapeutic and prophylactic toothpaste improve periodontal status of patients with diabetes.

Relevance. The necessity of somatic health evaluation in patients with local inflammatory periodontal processes is determined by frequent relapses of the periodontal diseases. Low effectiveness of the widespread treatment methods and the lack of long-term remission of the disease is caused by the insufficient assessment Based on the clinical examination, diagnosis and laboratory tests, to determine the correlation between the somatic pathology and adaptability of the adult body in patients with localized periodontitis due to trauma. Materials and methods. Advanced examination was carried out in 169 subjects, aged 45-75 y.o., with mild to moderate localized periodontitis due to trauma. The somatic condition was assessed by laboratory and functional tests. Results. 1-2 somatic diseases were revealed in 39.3% of patients with mild localized periodontitis caused by trauma (periodontal pocket depth up to 4 mm). 3 and more comorbidities were detected in 35.3% of patients. Gastrointestinal problems, cardiovascular diseases, allergic conditions, anemiа and respiratory diseases were most frequently encountered. In moderate localized periodontitis caused by trauma and periodontal depth of 4-6 mm, 23.9% of patients had 1-2 comorbidities, while 62.7% of patients had 3 and more diseases. Gastrointestinal problems, cardiovascular diseases, anemia, allergic conditions and urogenital diseases were more frequent. A strong correlation was revealed between the type of adaptive reaction and the degree of localized periodontitis caused by trauma. Thus, the risk of moderate localized traumatic periodontitis development increases by 1.8 times in tense adaptive reaction, and by 3.45 times - in poor adaptation reaction. Conclusion. Functional condition of the body adaptability, which is essential for the development of the treatment strategy, could be determined by the evaluation of the physical condition, peripheral blood tests (entropy of WBC differential, C-reactive protein) in patients with localized periodontitis caused by trauma. The adaptive reaction tension and decrease were detected in patients with 3 or more comorbidities.

Introduction. Periodontal diseases are among the most common chronic infections in humans. Chronic low-level bacteremia and a septicemic inflammatory response have been suggested as a pathogenetic link between periodontal disease and atherosclerosis, diabetes and other systemic diseases. All this significantly increases the relevance of the search for the means for treatment and prevention of periodontal diseases. The aim of the present study was to evaluate blood count and the antioxidant capacity of venous blood, blood plasma, and serum in patients with periodontitis and control subjects with healthy periodontal tissues, and to investigate the effect of the homeopathic medication Traumeel S on the antioxidant capacity of venous blood, plasma, and serum. Material and Methods. The study was performed using venous blood of 21 individuals with chronic periodontitis and 22 healthy subjects. Reduction properties of venous blood, blood plasma, and serum were investigated using the method of reduction of nitroblue tetrazolium, proposed by Demehin et al. Results. The data showed that there was no significant difference in venous blood hemoglobin levels or erythrocyte counts between the groups, while significantly higher leukocyte counts were observed in the periodontitis group (P<0.05). The antioxidant capacity of blood plasma was significantly higher in the periodontitis group than it was in the controls (P<0.05). Meanwhile, the antioxidant capacity of serum was significantly lower in the periodontitis group as compared with controls (P<0.05). The preparation Traumeel S had no effect on the antioxidant capacity of venous blood or blood plasma in the studied groups. Conclusions. Compared to healthy individuals, the antioxidant capacity of blood plasma in patients with periodontitis was higher, while the antioxidant capacity of serum was lower. The homeopathic medication Traumeel S had no effect on the antioxidant capacity of venous blood, blood plasma, or serum. Our findings concerning the elevated leukocyte counts in venous blood of patients with periodontitis confirm the presumption that periodontal diseases cause low-grade systemic inflammation induced by the host response to periodontal bacteria.

Abstract Introduction. Periodontitis is a group of inflammatory disorders affecting periodontal tissues. This condition manifests by a progressive destruction of the alveolar bone, subsequently leading to tooth loss. World Health Organization introduced Community Periodontal Index of Treatment Needs in order to gain data regarding periodontal health and treatment needs of people with periodontitis Aim. To evaluate the periodontal status of citizens living in the city of Lublin and its surrounding, using Community Periodontal Index of Treatment Needs (CPITN). Material and methods. Community Periodontal Index of Treatment Needs was used to assess the periodontal status among 180 patients aged 35-44 residing in Lublin and the area around it. Results. Periodontal diseases have been observed in over 90% of the examined population. Treatment need index TN1 has referred to 26.11% of the patients, TN2 – 61.67% and TN3 – 2.22% respectively. Conclusions. Patients who visit the dentist regularly have a better periodontal status as compared to groups randomly selected.

Summary. The research of periodontal diseases provides the basis for a generalized conclusion that the occurrence of pathological changes in the teeth-containing tissues occurs when the general state of the organism changes, due to the action of a number of endogenous and exogenous factors that determine the approaches to the treatment and prevention of these diseases. Despite the automation and mechanization of poultry farms, an increasing percentage of poultry workers are exposed to adverse productive factors, which lead to occupational diseases, in particular periodontal diseases.The aim of the study – to evaluate the effectiveness of the developed scheme of treatment of chronic catarrhal gingivitis in poultry factory workers according to clinical observations and paraclinical indices.Materials and Methods. To evaluate the effectiveness of the treatment of chronic catarrhal gingivitis (CCG), there were 39 employees of poultry farms, whо were divided into 2 equal groups: the main (20 persons), in which treatment was performed according to the scheme we developed and control (19 people), where patients were treated for according to the “Protocols of the Ministry of Health of Ukraine” the provision of medical care in the specialty “Therapeutic Dentistry”.Results and Discussion. In case of poultry factory workers, patients with CCG, 1 month after treatment, the index of PI in the main group was (0.45±0.03) points, the value of the index of PMA decreased by 2 times: (12.28±2.28) %, the value of the index OHI- S corresponded to satisfactory hygiene: (0.98±0.02) points. Six months after treatment, patients in the main group who tested for stabilization of the periodontal index of PI with a value of (0.45±0.04) points, a decrease in the PMA index to (11.79±2.23) and an OHI-S index of (1.08±0.01) score. In 12–18 months, the value of the index of PI in the main group was (0.63±0.23) points, the index PMA fell to (14.37±2.26) %. The condition of oral hygiene according to the OНI-S index in the main group was assessed as “satisfactory” with a value of (1.28±0.04) points.Conclusions. In the main group, the elimination of the inflammatory process in the gums was clinically observed at all stages, which was confirmed by a decrease in the periodontal indices PMA up to (11.79±2.23) %, PI up to (14.37±2.26) %, a decrease in the index of the hygiene index OHI-S to (1.08±0.01) points. Thus, the proposed therapeutic model allowed to increase the efficiency and reduce the duration of treatment of chronic catarrhal gingivitis in workers of poultry farms.
