The aim of the study was to evaluate the effectiveness of psychological (adherence to treatment) and accompanying drug therapy depending on the form of pulmonary tuberculosis. The results of complex therapy of 326 patients with various forms of pulmonary tuberculosis are considered. All patients received etiotropic treatment, taking into account the sensitivity of the pathogen to anti-tuberculosis drugs, as well as support therapy, including psychological support and, if indicated, hepatoprotective therapy with Remaxol. When analyzing the dependence of the psychoemotional state of patients on the form of the tuberculous process in the lungs, it was noted that high adherence to therapy was observed in one third of patients in all groups (DTL — 26.1%, ITL — 28.6%, FCTL — 32.8% and ATL — 31.8%, respectively, P>0.05). The results obtained may be associated with a high level of resistance of mycobacterium tuberculosis excreted from pa tients: out of 232 patients with low adherence to treatment (NUP and UUP), 97 (36.2%) showed drug resistance of the pathogen to two or more etiotropic drugs, which, in turn, caused prescription of reserve drugs. Thus, the determination of the level of adherence to treatment is justified and is aimed at correcting the psychological support of patients with pulmonary tuberculosis, which makes it possible to increase the effectiveness of therapy in these patients. The appointment of the latter in a number of patients led to the development of hepatotoxic reactions, which served as the basis for the inclusion in the therapy regimens of the accompaniment of Remaxol (intravenous drip of 400 ml, course No. 5–7). Pronounced hepatoprotective (improved transaminase parameters) and insignificant immunoprotective (positive dynamics of the cytokine profile) effects were noted during treatment. The noted changes in the immunogram can be associated with the proven antioxidant and membrane stabilizing effect of succinic acid, which is part of Remaxol. These changes require further research.

Tuberculosis is a severe infectious disease that negatively affects not only the physical condition of patients, but also the psychological condition of patients. The first thing — the shock of the first diagnosis of tuberculosis for the patient. The second thing — when the patient realizes that he must be treated for an unusually long time. Objective — to learn the impact of providing psychological support for the patients with tuberculosis in a phthisiatric hospital in the form of individual counseling and group classes for patients. Materials and methods. A prospective surgical study was provided to establish the level of knowledge, level of adherence to treatment of patients with tuberculosis and the degree of violation of their psycho-emotional state at the beginning of treatment, and individual counseling and group classes for patients. The study included 335 patients with multidrug-resistant pulmonary tuberculosis, who received treatment at the clinic of the National Institute of Phthisiology and Pulmonology named after F.G. Yanovsky NAMS of Ukraine on short-term and individualized regimens of antimycobacterial therapy in the framework of scientific developments. Patients received questionnaires before treatment and before discharge, counseling on tuberculosis and their psychological state, various group classes. Results and discussion. A comprehensive approach of treatment using different methods of psychological support allowed increasing the level of adherence to treatment by 32% in the group of patients who received a group classes, compared with those patients who didn’t; reduce the level of stigma by 40.5%, halve the severity anxiety disorders. Conclusions. Comprehensive psychological support should be a mandatory component in the treatment of patients with multidrug-resistant tuberculosis, which can significantly reduce stigma, improve the psychological state of patients and their adaptation in society.

Abstract The labeling of foods containing ingredients derived from soybean is recommended in Japan because of an increasing number of patients who are allergic to soybeans. To ensure proper labeling, a novel sandwich ELISA kit for the determination of soybean protein in processed foods (FASTKIT Ver. II, Soybean, Nippon Meat Packers, Inc.; soy kit) has been developed. Five types of incurred samples (model processed foods: rice gruel, sausage, sweet adzuki bean soup, sweet potato cake, and tomato sauce) containing 10 g soybean soluble protein/g food were prepared for use in interlaboratory evaluations of the soy kit. The soy kit displayed a sufficient RSDR value (interlaboratory precision: 9.313.4 RSDR) and a high level of recovery (97114) for all the incurred samples. The RSDr value for the incurred samples was mostly <4.8. The results of this interlaboratory evaluation suggest that the soy kit can be used as a precise and reliable tool for the determination of soybean proteins in processed foods.

Less onerous, compared with warfarin, treatment with direct oral anticoagulants (DOA) can lead to better adherence to treatment of patients with atrial fibrillation (AF). However, in a certain number of patients with AF, who were recommended by DOA, cardioembolic stroke recurs, which is largely due to the patients’ failure to comply with medical recommendations. The appointment of DOA as first-line drugs does not guarantee a high adherence of patients with non-valvular AF. For elderly and old patients with AF and numerous comorbidities, the proposal of a simpler pharmacotherapy regimen is especially important. In a number of large modern studies performed in clinical practice, high adherence to rivaroxaban therapy has been established, which may be a result of taking this DOA 1 time per day, its safety and effectiveness.

Relevance. Rehabilitation of patients with moderate to severe generalized periodontitis is a leading problem in periodontology. It was the determination of the prospects for immediate implantation in patients with chronic periodontitis, combined with the pathology of the tooth root and maxillary sinus.Materials and methods. A group of 94 people with periodontitis and chronic odontogenic rhinosinus was observed who underwent sinus surgical treatment, tooth extraction and one-stage implantation with FRP growth factor according to the author's technology.Results. The method showed high efciency on the basis of assessing the clinical, aesthetic result and restoration of bone density after surgery.Conclusions. The developed technology is a promising direction, it allows to combine a high level of sanation of alveolar tissue with the advantages of immediate implantation, prevents bone atrophy, helps reduce the duration of treatment and the number of surgical and orthopedic interventions.

The set of scientific-methodological tools to secure the mechanism of economic safety management of tourism entrepreneurship is examined as an aggregate of methods, tools, and conceptual activities directed at maintaining the high level of economic safety of tourism entrepreneurship. The features of managing the tourism enterprise and economic safety are analyzed. The basic valuation principles of the reliability and efficiency of the economic safety of the tourism enterprise are determined. The basic tasks of ensuring the economic safety of a small enterprise are outlined. The need to use the integrated approach that secures more opportunities to avoid threats and limits the danger of their emergence is emphasized. The most important principles for securing the economic safety of the tourism enterprise on the microeconomic level are described. Possible practical methods of risk management for the implementation of adopted decisions are proposed. The paper proves the fact that the complex nature of the management of the economic safety of the tourism enterprise and securing the sufficiently efficient management system of detecting and eliminating the threats are provided by the establishment of the management of the economic safety system of the tourism enterprise and its functional components. Research of the methodical approaches to the management of the tourism enterprises’ economic safety allows building and describing the functional structure of the mechanism of management of the tourist enterprise’s economic safety (it is formalized and described by 5 functions: determination of aims; planning; organization and adjusting; motivation and stimulation; control and monitoring).

The determination of natural radionuclide concentrations plays an important role for assuring public health and in the estimation of the radiological hazards. This is especially true for high level radiation areas. In this study, 226Ra, 228Ra and 238U concentrations were measured in well waters surrounding eight of the high-level natural radiation areas in northern Vietnam. The 226Ra, 228Ra and 238U activity concentrations vary from <1.2 × 10−3–2.7 (0.46), <2.6 × 10−3–0.43 (0.07) and <38 × 10−3–5.32 Bq/L (0.50 of median), respectively. 226Ra and 238U isotopes in most areas are in equilibrium, except for the DT-Thai Nguyen area. The calculated radiological hazard indices are generally higher than WHO (World Health Organization) recommendations. Average annual effective dose and excess lifetime cancer risk values due to drinking well water range from to 130 to 540 μSv/year and 7.4 × 10−6 to 3.1 × 10−5, respectively.

Abstract Previous studies have shown that the Ancient Egyptians used malted wheat and barley as the main ingredients in beer brewing, but the chemical determination of the exact recipe is still lacking. To investigate the constituents of ancient beer, we conducted a detailed IR and GC-MS based metabolite analyses targeting volatile and non-volatile metabolites on the residues recovered from the interior of vats in what is currently the world’s oldest (c. 3600 BCE) installation for large-scale beer production located at the major pre-pharaonic political center at Hierakonpolis, Egypt. In addition to distinguishing the chemical signatures of various flavoring agents, such as dates, a significant result of our analysis is the finding, for the first time, of phosphoric acid in high level probably used as a preservative much like in modern beverages. This suggests that the early brewers had acquired the knowledge needed to efficiently produce and preserve large quantities of beer. This study provides the most detailed chemical profile of an ancient beer using modern spectrometric techniques and providing evidence for the likely starting materials used in beer brewing.
