Sthoulya is one of the most effective disease which affect someone social, physical and mental features. As per modern view it is a precursor to coronary heart disease, high blood pressure, diabetes mellitus and osteoarthritis which have been recognized as the leading killer diseases of the millennium. Sthoulya is a state of increased Vikruta vruddhi of Medodhatu. It is one of the Satarpanottha vikaras. The drug Haritaki are having Laghu and Ruksha Guna which are opposite Guna to that of the Sthaulya. Objectives: Practical evaluation of Sthoulyahara effects of Haritaki. Results – 30 patients had completed the trial; no adverse effect were reported. All patients get significant result.

It is one of the Santarponottha Vikaras where a physician needs to apply the principle of Vishesha which can restore the unhealthy increase of components to the previous undiseased form. The drug Haritaki and Amalaki are having Laghu and Rooksha Guna which are opposite Gunas to that of the Sthoulya. Objectives - Practical evaluation of the Sthaulyahara effect of Haritaki and Amalkai based on the principle of Hrasa Hetur Visheshascha. Results - 60 patients (92%) had completed the trial, no adverse effect were reported. Both the groups had improved in the clinical trials, overall statistical significance was observed in the scores of both the groups. Discussion - By this statistical result we can concluded that Group A patients were more releived than Group B who were administered Haritaki Choorna. The hypothesis decided for the study was ‘Vishesha’ is the prime cause for Hrasa.Here it was clear that Vishesha applied was Guna Vishesha. Here significant results itself shows that Vishesha has done its role in reducing the obesity (Hrasa) in better way in both the Groups.

<p><strong>Letter to the editor:</strong></p><p>The world medical community has categorised atrial fibrillation (AF) as one of the three cardiovascular ‘epidemics of the 21st century’, along with chronic heart failure and diabetes mellitus [1]. In recent years, the prevalence of AF has increasing steadily. However, the exact cause for the increase in the incidence of AF<br />cannot be explained only by the increase in life expectancy, prevalence of cardiac valve disease or prevalence of myocardial infarction [2].</p><p>Although AF occurs in individuals with various manifestations of coronary heart disease, it is increasingly being diagnosed in patients with arterial hypertension without coronary heart disease [3]. AF causes serious cardiovascular complications; thus, a deep understanding of its pathogenetic aspects and a comprehensive study that considers comorbid pathologies for identifying the predictors of the development and progression of AF are required [4].</p><p>Hereditary factors can play a significant role in the development of AF and hypertension; consequently, the worldwide practice of scientific research in basic medicine pays significant attention to the molecular genetics methods of analysis.</p><p>This study aimed to evaluate the genetic determinants in patients with hypertension with AF progression accompanied by various extra-cardiac comorbid pathologies.</p><p>This prospective cohort study included 167 patients with a paroxysmal and persistent form of AF and stage III hypertonic disease without coronary heart disease. The average age of the patients was 53.3 ± 7.1 years. DNA isolation from blood leucocytes was performed using phenol–chloroform extraction. The rs1378942 polymorphism of the CSK gene, the rs220073 polymorphism and the -174G/C polymorphism (rs1800795) of the IL6 gene were assessed using polymerase chain reaction-restriction fragment length polymorphism. The statistical hypotheses were considered significant at a critical level of p = 0.05, i.e.<br />the difference was considered statistically significant at p &lt; 0.05. The lower limit of evidentiary power was equal to 80%.</p><p>This study reported associations between the rs1378942 polymorphism of the CSK gene, the rs1800795 polymorphism of the IL6 gene and the rs220073 polymorphism and the progression of AF in combination with the following associated diseases: hypertension, chronic obstructive pulmonary disease, hypothyroidism, type 2 diabetes mellitus and abdominal obesity. The relative risk of the progression of AF in carriers of the allele C was 1.94 times higher than that in carriers of the allele A [95% confidence interval (CI), 1.21–3.09]. Carriage of the AA genotype was conditionally protective against the progression of AF (relative risk, 0.41; 95% CI, 0.21–0.80; p = 0.010).</p><p>Associations of the rs1378942 and rs1800795 polymorphisms with the risk of recurrence of AF in combination with certain diseases were also found. In addition, associations were identified between rs1378942 and glomerular filtration rate, systolic and diastolic blood pressure, left atrial wall thickness and glucose, high-density lipoprotein (HDL) cholesterol, triglyceride and creatinine levels; between rs220073 and levels of triglycerides, atherogenic index, creatinine, fibrinogen and the number of months before the development of relapse and between rs1800795 and HDL cholesterol, creatinine and galectin-3 levels and diastolic blood pressure.</p><p>The secondary form of AF as a multi-factorial disease develops under the influence of many factors of both the external environment and hereditary nature. The complexity of the etio-pathogenesis of the disease makes it extremely difficult for researchers to identify the factors that play a leading role in the development of the pathological process. Currently, associative studies of AF with polymorphisms of &gt;260 genes have been conducted, and genome-wide associative studies have been performed as well. The reproducibility of the results depends on several factors: age, sex, comorbidities, ethnicity, penetrance, expressiveness, pleiotropy, various epigenetic influences and many more.</p><p>Despite the limitations of the sample, our study adds to the data material already available that can serve in the prognostic assessment of the development and progression of AF. Further studies will allow the development of a personalised algorithm for predicting the progression of AF in hypertension combined<br />with extra-cardiac diseases. Fragmented investigation has masked the overall picture for causes of cardiovascular disease (CVD). Among the risk factors for CVD, high blood pressure (BP) is associated with the strongest evidence for causation and it has a high prevalence of exposure. Biologically, normal levels of BP are considerably lower than what has typically been characterized as normal in research and clinical practice. We propose that CVD is primarily caused by a right-sided shift in the population distribution of BP. Our view that BP is the predominant risk factor for CVD is based on conceptual postulates that have been tested in observational investigations and clinical trials. Large cohort studies have demonstrated that high BP is an important risk factor for heart failure, atrial fibrillation, chronic kidney disease, heart valve diseases, aortic syndromes, and dementia, in addition to coronary heart disease and stroke. In multivariate modeling, the presumed attributable risk of high BP for stroke and coronary heart disease has increased steadily with progressive use of lower values for normal BP. Meta-analysis of BP-lowering randomized controlled trials has demonstrated a benefit which is almost identical to that predicted from BP risk relationships in cohort studies. Prevention of age-related increases in BP would, in large part, reduce the vascular consequences usually attributed to aging, and together with intensive treatment of established hypertension would eliminate a large proportion of the population burden of BP-related CVD.

The unfavorable trend of loss of health of teachers is determined by the growth of neglected forms of diseases and the influence of seasonal factors. Purpose. To study the influence of seasonal factors on the dynamics of primary morbidity of teachers of general educational institutions. Methods. The study of the incidence was carried out on the data of the appeal of teachers for medical care and data of medical examinations. The seasonal oscillation index was used to determine seasonal factors. Results. The incidence of teachers living in Omsk had a clear tendency to worsen in the following nosological forms: anemia, diabetes, obesity, peripheral nervous system diseases (neuritis, neuralgia, etc.), eye disease (myopia), diseases characterized by high blood pressure, coronary heart disease from 1.3 to 3.6 times (p<0.05), most of these forms are detected during medical examinations. In the structure of primary morbidity, the first three rank places were occupied by diseases of the respiratory, circulatory, endocrine system and metabolism. The total share of the first three groups of the above nosologies in the formation of pathologies was about 65%, and there was an increase in the average duration of temporary incapacity from 3 to 15 days. In addition, the effect of seasonal factors on the level of primary morbidity in diseases characterized by high blood pressure, diabetes, anaemia, which is associated with the cyclicality of the educational process and the date of medical examinations. Conclusions. 1. The deterioration of the health of teachers is due to an increase in primary morbidity, aggravated forms of diseases, including coronary heart disease, diabetes, anaemia, obesity, as well as an increase in the duration of days of temporary incapacity in these nosological forms. 2. Seasonal risk factors associated with the cyclicality of the learning process have been identified. Scope of application. INTRODUCTION: High blood pressure (BP) is ranked as the third most important risk factor for attributable burden of disease in south Asia. Raised blood pressure is a major risk factor for chronic heart disease, stroke, and coronary heart disease. WHO rates hypertension as one of the most important causes of premature death worldwide[i].  In India 57% of all stroke deaths and 24% of all coronary heart disease (CHD) deaths are due to hypertension. Other than coronary heart disease and stroke, complications include peripheral vascular disease, heart failure, renal impairment, retinal hemorrhage, and visual impairment. MATERIAL AND METHODS: Study population was adult patients who had been receiving anti-hypertensive treatment at hospital for at least 6 months. Interviews were conducted with all the participants and the questionnaires were filled. Demographic and anthropometric data were obtained for the patient who includes height and weight, BMI (Body Mass Index), personal medical history was obtained, and diagnosis of hypertension and other co-morbid conditions were recorded. RESULTS: A total of 200 patients were included in this study were observed and traced up to 6 month period of the study. 200 patients were included in the study of which 117 (58.50%) were male and 83 (41.50%) were female. Mean age of male patients was 52 ± 18.29 while in female it was 53 ± 17.88. We have divided our patients into four groups in <20 years only one male patient (0.85%) was having hypertension. In 20 – 39 years age group 16 (13.67%) male and 8 (9.63%) female were observed. In 40 to 59 age group 47 (35.04%) male and 31 (37.35%) female were observed. In >60 age group there were 53(45.30%) male and 44 (53.01%) female were observed. Maximum number of cases were observed in the >60 age group in both the sexes. 46 male (39.32%) and 35(42.17%) female had history of hypertension with Less than 5 years. while 71 (60.68%) male and 48 (57.83%) female had hypertension for more than five years. Out of 117 male 13 (11.11%) were normal weight, 85 (72.65%) were overweight and 19 (16.24%) were obese. In female 4 (4.82%) were normal weight, 69 (85.13%) were overweight and 10 (12.50%) were obese. Hypertension was most prevalent in overweight group in both the sexes. Diabetes was observed in 71 (60.68%) male and 56 (67.47%) female. 46 (39.32%) male and 27(32.53%) of female had myocardial infarction.   Before start of the study 68 (58.12%) had controlled blood pressure, 42 (35.90%) had uncontrolled BP, Hypertensive crisis was seen in 5 (4.27%) patients and Resistant hypertension was seen in 2 (1.71%) cases. After study period 58 (49.57%) had controlled blood pressure, 22 (18.80%) had uncontrolled BP, Hypertensive crisis was seen in 2 (1.71%) patients and Resistant hypertension was seen in 1 (0.85%) case. CONCLUSION:  Chronic uncontrolled high blood pressure which can be reduced by proper management, counselling and lifestyle modification. Prevalence of hypertension in males was slightly higher than female counterparts .Control of hypertension was more in female patients. Throughout the COVID-19 pandemic, the main risk factors associated with the progression to severe disease or death have been typically advanced age, diabetes mellitus, obesity, high blood pressure, heart disease, and chronic pneumopathy. Because of their immunosuppression status, persons with HIV were also expected to have a higher susceptibility to infection or a poor clinical evolution. So far, this has not been confirmed to happen, giving way to hypotheses about the role of immunosuppression or the use of antiretrovirals, which could explain this paradox. In this article we present the existing data on the epidemiology and characteristics of HIV-COVID-19 co-infection, discuss the available evidence on the possible factors involved in the evolution of individuals affected by both viruses, analyze other determinants that may negatively affect persons with HIV during the pandemic, and present recommendations for the prevention and care of COVID-19 infection in the context of HIV.

Considerable evidence suggests that diabetes mellitus and hypertension are influenced by genetic factors. Studies in humans have associated glucocorticoid receptor (GR) polymorphisms with high blood pressure, insulin sensitivity, body mass index, increased visceral fat, and variations in tissue-specific steroid sensitivity. The N363S polymorphism of the GR results in an asparagine to serine amino acid substitution in a modulatory region of the receptor. Phosphorylation of serine residues in this region has been shown to enhance transactivation of GR responsive genes. The aim of this study was to investigate the association between the 363S allele and risk factors for coronary heart disease and diabetes mellitus in a population of European origin living in the northeast of the United Kingdom. Blood samples from 135 males and 240 females were characterized for 363 allele status. The overall frequency of the 363S allele was 3.0%, 23 heterozygotes (7 males and 16 females) but no 363S homozygotes were identified. The data show a significant association of the 363S allele with increased waist to hip ratio in males but not females. This allele was not associated with blood pressure, body mass index, serum cholesterol, triglycerides, low-density lipoprotein and high-density lipoprotein cholesterol levels, and glucose tolerance status. The results of this study suggest that this GR polymorphism may contribute to central obesity in men. Further studies are required to elucidate the properties of GR363S at a molecular level.

High blood pressure (hypertension) is arguably the most important preventable cause of premature microvascular and macrovascular disease and the associated morbidity and mortality in people with diabetes. This chapter will review key aspects of the epidemiology and pathophysiology of hypertension in people with diabetes, as well as recommended approaches to its clinical evaluation and treatment.
