A Systematic Review on COVID 19 Treatment and Management

There is a new world health crisis threatening the public with spread of COVID-19 (Coronavirus Disease-2019). The virus outbreak has been declared a public health emergency of international concern by World Health Organization (WHO). The pandemic of coronavirus disease 2019 (COVID-19) caused by the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) presents an unprecedented challenge to identify effective drugs for prevention and treatment. We here summarize the current clinical characteristics data to guide potential COVID-19 about Prevention, Diagnosis, Treatments and Prevention of COVID-19. In this review, we extracted data from various Research Report, WHO guidelines and other articles. It is important to aware the readers that new data updating nearly every hour regarding clinical characteristics, diagnosis, treatment strategies, and outcomes COVID-19. All over the world the disease has caused varying degrees of illness. Patient shows various symptoms usually fever, cough, sore throat, breathlessness, fatigue, and malaise among others. The disease is being rehabilitate through general treatment, symptomatic treatment, by using antiviral drugs, oxygen therapy and by the immune system. It is necessary to recognize the potential cases as soon as possible and isolate the suspected people from the confirmed cases of COVID-19, to avert the potential transmission of infection to other patients and health care staff.

Novel Coronavirus are very harmful virus. This viruses have positive single stranded RNA genome and enveloped which is called nucleocapsid. The family of this virus is Coronaviridae. This virus originated from species of avian and mammalian. This virus effect on upper respiratory tract in humans. Many species of these novel coronaviruses (HCoVs) are named as HCoV-HKU1, HCoV-NL63. Predominant species of this virus is Middle East respiratory syndrome (MERS-CoV) across the world.  In both adults and childrens HCoV-HKU1 sp. is causes for chronic pulmonary disease and HCoV-NL63 species causes for upper and lower respiratory tract disease. Most recent species of this virus is MERS-CoV. This species caused for acute pneumonia and occasional renal failure. The new strain of novel Coronavirus is SARS-CoV-2. This strain causes for the Coronavirus Disease 2019 (COVID-19). This disease named by the World Health Organization. Now world fighting against COVID-19 and according to the recent statistics report of world about the COVID-19 cases approx 22.6M confirmed cases and 792K death cases appeared and recovered 14.5M. COVID-19 disease starts to spread from December 2019 from china. Covid-19 disease is emerged in Wuhan seafood market at Wuhan of South China and then rapidly spread throughout the world. The corona virus outbreak has been declared a public health emergency of International concern by World Health Organization (WHO). In this article we summarize the current clinical characteristics of coronavirus and diagnosis, treatments and prevention of COVID-19 disease. In this review article, we analyze data from various Research Reports like WHO guidelines and other articles. It is very important to the readers that new data of COVID-19 updating nearly every hour of day regarding clinical characteristics, diagnosis, treatment strategies, and outcomes COVID-19 disease. The degree of COVID-19 disease varying throughout the world. COVID-19 affected patient shows various symptoms like fever, cough, sore throat, breathlessness, fatigue, and malaise among others. The COVID-19 disease is being treated through general treatment like symptomatic treatment, by using antiviral drugs, oxygen therapy and by the immune system. There is no vaccine or therapeutic strategies for deal with this disease yet. In this critical situation preventive measures must be require.  A very important issue in preventing viral infection is hand hygiene. There are other entities that people can boosting the immune system and help in infection prevention.

<p>COVID-19, the respiratory illness caused by the novel SARS-CoV-2 virus has rapidly emerged as a serious global health concern. The world health organization (WHO) and has enacted various protocols for healthcare institutions internationally in an attempt to slow the spread of disease, including adjustments for procedures performed by otolaryngologists. Based upon published evidence we highlight different strategies to reduce infection transmission during endoscopic sinus surgery. All patients undergoing endoscopic sinus surgery should be discussed with health care staff to be involved in procedure. The patients should be divided into elective, semi-elective, and urgent/emergency. COVID-19 test should be done in all patients prior to surgery along with high resolution computed tomography (HRCT) chest. Irrespective of COVID screening results, it is advisable to treat all patients undergoing aerosol generating procedures as COVID-19 positive, and the whole theatre team should wear personal protection equipment (PPE) as per current national guidelines. Preoperative nasal decongestion to be undertaken with atomization devices. Consider telephone/video clinic follow up appointments unless clinical examination is considered essential. Considering the current health crisis caused by COVID-19, the following guidelines are recommended for performing nasal surgeries: patient visits must be limited to only urgent/emergent cases for the time being. Only vital personnel should remain in the OT while performing any nasal endoscopic procedure as this will both reduce exposure and conserve vital personal protective equipment. Five levels of protection are recommended for the person performing the procedure.</p>

PurposeAs the world faces a new health crisis threatening people with the spread of Covid-19, this study aims to summarize the key information of Covid-19 related to disease characteristics, diagnosis, treatment and prevention along with the lessons learned from Thailand.Design/methodology/approachThe narrative review was synthesized from various sources such as the World Health Organization; Centers for Disease Control and Prevention; Ministry of Public Health and other related news; articles in ScienceDirect, PubMed, Google Scholar; and the author's perspective regarding the lessons learned from Thailand with keywords of “Covid-19” and “Coronavirus” from January to August 2020. Google Trends was used to set common questions.FindingsCovid-19 is the seventh family of coronaviruses that cause various symptoms related to respiratory systems. The disease can be treated through general and symptomatic treatment, by using antiviral drugs. As of July 2020, there are four potential vaccine candidates ChAdOx1 nCoV-19, mRNA-1273, Ad5-nCOV and BNT162b1. The recommendations for Covid-19 prevention are physical distancing, face masks, eye protection and hand washing. Thailand is now considered as low-risk for Covid-19 possibly because of (1) soft policy by government actions, (2) village health volunteers, (3) integration of technology and (4) fact-based communications.Originality/valueThis study summarized the key points about Covid-19, clarified some misunderstandings and shared strategic actions from Thailand, which can be adapted according to the different capacities and situations in other countries.

Background: In the battle against the Coronavirus Disease 2019 (COVID-19) pandemic, medical care staff, especially nurses, are at a higher risk of encountering psychological health issues and distress, such as stress, tension, burdensome indications, and, most importantly, fear. They are also at higher risk of becoming infected and transmitting this virus. In Saudi Arabia, it was noticed that the healthcare workforce suffered from anxiety, and that this more evident in women than men. Objective: This study aimed to assess the knowledge of nurses regarding COVID-19 and the level of anxiety toward the COVID-19 outbreak in the current pandemic situation. Design: A cross-sectional design was used and a validated self-administered online questionnaire with a set of questions related to COVID-19 was distributed to 87 participating nurses. Results: The results showed that more than half of the nurses (71.90%) had an adequate and good knowledge about the causes, transmission, symptoms, treatment, and death rate of COVID-19. The main sources of information for the nurses were social media (51.7%) and the World Health Organization and the Ministry of Health (36.8%). Conclusions: The results allowed the conclusion that, though the nurses had satisfactory knowledge about COVID-19, more than 50% of them experienced mental health issues such as anxiety. To address this, along with providing more knowledge about COVID-19, nurses should be supported in managing their anxiety.

Colorectal cancer (CRC) is the World’s third most frequently diagnosed cancer type. It accounted for about 9.4% mortality out of the total incidences of cancer in the year 2020. According to estimated facts by World Health Organization (WHO), by 2030, 27 million new CRC cases, 17 million deaths, and around 75 million people living with the disease will appear. The facts and evidence that establish a link between the intestinal microflora and the occurrence of CRC are quite intuitive. Current shortcomings of chemo- and radiotherapies and the unavailability of appropriate treatment strategies for CRC are becoming the driving force to search for an alternative approach for the prevention, therapy, and management of CRC. Probiotics have been used for a long time due to their beneficial health effects, and now, it has become a popular candidate for the preventive and therapeutic treatment of CRC. The probiotics adopt different strategies such as the improvement of the intestinal barrier function, balancing of natural gut microflora, secretion of anticancer compounds, and degradation of carcinogenic compounds, which are useful in the prophylactic treatment of CRC. The pro-apoptotic ability of probiotics against cancerous cells makes them a potential therapeutic candidate against cancer diseases. Moreover, the immunomodulatory properties of probiotics have created interest among researchers to explore the therapeutic strategy by activating the immune system against cancerous cells. The present review discusses in detail different strategies and mechanisms of probiotics towards the prevention and treatment of CRC.

One year after the declaration of the COVID-19 pandemic by the World Health Organization (WHO) and despite the implementation of mandatory physical barriers and social distancing, humanity remains challenged by a long-lasting and devastating public health crisis. Non-pharmacological interventions (NPI) are efficient mitigation strategies. The success of these intense NPI is dependent on the approval and commitment of the population. The launch of a mass vaccination program in many countries in late December 2020 with mRNA vaccines, adenovirus-based vaccines, and inactivated virus vaccines has generated hope for the end of the pandemic. Current issues: The continuous appearance of new pathogenic viral strains and the ability of vaccines to prevent infection and transmission raise important concerns as we try to achieve community immunity against SARS-CoV-2 and its variants. The need of a second and even third generation of vaccines and the possibility of potentially harmful side-effects of the vaccines (i.e. venous thromboembolism ) have already been acknowledged. Perspectives: There is a critical and urgent need for a balanced and integrated strategy for the management of the COVID-19 outbreaks organized on three axes: (1) Prevention of the SARS-CoV-2 infection, (2) Detection and early diagnosis of patients at risk of disease worsening, and (3) Anticipation of medical care (PDA). Conclusion: The “PDA strategy” integrated into state policy for the support and expansion of health systems and introduction of digital organization (i.e. telemedicine, artificial intelligence and machine learning technology) is of major importance for the preservation of citizens’ health and life world-wide.

AbstractBackgroundBoth psychodynamic group therapy (PGT) and clonazepam are used as treatment strategies in reducing symptoms of generalized social anxiety disorder (GSAD). However, many individuals remain symptomatic after treatment with PGT or clonazepam.MethodFifty-eight adult outpatients with a diagnosis of GSAD according to DSM-IV were randomized to 12 weeks PGT plus clonazepam or clonazepam. The Clinical Global Impression-Improvement (CGI-I) Scale was the primary efficacy measure. Secondary efficacy measures included the Liebowitz Social Anxiety Scale (LSAS) total score, the World Health Organization Instrument to Assess Quality of Life—Brief (WHOQOL-Bref) Scale and the Beck Depression Inventory (BDI).ResultsCGI-I data from 57 patients (intent-to-treat population) showed that patients who received PGT plus clonazepam presented significantly greater improvement than those who received clonazepam (P = 0.033). There were no significant differences between the two groups in the secondary efficacy measures.ConclusionsOur study suggests that the combination of PGT with clonazepam may be a promising strategy for the treatment of GSAD, regarding gains in the global functioning. However the present study failed to detect more specific changes in social anxiety symptomatology between the two groups.

Coronavirus disease 2019 (COVID-19) is a pandemic disease of modern time with unique and rapid transmission rate and affected almost all the nations without respecting any border. Coronavirus disease 2019 (COVID-19) is arguably the biggest health crisis the world has faced in 21st century. It is an infectious disease and declared pandemic by the World Health Organization. The coronavirus disease 2019 (COVID-19) outbreak, which originated in Wuhan, China, has now spread to 192 countries and administrative regions infecting nearly 800,000 individuals of all ages as of 31 March 2020. Though most infected individuals exhibit mild symptoms including fever, upper respiratory tract symptoms, shortness of breath, and diarrhoea, or are asymptomatic altogether, severe cases of infection can lead to pneumonia, multiple organ failure, and death. Globally, at least 7900 deaths have been directly attributed to COVID19, and this number is expected to rise with the ongoing epidemic. This is particularly crucial as the current outbreak involves a new pathogen (SARS-CoV-2), on which limited knowledge exists of its infectivity and clinical profile. Research is in progress on therapeutic efficacy of various agents including anti-malarials (Chloroquine and Hydroxychloroquine), antiviral drugs, and convalescent serum of recovered patients. Unani system of medicine is one of the traditional systems of medicine which is being explored for providing preventive, supportive and rehabilitative care to patients. Unani system of medicine has a detailed description of drugs that are utilized in many infectious diseases, including respiratory infections. Immune response is essential to eliminate virus and to preclude disease progression to severe stages. Therefore, it is important to summarize the evidence regarding the preventive measures, control options such as immune-stimulator and prophylactic treatment in Unani medicine against Covid19. This review summarizes various pharmacological actions of Unani formulation Tiryaq-e-Arba in Unani literature and various reported pharmacological activities which can possibly provide prevention, control and reduction of complications of this deadly disease.

The coronavirus disease 2019 (COVID-19) pandemic is a general health crisis and has irreversible impacts on human societies. Globally, all people are at risk of being exposed to the novel coronavirus through transmission of airborne bioaerosols. Public health actions, such as wearing a mask, are highly recommended to reduce the transmission of infectious diseases. The appropriate use of masks is necessary for effectively preventing the transmission of airborne bioaerosols. The World Health Organization (WHO) suggests washing fabric masks or throwing away disposable masks after they are used. However, people often use masks more than once without washing or disposing them. The prolonged use of a single mask might—as a result of the user habitually touching the mask—promote the spread of pathogens from airborne bioaerosols that have accumulated on the mask. Therefore, it is necessary to evaluate how long the living components of bioaerosols can be viable on the masks. Here, we evaluated the viability of airborne Bacillus subtilis (B. subtilis) in bioaerosols filtered on woven and anti-droplet (non-woven) face masks. As a simulation of being simultaneously exposed to sand dust and bioaerosols, the viability rates of bioaerosols that had accumulated on masks were also tested against fine dust and airborne droplets containing bacteria. The bioaerosols survived on the masks immediately after the masks were used to filter the bioaerosols, and the bacteria significantly proliferated after one day of storage. Thereafter, the number of viable cells in the filtered bioaerosols gradually decreased over time, and the viability of B. subtilis in bioaerosols on the masks varied, depending on the mask material used (woven or non-woven). Despite the reduction in viability, bioaerosols containing living components were still found in both woven and anti-droplet masks even after six days of storage and it took nine days not to have found them on masks. The number of viable cells in bioaerosols on masks significantly decreased upon exposure of the masks to fine dust. The results of this study should provide useful information on how to appropriately use masks to increase their duration of effectiveness against bioaerosols.

In the past 100 years, the world has faced four distinctly different pandemics: the Spanish flu of 1918-1919, the SARS pandemic of 2003, the H1N1 or “swine flu” pandemic of 2012, and the ongoing COVID-19 pandemic. Each public health crisis exposed specific systemic shortfalls and provided public health lessons for future events. The Spanish flu revealed a nursing shortage and led to a great appreciation of nursing as a profession. SARS showed the importance of having frontline clinicians be able to work with regulators and those producing guidelines. H1N1 raised questions about the nature of a global organization such as the World Health Organization in terms of the benefits and potential disadvantages of leading the fight against a long-term global public health threat. In the era of COVID-19, it seems apparent that we are learning about both the blessing and curse of social media.
