Prevention, identification, and treatment options for the management of bed bug infestations

In the past decade, bed bug infestations have been increasing worldwide. Historically, studies have failed to provide evidence for the transmission of human diseases through bed bugs, but recent evidence is lacking. Although physical reactions to bed bug bites continue to be documented, evidence regarding the mental health effects arising from bed bug infestations is limited to anecdotes and case reports. This report provides an update and summary of the bed bug management strategies discussed during a workshop at the 2010 Canadian Public Health Association Conference. “Best practices” regarding prevention, identification, and treatment options are presented, using evidence from existing evaluative studies. Awareness of bed bug behaviour and proper building maintenance can prevent transfer of bed bugs from belongings, including second-hand items, and reduce entry points and harbourage sites. In addition to inspection by trained professionals, early recognition of clinical symptoms and environmental signs of an infestation are important to avoid further spread of bed bugs and to allow implementation of safe and effective treatment options. Early findings may indicate a need for special support for vulnerable individuals with mental health issues potentially exacerbated by bed bug infestations. Preparing units for treatment, including the removal of clutter, is essential. However, physical and financial limitations are challenges for managing bed bug infestations, especially for vulnerable populations. Current treatment options such as application of heat or pesticides can be effective when properly implemented, but there are concerns with pesticide resistance. An Integrated Pest Management approach is recommended and should include ongoing monitoring and prevention that are essential for positive treatment outcomes. Regulatory officials, building management, and pest management professionals are encouraged to collaborate using a systematic approach to address bed bug infestations.

Background: Bed bugs as blood-sucking insects have become a public health problem in urban communities through­out the world. The objective of this study was to determine the effects of resident education and self-implementation of Integrated Pest Management (IPM) strategy for eliminating bed bug infestation in infected apartments in Ahvaz City, Iran. Methods: This interventional study was conducted in seventy apartments infested with bed bug (Cimex lectularius) in Ahvaz City, southwest Iran, during a 6-month period. The bed bug infestations reported to Health centers were in­spect­ed visually and confirmed by medical entomology experts. Then, the heads of the households were received tech­niques/ tools of the IPM program and trained by medical entomology experts before self-implementation of control methods. Finally, the infected apartments were inspected by the experts at 1, 3, and 6 months after intervention and data were rec­orded in a checklist. Results: From the seventy infected apartments, 57%, 28% and 15% were considered as low, moderate, and high level infestation respectively. The bed bug infestation was eliminated from 53 apartments (76%) after one month and it reached to 62 apartments (88%) by the end of third month. Finally, after six months of applying IPM program, bed bugs infestation was eliminated from all infected apartments (100%). Residents expressed their 100% satisfaction with ap­plying the bed bug IPM strategy. Conclusion: Training residents to implement the IPM program can reduce pest control costs, the volume of pesticides consumed, and human exposure to chemical pesticides, resulting in increased human and environmental health and safety.

The purpose of the study was to summarize the available data on etiology, diagnosis, clinical symptoms and signs as well as on various approaches to the treatment of neuroendocrine cervical tumors.Material and Methods. The relevant sources were searched in the PubMed and cochrane Library systems, and publications from 1980 to 2019 were analyzed, 53 of which were used to write this review. We also included 6 case reports from N.N. Petrov National Research Center of Oncology.Results. Neuroendocrine tumors (NETs) are extremely rare and aggressive malignancies of the female genital tract, among which NETs of the cervix are the most common. Due to the rarity of these tumors, there are currently no treatment standards based on prospective, well-planned clinical trials. For these reasons, NETs present a significant therapeutic challenge for clinicians. Case reports. Six patients ranged in age from 32 to 71 years, with a median age of 46 years, were diagnosed with large-cell neuroendocrine carcinoma (4 patients) and small-cell neuroendocrine carcinoma (2 patients). One patient with stage IIIA dropped out of the follow-up schedule. Out of 5 followed up patients, 2 patients with stage IIIA and IIB died of disease progression after 6 and 11 months, respectively. One patient with stage IB1 is in remission for 16 months. Two patients with stage IIB continue to receive primary treatment.Conclusion. Neuroendocrine carcinoma of the cervix is a rare variant of cervical cancer. The choice of treatment options is decided only by a multidisciplinary team of doctors, and further research is required.

Abstract Bed bugs spend most of their lives hiding in harborages, usually in the seams of mattresses and box springs and in crevices of bed frames. For insecticidal products that target these shelters, the repellency of the products for bed bugs may influence their duration of contact. Bed bugs are known to avoid contacting surfaces treated with certain insecticides. The fungal biopesticide Aprehend contains spores of the entomopathogen, Beauveria bassiana. It is sprayed around bed frames, box springs, and furniture where bed bugs are likely to walk, which includes potential shelters. Here, I investigated the influence of a permethrin-impregnated cover, ActiveGuard, on bed bug sheltering behavior and the effectiveness of combining ActiveGuard with Aprehend. Bed bugs avoided harboring in a shelter constructed with ActiveGuard compared to a nontoxic encasement-type cover. This avoidance behavior reduced mortality induced by ActiveGuard shelters compared to forced continuous contact on the ActiveGuard cover. However, while bed bugs also avoided Aprehend-treated ActiveGuard shelters, the combined treatment induced almost complete mortality and more quickly than Aprehend-treated shelters made of the encasement-type cover. This suggests compatibility between the two integrated pest management (IPM) tools even though the bed bug’s avoidance behavior would suggest otherwise. Since Aprehend is highly effective against pyrethroid-resistant bed bugs, its use would provide more effective control where bed bug populations are more resistant to the permethrin-impregnated cover.

Background: In December 2019, an outbreak of a novel coronavirus disease (COVID-19; previously known as 2019-nCoV) was reported in Wuhan, Hubei province, China, which has subsequently affected more than 200 countries worldwide including Europe, North America, Oceania, Africa and other places. The number of infected people is rapidly increasing, while the diagnostic method of COVID-19 is only by nucleic acid testing. Objective: To explain the epidemiological characteristics, clinical features, imaging manifestations and to judge diagnostic value of COVID-19 by analyzing the clinical data of COVID-19 suspected and confirmed patients in a non-outbreak, Shanghai, China. To clarify the early epidemiology and clinical characteristics about COVID-19. Methods: Cross-sectional, single-center case reports of the 86 patients screened at Zhoupu Hospital in Pudong New District, Shanghai, China, from January 23 to February 16, 2020. Epidemiology, demography, clinical, laboratory and chest CTs were collected and analyzed. The screened patients were divided into COVID-19 and non-COVID-19 based on nucleic acid test results. Results: Of the 86 screened patients, 11 were confirmed (12.8%) by nucleic acid testing (mean age 40.73 ± 11.32, 5 males). No significant differences were found in clinical symptoms including fever, cough, dyspnea, sore throat, and fatigue (P > 0.05). No statistical difference was observed in plasma C-reactive protein (CRP) between the two groups (COVID-19 and non-COVID-19 ) of patients (P = 0.402), while the white blood cell count and lymphocyte count of the confirmed patients were slightly lower than those of the suspected patients (P < 0.05). Some non-COVID-19 chest CTs also showed subpleural lesions, such as ground-glass opacities (GGO) combined with bronchiectasis; or halo nodules distributed under the pleura with focal GGO; consolidation of subpleural distribution or combined with air bronchi sign and vascular bundle sign, etc. Conclusion: The early clinical manifestations and imaging findings of COVID-19 are not characteristic in non-outbreak areas. Etiological testing should be performed as early as possible for clinically suspected patients.

OBJECTIVE Arteriovenous malformations (AVMs) in the cerebellopontine angle cistern (CPAC) are specific lesions that can cause neurovascular compression syndromes as well as intracranial hemorrhage. Although case reports describing the CPAC AVMs, especially those presenting with trigeminal neuralgia (TN), have been accumulating by degrees, the pathophysiology of CPAC AVMs remains obscure. The authors' purpose in the present study was to evaluate the clinical and radiographic features of CPAC AVMs as well as the treatment options. METHODS This study defined a CPAC AVM as a small AVM predominantly located in the CPAC with minimal extension into the pial surface of the brainstem and closely associated with cranial nerves. All patients with CPAC AVMs treated in the authors' affiliated hospitals over a 16-year period were retrospectively identified. Clinical charts, imaging studies, and treatment options were evaluated. RESULTS Ten patients (6 men and 4 women), ranging in age from 56 to 77 years (mean 65.6 years), were diagnosed with CPAC AVMs according to the authors' definition. Six patients presented with hemorrhage, 3 with TN, and the remaining patient developed a hemorrhage subsequent to TN. Seven AVMs were associated with the trigeminal nerve (Group V), and 3 with the facial-vestibulocochlear nerve complex (Group VII–VIII). All patients in Group VII–VIII presented with the hemorrhage instead of hemifacial spasm. Regarding angioarchitecture, the intrinsic pontine arteries provided the blood supply for all CPAC AVMs in Group V. In addition, 5 of 7 AVMs with hemorrhagic episodes accompanied flow-related aneurysms, although no aneurysm was detected in patients with TN alone. With respect to treatment, all patients with hemorrhagic presentation underwent Gamma Knife surgery (GKS), resulting in favorable outcomes except for 1 patient who experienced rebleeding after GKS, which was caused by the repeated rupture of a feeder aneurysm. The AVMs causing TN were managed with surgery, GKS, or a combination, according to the nidus-nerve relationship. All patients eventually obtained pain relief. CONCLUSIONS Clinical symptoms caused by CPAC AVMs occur at an older age compared with AVMs in other locations; CPAC AVMs also have distinctive angioarchitectures according to their location in the CPAC. Although GKS is likely to be an effective treatment option for the CPAC AVMs with hemorrhagic presentations, it seems ideal to obliterate the flow-related aneurysms before performing GKS, although this is frequently challenging. For CPAC AVMs with TN, it is important to evaluate the nidus-nerve relationship before treatment, and GKS is especially useful for patients who do not require urgent pain relief.

Bed bugs have recently re-emerged as human pests worldwide. In this study, two surveys queried licensed pest management companies in Ohio (Midwest USA) about their experiences managing bed bugs. A primary objective was to assess the magnitude and spread of bed bug infestations statewide based on companies’ treatment records from 2005 and 2011 (first survey) and 2016 (second survey). The survey response rates were 35.6% and 31.6%, respectively. Treatment data from 2005 indicated that Ohio’s bed bug problem likely started in the SW corner of the state in Hamilton County (includes city of Cincinnati), since it totaled five times more treatments (approximately 4500) than second-ranking, centrally located Franklin County (Columbus). In the first half of 2011, more than 15,000 treatments were performed in these two counties. In 2016, treatments reached nearly 38,000 in Franklin County and in NE Ohio in the three combined counties that include Cleveland-Akron-Canton. Bed bug problems expanded statewide during an 11 y period, with an estimated 100+ treatments in 7 counties in 2005, 45 counties in 2011, and nearly all 88 counties in 2016. Apartments/condos and single-family residences comprised the largest share of bed bug work. Residents misused many pesticides and household cleaners trying to eliminate bed bugs. Many also discarded unwrapped infested furniture, which may further spread these bugs. More public education is needed to stop such practices. This study shows that bed bug problems grow and spread quickly; federal, state, and local officials and the public should immediately deal with bed bugs.

Developmental anomalies are one of the challenging clinical conditions to achieve successful treatment outcomes. Among these, Palatogingival groove [PGG] is a rare developmental anomaly usually found on the palatal aspect of maxillary anterior teeth. As self-cleansing is difficult in such patients, it can lead to serious localized periodontal lesion or can occur in combination with endodontic lesion. It is our sole responsibility being a specialist in the field of dentistry, to early diagnose and provide treatment for such conditions. On the basis of PGG extension to the pulp, it can be classified as simple or complex; accordingly, plethora of treatment options are available. Clinically taking interdisciplinary treatment modalities into consideration, curettage of the involved gingival tissues, elimination of the groove by grinding (saucerization), or by sealing with a Biocompatible material which might enhance the healing potential. Incase if the groove extends beyond the middle-third of the root, surgical flap debridement along with combined use of barrier membrane and alloplastic bone graft are recommended.This article represents a rare clinical case reports in management of PGG defect with an innovative conservative approach for simple, minimal groove extensive case and groove extension beyond 1/3 of the root region, treated with surgical multi-interdisciplinary approach.

Background: Duodenal duplication cysts are rare gastrointestinal tract malformations. Most patients experience symptom onset in the first decade of life. This review aims to examine clinical presentation, management strategies and outcomes of duodenal duplication cysts in childhood. Methods: A Pubmed/Medline (http://www.ncbi.nlm.nih.gov/pubmed/) search in October 2019 for articles published since 1999 using the keywords “duodenal duplication cyst,” “child” and “newborn” was carried out. Clinical symptoms, complications, diagnostic examinations, treatment options and outcomes were analyzed and tabulated. Results: There were 41 citations in the literature providing adequate descriptions of 45 cases of duodenal duplication cysts. The age of presentation ranged from newborn to 18 years. The median interval between initial presentation and definitive diagnosis and treatment was 17 months (range: 2 months to 12 years). Overall, 67% of cases presented with abdominal pain, and 43% were complicated with pancreatitis. Different surgical and endoscopic therapeutic strategies were reported. Conclusions: Duodenal duplication cysts may be associated with life-threatening complications and/or recurrent symptoms, impairing quality of life. Early recognition of patients who demonstrate suggestive signs and symptoms is important to ensure success of treatment. This review may be useful to highlight the main diagnostic aspects and limit the risk of a delayed diagnosis.

Terson's sign (TS) is classically defined as vitreous hemorrhage associated with subarachnoid hemorrhage of aneurysmal origin, being an important predictor of severity, indicating greater morbidity and mortality when compared to patients without the sign. The objective of this study is to review the relationship of Terson syndrome/Terson sign with the prognosis of aneurysmal subarachnoid hemorrhage. A search for original articles, research and case reports was performed on the PubMed, Scielo, Cochrane and ScienceDirect platform, with the following descriptors: Terson sign and subarachnoid hemorrhage. Retrospective, prospective articles and case reports published in the last 5 years and which were in accordance with the established objective and inclusion criteria were selected. Ten (10) articles were selected, in which the available results show an unfavorable prognostic relationship of TS and subarachnoid hemorrhage, because these patients had a worse clinical status assessed on the Glasgow scales ≤ 8, Hunt & Hess > III, Fisher > 3, in addition to intracranial hypertension and location of the aneurysm in the anterior communicating artery complex. The early recognition of this condition described by Albert Terson in 1900 brought an important contribution to neurosurgery, being recognized until nowadays.
