Recurrent paediatric pinworm infection of the vagina as a potential reservoir for Enterobius vermicularis

AbstractEnterobius vermicularis infection remains one of the most common parasitic infections, particularly prevalent in children. Enterobiasis, although not usually dangerous, may cause significant morbidity. Elimination of the parasite from a family or an institution often poses problems, either because of an incomplete cure or re-infection. While there have been limited reports of ectopic enterobiasis throughout the world, ours is probably one of the rarest reports of recurrent vaginal E. vermicularis infection in the absence of any gastrointestinal symptoms despite complete treatment. A 4-year-old girl presented with recurrent episodes of vulval itching on 3–4 occasions over 2 years. There was no pruritis ani nor urinary/gastrointestinal complaints. The vulva was inflamed with 4–5 living worms, 6–7 mm in length, emerging from the anterior vaginal fornix, but with no vaginal discharge. Direct microscopic examination of vaginal swabs revealed adult worms of Enterobius but no eggs. Repeated stool samples from the patient, parents and a sibling were negative. The patient was treated orally with 100 mg of mebendazole for 3 days followed by two more courses at 3-week intervals over a period of 3 months. Recurrent vaginal enterobiasis despite complete treatment and in the absence of any gastrointestinal involvement suggests that the vagina is a potential reservoir for E. vermicularis, which supports the theory of rare ectopic enterobiasis through the ascending pathway of the female genital tract.

Abstract Background A wide variation of causes can lead to gastrointestinal symptoms in children- an infection with parasites is one of them. The expansion of international travel might lead to an increase in testing children for a correspondent infection. Currently there are no guidelines available, which patients should be tested for a possible parasitical infection. The aim of the study was to characterize Swiss children suffering from intestinal parasites, in order to provide more knowledge for the clinician who should be tested. Methods This is a retrospective study of Swiss pediatric patients, whose stools have been tested for parasites and helminths. Results A total of 1855 stool samples, belonging to 572 different children with an average age of 7.9 years, were tested within a 10-year period. The prevalence of a positive result was 4.2%, of which all were positive for Blastocystis, and 12.5% had a co-infection with Endolimax nana. Conclusion Immigrants, immune compromised children with diarrhea and pediatric patients with bloody or protracted diarrhea should have 2 different stool specimens examined for a possible parasitical infection.

Status of intestinal parasitic infections among primary school children in Rivers State, Nigeria, was investigated between January and December 2011. A total of 3,826 stool samples were collected from school children (1,828 males and 1998 females) in 36 primary schools from 13 local government areas of Rivers State. The samples were analyzed using wet saline/iodine and formol ether concentration methods. Of the 3,826 stool samples examined, 1059 (27.66%) were positive for different intestinal parasites, namely,Ascaris lumbricoides(51.78%), hookworm sp. (25.0%),Trichuris trichiura(15.18%),Strongyloides stercoralis(7.14%),Taeniasp. (0.89%), andEnterobius vermicularis(0.01%). The prevalence of the infection was generally higher in males (57.60%) than females (42.40%). The differences were not statistically significant (P>0.05). Among these intestinal parasites,Ascaris lumbricoides, hookworm sp., andTrichuris trichiurawere found in all the 13 local government areas studied whileStrongyloides stercoraliswas found in 12,Taeniasp. in five, andEnterobius vermicularisin only one community in Ahoada Local Government Area. The overall infection rate remains high and would require coordinated deworming of the school children within the state.

Background. Infection with intestinal parasites is widespread worldwide, especially in developing countries. Intestinal parasites are known as one of the leading causes of diarrhea in both immunocompetent and immunocompromised subjects, but cancer patients are highly susceptible to contamination, and it can be deadly for them. This study aimed to estimate the prevalence of intestinal parasites in immunocompromised patients in Ahvaz. Material and Methods. In this descriptive cross-sectional pilot case-control study, fecal samples were collected from 52 children with malignancies hospitalized in Baqaei2 hospital in Ahvaz. A questionnaire including demographic information, type of cancer, type of gastrointestinal symptoms, and laboratory diagnosis was completed for each patient. The collected specimens were examined by direct smear, Logul staining, and concentration. Result. The 52 stool samples were collected, 46% were female and 54% male. The age range of children enrolled in the study was from 4 months to 16 years. Of these stool samples, 38.38% were infected with a variety of parasitic intestinal infections (helminths and protozoa). In this study, protozoan parasites, Blastocystis (23%), Chilomastix mesnili (1.92%), Endolimax nana (7.7%), and Entamoeba coli (1.92%), and helminth infection, Strongyloides stercoralis (3.84%), were observed and statistical analysis showed that there was a significant relationship between gastrointestinal symptoms and parasitic infection in children with cancer. Conclusion. Blastocystis and Endolimax nana are the most prevalent gastrointestinal parasitic protozoans that infect individuals admitted to Baqaei2 Hospital of Ahvaz, Iran. Since parasitic intestinal infections in immunocompromised patients lead to fatal diarrhea, children with parasitic infections must be carefully identified and treated.

SummaryWe are reporting the case of a nine-month-old Pakistani female with complaint of growth retardation who presented multiple intestinal parasitic infections. Probably because of contamination with fecal matter, the initial microscopic examination of the urinary sample revealed the presence of eggs of Enterobius vermicularis, cysts of Entamoeba coli, and an organism similar to mites. Stool samples were obtained after two weeks and microscopic investigation confirmed the presence of Enterobius vermicularis eggs, cysts of Entamoeba coli, and hookworm eggs. The patient was immediately subjected to mebendazole therapy associated with trimethoprim-sulfamethoxazole, to which she responded well. Follow-up stool re-examinations performed 15 and 30 days after the treatment tested negative for all parasitic ova and cysts. This study reflects the importance of considering multiple parasitic infestations in low socio-economic populations and highlights the need of improving poor hygienic conditions to prevent such infections, in particular in children.

Intestinal parasitic infestations are a common finding in the developing world, however, the patterns of parasitic distribution and rates are different elsewhere. The prevalence of intestinal parasitic infestation and chemical analysis of stool sample of the school children of age group 4-12 years in Dadeldhura district, far western region of Nepal. Out of 530 stool samples, 165 (31.13%) were found to be intestinal parasites positive. Among the total parasites, Hymenolepsis nana (46.56%) and Giardia lamblia (7.47%) were found in high prevalence as helmiths and protozoa respectively. Among the total stool samples 159 (30%) showed positive occult blood test. The prevalence of parasitic infection was found high in the children of age group 4-6 years (38.18%) than others though the result was not significant statistically (P>0.05). Highest prevalence of parasites was found in those not following the hygienic conditions (36.17%) than those following (18.83%). Similarly, high proportion of the parasites were found in children having gastrointestinal symptoms (49.23%) than in children without the gastrointestinal symptoms (13.70%) though the result was not significant statistically (P>0.05). The study showed the children using the water directly from the source had higher prevalence of parasites (32.13%) than those using treated water (14.28%). This study presents the high prevalence of stool parasites in school going children of Dadeldhura district. Key words: Parasitic infection, School children, Stool samples, Dadeldhura, Nepal.

Abstract Background: Blastocystis is a parasite that inhabits human intestinalis. It is commonly identified in asymptomatic individuals, while its pathogenic role is underestimated. In Syria, studies on this parasite are very few, and it is not documented in the laboratory reports. The present study aimed to evaluate the sensitivity of three different diagnostic methods in the detection of Blastocystis among patients with no specific or clear symptoms. Methods: Stool samples were collected from 70 patients suffering from various gastrointestinal symptoms. All samples were examined microscopically using iodine staining smears, and after in vitro cultivation at 37°C for 48- 72 h using Jones' medium. Molecular detection of Blastocystis was determined by fragment amplification of the SSU rRNA gene using PCR. Results: Blastocystis was identified in: 49 cases (70%) by direct microscopic examination, in 60 isolate (85.7%) by in vitro culture and in 64 (91.4%) of cases using molecular detection. Comparative analysis revealed that the sensitivity of microscopic detection for Blastocystis was 73.4% while it was 90.6% for in vitro culture and 95.9 - 96.7% using PCR detection method. Blastocystis was found alone in 32 (65.3%) of cases, while co-infection was detected in 17 (34.7%) samples. Conclusions: Our findings highlighted the importance of considering Blastocystis in laboratory diagnosis. Molecular methods are recommended for screening clinical specimens for Blastocystis infection especially among individuals with no common particular symptoms. If not applicable, two different diagnostic techniques are required for accurate diagnose of this parasite.

Fecal-orally transmitted cyclosporiasis is frequent in remote resource-limited settings in Central and South America with poor hygiene conditions. In this study, we aimed at assessing seasonal effects on the epidemiology of colonization or infection with C. cayetanensis in Colombian indigenous people living under very restricted conditions. In the rainy season between July and November and in the dry season between January and April, stool samples from indigenous people with and without gastrointestinal symptoms were collected and screened for C. cayetanensis applying in-house real-time polymerase chain reaction (PCR). In the rainy season and in the dry season, positive PCR results were observed for 11.8% (16/136) and 5.1% (15/292), respectively, with cycle threshold (Ct) values of 30.6 (±3.4) and 34.4 (±1.6), respectively. Despite higher parasite loads in the rainy season, fewer individuals (2/16, 12.5%) reported gastrointestinal symptoms compared to the dry season (6/15, 40%). In conclusion, considerable prevalence of C. cayetanensis in Colombian indigenous people persists in the dry season. Low proportions of gastrointestinal symptoms along with higher parasite loads make colonization likely rather than infection.

Background. Neglected tropical diseases continue to be one of the leading causes of morbidity and mortality in the developing world. Psychiatric patients are among groups at risk for parasitic infection although control and monitoring programs largely overlook this population. This study aimed at determining prevalence and factors associated with intestinal parasitic infection among patients admitted to a psychiatric facility.Method. The study followed cross-sectional design; all the residing patients that met the inclusion criteria were included in the survey. Stool samples were collected and examined by direct wet preparation and formol-ether concentration. Data were analyzed with STATA version 12.1; Chi-square test was computed to determine the level of significance atpvalue < 0.05.Results.Of all 233 patients who returned the stool samples, 29 (12.45%) screened were positive for an intestinal parasite. There was no significant association between parasite carriage and age, sex, or duration of hospital stay.Conclusion. The study shows that intestinal parasitic infection is common among patients in a psychiatric facility and highlights that parasitic infections that enter through skin penetration may be a more common mode of transmission than the oral route. Furthermore, the study underscores the need for surveillance and intervention programs to control and manage these infections.

AbstractA cross-sectional survey was conducted to determine the prevalence and intensity of soil-transmitted helminths among pre-school and school-aged children attending nursery and primary schools in Ile-Ife. Single stool samples were collected between January and March, 2009 from 352 children randomly selected from a total of 456 children attending both private and government schools. The stool samples were processed using the modified Kato–Katz technique, and then examined for the eggs of soil-transmitted helminths (STHs). One hundred and twenty-one (34.4%) samples were positive for STH eggs. The overall prevalences of Ascaris lumbricoides, Trichuris trichiura and hookworm were 33.2%, 3.7% and 0.9%, respectively. The prevalence of STH infection in government schools (47.8%) was significantly higher than in private schools (16.1%) (P < 0.001). The most common type of mixed infection was the combination of A. lumbricoides and T. trichiura (6.8%). The prevalence and intensity of A. lumbricoides rose with age. The lowest prevalence and intensity (7.7%; 0.240 ± 0.136 eggs per gram (epg)) were recorded in the 2- to 3-year-old age group, while the highest prevalence and intensity (58.7%; 1.820 ± 0.237 epg) were recorded in children aged 10 years and above. A questionnaire survey indicated that 73% of the children attending private school had been treated with anthelminthics less than 2 months prior to the collection of stool specimens, while 43% of the children attending government school received anthelminthic treatment during the same period. The findings indicate that STH infections are endemic among schoolchildren in Ile-Ife and that the burden of parasitic infections is greater in government schools than in private schools.
