Abstract Wide pedestal Quiescent High confinement (QH) mode discovered on DIII-D in recent years is a stationary and quiescent H-mode with the pedestal width exceeding EPED prediction by at least 25%. Its characteristics, such as low rotation, high energy confinement and ELM-free operation, make it an attractive operation mode for future reactors. Linear and nonlinear simulations using BOUT++ reduced two fluid MHD model are carried out to investigate the bursty broadband turbulence often observed in the edge of wide-pedestal QH-mode plasmas. Two kinds of MHD-scale instabilities in different spatial locations within the pedestal were found in the simulations: one mild peeling-ballooning (PB) mode (γ_PB<0.04ω_A) located near the minimum in Er well propagating in ion diamagnetic drift direction; and one drift-Alfvén wave (DAW) locates at smaller radius compared to Er well propagating in the electron diamagnetic drift direction and unstable only when the parallel electron dynamics is included in the simulation. The coupling between drift wave and shear Alfvén wave provides a possible cause of the experimentally observed local profile flattening in the upper-pedestal. The rotation direction, mode location, as well as the wavenumber of these two modes from BOUT++ simulations agree reasonably well with the experimental measurements, while the lack of quantitatively agreement is likely due to the lack of trapped electron physics in current fluid model. This work presents improved physics understanding of the pedestal stability and turbulence dynamics for wide-pedestal QH-mode.

Abstract Background Osteoma is a benign tumor of the bones, which can be classified as central or peripheral. The occurrence in the jawbones is uncommon, but when it occurs, there is a greater prevalence of the mandible. The etiology is still unknown, and the hypothesis of its development is debated. Case presentation A 35-year-old Caucasian man presenting a tumor lesion in the right jawbone that had been growing for 8 years sought medical service complaining of speaking impairment. According to the patient, the tumor appeared shortly after a minor trauma caused by tooth extraction. The diagnosis of the lesion was made through clinical, radiographic, and histological methods, and the surgical treatment was successful and satisfactory for the patient as well as the surgical team, despite a short follow-up. Conclusion Etiopathogenesis of osteoma is not determined in the majority of cases. In the present report, it was possible to hypothesize the association between a minor trauma and the development of the tumor, reinforcing the reactive theory of tumor development. The uncommon location of the osteoma, as well the possibility of identifying the possible cause of the lesion, makes this case particularly interesting.

Occipital neuralgia (ON) is a condition defined as a headache characterized by paroxysmal burning and stabbing pain located in the distribution of the greater occipital nerve (GON), lesser occipital nerve (LON), or third occipital nerves (TON). This condition can be severely impairing in symptomatic patients and is known to have numerous etiologies deriving from various origins such as trauma, anatomical abnormalities, tumors, infections, and degenerative changes. This study reports four cases of a previously undescribed anatomical variant in which the (spinal) accessory nerve (SAN) fuses with the LON before piercing the sternocleidomastoid (SCM). The fusion of these two nerves and their route through the SCM points to a potential location for nerve compression within the SCM and, in turn, another potential source of ON. This anatomical presentation has clinical significance as it provides clinicians with another possible cause of ON to consider when diagnosing patients who present with complaints of a headache. Additionally, this study explores the prevalence of piercing anatomy of the LON and GAN and discusses their clinical implications.

Abstract Stokes parameters have been measured by using a polarimeter consisting of a rotating phase plate before a fixed polarizer for bullet-shaped red, green and blue LEDs at 3 different directions of 0°, 45° and 90° from the principal axis. The degree of polarization is minimum at the observation angle 0° (observed head-on) for all colors as expected but has non-zero values (1-9%). As for the possible cause for the partial polarization, it is likely to be brought by striae inside the transparent epoxy resin that can be easily visible. Data at observation angle 90° have features common for all colors; the degree of polarization is highest, the long axis azimuth of the polarization ellipse is nearly in the horizontal direction, and the ellipticity is small. These features can be explained as follows. At observation angle 90°, only small fraction of the beam emitted nearly horizontally is detected possibly through multireflection (the plane of incidence is in the vertical plane) inside the top- and bottom-surfaces (in the horizontal direction) of the chip substrate. Since the reflectance for s-polarization (horizontal component) is higher than that for p-polarization, the emerging beam becomes horizontally polarized. The hypotheses that geometrical asymmetry generates polarization is experimentally supported.

The aim of this paper is to determine the possible cause of lagging Czech companies in the field of HR transformation. The basic source of data is data from the Czech Statistical Office. The paper uses the method of classification analysis of graduates in the field of information and communication technologies. The paper is divided into two parts, where the first part deals with the evaluation of tabular data and the second with the testing of the established hypothesis (H0). The number of graduates in the field of information and communication technologies in the Czech Republic has been steadily declining since 2015, although the results achieved so far do not indicate a significant change, which could be the main cause of Czech companies lagging behind in HR transformation. The low involvement of graduates in this field is also caused by older and backward employees in companies, who do not like to change established systems.

Environmental monitoring is essential for society to maintain balance with the environment, given this fact, several methods from the science of geoprocessing and remote sensing were created to assist in the task. The present work used the Universal Soil Loss Equation to verify the spatiality of laminar erosions in the municipalities of Mandaguaçu and Presidente Castelo Branco, both in Paraná, in addition to comparing their results with data from the Digital Vegetation Index (NDVI) to obtain relationships between the loss of soils and the development or lack of vegetation. The results identified several foci of laminar erosion in the study area, as a possible cause, soils from the highly erodible Caiuá Sandstone can be considered, as well as topographic factors of the EUPS LS factor. It should be noted that the use of digital tools, satellite images and map algebra is not essential for environmental monitoring studies.

Review question / Objective: To assess the possible cause of events, the incidence of grade 3 hyperglycemia after treating Sofosbuvir/Velpatasvir/Voxilaprevir in HCV infections. Condition being studied: Sofosbuvir, velpatasvir, and voxilaprevir (SOF/VEL/VOX) is an effective, safe rescue therapy regimen for patients have previously been treated failure. Initiating Direct-Acting Antiviral (DAA) treatment for HCV infection with diabetes have experienced hypoglycemia, it could improve insulin resistance due to clean HCV. However, some studies shown that SOF/VEL/VOX has Grade 3 hyperglycemia adverse events. This finding contradicts that other DAAs studies. Information sources: Conducting a comprehensive literature search on the pubmed, Cochrane, clinicalkey, Embase, and MEDLINE electronic databases.

Abstract COVID-19 is the most devastating pandemic situation we have experienced in our age, affecting all systems. Although it affects all systems, it shows its most important effect through thrombophilia. Therefore, the possible cause of sudden death due to COVID-19 may be embolism caused by thrombophilia. D-dimer amounts increase due to COVID-19. The thrombosis is associated with sudden death in COVID-19 disease in populations. Since individuals with thrombophilia will be more prone to death due to COVID-19, it may be appropriate to administer low doses of Clexane (Enoxaparin sodium) or low-weight heparin for prophylactic purposes in order to consider these individuals at high risk and to prevent deaths. Moreover, in order not to risk the lives of healthcare professionals with thrombophilia, it would be appropriate to keep them away from individuals with COVID-19 disease and to employ them in different healthcare services according to their fields of expertise. It should also not be forgotten that different symptoms related to COVID-19 appear day by day, these different symptoms probably show that the virus has undergone mutations in order to survive, but no matter what, its effect on thrombophilia has not been eliminated yet. This compilation aims to present the reasons and causes of death due to COVID-19, possible treatment options, and thrombophilia panel tests and new parameters that may have a place in the meticulous interpretation of these tests and possible etiopathology in the light of current information. Therefore, presenting this information in a rational manner and keeping the parameters of the thrombophilia panel under strict control predict that the deaths due to the virus will be partially reduced.

Purpose: This retrospective study evaluated the survival rates of implants compromising adjacent teeth and the associated complications. Methodology: Medical records and orthopantomographic images of 1,132 patients and 1,478 implants were retrospectively analyzed. Finally, 96 patients (52 females, 44 males) with 111 malpositioned implants were included in the study. The mean follow-up of the study was 32 ± 14 months. The patients were divided into two core groups: 1) adjacent teeth and dental implants were considerably close but tangent to each other (TAN), and 2) dental implant cutting the roots of the adjacent tooth (CUT). In addition, the CUT group was divided into two subcategories considering the possible cause of malangulation as angled implant (AI) or angled adjacent tooth (AT). Damage to adjacent teeth, future treatment requirements, and the survival rates of the implants were recorded. Results. Among the 111 implants, 4 (3.6%) implants failed, all of which belonged to the CUT category and the AI subgroup. Among the 88 preoperatively vital adjacent teeth, root canal treatment was performed in 18 (20.5%) teeth, whereas 2 (2.3%) teeth were extracted due to malpositioned implanting in follow-ups. Conclusions. The placement of implants too close to the adjacent teeth and even cutting direction did not have a statistically significant effect on the survival rates of implants. However, this could cause adjacent teeth to undergo unnecessary root canal treatment or extraction. Clinical relevance. Patients with malpositioned adjacent teeth or dilacerated root(s) adjacent to the edentulous area are at a higher risk for malpositioned implant complications. Most implant malposition complications are observed in the first premolar region (37% cases). Therefore, more attention should be given while placing implants in the first premolar region.

The prevalence of the chronic lymphoproliferative diseases is increasing worldwide with increase of the population age. It is known that the presence of comorbidities in such patients plays an important role in predicting treatment outcomes. The aim of the work was to study the prevalence and determine the structure of respiratory symptoms and comorbid pulmonary pathology in patients with chronic lymphoproliferative diseases (CLPD) in the Dnipro region of Ukraine. After analyzing 986 cards of inpatients of the hematology department, whose average age was 65 (56; 69) years, it was determined that 9.0% of patients had at least one chronic respiratory disease, the most common among which were chronic bronchitis, chronic obstructive pulmonary disease, as well as community-acquired pneumonia. Dyspnea and tachypnea are also common among patients with CLPD without established respiratory comorbidity, cardiovascular disease, or anemia. Based on the data obtained, we can recommend a study of the respiratory function and pulse oximetry, as well as a thorough collection of anamnesis of smoking and analysis of the results of chest computed tomography in all patients with CLPD in order to identify the possible cause of shortness of breath and establish the presence of respiratory comorbidity.
