Within aerospace and defence sectors, maintaining asset availability during operational service has become more important than quality of service throughout the system life cycle. This requires organisations to establish cost effective strategies to manage uncertainties within their value led services e.g. maintenance activities. In large organizations, it is not always apparent whose decision affects the outcome the most. Often, accountability moves away from the designated organization personnel in unforeseen ways, and depending on the decisions of individual decision makers, the structure of the organization, or unregulated operating procedures may change. This can have far more effect on the overall reliability leading to inadequate troubleshooting, repeated down-time and reduced availability. This chapter focuses on discussing the No Fault Found problems in aviation and highlights the drivers that influence its decision making process. It further articulates the contents of tacit knowledge and discusses the knowledge gaps with current management policies.

Many organisations are currently dealing with long standing legacy issues in clean up, decommissioning and demolition projects. Industry is required to ensure that all bulk articles, substances and waste arisings are adequately characterised and assigned to the correct disposal routes in compliance with UK legislation and best practice. It is essential that data used to support waste sentencing is of the correct type, quality and quantity, and that it is appropriately assessed in order to support defensible, confident decisions that account for inherent uncertainties. AMEC has adopted the Data Quality Objectives (DQO) based methodology and the software package Visual Sample Plan (VSP) to provide a better, faster, and more cost effective approach to meeting regulatory and client requirements, whilst minimising the time spent gathering data and assessing the information. The DQO methodology is based on a scientific approach that requires clear objectives to be established from the outset of a project and that there is a demonstration of acceptability of the results. Through systematic planning, the team develops acceptance or performance criteria for the quality of the data collected and for the confidence in the final decision. The systematic planning process promotes communication between all departments and individuals involved in the decision-making process thus the planning phase gives an open and unambiguous method to support the decisions and enables the decision-makers (technical authorities on the materials of concern) to document all assumptions. The DQO process allows better planning, control and understanding of all the issues. All types of waste can be sentenced under one controllable system providing a more defensible position. This paper will explain that the DQO process consists of seven main steps that lead to a detailed Sampling and Analysis Plan (SAP). The process gives transparency to any assumptions made about the site or material being characterised and identifies individuals involved. The associated calculation effort is reduced using the statistically based sampling models produced with VSP. The first part of this paper explains the DQO based methodology and Visual Sample Plan and the second part shows how the DQO process has been applied in practice.

The long term management of a production asset raises several major issues among which rank the technical management of the plant, its economics and the fleet level perspective one has to adopt. Decision makers are therefore faced with the need to define long term policies (up to the end of asset operation) which take into account multiple criteria including safety (which is paramount) and performance. In this paper we first remind the reader of the EDF three-level methodology for asset management. We then focus on the knowledge model and on the software tools that implement this methodology in order to gather, preserve, share, maintain and exploit the expert knowledge needed for asset management and to allow decision makers to define, evaluate and analyze long term plant operation and maintenance policies. Lastly, as the quality of the processed plant level evaluations (operation & maintenance strategies are evaluated, at a plantlevel, through a set of technical and economic indicators) and their interpretation relies on the quality of the knowledge captured in the tools, we focus on the definition of a “adaptative” user interface — based on Electronic Structured Documents — that allows technical/strategic experts and decision makers to consult the useful pieces of knowledge in a context dependent way. Such an interface, which, in a near future, should be fully implemented in the tools will facilitate the validation of the knowledge-base content and the analysis of the processed results.

Chronic spontaneous urticaria (CSU) is a common, debilitating skin disorder associated with impaired quality of life and psychological comorbidity. Symptoms can be difficult to control and many individuals will not respond to first line treatment. Due to the chronic and unpredictable nature of the disorder, patients frequently have repeated healthcare attendances. Despite this, little is known about healthcare resource utilization internationally. Furthermore, there is no Irish data to inform fundholding decision makers. Omalizumab is an anti IgE monoclonal antibody used in refractory urticaria. It is a comparatively high cost medicine and access to this treatment can be challenging. Recent assessments of omalizumab compared with usual care suggest that omalizumab is a cost-effective treatment for refractory urticaria. We carried out a retrospective review of 47 patients commenced on omalizumab. We evaluated unplanned primary and secondary care attendances and urticaria symptomatology before and after treatment. As expected, patients with refractory disease that were commenced on omalizumab had objective improvements in urticaria symptoms. Importantly, we show that this is reflected in a dramatic reduction in unplanned healthcare interactions at primary care and emergency departments. These data suggest that omalizumab may benefit these patients by reducing disease activity and thereby reducing the need for unplanned healthcare interactions.

Mental disorders impose considerable socioeconomic costs due to their episodic/chronic nature, their relatively early ages at onset, and the highly disabling nature of inadequately treated mental illness. Despite substantial increases in the volume of mental health treatment for disorders in the past two decades, particularly pharmacotherapies, the level of morbidity and mortality from these disorders does not appear to have changed substantially over this period. Improving outcomes will require the development and use of more efficacious treatments for mental disorders. Likewise, implementation of cost-effective strategies to improve the quality of existing care for these disabling conditions is required.

Purpose To review the effectiveness of routine visits and routine tests in detecting isolated locoregional recurrences in asymptomatic patients after treatment for early-stage invasive breast cancer. Methods Systematic review and meta-analysis. The proportion of isolated locoregional recurrences diagnosed during routine visits or routine tests in asymptomatic patients was compared with the proportion of isolated locoregional recurrences in symptomatic patients. Results Twelve studies that involved a total of 5,045 patients and 378 isolated locoregional recurrences were identified. Pooling data showed an overall estimate of 40% of isolated locoregional recurrences diagnosed during routine visits or routine tests in asymptomatic patients (95% CI, 35 to 45). Of these, 47% (95% CI, 39 to 54) were diagnosed after mastectomy, and 36% (95% CI, 28 to 43) were diagnosed after breast-conserving therapy (relative risk, 1.327; 95% CI, 1.014 to 1.738). Apart from differences in therapy, we have not been able to discern subgroups of patients for whom results were different. Conclusion Approximately 40% of isolated locoregional recurrences are diagnosed during routine visits and routine tests in asymptomatic patients treated for early-stage invasive breast cancer. We could not assess whether these were detected by either physical examination or other tests, nor if the detection of asymptomatic isolated recurrences had any influence on potential for cure or quality of life. As the overall quality of the included studies and the overall incidence of isolated locoregional recurrences are low, this systematic review highlights the need for prospective comparative studies on cost-effective strategies for the follow-up of patients after a diagnosis of breast cancer.

AbstractEconomic evaluations can help decision makers identify what services for children with neurodevelopmental disorders provide best value-for-money. The aim of this paper is to review the best available economic evidence to support decision making for attention deficit-hyperactivity disorder (ADHD) and autism spectrum disorder (ASD) in children and adolescents. We conducted a systematic review of economic evaluations of ADHD and ASD interventions including studies published 2010–2020, identified through Econlit, Medline, PsychINFO, and ERIC databases. Only full economic evaluations comparing two or more options, considering both costs and consequences were included. The quality of the studies was assessed using the Drummond checklist. We identified ten studies of moderate-to-good quality on the cost-effectiveness of treatments for ADHD and two studies of good quality of interventions for ASD. The majority of ADHD studies evaluated pharmacotherapy (n = 8), and two investigated the economic value of psychosocial/behavioral interventions. Both economic evaluations for ASD investigated early and communication interventions. Included studies support the cost-effectiveness of behavioral parenting interventions for younger children with ADHD. Among pharmacotherapies for ADHD, different combinations of stimulant/non-stimulant medications for children were cost-effective at willingness-to-pay thresholds reported in the original papers. Early intervention for children with suspected ASD was cost-effective, but communication-focused therapy for preschool children with ASD was not. Prioritizing more studies in this area would allow decision makers to promote cost-effective and clinically effective interventions for this target group.

The results of elements development of varietal agrotechnics of a new high-potential line 238h of winter triticale presented. It was identified that under the conditions of 2020 the most cost effective fertilizer system is an early spring application of NPK (S) (15-15-15 (10)) at a dose of 200kg/ha. The profitability level of this fertilizer was 88,9%.

Growth in telecom infrastructure and provision of modern telecom services to consumers at a cost based tariff helps growth of national economy. Modern telecoms serve as the engine of growth of national economy. Following the global trends of liberalisation and deregulation in telecoms monopolies which have thus far been providing inefficient communication at a greedily high tariffs are falling apart. Mergers in telecoms are not for increasing the size of the monopoly but to provide more efficient and cost effective services to the consumers. In Pakistan the erstwhile T&T department played a needful role at its time. Conversion of the department into a corporation and then into a company were steps necessary for following the global trends. Need now is to continue this trend further, eliminate the monopolistic approach by allowing more players in the field thus permitting the market forces to decide the provision of better quality of modern services at competitive price.

Abstract Today, failure analysis involving flip chip [1] with copper pillar bump packaging technologies would be the major challenges faced by analysts. Most often, handling on the chips after destructive chemical decapsulation is extremely critical as there are several failure analysis steps to be continued such as chip level fault localization, chip micro probing for fault isolation, parallel lapping [2, 3, 4] and passive voltage contrast. Therefore, quality of sample preparation is critical. This paper discussed and demonstrated a quick, reliable and cost effective methodology to decapsulate the thin small leadless (TSLP) flip chip package with copper pillar (CuP) bump interconnect technology.
