Should fertile women quit drinking alcohol to produce better quality oocytes?

Summary Alcohol consumption has long been shown to affect both fetal health and pregnancy. In this study, antral follicle count, maturation level of oocytes including morphological assessment and number of metaphase I (MI), metaphase II (MII) and germinal vesicle (GV) stage oocytes obtained from young women (age < 30 years old) with or without alcohol consumption were investigated. In total, 20 healthy women who were social drinkers and 36 healthy women who do not consume alcohol were involved in this study. Women in both study and control groups were undergoing controlled ovarian stimulation. The antral follicle count and the number and quality of the oocytes retrieved were evaluated and recorded. In total, 635 antral follicles, 1098 follicles and 1014 oocytes with 820 MII, 72 MI and 78 GV stage oocytes were collected from the social drinkers. In the control group, 628 antral follicles, 1136 follicles and 1085 oocytes with 838 MII, 93 MI and 102 GV stage oocytes were evaluated. The results of this study showed that the antral follicle count was very similar in both groups. The number of oocytes and MII stage oocytes was slightly higher in the control group, although it was not a significant difference. This study showed that although the consumption of alcohol may have adverse effects post-implantation, it may not have a solid effect during oogenesis in young women. The results of this study are especially important in clinical settings as some women who are social drinkers undergo in vitro fertilization treatments.

Background: The objective of the present study was to establish the role of AFC as a marker of ovarian reserve in fertility-proven and in sub-fertile Indian women, and to establish the baseline cut-off AFC values for Indian women.Methods: This is an Observational Case-Control type of study. Test subjects (n=30, age range 20-35 years) were selected randomly from all the women coming to our Out-Patient Department with Primary Infertility. Healthy female volunteers (n=30, age range 20-35years) were recruited from the local population. Statistics: Student t test was applied to compare the mean Antral Follicle count between the case and control group. Pearson correlation test was applied to assess the correlation between age and AFC for case and control group.Results: The results of the study show that there is a significant difference between the number of antral follicles between fertile and infertile women (p < 0.001). The baseline cut-off for successful pregnancy was established as an AFC of 12. There exists negative correlation (r = -0.249) between age and AFC case group indicating as the age increases AFC decreases.Conclusions: The findings from this study help in strengthening the idea that AFC serves as a marker for ovarian reserve. Our observation indicates that the number of antral follicles is lower in the subfertile patients as compared to the fertile group (in all age groups), in view of the significantly lower median AFC in women of the former group (P < 0.001).


Introduction: Poor ovarian responders are the most challenging patients in reproductive medicine and no successful treatment has been proposed. Androgens are thought to play an important role during early folliculogenesis and diminished levels are associated with decreased ovarian sensitivity to follicle-stimulating hormone. This study aimed to determine whether pretreatment with testosterone improves the results in poor responders undergoing in vitro fertilisation (IVF). Materials and methods: This observational pilot study enrolled 33 poor responders undergoing IVF. Eleven patients were pretreated with 250 mg intramuscular testosterone and compared to a control group of 22 patients. The participants were tested for free testosterone, dehydroepiandrosterone sulfate, sex hormone binding globulin, and anti-mullerian hormone (AMH). Results: The two groups had similar baseline characteristics. Significant improvement was reached in the hormones free testosterone, dehydroepiandrosterone sulfate, and sex hormone binding globulin in the testosterone-pretreatment group. No difference was detected in antral follicle count (5.06 versus 4.24); AMH (0.51 versus 0.53), mature oocytes (2.2 versus 2.32), and the number of embryos (1.2 versus 1.33) between the study and control groups, respectively. There was a slow improvement in fertilisation rate but without any significance (62.97% versus 57.61%). However, the cancellation rate of the ovarian stimulation was much greater in the control group (18.18%) in comparison with the study group (0.0%). Pregnancy rate (PR) in the testosterone group was higher than controls (PR per cycle: 27.3% versus 4.6; p=0.09). Conclusion: Based on the limited number of patients studied, pretreatment with testosterone seems to improve PR and cancellation rate in poor responders but failed to affect antral follicle count, AMH, and the number of mature oocytes and embryos. Given these results, further research would provide more certainty.

Aromatase is the key enzyme, which converts androgens into estrogens. To study the role of aromatase in pathogenesis of endometriosis 57 patients and 15 healthy women of reproductive age were examined. Aromatase activity was detected by reaction of estrogens on aromatase inhibitor letrozol intake. Ovarian source of detected aromatase activity was proved by suppression of reaction on letrozole intake during therapy with gonadotropin- releasing hormone agonist. Aromatase activity in recalculation on antral follicle in endometriosis patients withII–IV stages was higher than in healthy women, though total ovarian aromatase activity was not differed from control group because of low number of antral follicles in endometriosis patients. The disturbance of folliculogenesis in endometriosis is connected probably with hyperestronaemia, which has ovarian and extragonandal origin.

SummaryInfertility is the most common issue in the field of reproductive medicine. Many factors affect fertility status, including life-style choices such as cigarette smoking or alcohol use. The aim of this study was to investigate the effects of cigarette smoking on oocyte quality as well the quantity in young fertile women. In total, 56 young fertile women who were undergoing oocyte donation programmes were included in this study. The effects of cigarette smoking on antral follicle count, number of follicles and oocytes retrieved and morphology were assessed by an expert embryologist. The results of this study showed that cigarette smoking did not have a significant effect on the follicle count or the number of oocytes retrieved from young and fertile women. However, a significant difference was observed in the morphological assessment. In conclusion, although cigarette smoking does not seem to affect oocyte development, it had an effect on cytoplasmic anomalies and therefore may lower pregnancy chance. Therefore, it is crucial to give proper counselling to patients who are trying to become pregnant both naturally and by in vitro fertilization.

Objectives: To determine the effect of oral cypermethrin exposure on the decrease of Bcl-2 expression in granulosa cells and antral follicle count in Rattus norvegicus ovary.Materials and Methods: This was a true experimental study using post-test control group design. This study used 24 Rattus norvegicus which were divided into 4 groups, the control group, treatment group (P1) exposed to cypermethrin 10 mg/kg BW, treatment group (P2) exposed to cypermethrin 15 mg/kg BW and treatment group (P3) exposed to cypermethrin 20 mg/kg BW. The treatment groups were exposed to cypermethrin for 28 days. A surgery was conducted in proestrus phase and ovarian organs were taken. Bcl-2 expression examination was performed by using immunohistochemical method and the antral follicle count was assessed with the Hematoxillin Eosin (HE)Results: This study showed the decrease of Bcl-2 expression in cypermethrin-exposed group compared to control group. There was a decrease in antral follicles count in cypermethrin-exposed group compared to control group.Conclusion: Cypermethrin can decrease Bcl-2 expression and antral follicles count in Rattus norvegicus ovaries.

The use of juvenile invitro embryo transfer (JIVET) is limited by variation between prepubertal lambs in ovarian response to exogenous gonadotrophins. In cattle, anti-Müllerian hormone (AMH) is a predictive endocrine marker of antral follicle count. In this study we measured plasma AMH concentrations in lambs at 3 and 5 weeks of age and determined associations between AMH concentrations and ovarian response to gonadotrophins and invitro blastocyst production at 6–8 weeks of age in a JIVET program. At 5 weeks, AMH (n=38) was positively correlated with surface antral follicle count (r=0.87, P&lt;0.001), blastocysts produced (r=0.92, P&lt;0.001) and blastocysts produced as a proportion of oocytes collected (r=0.44, P&lt;0.01) or cleaved (r=0.43, P&lt;0.01). Similar associations were observed between AMH at 3 weeks (n=30) and follicle number (r=0.70, P&lt;0.05) and blastocysts produced (r=0.87, P&lt;0.05). Lambs with high (&gt;2.2ngmL−1) compared with medium (0.4–2.2ngmL−1) and low (&lt;0.4ngmL−1) AMH at 5 weeks had more antral follicles (mean (±s.e.m.) 118.7±13.9 vs 68.2±8.1 and 30.4±12.3 respectively; P&lt;0.05) and more blastocysts produced (mean (±s.e.m.) 54.9±6.9 vs 18.9±4.0 and 7.5±6.1 respectively; P&lt;0.05). These results suggest that AMH concentration at 5 weeks of age can be used to select donor lambs which enhance the success of JIVET programs.

Aim: To evaluate the surface detail reproduction of dental stone this is immersed in different disinfectant solution and studied under stereomicroscope. Methodology: Total number of 30 specimens of dental stone (Type III) were made with measurements of 1.5cm diameter and 1cm height .This samples are divided in to 3 groups group A,B,C. were A is immersed in Distilled water which was taken as control group ;B is immersed in 2% Glutaraldehyde and C is immersed in 5%sodium hypochlorite. Each specimen were immersed in the disinfectant solution for 15 minutes and dried under room temperature for 24 hrs. After 24 hrs each specimens are studied under stereomicroscope for surface details. Result: The results showed no significant difference in the surface irregularities and porosities for a group 1 and group 2 except group 3 which showed significant increase in the porosities, surface irregularities and erosions after disinfection with 5% NaHOCl by immersion method. Conclusion: The surface detail reproduction capacity of die stone was adversely affected when 5% Sodium hypochlorite was used as disinfectant solution when compare d to control group and 2% Glutaraldehyde

AbstractFracture resistance of endodontically treated teeth restored with post. Aims: This study aims to compare the fracture resistance of endodontically treated teeth restored with resin fiber and stainless steel post. Commercially available prefabricated resin fiber post(Dentsply Maillefer Easy Post), prefabricated stainless steel post(Coltene/Whaledent Parapost) were used. Methods and Material: Forty five maxillary central incisors were obturated and divided into 3 groups: Control Group (Group I) without any post (n = 15), Resin Fiber Post Group (Group II) (n = 15) and Stainless Steel Post Group (Group III) (n = 15). In all Groups except control group, post space was prepared; a post was cemented, and a core build-up was provided. All the specimens were subjected to compressive force under a universal testing machine until fracture. Statistical analysis used: The results were analyzed using the variable analysis test (ANOVA). Results: One-way analysis of variance revealed significant difference among test groups. The control group demonstrated highest fracture resistance (925.2183 N), followed by the resin fiber post group (486.7265 N) and stainless steel post group (423.539N). Conclusions: Teeth restored with resin fiber post showed higher fracture resistance values than prefabricated stainless steel post.

AbstractHydrosalpinx is a disease characterized by the obstruction of the salpinx, with progressive accumulation in the shape of a fluid-filled sac at the distal part of the tuba uterina, and closed to the ovary. Women with hydrosalpinges have lower implantation and pregnancy rates due to a combination of mechanical and chemical factors thought to disrupt the endometrial environment. Evidence suggests that the presence of hydrosalpinx reduces the rate of pregnancy with assisted reproductive technology. The main aim of the present is review to make an overview of the possible effects of hydrosalpinx on in vitro fertilization (IVF). We conducted a literature search on the PubMed, Ovid MEDLINE, and Google Scholar data bases regarding hydrosalpinx and IVF outcomes. Hydrosalpinx probably has a direct toxic effect on sperm motility and on the embryos. In addition, the increasing liquid inside the salpinges could alter the mechanisms of endometrial receptivity. The window of endometrial receptivity is essential in the implantation of blastocysts, and it triggers multiple reactions arising from the endometrium as well as the blastocysts. Hydrosalpinx could influence the expression of homeobox A10 (HOXA10) gene, which plays an essential role in directing embryonic development and implantation. Salpingectomy restores the endometrial expression of HOXA10; therefore, it may be one mechanism by which tubal removal could result in improved implantation rates in IVF. In addition, salpingectomy does not affect the ovarian response, nor reduces the antral follicle count. Further studies are needed to establish the therapeutic value of fluid aspiration under ultrasonographic guidance, during or after oocyte retrieval, in terms of pregnancy rate and ongoing pregnancy.

ABSTRACT Objective:  To evaluate the proliferation and morphology of human osteoblasts cultured on two brands of mini-implants after 24, 48, and 72 hours, in addition to the chemical composition found on their surface. Materials and Methods:  Two brands of mini-implant (Morelli and Neodent) were evaluated; polystyrene was used as a control group (n  =  3). Osteoblasts were cultured on the surface of sterilized mini-implants in a CO2 incubator at different time periods (24, 48, and 72 hours). Osteoblast proliferation was quantified by scanning electron microscopy using up to 5000× magnification, and cell morphology was analyzed by a single observer. For the chemical analysis, spectroscopy X-ray fluorescence was used to identify and quantify chemical components on the surface of the mini-implants. Results:  Two-way ANOVA showed no significant interaction between the factors studied (P  =  0.686). A Tukey test revealed no significant difference in osteoblast proliferation between the mini-implants at all studied periods; however, a difference in cell proliferation was detected between the Neodent and the control group (P  =  .025). For all groups, time had a direct and positive effect on osteoblast proliferation (P &lt; .001). The significant elements present in both brands of mini-implants were titanium, aluminum, vanadium, and iron. Conclusions:  Osteoblast proliferation was present on the mini-implants studied, which increased over time; however, no significant difference between brands was observed. No difference was seen between the mini-implants evaluated in terms of chemical composition. Cell adhesion after 72 hours suggests that areas of bone remodeling can be achieved, thus initiating the process of mini-implant anchorage.
