Background:  Several prognostic markers are used in clinical stages of Chronic Lymphocytic Leukemia (CLL); including lactate dehydrogenase enzyme (LDH), CD38 and Zeta associated protein kinase (ZAP70). This study aimed to compare LDH with ZAP70 and CD38 as prognostic markers in Sudanese patients with CLL. Materials and Methods: Fifty newly diagnosed patients with CLL were enrolled in this cross-sectional study, the age of patients ranged between 36 - 85 years, with mean of 62 years. Of total patients; there were 34 males (68%) and 16 females (32%). Blood samples were obtained and LDH measurement was done by using Dirui CS-T240 automated analyzer while CD38 and ZAP-70 were measured by flowcytometry. Data were analyzed by statistical package for social science (SPSS) computer software program version 21. Results: The present study revealed a significant association between LDH and Rai staging (p. value= 0.002), when LDH compared with ZA-70 and CD38 as reference prognostic markers; poor correlation between CD38 and LDH was found (R2= 0.086, P value= 0.034) and no correlation between LDH level and ZAP-70 expression (R2= 000, P value= 0.960). In conclusion: According to the outcomes of this study; ZAP-70 and CD38 cannot be substituted by LDH as a prognostic marker.

Background: The clinical course of chronic lymphocytic leukemia is highly variable. The determination of ZAP70 and CD38 is increasingly utilized as prognostic factor for chronic lymphocytic leukemia. The aim of conducting this study was to investigate the frequency of CD38 and ZAP70 expression among Sudanese Chronic lymphocytic leukemia (CLL) patients and to relate them to the Binet and Rai clinical staging systems. Method: A total of 93 patients (mean age; 62.29 ± 11.68, sd) were enrolled in this cross-sectional study. CD38 and ZAP70 expression levels were measured with four color flowcytometry using the cut-off values of 20% for ZAP70 and 30% for CD38 expression. Staging was assessed by using clinical examination and CBC for all patients. Data were analyzed using the Statistical Package for Social science for Windows (SPSS), version 22. Results: There were 93 CLL patients and the median age of the group was 63 years (36–95 years). About 71% of the patients presented with lymphadenopathy, 53.8% with splenomegaly, 73.1% with anemia, and 45.2% with thrombocytopenia. There was higher frequency of Binet stage C and Rai stage IV (62 [66.6%] patients and 34 [36.5%] patients, respectively). In addition, CD38 and ZAP70 showed higher frequency among Binet and Rai advance stages. ZAP70 and CD38 positivity were detected in 21 patients (22.6%) and 31 patients (33.3%), respectively. There was no statistically significant association between ZAP70 and CD38 and clinical staging systems (P-value > 0.05). Conclusion: No significant association was observed between Flowcytometric (CD38 and Zap70) Prognostic Indicators and clinical staging systems. Introduction: Heart Failure (HF) is a global health problem, and despite much progress in new therapies, its prognosis remains poor. This is due to a number of factors including lack of adherence to HF medications. Previous studies have shown high sensitivity C-reactive protein (hsCRP) to change with HF progression, and that plasma hsCRP levels can be used to predict HF progression and assessment of adherence to medications. This association however has not been studied in Sub-Saharan African (SSA) countries, including Tanzania. Aim: To study the association between hsCRP levels and HF medications adherence among HF patients attending the Jakaya Kikwete Cardiac Institute (JKCI) in Dar es Salaam, Tanzania. Materials and Methods: This was a hospital based cross-sectional study, where all eligible adult HF patients were consecutively enrolled as they attended their regular clinics, until the sample size was reached. Patients’ socio-demographic and clinical information was collected through interviews and chart reviews. An 8-item Morisky tool was used to assess patients’ adherence to HF medications. Blood was collected and analysed for hsCRP, Complete Blood Count (CBC) and cholesterol panel. hsCRP was considered elevated when it was >5 mg/L. Data were analysed using Statistical Package for Social Sciences (SPSS) computer software, version 21. All tests were 2-sided, and a p-value of <0.05 was considered to indicate a significant association. Results: In total, 210 HF patients were enrolled. Their mean age was 54±15.9 years and 53.8% were females. In the total population, the mean hsCRP was 7.15±4.94 mg/L, and 122 (58.1%) patients had elevated hsCRP. Poor medications adherence was present in 138 (65.7%) patients. Patients with poor medications adherence were more likely to have elevated hsCRP levels (68.1%) when compared to patients with good adherence (38.9%), p<0.001. In multivariate logistic regression analysis, elevated hsCRP independently predicted poor medications adherence (OR=4.27, 95% CI 2.14-8.51, p<0.001), irrespective of other variables. Conclusion: HF patients with poor medications adherence are 4-times more likely to have elevated hsCRP, irrespective of other clinical and socio-demographic factors in this study cohort. These findings suggest that hsCRP levels can be considered as surrogate to HF medications adherence.

Objectives: To find the frequency of complete heart block in acute inferior wallmyocardial infarction and its relation with severe obstructive disease of the infarct related artery.Study Design: Cross sectional study. Setting: Department of Cardiology Choudhary PervaizElahi Institute of Cardiology, Multan. Period: June 2016 to June 2017. Methodology: Noneprobably consecutive sampling technique was used. Collected data was entered and analyzedthrough statistical/computer software SPSS vrs 22, mean ± SD was calculated for quantitativedata and variables like, age and frequency (percentage) was calculated for qualitative datalike gender, dominant artery and CHB. P value ≤ 0.05 was considered as significant. Results:A total number of n=234 (100%) patients were included in this study, both male and femalegenders. Distribution of Gender showed that male patients were more than females patientsi.e. n=198 (84.6%) and n=36 (15.4%) respectively. The main outcome variable of this studywas severity of disease. It was observed that, in with CHB group, severity of disease noted as93.2% (n=109) patients while in without CHB group it was 57.3% (n=67). Conclusion: Inferiorwall MI has severe adverse effects when combines with complete AV nodal heart block. Severityof disease was significantly associated with gender. Almost all times infarct related artery wasdominant right coronary artery.

Objectives: In this study we investigate the effectiveness of lycopene in themanagement of oral submucous fibrosis (OSF) when intralesional steroids also given incombination. Study Design: Cross sectional study. Setting: Dental Section Allied Hospital,Faisalabad. Period: Eight months from September 2016 to April 2017. Methodology: This crosssectional study was initiated after approval from hospital ethical board. Collected Informationwas entered in SPSS computer software version 23 and analyzed for possible results. Meanand SD was calculated and presented for quantitative data like age, VAS score and mouthopening. Frequency (percentages) were calculated and presented for qualitative data such asgender. Post stratification statistical chi square test was used to see effect modification. P value≤ 0.05 was considered as significant. Results: Overall, there were 100% (n=354) patients.Total patients who were selected for the study had the habit of chewing areca nut either innatural form or in the form of Pan Masala, gutkha or mawa. Gutkha is the most common formof areca nut used, i.e. 65.8% (n=233) patients were using it. In our study average increase inmouth opening from baseline (before treatment) was 5.12±1.03 mm in group A. 4.20±1.26mm in group B and 3.40±0.19 mm in group C at six week. Conclusion: When Lycopene usedcombined with intralesional steroids and Hyaluronidase its effectiveness is better in improvingthe mouth opening its symptoms in patients with Oral Submucous Fibrosis (OSF). It was alsofound that there was no side effects.

Background: Studies conducted in recent years have proposed a protective effect of vitamin D3 (VitD3) against various types of cancers. Thus, in this research, we investigated the serum levels of VitD3and its relation with prognostic clinical factors in chronic lymphocytic leukemia (CLL). Method: In this cross-sectional study, 86 CLL patients were investigated for a one-year period and evaluated for the relationship between Vit D3serum levels, Rai stages, age, gender, and pathological factors. A control group was added to the study in order to compare the VitD3 levels of CLL patients with that of the normal population. Results: The mean serum VitD3level in CLL patients was 29.76 ± 18.75 ng/ml, and most of the patients were in stage I and II. There was no significant association between age, sex, and Rai stages of CLL with VitD3 level. Except for splenomegaly (P < 0.001), the leukemia-related phenotypes, complications, and prognostic factors had no meaningful association with VitD3 (P > 0.05) level. VitD3 serum level was meaningfully different between case and control groups (P < 0.001). Conclusion: In the current research, there was no significant relationship between VitD3serum levels and investigated variables, except splenomegaly; these findings are in concordance with other studies.

Background: The visual evoked potentials (VEP) is a valuable tool to document occult lesions of the central visual channels especially within the optic nerve. Objectives: The purpose of the present study was to observe the findings of first few cases of VEP done in the neurophysiology department of the National Institute of Neurosciences (NINS), Dhaka, Bangladesh. Methodology: This cross-sectional study was conducted in the Department of Neurophysiology at the National Institute of Neurosciences and Hospital, Dhaka, Bangladesh from September 2017 to March 2020. All patients referred to the Neurophysiology Department of NINS for VEP were included. Pattern reversal VEPs were done using standard protocol set by International Federation of Clinical Neurophysiology (IFCN). Results: The mean age of the study population was 30.70 (±12.11) years (6-68 years) with 31 (46.3%) male and 36 (53.7%) female patients. The mean duration of illness was 8.71 (±1.78) months (3 days- 120 months). Most common presenting symptom was blurring of vision (37.3%) and dimness of vision (32.8%). Patterned VEP revealed mixed type (both demyelinating and axonal) of abnormality in most cases [29(43.35)]. The most common clinical diagnosis was multiple sclerosis (29.85%) and optic neuropathy (26.87%). In the clinically suspected cases of multiple sclerosis, optic neuropathy and optic neuritis most of the cases of VEP were abnormal and the p value is 0.04 in optic neuropathy and optic neuritis. Conclusion: The commonest presentation of the patients in this series were blurring of vision and dimness of vision. The most common clinical diagnosis for which VEP was asked for, was optic neuritis and multiple sclerosis. Most abnormalities were of mixed pattern (demyelinating and axonal). Breast cancer is one of the most common illness that killed woman. One of the therapy to cure breast cancer is chemotherapy. Chemotherapy has side effect either physical and psychology, that caused people who’s in chemo therapy, prone to stress. Stress effected by many factors, such as characteristic and chopping strategy that patient has been using. The aim of this research is to get a perspective about the relation of characteristic and chopping strategy with breast cancer patient stress, whose in chemo therapy. This research using cross sectional study and taking sample by using accidental sampling method. The data analyzed by using bavariat and multivariat with variable result shows that breast cancer patient stress who has chemo therapy realted to age characteristic (p value = 0.00) the time since they diagnosed with cancer (pvalue = 0.03), how long they have chemo therapy (pvalue = 0.00) and chopping strategyby looking social support (pvalue = 0.00) looking for spiritual (pvalue = 0.00) with dominan variable which related to stress is chopping strategy to looking spiritual support (coeffecients B = -1.139).   Key words : Breast cancer, chemotherapy, stress
