Practicing a piano is important for a pianist. A good training program certainly needs to be designed for every pianist. Long time and self-discipline as the basis for training, is actually not enough to get maximum results. Complaints such as saturation, boredom, finger injury, even frustration are always a cliché problem that often occurs. Creating a training program that is right on target and efficient is very necessary to be designed, so that training can be fun, stimulate and of course get a fast and significant development. Then what kind of exercise program should the pianist know in order to get maximum results? The following will explain some of the suggestions and trick that can help a pianist, so he can make the best use of training time.

Objective: The primary aim of the present article was to identify palliative care patient populations who are willing to participate in and able to complete a group exercise/physical training program designed specifically for the individual patient.Method: We conducted a prospective phase II intervention study examining the willingness and ability of palliative care cancer patients to participate in a group exercise physical training program. Patients who were diagnosed with incurable cancer and had a life expectancy of less than 1 year at two outpatient clinics were invited to participate in an exercise program in the hospitals. The groups met twice a week over a 6-week period. Results: One hundred one consecutive patients were asked for inclusion. Sixty-three patients agreed to participate. Sixteen (25%) of the 63 patients dropped out after consent was given, but before the program started due to medical problems, social reasons, or death. Thus, 47 patients started the exercise program. Thirteen patients withdrew during the program due to sudden death, medical problems, or social reasons. The most frequent reasons for withdrawal were increased pain or other symptoms. Thirty-four patients completed the exercise program.Significance of results: A high proportion of incurable cancer patients were willing to participate (63%) in a structured exercise program. The attrition rate was high, but despite being severely ill, 54% of the patients completed the exercise period. This shows that a physical exercise program tailored to the individual patient is feasible in this population.

The purpose of this study was to assess the effect of a six week exercise program to reduce the muscle tone of the trapezius and musculoskeletal discomfort (MED) of office workers. Twenty-six workers performed an exercise program based on: (1) stretching of cervical and/or dorsal region; (2) joint mobility of shoulders and rachis; (3) strengthening deep stabilizer and core muscles; and (4) scapula stabilizing exercises. A Myoton device was used to evaluate trapezius tone and the Cornell Musculoskeletal Discomfort Questionnaire was used to assess changes in MED at three points of evaluation: at the beginning (Pre_1) and at the end of the workday (Post_1), and after the training program (Pre_2). The Wilcoxon test and Cohen’s d were performed to examine differences and effect sizes between evaluations. Main results show that trapezius tone remained constant during the workday, but decreased in the dominant upper trapezius (p = 0.003, ES = −0.60) and increased in the non-dominant middle trapezius (p = 0.016, ES = 0.45) after the exercise program, which eliminated significant muscle asymmetries. MED significantly decreased in the neck (p = 0.027, ES = −0.60) and upper back (p = 0.046, ES = −0.67). In conclusion, MED appears to improve in office workers after a six week training program, which may be explained by a decrease in trapezius tone and increase in the left middle trapezius tone.

While emerging studies have demonstrated the benefit of exercise in Substance Use Disorder (SUD) recovery outcomes, lack of motivation to engage in exercise has been indicated as one of many perceived barriers that contribute to low recruitment and adherence rates in SUD treatment. The current study aimed to explore participants’ perceptions of attending a supervised exercise program (boot camp workouts, walking/running practice, and a race event) while in treatment for SUD. A total of 109 participants were recruited to a 14-week exercise training program and 61 chose to participate in, and completed, a race at the close of the program. Interviews were conducted during weeks 6 through 14 and data were examined using Thematic Analysis. Three main themes were identified: (1) pushing forward recovery through running, (2) gaining a sense of achievement by crossing the finish line, and (3) building a sense of belonging in the program. Implications for SUD recovery programs are discussed.

Abstract Objectives Visceral adipose tissue (VAT) plays pivotal roles for an increased risk of cardiometabolic disease through triggering inflammatory process. This prospective study aimed to evaluate whether the exercise training could reduce inflammatory activity of VAT assessed by 18F-fluorodeoxyglucoase (FDG) positron emission tomography-computerized tomography (PET/CT). Methods Twenty-three overweight women who participated in exercise training program were included. Exercise training program was composed of aerobic exercise (45 min/session, 300 Kcal/day) and muscle strength training (20 min/session, 100 Kcal/day) 5 times per week for 3 months. They underwent F-18 FDG PET/CT before starting exercise program (baseline) and after completion of 3 months exercise program. Anthropometric data, clinical laboratory data, VAT area, and maximum standardized uptake value (SUVmax) of VAT were compared between baseline and after completion of the total exercise program. Results Baseline VAT SUVmax showed significant correlation with body weight, body mass index (BMI), waist circumference, and hip circumference. VAT SUVmax was significantly reduced by exercise training program. Exercise training program also reduced body weight, BMI, waist circumference, hip circumference, VAT area, subcutaneous adipose tissue area and blood pressure. The change of VAT SUVmax was significantly higher than the change of VAT area. Conclusions Exercise training could reduce the inflammatory activity of VAT assessed by F-18 FDG PET/CT. Thus, exercise would be a promising non-pharmacological strategy to reduce inflammatory activity of VAT. Furthermore, F-18 FDG PET/CT could be useful to evaluate the effect of therapeutic intervention targeted to inflammatory VAT activity.

Backgroun/rationale: Adults with physical disabilities are an underserved population among Americans who participate in adaptive shooting sports. A basic rifle marksmanship clinic has potential to promote a low impact outdoor physical activity for adults with physical disabilities as leisure skill development. Methods: This grant-funded service project of a therapeutic recreation (TR) graduate student utilized program design and evaluation. The specific program design included best practices for teaching basic rifle marksmanship and measured participants’ performance outcomes related to shooting skills and knowledge. A half-day rifle marksmanship clinic was conducted as a university-funded community service grant which provided rifle instructor certification, ammunition and supplies. The clinic was a collaboration between physical therapy (PT) and TR faculty and PT, OT, and TR majors. Results: All participants gained shooting knowledge and skill through this program. A shooting clinic format with classroom instruction followed by practice on a live firing range was effective. The use of a leisure partner for each participant helped sustain interest and continued support to develop skill after the clinic. Doing an in-home exercise program prior to the clinic reduced muscle soreness from the intensity of target practice in a clinic format. Conclusions: TR specialists may promote shooting sports as treatment for functional improvement or as leisure skill development. They may collaborate with physical and occupational therapy to emphasize psychosocial aspects of shooting sports.Recommendations: Shooting skills instructors could expand live fire training time at clinics, start training with virtual shooting or air or laser rifles to assess participant readiness, promote inclusive practice specific to shooting sports, and apply empowerment evaluation as a signature program strategy.

Training managers in the U.S. Army face a continuing problem of meeting increasing training demands with limited instructional resources, particularly qualified training personnel. In most aviation unit flight training programs, instructor pilots (IPs) serve a dual role of academic lecturer and in-the-cockpit instructor. To reduce the amount of IP time required to administer training in a reserve aviator training program, a 19-day training program was developed that incorporates home study and features of the Personalized System of Instruction (PSI). Forty-seven aviators were trained using this program. Following one year of no training, 24 of the aviators returned for refresher training using the same program. The results indicate that the home study/PSI approach reduces on-site academic training time and virtually eliminates IP time on academic topics. The results also suggest that an academic training program based on PSI can be used successfully while conserving the scarce instruction resource of IP time.

Mangrove fruits can be used as an alternative food stuffs.  Household businesses in Mempawah Regency processed mangrove fruits into products in the form of dodol, syrup, and flour. Nevertheless, the processing done by the community was very simple and required a long time. One disadvantage of household businesses in managing their businesses was the poor financial management, specifically related to the accounting bookkeeping process.  This service activity aimed to provide accounting training for household businesses so that they can have better financial management knowledge and procedures.  The method used was the accounting training practice. This training activity was participated in by the mangrove fruit processing household business operators which was carried out in Sungai Bakau Besar Laut Village, Siliki Pinyuh Sub-district, Mempawah District. The results of the training showed an increase in the knowledge and abilities of the participants in accounting bookeeping and the participants practiced themselves in their respective businesses. To support the sustainability of this training program, a mentoring program from the government and academia are needed.

Background Enhancing resilience and reducing stress are critical to increasing the readiness and performance of military personnel in operational settings. Stress and resilience programs can improve service members’ ability to manage their stress during both normal and extreme operational conditions, and have the potential to enhance safety at sea. Implementing an efficient and effective application-based training program in a military setting has unique challenges, including minimizing intrusion on the command’s training time and technological considerations (eg, Wi-Fi and Bluetooth technology restrictions). Objective An enhanced version of a training program called the Stress Resilience Training System (SRTS) was implemented into the operating environment of a naval vessel. The system contains an in-person workshop, regular mentoring in resilience-building techniques, and an iPad-based app used for biofeedback of heart rate variability coherence and training support. This work describes the lessons learned from the implementation process of the app component of the program, based on responses from active duty service members. Methods Crew members aboard a Navy vessel (N=92) volunteered to participate in the evaluation of the 10-week program. All ship personnel were provided with an initial 2.5-hour workshop, mentorship, and iPads containing the SRTS app to use for the duration of the 10-week program. Participants rated different components of the training, and their app usage during the course of the study was recorded. Results Participants somewhat agreed that the app was appropriate for military service members (mean 2.51, SD 1.14; response options 0=strongly disagree to 4=strongly agree for all ratings) and were somewhat likely to recommend the app to fellow service members (mean 2.53, SD 1.03). Ratings of the workshop’s relevance to military readiness (mean 2.68, SD 0.95) and of the instructors’ credibility (mean 3.22, SD 0.92) were higher than ratings of the app. Additionally, usage of the app was low and highly variable (mean 42.26, SD 60.53; range 0 to 312.54 minutes). Anecdotal evidence provided by crewmembers suggested that using the app on an iPad was cumbersome and that the Wi-Fi was often inaccessible, making the iPad a less valuable tool overall. Conclusions This implementation of the app component of SRTS raised questions regarding the suitability of the technological format for this operational setting. User ratings and participant comments suggested that the technology was not the most successful component of the program. The implementation in the iPad format was not conducive to the operational setting and the inconvenience of this format may have deterred participants from using it in settings where a personal or more compact device may have been more appropriate. Recommendations going forward include making the app component available for use on smartphones operating on both iOS and Android platforms to make it user friendly, accessible, and more engaging, which, in turn, is expected to increase usage and uptake of the program’s techniques. Furthermore, incorporating more engaging content, gamification, and tracking and reporting user progress into the overall app may enhance motivation to use more components of the app and increase usage, ultimately enhancing its impact on resilience.
