Biological variation in urine samples used for analyte measurements

Abstract To determine the influence of biological variation on the reliability of data from different types of urine specimens, we measured nine analytes in first-morning, randomly collected, and 24-h samples of urine from 53 healthy individuals (14 men and 39 women). The urines were collected once a week for 10 weeks. The data obtained were used as a basis for specimen collection and to gain insight into the influence of urine quantities in the diagnosis, screening, and monitoring of patients. We found that 24-h urine expressed in output rather than concentration units is the most reliable specimen for diagnosis and monitoring for most of the analytes studied. On the basis of the ratio between estimated within- and between-subject variation, the tests with greatest medical usefulness for diagnosis and screening of specific pathologies are those measuring protein and sodium. Moreover, the results indicate that urine creatinine may be a poor test for diagnosis, monitoring, and screening.

Abstract Analytical, intra-individual, and interindividual components of variation for sodium, potassium, urea, creatinine, calcium, and phosphate were estimated from results for 24-h urine specimens collected from 15 apparently healthy individuals every four weeks for 40 weeks. Expressed as output, mean values differed for men and women, except for calcium. Our data on intra-individual variation were similar to those obtained for 10 men by Shephard et al. (Clin Chem 1981;27:569-73). Calculated analytical goals are easily attained by current methods. Reference values for urine creatinine are useful only when expressed as output and stratified according to gender. The ratios of intra- to interindividual variation generally increase on such stratification; separate reference values for men and women are therefore required for analytes expressed as output. Measurements of sodium and potassium in urine should be reported as concentration, but output terms are favored for the other analytes. Differences for two serial results from an individual must be rather large to differ statistically.

Abstract Intra-individual, interindividual, and analytical components of variation for each of 10 urine analytes were estimated from results obtained on analysis of urine specimens from 10 apparently healthy young men. In spot urines, urea and osmolality showed strong individuality, but intra-individual variances were larger than interindividual variances for sodium, urate, phosphate, and glucose. Urea and osmolality displayed strong individuality in daily urines, whereas sodium (when results were expressed in output terms), urate, potassium, and total protein (when results were expressed in concentration units) had intra-individual variances larger than interindividual variances. In monthly urines, calcium and glucose, in concentration terms, and phosphate, in output terms, exhibited strongest individuality. Analytical variance was a significant percentage of the total variance for total protein and glucose; the analytical methods we used for these analytes were probably unsatisfactory. A series of analytical goals for imprecision, based on biological variation, were derived. Although these goals may not be applicable in all clinical situations, they represent the beginning of a data base in the literature.

This study examines how media representations of Palestinian-Israeli politicians, can help community members realize their capabilities. The study’s database is comprised of 1,207 interviews conducted with Palestinian-Israeli politicians on news and current affairs programs on the three national television channels and the two national radio stations in Israel, for 24 months (2016-2017). We identified and analyzed the differences in the modes of representation between national and local Palestinian-Israeli politicians and between Palestinian-Israeli parliament members in the Joint List and Palestinian-Israeli parliament members in Zionist parties, all through the capabilities prism. In this study, we demonstrated how different types of Palestinian-Israeli politicians may potentially affect the realization of different political functions and capabilities. Analyzing political representations in the media through the theoretical framework of the ‘capabilities approach’ contributes to a more comprehensive insight into the roles the media can play promoting people’s wellbeing and human rights, relative to traditional media theories.

This article explores the impact of online crime victimisation. A literature review and 41 interviews – 19 with victims and 22 with experts – were carried out to gain insight into this. The interviews show that most impacts of online offences correspond to the impacts of traditional offline offences. There are also differences with offline crime victimisation. Several forms of impact seem to be specific to victims of online crime: the substantial scale and visibility of victimhood, victimisation that does not stop in time, the interwovenness of online and offline, and victim blaming. Victims suffer from double, triple or even quadruple hits; it is the accumulation of different types of impact, enforced by the limitlessness in time and space, which makes online crime victimisation so extremely invasive. Furthermore, the characteristics of online crime victimisation greatly complicate the fight against and prevention of online crime. Finally, the high prevalence of cybercrime victimisation combined with the severe impact of these crimes seems contradictory with public opinion – and associated moral judgments – on victims. Further research into the dominant public discourse on victimisation and how this affects the functioning of the police and victim support would be valuable.

Abstract Objectives Thyroid biomarkers are fundamental for the diagnosis of thyroid disorders and for the monitoring and treatment of patients with these diseases. The knowledge of biological variation (BV) is important to define analytical performance specifications (APS) and reference change values (RCV). The aim of this study was to deliver BV estimates for thyroid stimulating hormone (TSH), free thyroxine (FT4), free triiodothyronine (FT3), thyroglobulin (TG), and calcitonin (CT). Methods Analyses were performed on serum samples obtained from the European Biological Variation Study population (91 healthy individuals from six European laboratories; 21–69 years) on the Roche Cobas e801 at the San Raffaele Hospital (Milan, Italy). All samples from each individual were evaluated in duplicate within a single run. The BV estimates with 95% CIs were obtained by CV-ANOVA, after analysis of variance homogeneity and outliers. Results The within-subject (CV I ) BV estimates were for TSH 17.7%, FT3 5.0%, FT4 4.8%, TG 10.3, and CT 13.0%, all significantly lower than those reported in the literature. No significant differences were observed for BV estimates between men and women. Conclusions The availability of updated, in the case of CT not previously published, BV estimates for thyroid markers based on the large scale EuBIVAS study allows for refined APS and associated RCV applicable in the diagnosis and management of thyroid and related diseases.

Abstract The study contributes to the knowledge of species composition and biogeographical affinities of savannas rodent in Gabon. Unlike small rodents in Gabonese forests, there is little data on the diversity of small rodents in Gabonese savannas. The diversity and distribution of rodent murid communities was studied in four different types of savanna in Gabon: Coastal Basin (South-West), Lopé/Okanda (in the Center), Batéké Plateaux (Southeastern) and Ngougnié/ N’yanga (in the South). A total of 428 individuals representing six species were captured over 11,920 trap nights. Trap success was highly variable (2.2–6.9 %). The most abundant species were Mus minutoides (69%) followed by Lemniscomys striatus (21.5%). Indices of species richness varied from 2 to 5 and diversity (Shannon and Weaver) was low in the four savannas with the highest value at Ngougnié/N’yanga (H′ = 1.2). Species distributions show that Gabonese savanna small rodents conform to four distribution types, with one species known from Zambesian savannas exhibiting austral affinities (Pelomys campanae: occurs in three southern savannas). This new information provides important insight into the biogeography of small rodents at a local and regional level. Moreover, the correspondence analysis highlighted an influence of local ecological factors on population abundance.

Three humic and one fulvic acid were degraded by mild chemical oxidation with peracetic acid, with special emphasis on the effects of this type of oxidation on N-containing components. The different types of N that were considered were NH4+-N, amino acid-N, amino sugar-N, NO2−-N + NO3−-N, and by difference from total N, "unknown" N. The behaviour toward mild chemical oxidation of all four preparations was essentially similar: there were decreases in mino acid-N, amino sugar-N and "unknown" N, increases in NH4+-N, NO2−-N + NO3−-N with one material, and in N-gases. The "unknown" N was not inert. Between 16.6 and 59.1% of the latter appeared to be converted, as a result of mild chemical oxidation, to NH3 and N-gases which were expelled from the systems. The results presented provide an insight into what happens to N-containing humic components as a result of mild oxidation.

We assessed the analytical and biological variation of pyridinium crosslinks in early morning, 2 h fasting, and 24 h urine specimens from 14 healthy adults over a 1 month period. The results were expressed both in terms of pyridinoline concentration and pyridinoline/creatinine ratio. The data obtained were used to select the optimum specimen for clinical purposes. We found that: ( a) early morning specimens are preferred; ( b) results should be expressed as pyridinoline/creatinine ratio; ( c) reference intervals should be stratified according to gender; ( d) the necessary analytical imprecision (CV≤ 9%), derived from biological variation, is not easily achieved by current methods; ( e) the difference between serial results from an individual must be > 50% to be statistically significant; and ( f) assessment of risk for osteoporotic fracture by means of the pyridinium crosslink assay would, in a significant number of patients, require analysis of multiple urine specimens.

: Diabetes mellitus (DM) is a type of chronic metabolic disease that has affected millions of people worldwide and is known with a defect in the amount of insulin secretion, insulin functions, or both. This deficiency leads to an increase in the amounts of glucose, which could be accompanied by long-term damages to other organs such as eyes, kidneys, heart, and nervous system. Thus, introducing an appropriate approach for diagnosis and treatment of different types of DM is the aim of several researches. By the emergence of nanotechnology and its application in medicine, new approaches were presented for these purposes. The object of this review article is to introduce different types of polymeric nanoparticles (PNPs), as one of the most important classes of nanoparticles, for diabetic management. To achieve this goal, at first, some of the conventional therapeutic and diagnostic methods of DM will be reviewed. Then, different types of PNPs, in two forms of natural and synthetic polymers with different properties, as a new method for DM treatment and diagnosis will be introduced. In the next section, the transport mechanisms of these types of nano-carriers across the epithelium, via paracellular and transcellular pathways will be explained. Finally, the clinical use of PNPs in the treatment and diagnosis of DM will be summarized. Based on the results of this literature review, PNPs could be considered one of the most promising methods for DM management.

Abstract Objectives A large number of people undergo annual health checkup but accurate laboratory criterion for evaluating their health status is limited. The present study determined annual biological variation (BV) and derived parameters of common laboratory analytes in order to accurately evaluate the test results of the annual healthcare population. Methods A total of 43 healthy individuals who had regular healthcare once a year for six consecutive years, were enrolled using physical, electrocardiogram, ultrasonography and laboratory. The annual BV data and derived parameters, such as reference change value (RCV) and index of individuality (II) were calculated and compared with weekly data. We used annual BV and homeostatic set point to calculate personalized reference intervals (RIper) which were compared with population-based reference intervals (RIpop). Results We have established the annual within-subject BV (CVI), RCV, II, RIper of 24 commonly used clinical chemistry and hematology analytes for healthy individuals. Among the 18 comparable measurands, CVI estimates of annual data for 11 measurands were significantly higher than the weekly data. Approximately 50% measurands of II were <0.6, the utility of their RIpop were limited. The distribution range of RIper for most measurands only copied small part of RIpop with reference range index for 8 measurands <0.5. Conclusions Compared with weekly BV, for annual healthcare individuals, annual BV and related parameters can provide more accurate evaluation of laboratory results. RIper based on long-term BV data is very valuable for “personalized” diagnosis on annual health assessments.
