Non-traditional scheme of HF-injection has been used in T-10 tokamak for ECRH at the 2nd harmonic of ECR in X-mode. Two HF-launcher systems, with focusing beams, injected power in opposite directions under toroidal angles +/-20˚. Input power from each launcher was about the same (Phf =0.8 ÷ 0.85MWt). Absorbed energy was deposed in central area of plasma column. The new phenomenon has been found in some shots with W-limiter and Li-coating in the regime mentioned above. The spontaneous rise of the electron density nearly in the all plasma column occurs simultaneously with the rise of Te in the wide region (0<r/a<0.8) at the steady-state of ECRH. The value of the energy confinement time abruptly rises by ~15%. The absolute value of the electron heat and density fluxes reduces abruptly in the whole plasma column similarly to that of at non-local (or global) L-H transitions found earlier in JET and JT-60U. The accumulation of tungsten and light impurities is absent. In a series of shots, without L-H transitions, measurements of X-ray spectra (PHA data) showed a significant difference in comparison with the ECCD co-injection by two gyrotrons.

BACKGROUND: Microsurgical flaps are widely used to treat complex traumatic wounds of upper and lower limbs. Few studies have evaluated whether the vascular changes in preoperative computed tomography angiography (CTA) influence the selection of recipient vessel and type of anastomosis and the microsurgical flaps outcomes including complications. OBJECTIVE: The aim of this study was to evaluate if preoperative CTA reduces the occurrence of major complications (revision of the anastomosis, partial or total flap failure, and amputation) of the flaps in upper and lower limb trauma, and to describe and analyze the vascular lesions of the group with CTA and its relationship with complications. METHODS: A retrospective cohort study was undertaken with all 121 consecutive patients submitted to microsurgical flaps for traumatic lower and upper limb, from 2014 to 2020. Patients were divided into two groups: patients with preoperative CTA (CTA+) and patients not submitted to CTA (CTA–). The presence of postoperative complications was assessed and, within CTA+, we also analyzed the number of patent arteries on CTA and described the arterial lesions. RESULTS: Of the 121 flaps evaluated (84 in the lower limb and 37 in the upper limb), 64 patients underwent preoperative CTA. In the CTA+ group, 56% of patients with free flaps for lower limb had complete occlusion of one artery. CTA+ patients had a higher rate of complications (p = 0.031), which may represent a selection bias as the most complex limb injuries and may have CTA indicated more frequently. The highest rate of complications was observed in chronic cases (p = 0.034). There was no statistically significant difference in complications in patients with preoperative vascular injury or the number of patent arteries. CONCLUSIONS: CTA should not be performed routinely, however, CTA may help in surgical planning, especially in complex cases of high-energy and chronic cases, since it provides information on the best recipient artery and the adequate level to perform the microanastomosis, outside the lesion area.

Abstract Number of pigs born alive has been a key factor of the increasing efficiency of the U.S. swine industry. However, with increased pigs in the uterus, birth weight has been negatively impacted, with more small or at-risk pigs being born per litter. In order to overcome these changes, a study testing three commercial oral drenches against a control to determine which would increase average daily gain and decrease preweaning mortality. In a completely randomized design, 877 one-day-old suckling pigs from a high-health farm were selected for the experiment if they appeared to be in the bottom 20% of bodyweight compared to their contemporaries. Selected pigs were given one of four drenching treatments: 1) none (control), 2) bioactive proteins (BP), 3) high energy sugars (HES), and 4) immunoglobulins (IgY). Pigs were weighed on d 1 and d 19 of age (weaning), with mortality tracked during the suckling period. Data were analyzed using SAS v 9.4 (Cary, NC), with pig as the experimental unit and an accepted alpha of 0.05. Treatment had no detected effect on birth weight, weaning weight, ADG, or mortality (P = 0.79, 0.96, 0.86, 0.38 respectively). Likewise, statistical contrasts were used to determine there was no detected impact (P &gt; 0.10) of drench, regardless of type, compared to the control in any measured response criteria. Interesting, pigs drenched with BP or IgY had numerically lower preweaning mortality (11.2 and 11.5% respectively), than those administered the control or HES (15.4 and 15.2%, respectively). In conclusion, this experiment showed no significant difference in the performance between piglets given no product vs. those drenched with bioactive proteins, high energy sugars, or immunoglobulins. However, additional research is warranted with greater replication or disease stressors to better understand if oral drenches may improve preweaning performance or mortality in different situations.

Environmental issues make the quest for better and cleaner energy sources a priority. Worldwide, researchers and companies are continuously working on this matter, taking one of two approaches: either finding new energy sources or improving the efficiency of existing ones. Hydrogen is a well-known energy carrier due to its high energy content, but a somewhat elusive one for being a gas with low molecular weight. This review examines the current electrolysis processes for obtaining hydrogen, with an emphasis on alkaline water electrolysis. This process is far from being new, but research shows that there is still plenty of room for improvement. The efficiency of an electrolyzer mainly relates to the overpotential and resistances in the cell. This work shows that the path to better electrolyzer efficiency is through the optimization of the cell components and operating conditions. Following a brief introduction to the thermodynamics and kinetics of water electrolysis, the most recent developments on several parameters (e.g., electrocatalysts, electrolyte composition, separator, interelectrode distance) are highlighted.

Background and objectives: For the last three decades, non-operative management (NOM) has been the standard in the treatment of clinically stable patients with blunt spleen injury, with a success rate of up to 95%. However, there are no prospective issues in the literature dealing with the incidence and type of splenic complications after NOM. Materials and methods: This study analyzed 76 pediatric patients, up to the age of 18, with blunt splenic injury who were treated non-operatively. All patients were included in a posttraumatic follow-up protocol with ultrasound examinations 4 and 12 weeks after injury. Results: The mean age of the children was 9.58 ± 3.97 years (range 1.98 to 17.75 years), with no statistically significant difference between the genders. The severity of the injury was determined according to the American Association for Surgery of Trauma (AAST) classification: 7 patients had grade I injuries (89.21%), 21 patients had grade II injuries (27.63%), 33 patients had grade III injuries (43.42%), and 15 patients had grade IV injuries (19.73%). The majority of the injuries were so-called high-energy ones, which were recorded in 45 patients (59.21%). According to a previously created posttraumatic follow-up protocol, complications were detected in 16 patients (21.05%). Hematomas had the highest incidence and were detected in 11 patients (14.47%), while pseudocysts were detected in 3 (3.94%), and a splenic abscess and pseudoaneurysm were detected in 1 patient (1.31%), respectively. The complications were in a direct correlation with injury grade: seven occurred in patients with grade IV injuries (9.21%), five occurred in children with grade III injuries (6.57%), three occurred in patients with grade II injuries (3.94%), and one occurred in a patient with a grade I injury (1.31%). Conclusion: Based on the severity of the spleen injury, it is difficult to predict the further course of developing complications, but complications are more common in high-grade injuries. The implementation of a follow-up ultrasound protocol is mandatory in all patients with NOM of spleen injuries for the early detection of potentially dangerous and fatal complications.

Introduction: Frailty in elderly trauma populations has been correlated with an increased risk of morbidity and mortality. The Score for Trauma Triage in the Geriatric and Middle-Aged (STTGMA) is a validated mortality risk score that evaluates 4 major physiologic criteria: age, comorbidities, vital signs, and anatomic injuries. The aim of this study was to investigate whether the addition of additional frailty variables to the STTGMA tool would improve risk stratification of a middle-aged and elderly trauma population. Methods: A total of 1486 patients aged 55 years and older who met the American College of Surgeons Tier 1 to 3 criteria and/or who had orthopedic or neurosurgical traumatic consultations in the emergency department between September 2014 and September 2016 were included. The STTGMAORIGINAL and STTGMAFRAILTY scores were calculated. Additional “frailty variables” included preinjury assistive device use (disability), independent ambulatory status (functional independence), and albumin level (nutrition). The ability of the STTGMAORIGINAL and the STTGMAFRAILTY models to predict inpatient mortality was compared using area under the receiver operating characteristic curves (AUROCs). Results: There were 23 high-energy inpatient mortalities (4.7%) and 20 low-energy inpatient mortalities (2.0%). When the STTGMAORIGINAL model was used, the AUROC in the high-energy and low-energy cohorts was 0.926 and 0.896, respectively. The AUROC for STTGMAFRAILTY for the high-energy and low-energy cohorts was 0.905 and 0.937, respectively. There was no significant difference in predictive capacity for inpatient mortality between STTGMAORIGINAL and STTGMAFRAILTY for both the high-energy and low-energy cohorts. Conclusion: The original STTGMA tool accounts for important frailty factors including cognition and general health status. These variables combined with other major physiologic variables such as age and anatomic injuries appear to be sufficient to adequately and accurately quantify inpatient mortality risk. The addition of other common frailty factors that account for does not enhance the STTGMA tool’s predictive capabilities.

AbstractTo carry out autothermal drying processes during the composting of biomass, a horizontal tubular reactor was designed and tested. A biodrying tunnel of the total capacity of 240 dm3 was made of plastic material and insulated with polyurethane foam to prevent heat losses. Municipal solid waste and structural plant material were used as the input substrate. As a result of autothermal drying processes, moisture content decreased by 50 % of the initial moisture content of organic waste of about 800 g kg−1. In the tested cycles, high temperatures of biodried waste mass were achieved (54–56°C). An appropriate quantity of air was supplied to maintain a satisfactory level of temperature and moisture removal in the biodried mass and high energy content in the final product. The heat of combustion of dried waste and its calorific value were determined in a calorimeter. Examinations of pyrolysis and gasification of dried waste confirmed their usefulness as biofuel of satisfactory energy content.

Background and Aim. The prevalence of childhood asthma varies among different nations and appears to have increased in recent years. It has been difficult to determine this prevalence precisely and compare the communities due to the lack of a standardized method. The International Studies of Asthma and Allergies in Childhood (ISAAC) has developed a written and video questionnaire to measure asthma prevalence and overcome these difficulties. This study was carried out to determine the prevalence of childhood asthma in Kashan, the central area of Iran, and to assess the agreement between the ISAAC written and video questionnaires and its gender relationship. Methods. Between December 2002 and June 2003, 2533 school children, aged 13-14 years, in Kashan secondary schools completed sequentially the ISAAC written and video questionnaires. The agreement between responses to the two questionnaires for reported “ever wheeze”, “exercise wheeze”, “night wheeze” and “night cough” was determined, using kappa coefficient and concordance. Results. The prevalence of asthma symptoms were significantly lower in the responses to the video questionnaire compared with the written questionnaire. Kappa coefficient showed only poor to fair agreement (κ = 0.13- 0.24) between video and written questionnaires, although the concordance between them always exceeded 60%. Conclusions. This study showed that reported asthma symptoms based on video questionnaires were significantly lower than the written questionnaire, and that there is low agreement between the two questionnaires which requires further investigation to explain the findings. In addition, this investigation revealed no significant difference between the genders for this low agreement.

Abstract Net energy efficiencies were calculated from data of an experimental herd with respect to type of cow, lactation number, stage of lactation and diet. The trial consisted of 71 Holstein-Friesians, 71 Jerseys and 71 Holstein-Jersey F1-crosses in 1st, 2nd and > 2nd lactation. Data were collected during 210 days of lactation, from calving to week 30 and included total dry matter intake, energy content of foods, milk yield, milk solids, body weight, body condition scores and several body measurements. The cows were divided into four feeding groups : high and low energy content of roughage as well as high and low proportion of concentrates. Net energy efficiency was calculated as the ratio of milk energy to total net energy intake after subtracting energy utilized for maintenance and body tissue change. Body tissue change was calculated either via body-weight changes or body condition-score changes. Due to the distribution of the efficiencies being skewed, efficiencies were transformed applying a Box-Cox transformation. Transformed net energy efficiencies were analysed using a repeated measurements design considering the sequential nature of the observations. Mixed models with a compound symmetry structure for the variance components were applied. Final models contained the fixed effects of type, lactation number, feeding group and the covariates week of lactation and its square. Holstein-Jersey crosses were more efficient than purebreds, second lactation cows were least efficient, cows given low energy roughage and a lower proportion of concentrates were more efficient than cows on the other diets. Least efficient were the cows belonging to the high energy roughage and higher proportion of concentrates group. The coefficients of determination of the final models were between 0·357 and 0·492.

Abstract Hydrogen is one of the important non-conventional energy sources because of its high energy content and non-polluting nature of combustions. The water splitting reaction is one of the significant methods for hydrogen generation from non-fossil feeds. In the present paper, the hydrogen generation has been experimentally investigated with water splitting reaction using metal aluminum in presence of potassium hydroxide as an activator under flow conditions. The rate of hydrogen generation was reported in the annular micro- reactor of 1 mm annulus using various flow rates of aqueous 0.5 N KOH ranging from 1 ml/min to 10 ml/min. The complete conversion of aluminum was observed at all the flow rates of aqueous KOH. The hydrogen generation rate was observed to depend on the flow rate of liquid reactant flowing through the reactor. At 1 ml/min of 0.5 N KOH, hydrogen generates at an average rate of 3.36 ml/min which increases to 10.70 ml/min at 10 ml/min of aqueous KOH. The Shrinking Core Model was modified for predicting the controlling mechanism. The rate of hydrogen generation was observed to follow different controlling mechanisms on various time intervals at low flow rates of aqueous KOH. It was observed that chemical reaction controls the overall rate of hydrogen generation at higher flow rates of aqueous KOH.
