Abstract Introduction: This study was conducted to evaluate clinical and radiographic findings induced by revascularization with and without platelet-rich plasma (PRP). Methods: Twenty patients with nonvital, immature anterior teeth were randomly categorized into 2 groups. Subsequent to minimal instrumentation of the root canal, disinfection was achieved with ledermix paste and followed by revascularization with and without PRP in groups 1 and 2, respectively. The cases were followed up clinically and radiographically at 6- and 12-month intervals. Results: Clinically, all cases were asymptomatic with complete resolution of signs and symptoms (except four cases which were considered as dropout). Radiographically, there was no significant difference in periapical healing, apical closure, and dentinal wall thickening in group I in comparison with group II. However, root lengthening was comparable for both of the procedures. Conclusions: Revascularization is a conservative and an effective method for inducing maturogenesis in nonvital, immature teeth. Supplementations with PRP can potentially improve the desired biological outcome of this regenerative procedure.

Objectives: Biopure MTAD™, a new root canal irrigant has shown promising results against the most common resistant microorganism, E. faecalis, in permanent teeth. However, there is lack of studies comparing its antimicrobial effectiveness with NaOCl in primary teeth. The purpose of this study was to compare the in vivo antimicrobial efficacy of NaOCl 2.5% and Biopure MTAD™ against E. faecalis in primary teeth. Study design: Forty non vital single rooted primary maxillary anterior teeth of children aged 4-8 years, were irrigated either with NaOCl 2.5% (n=15), Biopure MTAD™ (n=15) and 0.9% Saline (n=10, control group). Paper point samples were collected at baseline (S1) and after chemomechanical preparation (S2) during the pulpectomy procedure. The presence of E. faecalis in S1 & S2 was evaluated using Real time Polymerase Chain Reaction. Results: Statistical significant difference was found in the antimicrobial efficacy of NaOCl 2.5 % and BioPure MTAD™ when compared to saline (p>0.05). However, no statistical significant difference was found between the efficacies of both the irrigants. Conclusions: NaOCl 2.5% and BioPure MTAD™, both irrigants are equally efficient against E. faecalis in necrotic primary anterior teeth. MTAD is a promising irrigant, however clinical studies are required to establish it as ideal root canal irrigant in clinical practice.

Regenerative endodontics has introduced numerous procedures such as pulp implantation, revascularization, and postnatal stem cell therapy. Revascularization has been successfully implemented clinically nowadays, thus providing dentists with outrageous results. Platelet-rich fibrin (PRF) used either alone or along with bone graft promotes bone growth and vascularization. This matrix promotes migration, cell attachment, and proliferation of osteoblast that leads to bone formation. PRF consists of a packed fibrin complex consisting of leukocytes, cytokines, and glycoproteins such as thrombospondin. The usage of PRF has reported high success rates in surgical cases such as sinus lift procedures, healing of extraction sockets, and management of periapical abscesses. Compared to platelet-rich plasma, PRF is more economical, easy to prepare, and feasible to use in daily clinical practices. Revascularization compromised the induction of a blood clot into the root canal space, which emerged as a clinical triumph. This further led to platelet concentrates as an autologous scaffold on which revascularization could occur. The applications of PRF in regenerative endodontics are numerous, such as an agent for repairing iatrogenic perforation of the pulpal floor and for the revascularization of immature permanent teeth with necrotic pulps. It acts as a matrix for tissue ingrowth. Evidence of progressive thickening of dentinal walls, root lengthening, regression in the periapical lesion, and apical closure was reported. Further studies are needed to clarify the precise mechanism of action of PRF for dental pulp regeneration both in vitro and in vivo. The current review aims at the present uses of PRF in regenerative endodontics dentistry and its application with future recommendations and limitations.

Abstract Background: Accurate working length (WL) determination is necessary in achieving optimal healing by non-surgical root canal therapy in teeth with inflammatory apical root resorption. Electronic apex locators (EALs) are one of the mainstays in determination of WL of teeth. Aim: This study evaluated the accuracy of three EALs [RootZX (third generation), iPex (fourth generation) and Raypex 6 (modification of a fifth generation)] in determining the WL of teeth with simulated apical root resorption in permanent teeth. Materials and methods: Forty freshly extracted maxillary anterior teeth were collected and a 45° oblique cut was made at the root apex with a disc to simulate apical root resorption. Actual working length (AWL) was determined by direct visual method and was used as a control. Electronic working length (EWL) values were measured by three different apex locators that are RootZX (Group I), iPex (Group II) and Raypex 6 (Group III) at apex, 0.5 mm and 1 mm from apex. All values obtained were tabulated and statistical evaluation was carried out. Results: At apex, EWL obtained using iPex (p=0.05) showed a statistically significant difference from AWL. At 0.5 mm and 1.0 mm tolerance, iPex showed non-acceptability for WL measurement in 67.5% and 17.5% of samples compared to Root ZX (12.5% and 2.5%) and Raypex (7.5% and none) respectively. Conclusion: Within the limitation of this study, it can be concluded that Raypex 6 and RootZX show statistically significant accuracy in WL measurement compared with iPex in teeth with apical root resorption.

Management of non-vital immature anterior permanent teeth in children remains a challenge in paediatric dentistry and endodontics. Once the tooth becomes non-vital, root development ceases, rendering the tooth weak, and unable to withstand physiological forces of mastication. This results in a high rate of root fracture with poor prognosis in the medium to long term. Despite being endodontically treated, over 50 % of such teeth will be lost in the first 10 years following trauma. Traditional endodontic treatment does not contribute to any quantitative or qualitative increase in root dimensions resulting in life-long oral disability. The advancement in the science of Endodontics - Regenerative Endodontics has revolutionized current treatment strategies in treating the immature non-vital permanent tooth which has overcome the drawbacks of traditional treatment approaches. This report provides an insight view on successful maturogenesis of immature non-vital permanent incisors by induction of intracanal bleeding along with complications endured during the process. Regenerative endodontics is defined as “biologically based procedures designed to replace damaged structures, including dentin and root structures, as well as cells of the pulp - dentin complex”.1 Continued root development and reestablishment of pulp - dentine complex can be achieved by these procedures. Two major concepts in the regenerative endodontics are guided tissue regeneration (GTR) and tissue engineering. The base of these procedures is stem cells which can differentiate into the desired tissue component, growth factors or other tissue - inducing mediators and scaffold.2 Regenerative endodontic procedure (REP) includes induction of intracanal bleeding, placement of platelet rich fibrin (PRF), and platelet rich plasma (PRP). The goals of these procedures were elimination of signs and symptoms, continued root development, apical healing, and positive response to vitality testing. But preparation of PRP and PRF requires withdrawal of blood and sight of syringe can add emotional stress to young patients. Revascularization may be defined as the invagination of undifferentiated periodontal cells from the apical region in immature teeth.3,4 The nature of tissue formed after revascularization is not certain and can be confirmed by histological evaluation. Presence of blood supply is reliable. Revascularization considers only one facet - nature of newly formed tissue thus making term revascularization inaccurate and thus many authors disagreed with this term.5

Background: Autologous platelet concentrates such as platelet-rich plasma (PRP) and platelet-rich fibrin (PRF) have gained overwhelming popularity in regenerative endodontics. Clinical evidence reveals the lack of a particular advantage of using PRP or PRF over an evoked blood clot in promoting canal wall thickening and/or continued root development in immature necrotic teeth. Moreover, despite stimulating tissue repair and repopulating the root canals of immature and mature permanent teeth, the new vital tissue may not possess the functional activity of the native pulp tissue. Methods: To better understand the origin, nature, and long-term fate of the tissue types found within the pulp space, we critically examine all available histo-/morphological evidence for pulp–dentine complex regeneration using PRP and/or PRF, alone or together with an evoked blood clot, specialised or unspecialised primary cells, and other biomaterials. Results: Histological data from clinical studies is scant. Reportedly, the inner dentinal surface supports cementum-like tissue formation, but this interface likely deviates in structure and function from the native cementodentinal junction. Presence of bone-like tissue within the pulp space is intriguing since de novo osteogenesis requires closely coordinated recruitment and differentiation of osteoprogenitor cells. Compared to untreated necrotic teeth, an evoked blood clot (with/without PRF) improves fracture resistance. Tooth regeneration using PRF and dental bud cells is unreliable and the constituent neoformed tissues are poorly organised. Conclusion: PRP/PRF fail to demonstrate a significant advantage over an induced blood clot, alone. The true nature of neoformed tissues remains poorly characterised while their response to subsequent insult/injury is unexplored.

Alfa mangostin memiliki berbagai macam bioaktivitas dan merupakan major compound dalam eksrak kulit manggis (Garcinia mangostana L.), alfa mangostin memiliki aktivitas sebagai antioksidan, antiinflamasi dan antibakteri sehingga berperan dalam proses penyembuhan luka. Penelitian ini bertujuan untuk mengamati aktivitas ekstrak pericarp kulit buah manggis terpurifikasi yang mengandung > 90% alfa-mangostin dalam penyembuhan luka eksisi pada mencit putih jantan secara in vivo. Paramater yang diukur adalah persentase penyembuhan luka eksisi, waktu epitelisasi dan kerapatan serabut kolagen. Mencit dibagi menjadi dua kelompok, dimana kelompok I merupakan kelompok kontrol negatif (-) yang hanya diberi sediaan suspensi Na CMC 1%, sedangkan kelompok II merupakan kelompok perlakuan yang diberi sediaan suspensi ekstrak dengan konsentrasi 1%. Pada hari ke-5, ke-10 dan ke-15 diukur persentase penyembuhan luka dan diamati waktu epitelisasi serta kerapatan serabut kolagen. Hasil penelitian dari persentase penyembuhan luka dan waktu epitelisasi yang dianalisa dengan uji General Linear Model Repeated Measures memberikan perbedaan secara nyata (p<0,05), sedangkan untuk pengamatan serabut kolagen menunjukkan tidak ada perbedaan secara nyata (p>0,05) dan untuk waktu epitelisasi yang diuji menggunakan uji T Independent Sample memberikan perbedaan secara nyata (p<0,05), maka dapat disimpulkan bahwa ekstrak terpurifikasi dapat memberikan efek dalam penyembuhan luka eksisi pada mencit.   Alfa mangosteen is a major compound of mangosteen extract cortex and  hasvarious bioactivities, such as anti-oxidant, anti-inflammatory and anti-bacterial so that can be used as wound healing. This research aimed to evaluate the activity of Purified Mangosteen Pericarp Fruit Cortex which contained alfa mangosteen > 90% as excision wound healing  agent in male mice by in vivo study. The parameters observed were Excision wound healing, epithelialization time and collagen fiber density. Mice were divided into two groups, group I was the negative control group (-) which was only received 1% Na CMC suspension, while group II was the treatment group received 1 % extract suspension.  On the 5th, 10th and 15th days the percentage of wound healing was measured and the epithelialization time and density of collagen fibers was observed.The results of the percentage of wound healing and epithelialization time evaluation which were analyzed by the General Linear Repeated Measures test showed a significant differences (p <0.05), whereas observation of collagen fibers showed no significant differences (p> 0.05) and analysis of epithelialization time using Independent Sample T test showed a significant difference (p <0.05), it can be concluded that purified extract has excision wounds healing effect in mice.

ABSTRACT Background Cariogenic microorganisms are the most important cause for occurrence of dental caries. Dentifrices and mouthwashes containing antimicrobial substances are proven to be effective in the eradication of these pathogens from the oral cavity. Aim To evaluate the antimicrobial efficacy of fluoride, chlorhexidine (CHX), herbal, and xylitol containing toothpastes and mouthwashes against Streptococcus mutans (S. mutans) and Lactobacillus (LB) in subjects within the age group of 18 to 22 years at time intervals of 1, 3, and 6 months. Materials and methods One hundred subjects were randomly divided into four groups. Group I: fluoride, group II: chlorhexidine, group III: herbal, group IV: xylitol and instructed to use toothpastes and mouthwashes containing the specific agents. Salivary samples were collected to evaluate the levels of S. mutans and LB at baseline, 1, 3, and 6 months. Bacterial levels were evaluated using caries risk test (CRT) kit. Data were analyzed using analysis of variance and post hoc test. Results During intragroup comparison, S. mutans levels in group I showed statistically significant difference among the four time intervals. On intergroup comparison, S. mutans levels after 6 months for groups I, II, III, and IV were 1.12, 1.16, 1.28, and 1.4 respectively. Conclusion It can be concluded that fluoride, CHX, and xylitol showed a significant reduction in S. mutans and LB count after a time period of 6 months while herbal group did not show a significant reduction in S. mutans and LB count at any intervals. Objective: To evaluate the role of combination therapy of platelet rich plasma (PRP) with arthrocentesis and to compare it with arthrocentesis alone in the temporomandibular dysfuction (TMD) patients.Study Design: Quasi experimental study.Place and Duration of Study: Oral and Maxillofacial Surgery department, Armed Forces Institute of Dentistry, Combined Military Hospital Rawalpindi, from Jan 2017 to Jun 2019.Methodology: Forty-two patients diagnosed with refractory unilateral temporomandibular dysfuction, were included in the study. Patients were divided in two groups with 21 temporomandibular joints in each. Arthrocentesis alone was the control group (group I) and arthrocentesis with intra-articular injection of platelet rich plasma (group II) was the study group. Treatment outcomes were assessed and compared for all patients based on clinical parameters of pre and post treatment; for pain, maximal mouth opening and temporomandibular joint clicking sounds. Results: Out of 42 patients, 33 (79%) were females and 9 (21%) were males with mean age of 34.3 ± 8.4 years. There was statistically significant difference in both groups for all variables. The p-value of maximum mouth opening of both groups before and after treatment was 0.746 and 0.01, joint clicking sounds were present in 69% of our patients before the treament and it reduced to 14% after the treatment. There was marked gradual decrease in pain of both groups, group I (6.48 ± 1.470 to 1.81 ± 0.602) and group II (7.29 ± 1.007 to 1.19 ± 0.402). Objectives: The prevalence of typhoid fever was reportedly high, especially in the Asian continent, as many as 80% of cases came from slums inBangladesh, China, India, Indonesia, Laos, Nepal, Pakistan, and Vietnam. Due to many cases of antibiotic resistance in typhoid fever, various effortshave been made by combining antibiotic therapy or active compounds with adjuvants and herbs. Curcumin is an active compound found in manyherbal plants, especially in the Asian Continent. Curcumin has an antimicrobial effect, presumably due to its ability to bind Vitamin D receptors (VDR)as a potential ligand. This condition increases the expression of cathelicidin antimicrobial peptides and eradicates bacteria. Vitamin D will definitelybind to VDR as well; on this basis, this study wants to prove the effect of the combination of curcumin and Vitamin D therapy in inhibiting the growthof Salmonella typhi.Methods: This study is a true experimental pre- and post-test design using colony calculation method to investigate the effectiveness of curcumin andVitamin D in suppressing the growth of S. typhi bacteria in peritoneal fluid male mice strain balb/c. Mice were divided into five groups randomly, namely,the negative control groups, Group I (curcumin 200 mg/BB/day), Group II (curcumin 400 mg/BB/day), and Group III (curcumin 200 mg/BB/day andVitamin D 200 IU/day), and the positive control groups (antibiotic levofloxacin). The intervention was carried out for 5 days. After the 5th day, micewere then maintained for 3 weeks to determine the amount of colony growth in the post-intervention period.Results: The comparison of the results between each group gave significance in the average number of bacterial colonies of intraperitoneal fluid.Each group gave a significant difference of <0.05. Curcumin has an activity as an antimicrobial, the higher the dose, the greater the number of bacteriainhibited growth. After curcumin therapy, 200 mg/kg and 400 mg/kg for 5 days, a decrease in the number of bacterial colonies in the intraperitonealfluid was found. This study concluded that curcumin has an antimicrobial effect on S. typhi. The groups with combination therapy of Vitamin D andcurcumin intervention also gave the same results. Conclusion: Based on the results of this study, the combination of curcumin and Vitamin D is able to inhibit the growth of S. typhi bacteria, even upto 30 days after infection.

The aim of this study was to evaluate the apical microleakage of Biodentine and MTA orthograde apical plugs and to compare the effect of thickness of these biomaterials on their sealing ability. A total of eighty maxillary anterior teeth were used. The apices were removed by cutting with a diamond disc (Jota, Germany) 2 mm from the apical root end in an attempt to standardize the working length of all specimens to 15 ± 1 mm. Both materials were placed in 1–4 mm thickness as apical plugs root canal. Root canal leakage was evaluated by the fluid filtration technique. One-way ANOVA was used in order to determine normality of dispersal distribution of parameters; thereafter, results were analyzed by Kolmogorov-Smirnov test. Overall, between microleakage values of MTA and Biodentine regardless of apical plug thickness, no difference was observed. In terms of plug thickness, a statistically significant difference was observed between the subgroups of MTA and Biodentine (p<0.05). The apical sealing ability of Biodentine was comparable to MTA at any apical plug thickness.
