Abstract We evaluated "borderline" increases in overnight albumin excretion rates (AERs)—i.e., those between the upper 95th percentile of normal (7.6 μg/min) and the lowest value currently considered predictive of nephropathy (30 μg/min)—to determine their importance and to see whether glucose control influenced subsequent changes in the "borderline" AER values. Between 1985 and 1990, we studied 190 subjects with insulin-dependent diabetes mellitus (Type I), analyzing a mean of 6.5 timed overnight urine samples collected per subject. Above-normal AERs were associated with a significantly (by ANOVA) higher mean age (P = 0.03), longer duration of diabetes (P = 0.0002), and greater mean glycohemoglobin values (P = 0.002). The transition rate between borderline and abnormal AERs was significantly higher (P<0.0001, chi-square test) than the direct transition rate between normal and abnormal AERs, thus showing the borderline AER to be a definite intermediate stage. Good and poor glucose control were clearly associated with improvement and worsening, respectively, of the borderline AER values (P = 0.032, chi-square test of trend). More attention to borderline AER values is clearly indicated.

This study attempts to characterize a typical Chepang community in Chitwan district with reference to their economy at household level based on the study conducted in Lothar Village Development Committee. Chepang are considered to be one of the highly marginalized communities in Nepal having traditional subsistence based small economies. Their houses are small with mud floor, stone walls and straw roofs. One third of the Chepang households do not have toilets. They rear small number of mixed livestocks in a house eg. Cattle, buffaloes, poultry, goat and pig. They do not have household amenities like freeze, telephone, television, computer, motorcar and motorbike; but have mobile phones. More than ninty percent of Chepang go to jungle to collect one or the other types of edibles like githavyakur, wild fruits, and chiuri.Ninty five percent of Chepang people do not have bank account, thus rely on their friends and relatives for borrowing in household needs for money. Chi-square test reveals highly significant association between size of landholding and food sufficiency months, level of education and annual income, purpose of taking loan and sources of loan; as well as estimated annual income and account holding in bank.

Economic Literature, Vol. XIII August 2016, page 39-45

Introduction. The use of age- and risk-adjusted caries prevention requires up-to-date knowledge on the cariostatic effects of fluoride, as well as the methods and safety of fluoride prophylaxis. Aim. The aim of the study was to assess dentists’ knowledge of the safety and mechanism of anticaries effects of fluoride. Material and methods. An anonymous questionnaire was conducted among 212 dentists participating in dental training. The questions included in the questionnaire related to knowledge about water fluoridation, fluoride cariostatic mechanisms and the safety of fluoride prophylaxis. The chi-square test and the Spearman’s rank correlation coefficient were used for statistical analysis. A significance level of 0.05 was used. Results. Incorrect knowledge about water fluoridation was demonstrated by 26.9% of respondents in the country and 16.0% of respondents in Europe. Most respondents (80.7%) reported higher efficacy of exogenous vs endogenous anticaries approaches, and more than half of respondents (59.0%) considered the processes of remineralisation and demineralisation as the most important anticaries activity. Almost all respondents (95.7%) agreed that the use of fluoride for caries prevention is controversial, mainly due to its dose-dependent therapeutic or toxic effects (85.4%). According to about 20% of respondents, cariostatic fluoride doses have adverse effects on the general health, mainly in the form of bone fragility (10.4%). Conclusions. Lack of correct knowledge about the dominant cariostatic effects of fluoride may result in the choice of an inappropriate preventive method or avoiding local application of fluoride preparations for fear of adverse systemic effects, which may in turn lead to increased caries prevalence in a given population.

Abstract Background Maintaining the hips in flexion and abduction posture is a common recommendation to facilitate hip maturation in infants. Double diapering helps to maintain a baby’s hips in flexion and abduction posture, but the efficacy of double diapers in hip development has never been verified. We investigated whether double diapering results in greater improvement of the alpha angle in the first months of life. Methods This prospective randomised control study enrolled newborns with Graf type IIa hips. Babies were assigned to the double-diaper or single-diaper group. Parents were instructed on proper hip positioning, except for diapering. Change in the alpha angle from newborn to 1 month after birth, rate of improvement to bilateral Graf type I hips in 1 month, and number of ultrasound examinations and orthopaedic clinic visits in the first year were compared between the two groups. For babies with bilateral IIa hips, we used the averaged data from both hips, and for babies with unilateral IIa hips, we used the data from the IIa hips. Results Seventy newborns with 102 type IIa hips were included from January to December 2017. They were allocated to the double-diaper group (N=33) and single-diaper group (N=37). With a comparable sex ratio, gestational age, and newborn alpha angle, the double-diaper group had a greater increase of alpha angles in 1 month than the single-diaper group (+7.9° vs. +5.2°, t-test, p=0.011). Twenty-eight babies in the double-diaper group (84.8%) and 20 babies in the single-diaper group (54.1%) improved to having bilateral Graf type I hips (chi-square test, p=0.006). Subsequent clinical visits and hip ultrasounds before 1 year were significantly reduced in the double-diaper group. Conclusions Our results support double diapering for enhancing hip maturation in babies with Graf type IIa hips. The amount of improvement may not indicate double diapering as a treatment for hip dysplasia, but its effects in immature hips are noteworthy in reducing further clinical and ultrasound follow-ups.

ABSTRACT Background: Woman who become mother have gone through various life cycles, from pregnancy till childbirth. During this postpartum period, almost all mothers also breastfeed their babies. The process of restoring the body health after childbirth along with caring for the baby often leaves the mother physically and psychologically exhausted. So many mothers cannot provide exclusive breastfeeding. This is where the husband’s role is very important to provide support to the mother. This study aimed to analyze the influence husband’s role on the mother’s interest in giving exclusive breastfeeding. Subjects and Method: A cross-sectional study was conducted for 2 months on 46 postpartum mothers. This study conducted in Wonokromo, Surabaya, in 2020. The independent variable was the husband’s role and the dependent variable was the mother’s interest in giving exclusive breastfeeding. The data were collected using questionnaires and analyzed using Chi Square test. Results: The results showed that out of 21 (57.14%) mothers who had less husband’s role, most had a low interest in giving exclusive breastfeeding. Of the 25 mothers who have a good husband’s role, almost all (92%) have a high interest in exclusive breastfeeding (p< 0.001), and it was statistically significant. Conclusion: The better husband’s role in supporting his wife, the higher the mother’s interest in giving exclusive breastfeeding.
