Axial Spondyloarthritis

Ankylosing spondylitis (AS) is a chronic inflammatory arthritis affecting mainly the sacroiliac joints and spine, resulting in pain, stiffness, and reduced movement. Over the past decade there have been major advances in many aspects of the disease, including a broadening of the disease description to axial spondyloarthritis (axSpA). While the many advances have transformed the lives of patients with axSpA, they have also increased complexity for non-specialists in this area. This handbook contains a timely update of the key developments and current state of play in axSpA. It is intended primarily for the many healthcare professionals who encounter patients with this condition, in both primary and secondary care settings. It will also be of interest to the wider medical and research community.The handbook is written by rheumatologists with active research programmes and clinical expertise in these conditions. The topics covered include: • the clinical features • extra-articular manifestations and complications • the impact on patients’ lives • the major advances in genetics and pathogenesis • imaging advances • classification criteria and diagnosis (and the important differences between these) • treatment advances (particularly TNF inhibitors and upcoming biologics)

This article reviews the research trends in welfare sociology (sociological studies on social security and welfare), one of the many subfields of active research in sociology in Japan. For this purpose, several research streams formed from the 1970s to the 2000s are described, and some of the most important research results produced within these in the past two decades are introduced. In the latter part of this article, a broad overview of the research trends in Japanese welfare sociology is attempted by focusing on the contents of the journal published by the Japan Welfare Sociology Association (JWSA).

This introduction gives on overview of the sub-discipline of naval history since its emergence in the early eighteenth-century. It outlines the various social, cultural and political influences that have shaped the subject over the past three centuries, and discusses its relationship with the wider historical profession. The second half of the introduction sums up the current state of naval history, describing the many historiographies that now have a bearing on how the subject is conducted. Each contribution to the volume is introduced in this context, offering a precis of the chapters that follow.

Purpose There are countless factors that affect where an entrepreneur chooses to open a business that have been studied in the literature, including local socio-economic conditions, government policy, and agglomeration economies. One important aspect to the location decision that has not received as much attention from researchers thus far is the impact of crime on entrepreneurship. The purpose of this paper is to discuss the current literature on this topic, with a particular emphasis on the empirical issues present that have likely caused the research in this area to be scarce. Design/methodology/approach The authors conduct an analysis of the current state of the literature examining the relationship between crime and entrepreneurship. Looking at what has been done in the past, as well as improvements in the data, the authors discuss what has been done and what can be done in the future. Findings The authors discuss areas related to entrepreneurship and crime that the authors see as an emerging literature, based largely on the improvements in data and identification strategies that allow the authors to answer questions that the authors previously could not. Originality/value This paper is a review of the current literature, which also discusses areas that future researchers should consider and analyze further.

Ranked set sampling (RSS) is an approach to data collection and analysis that continues to stimulate substantial methodological research. It has spawned a number of related methodologies that are active research arenas as well, and it is finally beginning to find its way into significant applications beyond its initial agricultural-based birth in the seminal paper by McIntyre (1952). In this paper, we provide an introduction to the basic concepts underlying ranked set sampling, in general, with specific illustrations from the one- and two-sample settings. Emphasis is on the breadth of the ranked set sampling approach, with targeted discussion of the many options available to the researcher within the RSS paradigm. The paper also provides a thorough bibliography of the current state of the field and introduces the reader to some of the most promising new methodological extensions of the RSS approach to statistical data analysis.

AS has a major negative impact on patients’ quality of life. AS is part of a larger group of related spondyloarthritis (SpA) conditions and patients with AS often have extra-articular manifestations of these conditions. Over the past decade, there have been major advances in the understanding of the genetics and pathophysiology of the disease. Advances in imaging have allowed patients to be diagnosed without having to develop the radiographic structural damage that characterize AS, resulting in the concept of axial spondyloarthritis (axSpA). Together with the development of highly effective TNF inhibitors, these advances have transformed the management and outlook of patients with this condition. It is hoped that further advances in diagnosis, assessment and treatment of axSpA will lead to further progress in future.

The purpose of this study is to determine the impact of negative content that has an element of violence on the YouTube site on child development, determine the form of legal protection of children against negative content that has an element of violence on the YouTube site. This type of research used in this study is normative juridical. The approach used in this study is the statutory approach and the analytic approach, the method of gathering legal material through library research. While the method of analyzing the legal material is done qualitatively. The many negative impacts of the shows or content being watched are supported by data from the Indonesian Child Protection Commission (KPAI) which has recorded that in the past 4 years the number of violence against children has continued to increase. Forms of legal protection for children against negative content that has an element of violence on the YouTube site consists of legal protection that is preventive and repressive legal protection. During haemodialysis (HD) sessions, patients undergo alterations in the extracellular environment, mostly concerning plasma electrolyte concentrations, pH, and volume, together with a modification of sympathovagal balance. All these changes affect cardiac electrophysiology, possibly leading to an increased arrhythmic risk. Computational modeling may help to investigate the impact of HD-related changes on atrial electrophysiology. However, many different human atrial action potential (AP) models are currently available, all validated only with the standard electrolyte concentrations used in experiments. Therefore, they may respond in different ways to the same environmental changes. After an overview on how the computational approach has been used in the past to investigate the effect of HD therapy on cardiac electrophysiology, the aim of this work has been to assess the current state of the art in human atrial AP models, with respect to the HD context. All the published human atrial AP models have been considered and tested for electrolytes, volume changes, and different acetylcholine concentrations. Most of them proved to be reliable for single modifications, but all of them showed some drawbacks. Therefore, there is room for a new human atrial AP model, hopefully able to physiologically reproduce all the HD-related effects. At the moment, work is still in progress in this specific field.

The role of insurance in a person’s life is undoubtedly great because it serves as the most effective tool in case of loss of property, health problems, business protection, etc. The purpose of the article is to assess the current state of the insurance market in Russia and identify problems that arise during its functioning. The research methodology consists in summarizing and comparing data characterizing various components of the insurance market. The article analyzes the current state of the insurance market in Russia with a falling number of insurers by more than 20 % over the past 3 years, the share of top insurers continues to grow every year, over the past 3 years the growth has amounted to 12 %. Calculations that indicate a change in the level of concentration in the insurance market of the Russian Federation have been made. Based on the results of the analysis, it was concluded that there is monopolistic competition in the market. The main factors influencing the level of competition in the market under consideration, namely, barriers to entry into the industry and their impact, were studied Due to the relevance of the research, the analysis of the impact the pandemic has on the Russian insurance market was carried out and the prospects for the development of the market were considered. Identifying the weaknesses of insurers made it possible to determine the priority areas for further development: intensive digital transformation of business, marketing, advertising.

AbstractHuman emotion recognition is an active research area in artificial intelligence and has made substantial progress over the past few years. Many recent works mainly focus on facial regions to infer human affection, while the surrounding context information is not effectively utilized. In this paper, we proposed a new deep network to effectively recognize human emotions using a novel global-local attention mechanism. Our network is designed to extract features from both facial and context regions independently, then learn them together using the attention module. In this way, both the facial and contextual information is used to infer human emotions, therefore enhancing the discrimination of the classifier. The intensive experiments show that our method surpasses the current state-of-the-art methods on recent emotion datasets by a fair margin. Qualitatively, our global-local attention module can extract more meaningful attention maps than previous methods.
