Oral Submucous Fibrosis (OSMF) is a chronic debilitating disease characterised by progressive fibrosis. The disease is predominantly seen in South East Asia and mainly attributed to areca nut chewing habit. The classical presentation includes generalised blanching with palpable fibrotic bands in the oral mucosa. Patients experience severe burning sensation, trismus and difficulty in gustatory functions, thus resulting in compromised nutritional status. The OSMF has been grouped under Oral Potentially Malignant Disorders (OPMD) and has high rates of malignant transformation. Although randomised controlled trials on different treatment modalities have been conducted, currently there is no effective medical management. This paper highlights two cases of OSMF presenting with severe burning sensation and trismus, who underwent incisional biopsy for histopathological confirmation and to rule out dysplasia. Both the patients were treated with dexamethasone and hyaluronidase, biweekly for four weeks. Post-treatment there was significant improvement in the clinical symptoms, thus providing the patients with better quality of life.

Introduction: Oral Submucous Fibrosis (OSMF) is a potentially malignant disorder commonly found in the Southeast Asian region, classically characterized through restricted mouth opening and burning sensation in the mouth on consuming spicy food. The etiopathogenesis and treatment aspects of OSMF have been extensively researched and reviewed. However, the effect of this condition on the Quality of Life (QoL) of patients has not gained much consideration. The aim of our case report is to highlight the importance of evaluating the QoL in an OSMF patient by using discipline-specific questionnaire and also to present to the readers a review of the recently published research articles on the application of QoL questionnaires in OSMF patients. Results: It was observed that most of the researchers used generic and discipline-specific questionnaire for evaluating the quality of life in OSMF patients. Conclusion: Evaluation of QoL is recommended during the treatment of OSMF patients. There is a need to develop and employ more of disease-specific QoL instruments for OSMF patients.

Oral submucous fibrosis (OSF) is characterized by abnormal collagen deposition. It is a precancerous disorder and transforms into a malignant tumor in 1.5–15% of all cases. Symptoms include submucous fibrosis, ulceration, xerostomia, a burning sensation, and restricted mouth opening. All of these greatly interfere with patient quality of life. The present review introduces OSF from a molecular perspective and summarizes what is known about its underlying mechanisms, diagnostic biomarkers, and therapeutic interventions. In addition to the aggressive treatment of OSF, its prevention is also important. Future research should, therefore, focus on improving the oral health literacy of the patients susceptible to OSF.

BACKGROUND Medicinal plants and their extracts have been found to possess potent antiinflammatory, antioxidant and anti-cancer properties that slow down the cellular proliferation and malignant transformation rate of orally potential malignant disorders such as oral leukoplakia, oral submucous fibrosis and oral lichen planus. They are a potential source of antioxidants such as vitamin A, E, and C that neutralize the free radicals produced in various oral mucosal lesions thereby preventing the mucosal damage. They have been found to modulate cellular signalling pathways and, this activation influences the cellular defence mechanisms that protect the normal cells against reactive oxygen species or reactive metabolites and induces apoptotic death of cancerous cells, thus chemoprevention ensues to prevent, reverse or delay carcinogenesis. They have been studied to enhance tumour cell killing and inhibition of metastasis by increasing number of immunocompetent cells such as macrophages, cytotoxic T lymphocytes, natural killer cells and by recruitment of antibodies at the tumour site. Now a days, natural herbs or phytochemicals are gaining attention due to side effects and increased antimicrobial resistance that often occur with prolonged use of chemotherapeutic agents. Therapeutic effect of phytochemicals in oral PMDs is very promising as they prevent the progression of dysplasia from mild to severe. Literature has revealed significant reduction in burning sensation, difficulty in swallowing and speech, and pain associated with the oral mucosal lesion. However, more research should be conducted in future to establish their use as an adjunctive therapy in oral potentially malignant disorders. By this review, an attempt has been made to highlight therapeutic potential of diverse herbs such as aloe vera, tulsi, neem, curcumin, lycopene, green and black tea, peppermint etc. in the reduction of clinical symptoms of burning sensation and pain associated with the oral mucosal lesions, and discusses their indispensable role in prevention of oral cancer. ABSTRACTOral Squamous Cell Carcinoma (OSCC) is common preventable disease when diagnosed early, but mostly its progression follows transition from oral potentially malignant disorders (OPMDs) like Oral Submucous Fibrosis (OSF). However, it is difficult to predict possibilities of progression in these premalignant lesions hence, identification of molecular biomarkers would have major clinical impact in early diagnosis and better prognosis. In this context microRNA’s(miR’s) provide better opportunities in malignancy prediction and demarcation in OSF to OSCC transition as they perform key regulatory roles in many tumorigenic processes. Here, we computationally screened differentially expressed miR’s of OSCC and OSF from public databases followed by construction of protein interaction networks and enrichment analyses. The relevant miR’s were validated using qPCR of total 93 samplesincluding 34 OSCC, 30 OSF and 29 control blood and tissue samples. We identified significant down regulation of miR-133a-3p in OSCC compared to controls and interesting up-regulation compared to OSCC and control. miR-9-5p was up-regulated in OSF as well as OSCC and down-regulated in OSF compared to OSCC. Therefore, these two miR’s may serve as risk stratification biomarkers with validation in larger categorical datasets.

Oral potentially malignant disorders (OPMD) are diverse lesions associated with an increased likelihood for malignant transformation to oral cancer. The most frequently encountered OPMD in clinical practice are Oral Leukoplakia and oral submucous fibrosis (OSMF). The use of tobacco is consistently linked with leukoplakia and areca/betel nut with OSMF. Histopathological features of oral epithelial dysplasia, its varying grades are of immense clinical significance to evaluate its ability for malignant transformation. We report a case of homogenous leukoplakia and OSMF, two oral potentially malignant disorders occurring at the same site with high-grade dysplasia and its management using lasers.

Background: Oral habits can cause severe oral potentially malignant disorders. There is a need to deeply analyse the role of betel nut in the causation of these disorders. Patients must be educated in order to prevent them from grabbing these conditions as inspection leads to prevention. Aim: The aim of our study was to study and analyse the different patterns of quid usage among the participants with oral submucous fibrosis and leukoplakia in Central India population. Patients and Methods: The study was performed on 90 patients amongst which 45 patients were clinically diagnosed of oral submucous fibrosis & the other 45 patients were diagnosed of leukoplakia.  The cases were then subjected to a detailed questionnaire which aimed to extract information regarding the various types of quid usage habits, their frequency and duration. The data was recorded and statistical analysis was done. Results: The study included 90 subjects – 86 males & 4 females. Out of 6 types of quid habits it is found that type IV (processed areca nut), Type VI (combined quid usage) quid caused OSMF stage II, Stage III & homogenous leukoplakia respectively. There is a strong association between duration and frequency of habit in causation of OSMF & leukoplakia. Conclusion: The present study showed increased prevalence of OSMF & leukoplakia with type IV & VI quid usage among the population of Central India.

Oral submucous fibrosis (OSF) is a well-documented potentially malignant condition. It affects most commonly the adult patients of India and Indian subcontinent, only few cases affecting children have been reported in the literature. This paper presents three cases of OSF below the age of 12 years and reviews the aetiology, clinical presentation, treatment modalities in children with improved follow-up results. Clinical features like restricted mouth opening, burning sensation and history of betel nut chewing helps in the diagnosis of the patients, which can be confirmed by histopathological examination. Conservative management and oral physiotherapy in children help in improved mouth opening. Counselling the children, their friends and parents also plays a vital role. The present paper highlights that children on a larger scale are using tobacco products and further studies are required with larger sample size.

Oral submucous fibrosis (OSF) is known as a potentially malignant disorder, which may result from chemical irritation due to areca nuts (such as arecoline). Emerging evidence suggests that fibrogenesis and carcinogenesis are regulated by the interaction of long noncoding RNAs (lncRNAs) and microRNAs. Among these regulators, profibrotic lncRNA H19 has been found to be overexpressed in several fibrosis diseases. Here, we examined the expression of H19 in OSF specimens and its functional role in fibrotic buccal mucosal fibroblasts (fBMFs). Our results indicate that the aberrantly overexpressed H19 contributed to higher myofibroblast activities, such as collagen gel contractility and migration ability. We also demonstrated that H19 interacted with miR-29b, which suppressed the direct binding of miR-29b to the 3′-untranslated region of type I collagen (COL1A1). We showed that ectopic expression of miR-29b ameliorated various myofibroblast phenotypes and the expression of α-smooth muscle actin (α-SMA), COL1A1, and fibronectin (FN1) in fBMFs. In OSF tissues, we found that the expression of miR-29b was downregulated and there was a negative correlation between miR-29b and these fibrosis markers. Lastly, we demonstrate that arecoline stimulated the upregulation of H19 through the transforming growth factor (TGF)-β pathway. Altogether, this study suggests that increased TGF-β secretion following areca nut chewing may induce the upregulation of H19, which serves as a natural sponge for miR-29b and impedes its antifibrotic effects.
