Abstract Release of medical information must be subject to applicable procedures and must be with the patient's consent. Patients must make a stamped written statement that has authorized a third party to request medical data from a doctor. The purpose of this study is to determine the policy of releasing medical information and the flow of procedures for releasing medical information to the insurer. The research method in this study is to use a literature review design, namely research that examines research articles on the release of medical information to insurance parties by comparing, summarizing, and drawing conclusions. The search strategy used keywords and operator bundles used in this study, namely "medical records" or "information release" or "insurers". The result of the research is that a hospital is in the process of releasing medical information using policies in the form of SOPs, cooperation agreements with insurance parties, and orally. In addition, there are hospitals that have different procedures for releasing medical information because they do not only serve one insurance party, but there are several insurance parties that are served such as BPJS, Jasa Raharja, and Askes. However, in the process of releasing medical information, there are hospitals that are not yet in accordance with the flow of medical information release procedures that have been determined by the Hospital. Therefore, the hospital conducts outreach on the flow of procedures for releasing medical information so that the officer in charge has a better understanding of the release of medical information.

A pilot medical information system is being implemented and currently is providing services for limited categories of patient data. In one year, physicians’ diagnoses for 500,000 office visits, 300,000 drug prescriptions for outpatients, one million clinical laboratory tests, and 60,000 multiphasic screening examinations are being stored in and retrieved from integrated, direct access, patient computer medical records. This medical information system is a part of a long-term research and development program. Its major objective is the development of a multifacility computer-based system which will support eventually the medical data requirements of a population of one million persons and one thousand physicians. The strategy employed provides for modular development. The central system, the computer-stored medical records which are therein maintained, and a satellite pilot medical data system in one medical facility are described.

Historically, medical records have been maintamed by individual physicians to record specific information concerning patients. This information was often understandable only to the writer. The data were of outstanding events. This was thought to be sufficient documentation for patient care. Records are now read by others than the individual physicians. Groups of physicians working together often share the same patients and their records. Patients may have multiple sources of care. Our population has become more mobile which makes it necessary to transfer vast amounts of medical information. The medical record many times is the one instrument which gives a complete and continuous documentation of the patient's medical history. Third-party payers are requesting access to medical records to document services provided. Chart audit is being tested as a mechanism for evaluating physician performance. Records must reflect what the physician does in order to be useful in such an appraisal. Much clinical research on the delivery of health care depends on accurately kept records which are easily interpreted. A chart is also a legal document for the protection of the physician as well as the patient. Thus, records will be used in other than traditional ways. Proper confidentiality must be maintained when such uses are necessary. Physicians generally agree as to the essential content of a medical record. However, there is little unanimity as to the structure of the chart. No one system of keeping records is now appropriate for all situations. The maintenance of adequate charts requires additional cost in both time and money.

The ability to think creatively is very important to be developed in students, especially when learning is starting to shift to blended learning where students are required to be able to organize portions and also how to learn independently, for this reason, students in this era need to grow their creative abilities to be able to keep up with educational developments. This study aims to conduct studies related to factors that can foster creative thinking skills. The research method used in this study is the SLR (Systematic Literture Review) method. Data collection is done by collecting similar research articles and limiting it to the topic of mathematical creative thinking skills. The articles used in this study were 20 articles with the topic of creative thinking skills. Based on the literature review, the most influencing factors in creative thinking skills are problem-based models and open-ended approaches. For this reason, using the CPS model with an open-ended approach will provide a model with a new alternative approach that can be tried to improve creative thinking skills.

Introduction. The development of effective digital medicine tools is an intensive and complex process that requires the interdisciplinary efforts of a wide range of experts, from scientists and engineers to ethics experts and lawyers. Digital medicine products have great potential for improving medical measurement, diagnosis and treatment. One of the main challenges for the healthcare sector is to address the issue of fast, convenient and secure exchange of information about patients’ health. Service-oriented architectures of such products may accomplish many of the challenges facing healthcare systems. The purpose of the paper is to develop an information and software module ExchangeDMD to ensure the accumulation, storage and exchange of diagnostic medical data in accordance with modern medical information standards to maintain the interoperability function as one of the leading principles of digital medicine. Results. A special adaptive architecture of digital medicine infrastructure has been developed, which enables an integrated solution of data exchange between participants of providing medical services, which is carried out with the help of web services. The specifics of different types of medical information are analyzed and taken into account in accordance with the access regime for its processing. The module structure has been developed and implemented in software, which has three main levels: central virtual storage (virtual data center to implement certain functions), remote administration segment (technical support and administration network) and user segment (mobile devices and user-patient applications). Conclusions. The ExchangeDMD information and software module is designed to ensure the accumulation of patient data, integration between the various units within the system, as well as to ensure the management of this data by health care personnel. The ExchangeDMD module is built using the international standard HL7 CDA, which enables formalizing electronic medical records using RIM (information model links) to attract the necessary directories and classifiers when creating medical records and documents.

Abstract: The number of positive incidents, mortality and mortality due to Covid-19 is getting higher. In 2020, around 76.7% of elderly people died because of Covid-19 from the total number of patients who died due to Covid-19. This increasing number is not in line with the activity of cycling trends that were rife during the Covid-19 pandemic. So that there is a mismatch with the principles of health sports, namely sports to improve the health status of sports players. Therefore, this study is important to do to determine the relationship between cycling trends during the pandemic and increased immunity, especially for the elderly. The research method used is a literature review of various research articles that have been published in various journals indexed by Google Scholar, Research Gate, Scopus, and PubMed. The articles reviewed were articles related to cycling and the immunity of the elderly during the Covid-19 pandemic. Based on the analysis of the articles that have been done, there is a relationship between cycling and the immunity of the elderly. However, the SARS CoV-2 virus can infect healthy people without symptoms which then spreads to the elderly and causes high cases of mortality in the elderly. So that it requires active participation by all parties to prevent the spread of Covid-19 and improve the immune system in the midst of a pandemic with cycling.

The process of consolidating medical records from multiple institutions into one data set makes privacy-preserving record linkage (PPRL) a necessity. Most PPRL approaches, however, are only designed to link records from two institutions, and existing multi-party approaches tend to discard non-matching records, leading to incomplete result sets. In this paper, we propose a new algorithm for federated record linkage between multiple parties by a trusted third party using record-level bloom filters to preserve patient data privacy. We conduct a study to find optimal weights for linkage-relevant data fields and are able to achieve 99.5% linkage accuracy testing on the Febrl record linkage dataset. This approach is integrated into an end-to-end pseudonymization framework for medical data sharing.

Objective: Hospitals are legally responsible for the excellent service quality provided to patients based on statutory provisions. Hospital responsibilities in health services include criminal, civil and administrative liability. This study aims to determine the responsibility of the hospital in documenting medical resumes. Methods: The research method used is literature review, through review of research journals related to the completeness of medical resumes that have been published on accredited journal platforms in 2010-2020. This research was conducted in August-October 2020. Results: Analysis of 20 research journals relating to the completeness of medical resumes in the reviewed hospitals, it is known that the completeness of medical resume documents in the hospital is on average 75.80% Only 2 hospitals out of 20 hospitals documented medical resumes in accordance with the minimum hospital service standards. Based on the prevailing laws and regulations, the hospital is not criminally responsible for the incomplete documentation of medical resume, but the hospital is responsible civilian based on the principle of representative responsibility and administratively. Conclusion: The hospital has not optimally documented medical resumes in accordance with the minimum hospital service standards. As a result, the hospital is both civilly and administratively liable. Supervision of the doctor's performance in documenting medical resumes and the role of the medical record committee in ensuring medical staff complete all medical records of patients served is required.

The drafting of a medical computer file for pneumoconiosis at the Fund of Occupational Diseases was essentially based on an intuitive choice of medical information processing from a large experience. For statistical purposes, however, a more scientific selection of stored information is needed. Therefore, we checked out the medical data on 25,830 complete medical records. The frequency of all answer possib-ilities was tested question by question. A minority of questions had to be reexamined because the yled to insufficient answers. It will be possible in the future to improve the storage of medical information with this work method. A further investigation in which the results achieved by several physicians will be compared opens the possibility of rating the impact of subjective judgments in medical examinations.

A system for medical information has been developed. The system is a general and flexible one which without reprogramming or new programs can accept any alphabetic and/or numeric information. Coded concepts and natural language can be read, stored, decoded and written out. Medical records or parts of records (diagnosis, operations, therapy, laboratory tests, symptoms etc.) can be retrieved and selected. The system can process simple statistics but even make linear pattern recognition analysis. The system described has been used for in-patients, outpatients and individuals in health examinations. The use of computers in hospitals, health examinations or health care systems is a problem of storing information in a general and flexible form. This problem has been solved, and now it is possible to add new routines like booking and follow-up-systems.
