The purpose of this study was to analyze the factors that affect the waiting time for clinical laboratory examinations at RSU Haji Medan in 2021. This study is a mixed method. Using a total sampling of 17 people, namely all laboratory workers, the main informants in this study were clinical pathology doctors, room coordinators, clinical laboratory analysts at Haji Medan General Hospital (RSU) totaling 3 people and patients totaling 3 people. 1 additional informant, namely the Management section at RSU Haji Medan. The quantitative results are that there is a relationship between the qualifications of laboratory personnel having a value of p = 0.000, at the facility p = 0.044, there is no relationship between the transportation of specimens, the value of p = 0.099, there is a relationship between pre-analytical, analytical and pre-analytical problems. post analytic p value = 0.022, there is no electrical stability relationship p value = 0.235. Based on the results of research according to information and informants about the qualifications of officers running smoothly. have to send the sample to a private laboratory. Based on these results, it was concluded that the waiting time for laboratory results at RSU Haji has reached the standard that is in accordance with the minimum service standards at the Haji General Hospital in Medan City.

Introduction: Waiting time and length of stay ( LOS ) is one of indicators the quality of service in emergency room (ER ) that affects satisfaction patients, where speed and the accuracy is the essence service in ER. Research aims to understand the relations of waiting time and LOS with satisfaction patients in of emergency room Jombang general hospital. Method: This research using correlational design with cross sectional approached. The technique sampling is accidental sampling. The population were 1.204 people. And the sample size 60 people. Data collection used a questionnaire for measuring satisfaction patients and observation stopwatch a use for measuring waiting time and LOS. Data analyzed by statistical tests the spearman rank. Result: The results of the study obtained 31 people ( 51.7 % ) of respondents with waiting time slow, 24 people ( 40,0 % ) of respondents with los slow, while for the satisfaction of patients 31 people ( 51.7 % ) of a patient is satisfied with service. Statistical tests the spearman rank showsed that waiting time with satisfaction obtained  r value = 0,666, p value = 0,000. To LOS with satisfaction the r value = 0,350, p value = 0,006. There is a significant relation exists between waiting time and LOS with satisfaction patients. Discussion: Waiting time and LOS significant correlated with satisfaction patients in ER Jombang general hospital. The ER in Jombang general hospital is expected to have strong commitment, to give priority to the quality of service and interests of patients. Health workers is expected to give rapid service, right and respond to the problem of patients. And advanced research required about satisfaction patients in ER and the supporting factors.

This study aims to determine the factors that influence the performance of nurses and to determine the most dominant factors affecting the performance of nurses at the General Hospital Datu Beru Takengon 2021. The research design used a quantitative analytic survey with a cross-sectional design. The population is all nurses in the Inpatient Room at RSU Datu Beru Takengon as many as 200 people. The sampling technique used was random sampling. Data analysis was performed by univariate, bivariate and multivariate analysis with chi_square. The results of the Chi-Square statistical test analysis obtained p-value = 0.003 < 0.05, which means there is a relationship between workload and nurse performance, while the length of service period obtained p-value = 0.652 < 0.05, which means there is no relationship between length of service and nurse performance, the training variable obtained p-value = 0.004 < 0.05 which means there is a relationship between training and nurse performance, the discipline variable obtained p-value = 0.005 < 0.05 which means it means that there is a relationship between discipline and nurse performance and the motivation variable is obtained p-value = 0.002 < 0.05, which means there is a relationship between motivation and nurse performance. The results of the Multivariate test analysis showed that the variables that influenced the performance of nurses were workload, discipline and motivation, while the most dominant variable was the workload variable with p = 0.002 <0.05.

ABSTRAK   Berdasarkan data di RSUD Kota Prabumulih bahwa terdapat peningkatan jumlah kejadian perdarahan post partum, tahun 2014 sebanyak 178 orang, tahun 2015 sebanyak 246 orang dan tahun 2016 sebanyak 151 orang. Penelitian ini bertujuan untuk mengetahui hubungan umur ibu dan jarak kehamilan terhadap kejadian perdarahan post partum di RSUD Kota Prabumulih Tahun 2016. Penelitian ini merupakan penelitian analitik dengan rancangan cross sectional. Populasi yang digunakan adalah semua ibu bersalin di RSUD Kota Prabumulih, sebanyak 1.296 ibu bersalin dan 306 sampel. Pengambilan sampel dengan mengunakan random sampling, analisa data menggunakan analisa univariat dan bivariat dengan menggunakan uji statistik chi-square dengan derajat kemaknaan 0,05. Hasil penelitian ini menunjukan bahwa dari 288 ibu bersalin terdapat 151 yang mengalami perdarahan post partum sebagian besar adalah ibu dengan umur resiko tinggi sebanyak 43 orang  (40,9%) dan ibu dengan jarak kehamilan resiko tinggi sebanyak 21 orang (17,3%). Hasil uju chi-square umur didapatkan p.value 0,000 < α 0,05 dan uji chi-square untuk jarak kehamilan didapatkan p.value 0,000 < α 0,05. Maka ada hubungan umur ibu dan jarak kehamilan terhadap kejadian perdarahan post partum di RSUD Kota Prabumulih Tahun 2016. ABSTRACK   Based on the data at District General Hospital (RSUD) Prabumulih, there was increasing of post-partum bleeding, in 2014, there were 178 people. In 2015, there were 246 people. And in 2016, there were 151 people. The purpose of the study was to know relationship between maternal mother and the distance and old post-partum bleedingat District General Hospital (RSUD) Prabumulih in 2013. The study was analytic researchwith cross sectional design. Population thas was used in the study was all maternal mother ar District General Hospital (RSUD) Prabumulih. It was about 1.296 maternal mother and from 306. Random sampilng was done in the study, data analyses used univariate and bivariate analyses by using chi-square statistic test with significance level 0,05. The study result showed that from 306 maternal mother, there were 151 mother who experienced old post-partum bleeding, the large of that was high maternal mother 43 people  (40,9%) and mother age high distance 21 people (17,3%). The result of chi-square test was p value 0,000 < α 0,05 and chi-square test for age was p value 0,000 < α 0,05. It meant that there was relationship between maternal mother and the distance and of post-partum bleeding at District General Hospital (RSUD) Prabumulih in 2016.

ABSTRACT Low Birth Weight (LBW) was defined as infants born weighing less than 2.500 grams. WHO estimates that nearly all (98%) of the five million neonatal deaths in developing countries. According to City Health if Palembang Departement, infant mortality rate (IMR) in the year 2007 is 3 per 1000 live births, in 2008 four per 1000 live births, and in 2009 approximately 2 per 1000 live births. The cause of LBW is a disease, maternal age, social circumstances, maternal habits factors, fetal factors and environmental factors. LBW prognosis depending on the severity of the perinatal period such as stage of gestation (gestation getting younger or lower the baby's weight, the higher the mortality), asphyxia / ischemia brain, respiratory distress syndromesmetabolic disturbances. This study aims to determine the relationship between maternal age and educations mothers of pregnancy with the incidence of LBW in the General Hospital Dr Center. Mohammad Hoesin Palembang in 2010 This study uses the Analytical Ceoss Sectional Survey. The study population was all mothers who gave birth in public hospitals center Dr. Mohammad Hoesin Palembang in 2010 were 1.476 mothers gave birth with a large sample of 94 studies of maternal taken by systematic random sampling, ie research instument Check List. Data analysis was performed univariate and bivariate. The results of this study show from 94 mothers of LBW was found 45 people (47,9%) Which has a high risk age 26 LBW ( 27,7%) while the distance of low educations LBW (55,3%). From Chi-Square test statistic that compares the p value with significance level α = 0,05 showed a significant correlation between maternal age, where the p value = 0,002, of education mothers of pregnancy p value = 0,003 with LBW. In the general hospital center Dr. Mohammad Hoesin Palembang ini 2010. Expected to researches who will come to examine in more depth.   ABSTRAK Bayi Berat Lahir Rendah (BBLR) telah didefinisikan sebagai bayi lahir kurang dari 2.500 gram. WHO memperkirakan hampir semua (98%) dari 5 juta kematian neonatal di negara berkembang. Menurut Data Dinas Kesehatan Kota Palembang, Angka Kematian Bayi (AKB) pada tahun 2007 yaitu 3 per 1.000 kelahiran hidup, pada tahun 2008 4 per 1.000 kelahiran hidup, dan pada tahun 2009 sekitar 2 per 1.000 kelahiran hidup. Penyebab BBLR adalah penyakit, usia ibu, keadaan sosial, faktor kebiasaan ibu, dan faktor lingkungan. Prognosis BBLR tergantung dari berat ringannya masa perinatal misalnya masa gestasi (makin muda masa gestasi atau makin rendah berat bayi, makin tinggi angka kematian), asfiksia atau iskemia otak, sindrom gangguan pernafasan, gangguan metabolik. Penelitian ini bertujuan untuk mengetahui hubungan antara umur dan pendidikan ibu dengan kejadian BBLR di Rumah Sakit Umum Pusat Dr. Mohammad Hoesin Palembang Tahun 2010. Penelitian ini menggunakan survey analitik Cross sectional. Populasi penelitian ini adalah semua ibu yang melahirkan di Rumah Sakit Umum Pusat Dr. Mohammad Hoesin Palembang tahun 2010 sebanyak 1.476 ibu melahirkan dengan besar sampel penelitian 94 ibu melahirkan yang diambil dengan tehnik acak sistematik, instrumen penelitian yaitu check list. Analisis data dilakukan secara univariat dan bivariat. Hasil penelitian ini menunjukkan dari 94 ibu didapatkan kejadian BBLR 45 orang (47,9%) yang memiliki umur resiko tinggi 26 kejadian BBLR (27,7%) sedangkan yang pendidikan rendah 52 kejadian BBLR (55,3%). Dari statistik uji Chi-square yang membandingkan p value dengan tingkat kemaknaan α = 0,05 menunjukkan bahwa ada hubungan yang bermakna antara umur ibu p value (0,002) , pendidikan p value (0,003) dengan kejadian BBLR di Rumah Sakit Umum Pusat Dr. Mohammad Hoesin Palembang Tahun 2010. Diharapkan bagi peneliti yang akan datang untuk meneliti lebih mendalam.

INTRODUCTION. Among systemic vasopathies in children, IgA vasculitis Henoch Schoenlein (HS) is the most common, according to various authors, kidney damage is noted in 25-80 % and usually determines the prognosis of the disease.THE AIM of the study was to analyze clinical, laboratory, immunological, morphological characteristics, features of the course and treatment of nephritis associated with IgA vasculitis HS in children, as well as factors affecting the prognosis.PATIENTS AND METHODS. The study included 31 patients with morphologically verified nephritis due to IgA vasculitis HS (18 – boys, 13 – girls) aged 3 to 17 years, who were monitored at the Nephrology Department of the "2nd Children's City Clinical Hospital" of the National Center for Pediatric Nephrology and Renal Replacement therapy in Minsk from 2010 to 2019 yrs.The following parameters were analyzed: the clinical variant of kidney damage, laboratory tests (including the study of BAFF, RANTES lymphocyte activation molecules, pro-inflammatory IL1β, caspase1, TNFα, growth factors VEGF, TGF), 24 hours monitoring and office blood pressure measurements, ECHO cardiography with indicescalculation, ultrasound of the carotid arteries with the thickness of intima-media complex, morphological changes in the renal tissue, as well as treatment regimens.RESULTS. The contribution of deGal-IgA1, markers of T and B lymphocytes activation, pro-inflammatory and profibrotic molecules in the development of the disease is shown. Arterial hypertension was registered in 42 % of children, signs of heart remodeling according to the calculated indices in 19,3 %. Decrease level of adiponectin, vitamin D, leptin, increase concentration of obestatin, Pro-BNP, hs-CRP, and TSAT indicator classify patients with nephritis due to IgA vasculitis HS at moderate risk for the developmentof cardio-vascular disorders, which suggests the need for timely correction.CONCLUSION. In most cases, nephritis with IgA vasculitis HS has a benign course with rare relapses and progression to the end stage of chronic kidney disease (6,5 %).

Acute suppurative parotitis (ASP) is an acute infection of the parotid gland that necessitates hospitalization in some patients. The aim of this study was to evaluate clinical laboratory values including hydration, nutritional status, inflammatory markers and age, and to compare them with duration of hospitalization of patients with ASP. This is a retrospective chart review in a tertiary academic center. We investigated the factors affecting length of hospitalization in patients admitted to Galilee Medical Center with a diagnosis of ASP between 2010 and 2018. Of the 60 patients with ASP included in the study, 24 were male. The average age of patients was 60, ranging from 18 to 99. We found statistically significant correlations between length of hospitalization and patient age (r=0.3), C reactive protein (r=0.3), white cell count (WCC) at presentation (r=0.3), blood urea nitrogen to creatinine ratio (BUN:Cr) (r=0.2), and platelet levels at discharge (r=0.4). Examination of these factors on multivariate analysis found hospitalization duration was exclusively affected by patients’ level of dehydration as represented by BUN:Cr. Patient age, WCC levels at presentation, and platelet levels were not found to be statistically significant. Treatment and interventions should be planned accordingly.

Background:Musculoskeletal Ultrasonography (MSUS) is now a widely used tool for monitoring of rheumatoid arthritis (RA). Although there are many proposed sets of composite scores, a fixed set of joints may not be an ideal tool to assess a disease like RA, which affects many joints and tendons in different presentations. In previous study (1) U9 score was proven to be correlated with disease activity parameters.Objectives:To determine whether US assessment using U9 score is useful for monitoring response to treatment for RA or not?Methods:A prospective, multicenter study were conducted in period from July 2019 to December 2019. All recruited RA patients were subjected to: Disease activity assessment by clinical disease activity indices (CDAI and DAS28 ESR). Functional status assessment by (HAQ) and ultrasonographic assessment using U9 score which include 8 joints (bilateral wrists,2ndMCP,3RDMCP and knees) plus most clinically affected joint or tendon (one joint or one tendon). Most clinically affected joints from 48 joints. Any affected tendons could be choosing. All targeted joints were evaluated according to EULAR guidlines and by EULAR/ OMERACT combined score (0-3). Targeted tendons were scored (0-3).All patients received their treatment (biologic and non biologic DMARDs) according to the decision of the treating physicians. No specific therapy is needed. CDAI and DAS28 ESR, HAQ and U9 score were repeated after 3 months to detect the response to change after receiving the therapy.Results:One hundred and forty patients (23.6% were male) with mean age 39.26±11.30 were recruited from 4 tertiary referral university hospitals.There was a significant difference (<0.001) between the first and second visits as regards clinical, laboratory and ultrasonographic parameters. DAS 28 decreased form (5.29±1.21) to (3.95±0.99), ESR decreased from (42.12±15.24) to (26.84±12.32), HAQ2 improved from (0.652±0.350) to (0.510±0.237) and U9 total US score decreased from (13.56±5.18) to (8.02±4.28).There was significant correlation between U9 ultrasonographic score and clinical parameters at both visits (table 1).Table 1.correlation between U9 ultrasonographic score and clinical parameters.U9 at 1stvisitU9 at 2ndvisitDAS-28Pearson Correlation(P value)0.806<0.0010.790<0.001CDAIPearson Correlation(P value)0.787<0.0010.773<0.001HAQPearson Correlation(P value)0.431<0.0010.317<0.001We found that the most suitable cut-off value of U9 score to predict high disease activity was 11.5 (sensitivity 85.7% and specificity 80.6%), cut off value for moderate disease activity was 5.5(sensitivity 83.2% and specificity 88%) and cut off value for low disease activity was 3.5 (sensitivity of 83.3% and specificity 57.1%). AbstractPatient care is rapidly evolving toward the inclusion of precision genomic medicine when genomic tests are used by clinicians to determine disease predisposition, prognosis, diagnosis, and improve therapeutic decision-making. However, unlike other clinical pathology laboratory tests, the development, deployment, and delivery of genomic tests and results are an intricate process. Genomic technologies are diverse, fast changing, and generate massive data. Implementation of these technologies in a Clinical Laboratory Improvement Amendments-certified and College of American Pathologists-accredited pathology laboratory often require custom clinical grade computational data analysis and management workflows. Additionally, accurate classification and reporting of clinically actionable genetic mutation requires well-curated disease/application-specific knowledgebases and expertise. Moreover, lack of “out of the box” technical features in electronic health record systems necessitates custom solutions for communicating genetic information to clinicians and patients. Genomic data generated as part of clinical care easily adds great value for translational research. In this article, we discuss current and future innovative clinical bioinformatics solutions and workflows developed at our institution for effective implementation of precision genomic medicine across molecular pathology, patient care, and translational genomic research.
