Introduction: Diabetes mellitus (DM) is a major chronic disease. Prevalence of diabetes was 9% globally in 2014 and 9.3% in Canada and 7.2% in Alberta in 2015. Complications of the disease are numerous and frequent. Hypoglycemia is one complication of diabetes treatment. The objective of this study was to quantify and characterize presentations by adults to Alberta emergency departments (EDs) for hypoglycemia associated with type 1 (T1DM) or type 2 (T2DM) diabetes. Methods: A retrospective cohort study was conducted using data for Alberta for a five-year period (fiscal years 2010/11-2014/15). Data were sourced from an administrative database: National Ambulatory Care Reporting System (NACRS). Records of interest were those with an ICD-10-CA diagnosis of DM-associated hypoglycemia (i.e., E10.63, E11.63, E13.63, or E14.63). A descriptive analysis was conducted. Results: Data extraction yielded 7,835 presentations by 5,884 patients. The majority of presentations were by males (56.2%) and median patient age was 62. These episodes constituted 0.08% of presentations to Alberta EDs and they occurred at an event rate of 0.67 episodes per 100 patient-years (95% CI: 0.66-0.69). The annual rate of presentations decreased by 11.8% during the five-year period. Most presentations (63.4%) involved transportation to the ED via ambulance. Relative to LOS for ED presentations for all reasons, average length-of-stay (LOS) was 3.2x longer and 1.4x longer for discharged and admitted patients, respectively. For 27.5% of presentations, an X-ray was obtained. Most hypoglycemic episodes (65.2%) were considered to be of moderate severity while 34.3% were considered to be severe. None were mild because all involved access to an ED. The condition mainly (absolute terms) afflicted people with T2DM and urban areas; however, it disproportionately afflicted people with T1DM and rural areas. Conclusion: For a condition that is largely preventable with effective blood glucose management, DM-associated hypoglycemia incurs significant healthcare resource use. People with DM would be better served with more effective and safer euglycemic agents.

Abstract Background Child vaccination is recognised as a mechanism to improve child survival and health by reducing child deaths. However, there are significant inequalities in the coverage of different childhood vaccinations in both urban and rural areas. In this article, we examine the inequalities between rural and urban areas in the vaccination of children in Togo. Methods Using data from the 2017 MICS6 Survey, we examine the probability that a child aged 12–59 months will receive the required vaccinations and break down the sources of inequalities in immunisation coverage between rural and urban areas. This is done using the Oaxaca-Blinder decomposition technique, which identifies the sources and factors that explain differences in outcomes between groups. Results Analysis of the results shows that the characteristics of the child, the mother and the household influence the immunization status of the child. For example, children in rural areas are more likely to complete the required vaccinations. We find that a pro urban advantage in characteristics is observed. Conclusion There is also a need to specifically target vulnerable children in urban areas, in order to maintain the focus on women's empowerment and to pay special attention to children from poor households.

The present study was an attempt to analyze the farmers’ perception about causes and remedies of Monga in some selected areas of Lalmonirhat district of Bangladesh. In order to achieve the objectives, survey was conducted in three villages and one Char area in Hatibandha upazila of Lalmonirhat district. In total 90 samples were purposively taken of which 45 were from Char area, 25 from rural area, and 20 from urban area. The family size of the respondents revealed that medium family domination in Char area (the poorest area) is an indication of the consciousness of the poor people about population problem. Monga stricken people in terms of less consumption were higher in Char and rural areas compared to urban area. Lack of cultivable land, natural calamities, nonavailability of working facilities at non-agricultural sector, lack of working facilities at agricultural sector due to natural calamities and lack of credit availability and high interest rate were opined by the researcher as their causes of Monga. On the other hand, beef fattening, petty business, plant nursery and poultry farming were addressed as the mitigating activities of Monga undertaken by NGOs. old-age allowance, Hundred-days job scheme and Vulnerable Group Development (VGD) programme were observed as Monga mitigating activities undertaken by GOs in the study area. The difference between the performance of GO and NGO in Monga mitigation was wider in Char area than that of in rural and urban areas. The higher performance of NGOs was due to their better identification, selection and supervision strategy with perfect information. Drawbacks like, corruption, lack of proper management, lack of adequate financial support, etc., might be responsible for the poor performance of GO mitigating activities of Monga in the study area. The paper analyses construction and use of a selected indicator of pro-poor growth – the rate of pro-poor growth. It further explains the interpretation of this indicator in absolute and relative terms and indicates how economic growth affects poverty and inequality. The selected indicator is applied to the example of Indonesia and compares pro-poor growth in urban and rural areas of the country, examines regional disparities in terms of pro-poor growth for the period 1996–2019. From the absolute interpretation, pro-poor growth is observed in both urban and rural areas over the whole period. In relative terms, results of pro-poor growth for the first partial period (1996–2000) differ. While there was a relative pro-poor growth in the rural areas, there was a strong pro-poor growth in the cities with a significant decline in inequality observed (incomes of poor people increased while the average income of the whole population dropped). Indonesia achieved trickle-down growth in both rural and urban areas in two remaining periods (2000–2010 and 2010–2019).

Malnutrition is a major public health concern in Niger. The stunting rate in children in Niger is over 50%, one of the highest in the world. The purpose of this cross-sectional study was to examine children’s dietary diversity (CDD) and the maternal factors that impact CDD. A total of 1265 mother–child pairs were analyzed. Descriptive analysis was conducted to present maternal and child characteristics. To compare the mean scores of CDD in relation to the region, an independent sample t-test was conducted. A one-way ANOVA test was conducted to evaluate the CDD score by different age groups. A linear regression model was estimated to identify household, maternal and child factors that affect the CDD score. Our results indicate that most of the participants of our survey resided in rural areas and the majority (80.7%) of the mothers had no education. Factors such as region, children’s age, woman’s empowerment, vitamin A intake and wealth index were significant predictors of CDD (p < 0.05). The children residing in rural areas were more likely to have lower CDD scores (p < 0.05) than the children in urban areas, therefore becoming more susceptible to malnutrition.

The aim of our paper is to identify explanatory variables for income disparities between women and men across different regional types. Using data from the BA Employment Panel (BEP) descriptive statistics show that the gender pay gap grows wider from core regions to periphery. The main explanatory variables for the income differentials are vocational education in the men's case and size of enterprise in the women's case. Whereas in the case of women the importance of vocational status increases and the importance of size of enterprise decreases from rural areas to urban areas.

English
Qualified human resources with a good commitment to develop agricultural sector is one of the determining factors toward sustainable agricultural development. However, agricultural development deals with significant issue especially reduction in the number of young farmers. This paper aims to review structural changes from perspective of aging farmer and declined number of young farmers in Indonesia and other countries. Specifically, this paper identifies various factors causing the changes and describes the policies needed to support young workers to enter agricultural sector. The method used in this paper is both descriptive analysis and cross tabulation. The results show that aging farmers and young farmers decline in Indonesia keep increasing. The phenomena are also found in other countries in Asia, Europe, America and Australia. Various factors causing lack interest of young workers in agricultural sector, namely less prestigious, high risk, less assurance, unstable earning. Other factors are small size land holding, limited non-agricultural diversification and agricultural processing activities in rural areas, slow farm management succession, and lack of incentive for young farmers. To attract youth to enter agricultural sector, it is necessary to transform youth’s perception that agricultural sector currently is interesting and promising. The government needs to development agricultural industry in rural areas, introduces technology innovation, offers special incentives for young farmers, modernizes agriculture, and conducts training and empowerment of young farmers.

Agriculture is the primary sector in many provinces in Indonesia. In fact, most of the rural communities work in the agricultural sector. Nevertheless, the poverty level in rural areas remains high. Therefore, this study was aimed at investigating the performance of the agricultural sector in reducing the rural poverty level in Indonesia, and to investigate factors that contribute as a determinant in reducing rural poverty level in Indonesia. This study was significant, considering that the result was to contribute to government policy evaluation in the agricultural sector, especially in reducing poverty in rural areas. This study used quantitative analysis through multiple regressions with data panel from 2014 to 2017 from 33 provinces in Indonesia. This study revealed that the increase of agricultural sector share and the widening of the income distribution had caused an increase in poor people in a rural area. This finding also revealed that the income distribution gap was a determinant to the severity of rural poverty. The growth in the agricultural sector to contribute toward the economy could reduce rural poverty level in Indonesia. Meanwhile, agricultural financing, economic growth, inflation, and the farmer exchange rate had not significantly contributed to reducing the poverty level.

The previous chapters have provided a background in factors related to infant and maternal health and the various ways these individual and neighborhood characteristics can be studied in a GIS environment. Though rich in information for approaching investigations in urban areas, this book has yet to delve into these issues from a rural perspective, wherein some of the salient variables differ and certainly these data require additional understanding for use with GIS. Therefore, this chapter introduces some of the topics that could be included in health investigations in rural areas, along with some of the caveats for working with and interpreting the GIS results.
