Abstract Purpose We aimed to cross-sectionally investigate how work and sleep conditions could be associated with excessive fatigue symptoms as an early sign of Karoshi (overwork-related cerebrovascular and cardiovascular diseases; CCVDs). Methods We distributed a questionnaire regarding work, sleep, and excessive fatigue symptoms to 5410 truck drivers, as the riskiest occupation for overwork-related CCVDs, and collected 1992 total samples (response rate: 36.8%). The research team collected 1564 investigation reports required for compensation for Karoshi. Of them, 190 reports listed the prodromes of Karoshi, which were used to develop the new excessive fatigue symptoms inventory. Results One-way analyses of variance showed that the excessive fatigue symptoms differed significantly by monthly overtime hours (p < 0.001), daily working time (p < 0.001), work schedule (p = 0.025), waiting time on-site (p = 0.049), number of night shifts (p = 0.011), and sleep duration on workdays (p < 0.001). Multivariate mixed-model regression analyses revealed shorter sleep duration as the most effective parameter for predicting excessive fatigue symptoms. Multiple logistic regression analysis confirmed that the occurrences of CCVDs were significantly higher in the middle [adjusted ORs = 3.56 (1.28–9.94)] and high-score groups [3.55 (1.24–10.21)] than in the low-score group. Conclusion The findings suggested that shorter sleep duration was associated more closely with a marked increase in fatigue, as compared with the other work and sleep factors. Hence, ensuring sleep opportunities could be targeted for reducing the potential risks of Karoshi among truck drivers.

Serum high-sensitivity C-reactive protein (hsCRP) and serum uric acid (SUA) are biomarkers that predict chronic inflammation and cardiovascular dysfunction. Therefore, we aimed to investigate the association between sleep duration, hsCRP, and SUA in Korean women. Cross-sectional data from the Seventh Korea National Health and Nutrition Examination Survey was analyzed. The odds ratio (OR) and 95% confidence intervals (CIs) for an association between higher hsCRP (>2.0 mg/L) or higher SUA (>5.6 mg/dL) and sleep duration were calculated using multiple logistic regression analyses after adjusting for potential confounders. In total, 6151 women were included in the analysis. There was a U-shaped relationship between continuous sleep duration, hsCRP, and SUA. Compared to those who slept for 7–8 h, the ORs (95% CIs) for higher hsCRP were 1.43 (0.95–2.16) in short sleepers and 1.64 (1.09–2.48) in long sleepers after adjusting for confounders. Compared with those who slept for 7–8 h, the ORs (95% CIs) for higher SUA were 1.54 (1.04–2.26) in short sleepers and 1.94 (1.27–2.96) in long sleepers after adjusting for confounders. We found a U-shaped association between sleep duration, hsCRP, and SUA in Korean women. 7–8 h sleep was associated with lower level of hsCRP and SUA in Korean women.

Abstract Introduction Rotating shift work is known to adversely impact sleep. Napping is one of the strategies that workers can use to mitigate the effect of shift work on their sleep. In this study, we investigated the effect of chronotype on napping behavior in police officers involved in rotating shift work. Methods Actigraphy-based sleep measures and chronotype information was available from 74 police officers (20 women and 54 men; age [mean ± SD]: 32 ± 5.4 years) that participated in a 35-day field study during which they worked morning, evening, and night shifts. A generalized linear mixed model was used to assess the effect of shift type, chronotype, and their interaction on the likelihood to take a nap, adjusted for relevant covariates. In addition, linear mixed models were used to determine the effect of shift type, chronotype, and their interaction on sleep duration with and without taking into account napping duration. Results The likelihood to take a nap was influenced by an interaction between shift type and chronotype (χ2(2) = 11.2, p = 0.004). Earlier chronotype was linked to a lower likelihood to take naps during days with morning shifts and a higher likelihood during days with night shifts. Napping modulated the effect of shift type and chronotype on daily sleep duration, most notably during night shifts: while chronotype was associated with the duration of the main sleep period during night shifts, with the main sleep period being 1.7 h [95% C.I.: 0.6 – 2.8] shorter in the earliest chronotypes compared to the latest chronotypes, this effect was attenuated and no longer significant when napping duration was taken into account (difference in total sleep duration in latest chronotypes vs earliest chronotypes during nights shifts: 0.9 [−0.1 to 1.9] h). Conclusion Napping attenuates the chronotype-dependent effect of atypical work schedules on sleep duration in this population of shift-working police officers. These findings highlight the need to take into account chronotype when assessing the effect of shift work on sleep behavior. Objectives: The study aimed to investigate sex differences associated with sleep duration and the prevalence of thyroid disease among South Korean adults.Methods: This cross-sectional study included 17,555 adults who participated in the Korea National Health and Nutrition Examination Survey from 2016 to 2018. Sleep duration was categorized into three groups (insufficient, <7 hour; normal, 7-8 hour; prolonged, ≥9 hour). A chi-squared test was performed to assess baseline characteristics. Multiple logistic regression analysis was used to identify the association between general characteristic and the prevalence of thyroid disease. In subgroup analysis, multiple logistic regression analysis was performed to find the association between sleep duration and prevalence of thyroid disease by sex.Results: For men, insufficient and prolonged sleep durations were increased risk of thyroid disease compared with normal sleep duration (insufficient sleep: odds ratio, OR=1.85, 95% confidence interval, CI=1.19-2.87; prolonged sleep: OR=1.71, 95% CI=1.02-2.87).Conclusions: This study found that men with insufficient and prolonged sleep were associated with thyroid disease. Further studies are needed to identify the relationship between sleep duration, thyroid disease, and sleep quality by sex.

Aim: The aim of this study was to identify whether metabolite biomarker candidates for pancreatic cancer (PC) could aid detection of intraductal papillary mucinous neoplasms (IPMN), recognized as high-risk factors for PC. Materials & methods: The 12 metabolite biomarker candidates, which were found to be useful to detect PC in our previous study, were evaluated for plasma samples from patients with PC (n = 44) or IPMN (n = 24) or healthy volunteers (n = 46). Results: Regarding the performance of individual biomarkers of PC and PC high-risk IPMN, lysine exhibited the best performance (sensitivity: 67.8%; specificity: 86.9%). The multiple logistic regression analysis-based detection model displayed high sensitivity and specificity values of 92.5 and 90.6%, respectively. Conclusion: Metabolite biomarker candidates for PC are useful for detecting high-risk IPMN, which can progress to PC.

The prevalence of an epiretinal membrane (ERM) was elucidated using a dataset from a health examination program database in Japan. From the cohort database, 5042 eyes of 2552 subjects were included. The presence of an ERM, cellophane macular reflex (CMR), or preretinal macular fibrosis (PMF) was detected using color fundus photographs, and crude and age-standardized prevalence were obtained. To further assess the possible risk factors of ERM, background parameters were compared between ERM+ and − groups, and multiple logistic regression analysis was performed. ERM was detected in fundus photographs of 275 eyes (eye-based prevalence of 5.5%) from 217 subjects (subject-based prevalence of 8.5%). CMR was detected in 169 eyes (3.4%) of 138 subjects (5.4%), and PMF was detected in 106 eyes (2.1%) of 97 subjects (3.8%). By univariate analyses, compared with ERM− eyes or subjects, higher Scheie’s H grade (p < 0.0001), S grade (p < 0.0001), and glaucoma prevalence (p = 0.0440) were found in ERM+ eyes, and older age (p < 0.0001), more frequent histories of hypertension (p = 0.0033) and hyperlipidemia (p = 0.0441), and more frequent uses of medication for hypertension (p = 0.0034) and hyperlipidemia (p = 0.0074), shorter body height (p = 0.0122), and higher systolic blood pressure (p = 0.0078), and thicker intimal medial thickness (p = 0.0318) were found in ERM+ subjects. By multivariate analyses, older age (p < 0.0001, estimate = 0.05/year) was the only significant factor of ERM prevalence. Age-standardized prevalence of ERM was calculated to be 2.4%, 6.7%, and 13.3% for all ages, subjects older than 40 years, and subjects older than 65 years, respectively. We reported the prevalence of ERM and its subclasses in Japanese subjects. Since its prevalence is remarkably high in older subjects, an ERM can be seen as an important cause of visual impairment in Japan and in areas of the world where individuals live to an advanced age.

Although multilayered strategies including preventive behaviors should be adopted to mitigate coronavirus disease 2019 (COVID-19) transmission, evidence on the effectiveness of preventive behaviors against COVID-19 remains limited. This Internet-based prospective cohort study collected baseline data in November 2020 and follow-up data in February 2021, during the third wave of the epidemic in Japan. Among the 19 941 included participants, the percentages reporting that they always used a face mask, practiced hand washing/disinfection, gargling, and ensuring proper room ventilation were 85.4%, 36.0%, 51.1%, and 44.6%, respectively. Multiple logistic regression analyses revealed that less frequently practicing hand washing/disinfection (odds ratio [OR] = 1.20, 95% confidence interval [CI]: 1.10-1.32), gargling (OR = 1.20, 95% CI: 1.10-1.30), and ensuring proper room ventilation (OR = 1.38, 95% CI: 1.26-1.52) were significantly associated with self-reported COVID-19-like illness (CLI). These results suggest that personal preventive behaviors may be effective in reducing CLI, even when universal masking is practiced.

<b><i>Background:</i></b> Vestibular system is critical for maintaining balance and learning complex tasks. This study aimed to determine the frequencies, types, and predictors of vestibular dysfunctions (VDs) in children with type 1 diabetes (T1D) using videonystagmography (VNG). <b><i>Patients and Methods:</i></b> This study included 65 patients (children with T1D = 40; controls = 25). The patients underwent VNG. <b><i>Results:</i></b> Patients (boys = 15; girls = 25) had a mean age of 14.05 ± 1.82 years and duration of illness of 6.30 ± 2.84 years. The majority had frequent attacks of diabetic ketoacidosis (DKA) (65%) and hypoglycemia (40%). Dizziness was reported in 20%. VNG abnormalities were reported in 70% (<i>n</i> = 28), of them 71.43 and 28.57% had central and peripheral VDs, respectively. Dizziness was associated with peripheral VD. Compared to patients without VDs, those with VDs were older and had earlier age at onset and longer duration of diabetes (&#x3e;5 years), higher levels of HbA1c (&#x3e;7%), higher frequencies of DKA and hypoglycemic attacks, comorbid medical conditions, and diabetic complications. Multiple logistic regression analysis showed that presence of VNG abnormalities (VDs) was independently correlated with diabetes duration &#x3e;5 years (odds ratio [OR] = 4.52 [95% confidence interval [CI] = 3.55–7.04], <i>p</i> = 0.001), HbA1c% levels &#x3e;7% (OR = 3.42 [95% CI = 2.84–5.75], <i>p</i> = 0.001), and presence of hypoglycemic attacks (OR = 4.65 [95% CI = 2.85–7.55]). <b><i>Conclusions:</i></b> -VDs are prevalent in children with T1D and correlated with the duration and severity of diabetes and the occurrence of hypoglycemic attacks. Therefore, optimizing glycemic control and prevention and treatment of diabetic complications and comorbidities are important. Multidisciplinary follow-ups are required for early detection and management of diabetic VDs.

Introduction: This study sought to examine: (a) the associations between physical activity, sedentary time, and sleep duration, as discrete behaviors, with depression among adults with visual impairments; and (b) the impact of meeting none, one, two, or three of the guidelines for these behaviors on depression among adults with visual impairments. Materials: One hundred eighty-two ( Mage = 44.8) adults with visual impairments, recruited via email through two visual impairment organizations in the United States, completed the International Physical Activity Questionnaire–Short Form, a sleep duration question, the Major Depression Inventory, and a demographic questionnaire. Based on results from the questionnaires, dichotomous variables for meeting or not meeting physical activity, sleep, and sitting guidelines were created. Data were analyzed using three components: a descriptive analysis, Pearson product-moment correlation analyses, and hierarchical regression analyses. Results: Overall, 14.8% of participants were categorized as having some degree of depression. Meeting the sleep guideline was a significant negative predictor of depression scores in the hierarchical regression analyses. The number of guidelines met was a negative predictor for depression score controlling for other variables. Discussion: Adequate sleep, as well as meeting all three guidelines synergistically, was meaningful in influencing depression among this population. The current study’s results should prompt the continued examination of health-behaviors among adults with visual impairment using a more holistic 24-hour activity cycle framework. Implications for practitioners: This study supports the utilization of multi-behavioral interventions to reduce the risk of depression by enhancing physical activity and sleep, while reducing sitting time, among this population.
