Extreme professional working conditions of EMERCOM rescuers have a harmful effect to their health leading to high rate of digestive, respiratory, musculoskeletal and cardiovascular diseases. The morbidity frequency of these diseases at rescuers is higher, beyond all doubts, than at male population of the corresponding age, and it is connected with total years of occupational work and its intensity. This fact shows the necessity of establishing the system of medico-physiological support to rescuers of EMERCOM of Russia which must be seen as a significant part of their social protection and rehabilitation and must include six main components, i.e. consideration of conditions and character of activity in everyday and emergency situations; complex evaluation and monitoring of their health state; medical support during rescue work in emergency situations; psychological support; information and analytical maintenance of longstanding monitoring; development of hospital base and specialized medical assistance. An important component of the system of medico-physiological support to rescuers of EMERCOM of Russia is information and analytical maintenance of longstanding monitoring of professional load, health state and medico-social protection of the rescuers of EMERCOM of Russia. It was implemented by creating a Medical register of EMERCOM of Russia. We developed concepts, principals and organization methodical ground of multilevel system of medico-physiological support to rescuers of EMERCOM of Russia regarding their work in conditions of emergency situations. On the basis of professional work load studies, hygienic evaluation of work hardness and intensity, analysis of rescue work of State fire fighting services divisions of EMERCOM of Russia there were detected main stress factors, physiological and hygienic distinctive features of work.

Relevance. Professional activity of firefighters and rescuers refers to dangerous, stressful and heavy types of work associated with significant risk to life and health. Injuries, disability and mortality among the personnel of the Ministry of emergency situations directly depend on working conditions, occupational activities, the nature of the functions performed.Intention. To demonstrate effects of software products in the field of fire safety on meeting the information needs of managers and employees performing the tasks of prevention, extinguishing and elimination of consequences of fires and emergencies. Timely prevention and mitigation of fire and emergency consequences reduces material damage as well as the number of dead and injured people.Methodology. The article presents a brief description of the software products available in the Fund of computerized algorithms and programs of EMERCOM of Russia in the field of fire safety, which are designed to help in solving social and psychological problems of safety of the population and territories in emergency situations. The software products are designed to provide the heads of the subjects of the Russian Federation with the possibility of automating the planning of measures for the prevention and elimination of fires and emergencies. Software products contain information and references on prevention and elimination of fires and emergencies as well as guidance on the actions of officials and the population in emergency situations, both natural and man-made, and also regulatory and methodological documents for making decisions in the field of safety of the population and territories in emergency situations.Results and Discussion. Software products actually increase effectiveness of safety measures in emergency situations, preparedness of units of he State fire service, as well as help to make informed decisions in the field of social protection of employees. Based on one of the software products, the possibility of automating the presentation and analysis of statistical data on the number of fires, the number of fire fatalities, injuries and related direct material damage is shown.Conclusion. The use of software products in the field of fire safety ultimately reduces the risk of injury and death, the level of material losses from fires and other emergencies.

The specifics of professional activity of the employees of the State Penitentiary Service of Ukraine (SPS of Ukraine) have been revealed. It has been noted that specific features of professional activity of the employees of the State Penitentiary Service of Ukraine are due to: specific conditions of official activity; shortcomings in the organization of official activities; the presence of risk; constant communication with convicts; the occurrence of negative personal changes of employees; the presence of conflict interaction between the various services of penitentiary institutions. The extreme situations that arise in this activity and psychological consequences of the activity of the employees of the State Penitentiary Service of Ukraine in special conditions have been characterized. The author has distinguished the following types of extreme situations: transitive; ongoing; with the element of uncertainty; those that need readiness for extreme actions; those that simultaneously include unexpectedness and lack of time; with the receipt of false information; dominant state; critical situations. The stress-factors affecting the employees of the State Penitentiary Service of Ukraine in extreme situations of professional activity have been provided. Psychological consequences of the activity of the employees of the State Penitentiary Service of Ukraine in extreme conditions of professional activity include: the state of anxiety associated with manifestations of psycho-somatic illness; the state of fatigue that is manifested in the impossibility of performing professional activity; cases of “shock”, the emergence of which is associated with emergency situations; asocial behavior, which is manifested in the inflexibility or, vice versa, in the employee’s oppression. The directions of professional and psychological training of the employees of the State Penitentiary Service of Ukraine have been substantiated. The development and implementation of an integral program of psychological support for the development of the ability to cope with stress and self-regulation among the employees of the State Penitentiary Service of Ukraine has been offered.

In recent years, the surprising progress made in other areas of hematology (advances in the understanding of leukemogenesis, improved transplant techniques) has been conspicuously absent in the management of hematologic emergencies. And yet, every step toward greater knowledge, every new treatment option will be of little value unless we are able to manage the acute complications of hematologic diseases. These complications are better defined as hematologic emergencies, and they are characterized by a high rate of mortality. This review is based on a search of the literature that was initially confined to articles published in the journal Hematology from 2000 to 2009. The search was then extended to the Cochrane Library and to Pub Med in February 2010 with the following Keywords emergencies; urgencies; hematology. The same key words were employed in a search of the archives of Blood and the New England Journal of Medicine from 2000 to 2010. The results confirm that hematologic emergencies can be caused by hematologic malignancies as well as by non-neoplastic hematologic diseases. Within the former category; this review examines the causes; manifestations; treatment and prevention of disseminated intravascular coagulation; superior vena caval syndrome; spinal cord compression; tumor lysis syndrome; hyperleukocytosis; and hypercalcemia. We also review emergency situations associated with non-neoplatic haematological diseases; such as thrombotic thrombocytopenic purpura; drug-induced hemolytic anemia; and acute sickle-cell crisis.

 The problem of premature professional burnout among employees, including firefighters is of scientific and practical significance. The article describes the nature and types of professional stress, as well as activities of the Training Center of the Federal Fire Service in Chelyabinsk Region. It aims to identify guidelines in the normative labor functions of rescuers of the Ministry of Emergency Situations of Russia and firefighters for training programs to prepare fire service employees for stressful activities. To achieve this goal, labor functions of rescuers of the Ministry of Emergency Situations of Russia and firefighters were analyzed; training programs developed by the Training Center of the Federal Fire Service in Chelyabinsk region were assessed. The methods of comparative analysis and conversation with experts were used to identify pedagogical aspects of labor functions of fire service employees associated with their willingness to work under stress. The article established a relationship between the training programs and the development of readiness to perform labor functions under stress. The condition required for the training program to be efficient was determined. It involves attracting the psychological service of the Ministry of Emergency Situations to hold methodical meetings and seminars. The positive result of this training is an adequate response to stress factors. The research results may be used by teachers of training centers and universities and methodologists.

Abstract Background A number of refugees and asylum seekers have increased dramatically in recent years. More than 250 million people worldwide are considered as refugees (United Nations High Commission for Refugees, 2018), among whom more than 50% are children. General health and especially psychological health of the refugee and asylum seeking children are an emerging, however, not well explored issues. In this study, we aimed to review the literature on the psychological health of refugee children. Methods We performed a literature search and descriptive analysis of studies published until July 2019, through MEDLINE and Science Direct databases. We identified literature on psychological health state of refugee and asylum seeking children. We analysed results of 16 studies. Results The study found that refugee children are facing severe psychological health issues, such as Post-traumatic stress disorder (PTSD), depression, anxiety, sleep disorders, behavioural problems. We identified 3 phases of psychological trauma and stress among refugee children: stress due to conflict in their home countries, stress during migration, and upon the arrival to host country. Our analysis reviled integration difficulties such as racism and bulling as important stress factors for the refugee children. Studies found that prevalence of PTSD is more than 54% among refugee children residing in Norway, significantly higher than in reference population. The studies identified the need of regular mental health assessment and preventive care, psychological counselling services for the refugee children. All included studies showed significantly higher stress among refugee children when compared to local children in host countries. Conclusions Rehabilitation services and follow-up supportive programs should be implemented to improve the mental health status of refugee children; these interventions will also contribute to their better integration. Key messages Screening and regular monitoring provided by host countries healthcare system is crucial to identify high risk children. More research is needed to better investigate the psychological health state and needs of refugee children.

Background: Pediatric septic arthritis of the hip is a surgical emergency. Facing a hip flexum and fever, surgeons can use comparative ultrasound to confirm the diagnosis and to establish the surgical indication. While needle aspiration-irrigation is a simple and quick method, it gives rise to a high rate of recurrence. The purpose of our study was to demonstrate that hip arthroscopy by extracapsular approach is an effective and accessible alternative applicable in emergency situations. Methods: This is a report of 2 cases (7 years-135 cm; 13 years-143 cm) of needle aspiration-irrigation failure subsequently treated by hip arthroscopy. The X-ray and arthroscopy-assisted extracapsular technique was performed on a conventional orthopedic table, using a 30 degree arthroscope and a 3.5mm diameter motorized knife. Minimally invasive anterior longitudinal capsulotomy was performed by 2 anterolateral approaches via the proximal and distal tensor Fasciae Latae muscle, without dedicated instrumentation, followed by sampling, irrigation, removal of false membranes, synovial biopsies, partial synovectomy, cartilage assessment of the central then peripheral compartment and placement of a drainage. Results: In both cases, effusion completely disappeared and the inflammatory process gradually normalized with antibiotic therapy without any repeat procedure or complications related to the approach. Weight bearing was resumed in 3 weeks. Postoperatively, in 6 months and 15 months respectively, after resumption of sports activities hip examination was painless and no damage was noted on x-rays in either case. Conclusion: In our experience, hip arthroscopy by extracapsular approach can effectively address cases of failed needle aspiration-irrigation in pediatric septic arthritis of the hip. It could be provided as first-line emergency treatment in the event of purulent septic arthritis or late management, as a replacement for arthrotomy.

The authors describe the examples of possible offences of the law by temporary detention facilities officers, security and convoy officers, and other internal affairs officers responsible for criminal suspects convoying. Such offences are often connected with the disregard of rules of official duty performance or with the improper execution of duties, which leads to the occurrence of emergency situations, such as: an assault on a police officer by an individual or a group of individuals; hostage taking; escape or self-maiming attempts. Based on personal experience and judicial practice materials and regulating documents analysis, the authors consider the key measures aimed at the reduction of the emergency situations rate in the work of temporary detention facilities and security and convoy units. The professional activity of temporary detention facilities officers, as well as security and convoy officers, involves everyday contacts with  suspects, persons accused of crimes, and persons sentenced to imprisonment. Officers convoy them to courts, medical facilities, etc. Such duties become routine for experienced officers, which can lead to the loss of vigilance and cautiousness in the work with criminal suspects. On the other hand, inexperienced officers, not knowing the peculiarities of their service, can also commit dereliction of duty, or worse - neglect of their official duties. Therefore, the issues of control and preventive measures taken by senior officers aimed at avoiding offences of the law by their subordinates, become particularly significant.

The article is devoted to the partnership of social workers and ombudsmen as an influential tool for the citizens' rights protection. The common aspects of professional activity of social workers and ombudsmen in the citizens' rights protection of Ukraine and European countries are analyzed. Analyzed the annual reports of the National Ombudsman, clarified the dynamics of the receipt of citizens' reports about violations of their rights. There is a growing number of human rights violations in the field of social protection. The ombudsman has the right to make recommendations to the central executive bodies, which are ripe after the monitoring visits. Through the activities of social workers and the ombudsman, the state assumes the main functions of ensuring human rights in the social sphere. Foreign experience on the significant role of «natural allies» (social workers and ombudsmen) in drafting a formal protocol of strategic and tactical cooperation to optimize the residents' interests based on a friendly advisory approach have been highlighted. Most often, social workers and ombudsmen are forced to act as intermediaries between people and the state or other bodies to uphold justice and provide protection in cases where measures taken by the state in the interests of society as a whole threaten the rights and freedoms of individuals or groups. There is a need to intensify cooperation between the ombudsman's offices, social protection departments and social service providers (for example, through the joint Commissions on the quality of social services establishment, joint investigations into unsatisfactory social or medical care complaints). It seems relevant to expand the positions of specialized ombudsmen (for social protection (security), military ombudsman) as independent officials in communities (municipalities) for Ukraine.

Purpose. Studying the level of development of certain professionally significant psychological qualities of future specialists in extreme professions, such as volitional self-regulation, propensity to take risks, motivation to achieve success or overcome failure. Methods. Carrying out an ascertaining experiment: psychological testing using the techniques: «Motivation of success and fear of failure» (questionnaire by A.A. Rean), «Research of the propensity to take risks» (questionnaire of A.G. Shmelev), «Level of volitional self-regulation» (questionnaire A.V. Zverkov and E.V. Eydman). Findings. The results of studying the level of development of volitional self-regulation, self-control and perseverance, motivational orientation and propensity to risk of the 1st and 3rd year students of the University of Civil Protection of the Ministry of Emergency Situations of Belarus are presented. The conditions of professional activity of specialists in extreme professions are briefly considered. Strong-willed efforts for workers in extreme activities are necessary to overcome such external obstacles as difficulties in professional activities, various hindrances, confronting with other people or life circumstances. It can be concluded that the majority of respondents are characterized by a high level of volitional self-regulation, perseverance and self-control. The overwhelming majority of the first and third year students were diagnosed with pronounced motivation to achieve success. It has been found that almost all respondents participating in the study had an average level of propensity to take risks. Application field of research. The presented results can be used to work out a program for the development of professionally important psychological qualities, skills and abilities of extreme profile specialists aimed at forecasting the main conditions and regularities of their formation.
