Summary. Aim. Decrease the amount of postoperative complications and improve quality of life for patients by a chronic hemorrhoid (CH). Materials and methods. Both international and national foreign clinical protocols are analysed. The obtained data are confronted with our clinical experience. Results and their discussion. Materials and methods. In the looks of foreign colleagues to tactics of treatment of patients with CH are analysed. The obtained data are confronted from positions of the best medical practice of providing of medicaid to the patients with a CH. Results and their discussion. In spite of insignificant differences in the volume of operative treatment of patients of CH, international practice adheres to single approach on tactics of surgical decision of this problem. Among the radical methods of treatment recommend next methodologies as an operation of choice: hemorrhoidectomy Milligan - Morgan, Ferguson and Parks, that in all researches were marked as high-efficiency. Clinical experience of our clinic, that Milligan–Morgan hemorrhoidectomy is the operation of choice for patients with 3-4 degrees of fall of knots, and applications of modern methodologies allows to bring down lacks of this operation and to attain the level of complications less than 1 %. Conclusions. The necessity of development of compatible clinical protocol of medicare to the patients takes place with CH. Introduction of single home standards of providing of medicare to the patients with CH will allow to bring down the risk of development of complications and improve the results of treatment. The got results of own researches on providing of medicare to the patients with CH are comparable with the results of foreign colleagues.

Helminthiases of animals represent a serious problem for livestock production, especially for farms with high concentration of livestock, cause to livestock production the significant economic damage determined by death of animals, depression of efficiency, rejection of the organs struck with parasites and deterioration of production. Solutions of problems of a chemotherapy and prophylaxis of helminthiases of animals are assumed by existence of anthelmintic agents which high efficiency and safety is proved according to modern demands. For this purpose a certain order of scientific research at various levels, the most important of which is preclinical assessment of efficiency and safety on the basis of the international standards and ensuring comparable quality of results of researches, has to be carried out. Preclinical studies are conducted, as a rule, on adequate laboratory models of animals that allows to solve a series of important theoretical and practical questions, to tap new effective and safe substances and to offer for approbation in clinical conditions optimum doses and schemes of use of anthelmintic drugs for veterinary use.

One of the most effective methods of treatment of ureteral concretions is ureteroscopy with contact lithotripsy. The study analyzed the results of treatment of 96 patients with urolithiasis they were as follows: 44 patients underwent laser ureterolithotripsy, 52 persons had an open ureterolithotomy. When comparing different treatments for patients with ureterolithiasis, it has been found that the average duration of surgery when performing laser ureterolithotripsy was shorter and averaged 53 minutes. As for open ureterolithotomy it lasted in average 102 minutes. The average length of patients staying in clinic when performing laser ureterolithotripsy in average was 2.5 days, during open ureterolithotomy that index was 20 days. This is a significant advantage of endoscopic treatment of ureterolithiasis versus an open surgery (especially when a holmium laser is used in lithotripsy). By using endoscopic methods of treatment of ureteral concretions instead of an open surgery (ureterolithotomy), it has been possible to reduce the in−patient staying 8 times. Lithotripsy with a holmium laser occurred to be an effective method of destroying the ureteral concretions of any mineral composition, if the dimensions of these calculi did not exceed 2 cm. It should be emphasized that under these conditions, the localization of a stone and the duration of its stay in the ureter are also not important. In addition, the use of a holmium laser minimizes an injury to the ureter wall. The advantages of lithotripsy when it used with a holmium laser consist in its high efficiency in destruction of solid fixed and the ingrown stones. In case of combination of both a stone and ureteral stricture and in the presence of ligature calculi, the treatment with a holmium laser is also prescribed. Background. One of the most difficult in the clinical practice patients groups are patients with orofacial pain. A possible cause of pain in the face and mouth can be stylohyoid syndrome or Eagle syndrome. The clinical manifestations of the syndrome are observed in only 4-10.3% of cases among the total number of cases of elongated hyoid processes. Objective. To develop criteria for the choice of the surgical access for a safe removal of the hyoid process depending on its anatomy. Methods. In patients with the symptoms of stylohyoid syndrome, in the case of detecting an extended hyoid process during the clinical study, its removal was performed via an intraoral route (n=3). In the other patients (n=10) the removal was performed via an external access along the first cervical fold, or through the vertical retroaural access. Results. In patients with stylohyoid syndrome, we observed a high efficiency of the surgical treatment and rather short times of postsurgical rehabilitation, that significantly improved the quality of life. Conclusion. In our opinion, the surgical access through the first cervical fold is the most convenient way both from the technical viewpoint and from the viewpoint of a minimal cosmetic defect.

Introduction. Treatment of post-radiation damage to the rectum is a complex medical problem. The search for new methods of treatment for this category of patients based on modern achievements in regenerative surgery is urgent.The aim of the study was to evaluate the immediate and long-term results of treatment of post-radiation lesions of the rectum using microfat grafting.Materials and methods. The study involved 66 patients — women aged 28 to 83 years inclusive with complications of radiation therapy (rectovaginal fistulas, radiation proctitis, radiation ulcers).Results and discussion. After the treatment, there is a tendency to improve the condition. So, if before the procedure the average sizes of fistulas and ulcers were 1.67 ± 0.54 cm in diameter, then after 3 months these indicators were 1.39 ± 0.54, and after 6 months — 1.1 ± 0.56 cm Complete epithelialization of the defect was observed in all patients. The period for which it was possible to achieve complete epithelialization varies from 6 months to 2 years. On average, to achieve complete epithelialization of post-radiation damage to the rectum, most patients required 3 injections of autologous adipose tissue. Also, all patients showed an improvement in elastometry indices over time. As a result of treatment, there is a tendency to improve the quality of life of patients.Conclutions. The study allows us to conclude about the effectiveness of the treatment, as well as that in the long term, the treatment will also be effective, the likelihood of relapse is minimal.

Patients with secondary pyelonephritis have a high risk of developing chronic renal failure and a decline in the quality of life already in childhood. This pathology is prone to chronization, recurrence and progression with the formation of nephrosclerosis and the development of CRF, leading to disability of the body and requiring extraordinary interventions. Annually in the world among children it is fixed about 50 million cases of infection of urinary ways. In an open, prospective comparative study, the results of the analysis of the outcomes of the therapeutic and prophylactic efficacy of various therapies are presented in 85 children aged 4-15 years with chronic dysmetabolic pyelonephritis. Objective: to develop the principles of secondary prevention of chronic dysmetabolic pyelonephritis in children. Patients were divided into 2 groups depending on the method of treatment. 1 group consisted of 48 patients who used conventional methods of treatment. Group 2 consisted of 37 patients with children who received complex therapy: regional lymphotropic antibiotic therapy + electrophoresis with 0.5% euphillin. The results of our studies showed that after the conventional treatment there was no positive dynamics of the studied indicators of the functional state of the kidneys. A comparative assessment of the results of the study of the functional state of the kidneys and indices of endogenous intoxication showed a high efficiency of the combined use of regional lymphotropic antibiotic therapy + electrophoresis with euphyllin. Complex treatment, recommended by the authors, promotes renoprophylaxis: prevention of frequent recurrence of the disease and development of renal failure.

Need for optimal treatment approaches for adult patients with mucopolysaccharidosis is an urgent problem today. This happened dueto the fact that previously patients with MS rarely lived to adulthood and were observed mainly by pediatricians. But with the evolution of medical technologies and the emergence of modern methods of treatment and rehabilitation of such patients, the number of adult patients with MS is increasing. Clinicians have more and more questions in choosing a strategy for managing each specific clinical case. Currently, a comprehensive approach to treatment allows for the use of a multidisciplinary team to pay due attention to concomitant diseases that occur in patients with impaired GAG metabolism. Due to the rarity of this disease in the modern literature, there are as mall number of publications that cover only certain aspects of the disease, but there are no works devoted to a comprehensive approach to the treatment and rehabilitation of such patients. The analysis of literature data on diagnostics, methods of examination,options for conservative and surgical treatment depending on the type of detected pathology and functional needs of a patient with different types of MS is carried out. Recommendations for testing and evaluating functional status are described, and the most common options for conservative and surgical treatment that take into account specific problems that reduce the patient’s quality of life are presented. It is necessary to study the problem of MPs in adult patients in more detail, as well as to coordinate specialists who have experience in managing patients with metabolic disorders, in order to improve the results of treatment and rehabilitation of patients with this pathology.

Although North Korea is one of the most closed countries in the world, it has long been pursuing international cooperation with other countries in order to upgrade the quality of its film industry to international standards. Preceding studies on this topic have mainly focused on the political influences behind filmmaking in general and very few studies have exclusively dealt with North Korea’s international co-productions. In this respect, in order to develop a comprehensive understanding of the internalization strategy of North Korea’s film productions, this paper uses the global value chain as a framework for analysis. This approach helps understand the internationalization pattern of each value chain activity of film co-productions in terms of the film location and the methods for collaborating with foreign partners. By dividing the evolution of North Korea’s international co-productions into three periods since the 1980s, this paper finds that although North Korea has shown mixed results with different aspects of the film value chain, it has generally improved its internationalization over the three periods. This paper further provides strategic directions for North Korea by learning some of the successful Chinese experiences in the film sector regarding collaboration with foreign partners—to foster a win-win situation for all involved parties.

Health of dairy cows ensures human health, so it is important that dairy products do not contain antibiotics that are used to treat any inflammation, including mastitis. In Russia at present, the problem of mastitis in cows exists with both attached and loose housing of cows. Mastitis of dairy cows are the great problem in milking husbandry. Losses from mastitis in milk yield can reach 15-20%. The main reasons for the occurrence of nonspecific mastitis of cows are the shortcomings of the working components of the milking machine: the degree of deterioration of the nipple rubber and the violation of machine milking technology (the reduction in pre-milking time and vacuum fluctuations account for 70% of all causes). Treatment of mastitis with antibiotics is effective, but not environmentally friendly and unacceptable. For the treatment of inflammation of the udder, it is necessary to use more environmentally friendly methods of treatment, one of which is the exposure to ultra-high frequency electromagnetic field tested by medicine. Use of three types of medical-mobile milking machines with the same method of exposure to ultrahigh-frequency electromagnetic fields on cow's nipples through electrodes in milking cups: LPDA-1-UHF, LPDA-2-UHF and LPDA-UHF-30 M, is described in the article. Author proved that cows with subclinical forms of mastitis recovered faster during milking with exposure to the ultrahigh frequency than when treated by antibiotics, and milking with the ultrahigh frequency device helps to recover 82% of the affected quarters with clinical forms and 100% of cows with subclinical forms of mastitis or with udder irritation. The high efficiency of the method of exposure to electromagnetic fields of the ultrahigh frequency during machine milking by means of medical-mobile milking machines LPDA-UHF for the prevention and treatment of mastitis of cows was shown.

The immediate and long-term results of treatment of 30 patients with severe pain syndrome of the lumbar region, who underwent operations on denervation of DOS, were studied. The aim of the study was to evaluate the effectiveness of minimally invasive technologies for the treatment of pain in the lumbar region (denervation of DOS), to study the near and distant results of these treatment methods. Denervation DOS is an effective minimally invasive method for the treatment of facet syndrome caused by spondylarthrosis. It allows in the early and distant postoperative periods to significantly reduce the pain syndrome and improve the quality of life of patients.
