The Shutdown PSA Model of the TQNPP CANDU Reactor and the Outage Risk Management

This paper presents the characteristic of Outage Nuclear Safety Management Guideline (ONSMG) for Qinshan CANDU plant and the introduction of shutdown and low-power PSA (SPSA) of TQNPP. The SPSA model is used to assess the outage configuration risk of OT109. The result and its risk insight are discussed about the two different outage working plan. In conclusion, the risk of nuclear safety and the economic are discussed based on the risk assessment result.

Risk assessment in large organizations with extensive operational domains has been a challenging issue. Employing an efficient method along with realistic pair comparisons, applying subjective inferences of organization experts, and purging the intrinsic ambiguity of inferences, are not reflected in current airlines' safety management. Traditional two-dimensional risk assessment for risk management of safety hazards, however, is no longer sufficient to comply with this complexity. A new model for risk management and a novel formula for risk index calculation, based on a fuzzy approach, are presented in this study. In this new model, unlike in the traditional approach, the latent aftermath of safety reports, especially those which affect the continuity of the business, is also taken into account. In this model, along with the definition of a new structure for risk management, risk analysis should be restructured. To that end, a two-dimensional classic risk formula was replaced with three-dimensional (nonlinear) exponential ones, considering “the impact on the business” as a source of risk and hazard. For measuring the safety risk using the Fuzzy hierarchical evaluation method, considering experts' opinions, three criteria in four different operational fields were developed. This method employs a Fuzzy ANP to help quantify judgments, make qualitative judgments in the traditional method, and weigh the priority of elements contributing to risk. Also, it provides a tool for top-level as well as expert level management to monitor safety more precisely, monitor the safety level within their departments or organizations, set quantitative safety goals and provide feedback for improvement as well as find the most critical areas with the least cost. In this study, an airline has been selected as a case study for the risk assessment of reports based on the new model.

Every practical activity carried out by student at Aircraft Engineering Departement, then these activities have risks that need mitigation measures. In safety risk management requires departement to conduct an assessment of the risks that can occur from an activity using hazard identification, risk assessment and mitigation. By identifying types of hazards, assessing risks, getting results in the form of risk matrix and risk level and identifying mitigation actions that can be taken and accepted. Furthermore, it produces hazard identification and risk assessment & mitigation (HIRAM) documents, and controls risks to acceptable and tolerable levels with mitigation measures. Safety risk management which is one of the standards in implementing safety management system in Aircraft Engineering Departement can be fulfilled.

Patient safety is a system that makes patient care safer, including risk assessment, identificationand management of patient risks, incident reporting and analysis, the ability to learn from incidents andtheir follow-up, and implementation of solutions to minimize risks and prevent injury caused by errors.the result of carrying out an action or not taking the action that should be taken The importance of riskmanagement in the hospital is to minimize the number of unwanted events. Based on a preliminary studyof five medical record processing at the Dr Soediran Mangun Sumarso Wonogiri Regional Hospital, thebigger risk is filing, in the medical record unit there is no standard risk management operationalprocedure and there is no report related to risk management. This type of research is descriptive with across sectional approach. The subjects are the Facility Management and Safety coordinator, head ofmedical records, and head of filing. The object of the filing room and the risk management process. Theresearch instruments were observation guidelines and interview guidelines. Data analysis was carriedout by univariate. The results show that risk management at Dr Soediran Mangun Soemarso WonogiriHospital has been carried out. Safety Facility Management is a working group to plan risk managementin each unit. The medical record unit of Dr Soediran Mangun Soemarso Wonogiri Hospital implementsrisk management with an appropriate sequence of risk management processes starting fromcommunication consultation, scope of context criteria, risk assessment, risk treatment, monitoring andreview. In the filing section of the Dr Soediran Mangun Soemarso Wonogiri Regional Hospital, there isone important stage that is overlooked in the risk management process, namely the scope, context, andcriteria stages. The conclusion of this study is that the Facility and Safety Management section hasimplemented a risk management process in the medical record unit but has not been maximallyimplemented in the filing section.

UAVs performance is improved, in addition to GPS, they can find routes with artificial intelligence and various techniques, and can fly autonomously. The global UAV market was US$ 25.59 billion in 2018. It is estimated that the market will reach US$70.28 billion by 2029. The first three UAV manufacturers were DCL (USA), Parrot (France) and Yuneec (PRC) and are now being produced in many countries. Aircrafts are exposed to various risks: an aerodynamic stall due to inconsistent sensor readings and inadequate response, hitting a flock of geese or flying objects, pilot being unable to prevent the plane repeatedly nosediving despite following procedures. Therefore, procedures issued by aviation agency for aircraft type certification require an aircraft manufacturer (“applicant”) to demonstrate that its design complies with all applicable agency’s regulations and requirements. Safety risk management is a key component of a safety management system and involves fundamental activities such as identifying safety hazards, and assessing the risks and mitigation. Risk management is an integral component of safety management and involves some essential steps.Take into account any current mitigation measures and assess the seriousness in terms of the worst possible realistic scenario. The risk assessment considered five operational environments; remote, rural, suburban, urban, and congested. Reliability, availability, maintainability, and safety assessment are important study in the development of UAVs. This kind of study is mandatory to increase the reliability of the UAV, its availability, and to reduce repair and maintenance costs.

The safety of oil & gas pipelines is vital to the environment, people and properties. In this paper, the pipeline risk management is summarized. A management plan of pipeline integrity adopted worldwide based on intelligent inspection is introduced, and the actuality of pipeline risk management in China and the problems are mentioned. An integrated solution which emphasizes that the legislation on pipeline safety is the safeguard of it, to enhance pipeline safety management is the key for it and to design pipeline databases and risk assessment system based on digital computerized technology is the foundation of pipeline safety, is proposed. Only these three aspects are closely worked in, can the pipeline risk be effectively controlled, so that it is possible to reduce pipeline risk in maximum and keep pipelines operating safely and efficiently.

Safety risk assessment in dams is a traditional and modern approach to the engineering process of dams on safety systems so as to produce a design history that has a good overall record and integrity. With this approach for both old and new dams, this safety risk assessment aims to determine whether the risk the hazard can be tolerated or not, if it cannot be tolerated then there must be a solution for action to reduce the risk of danger so that the safety of the dam can be obtained. The existing risk assessment can be used as the basis for the construction of the dam to its maintenance, in this study discussed risk assessment safety on dams located in several countries such as in Asia, Europe and America. The methods implemented include the preparation of safety risks, risk analysis, risk evaluation and safety risk management. Based on research results The main risk factor that causes safety responsibilities is the Contractor's responsibility, namely the Non-technical project factor (40.54%), the 2nd rank is the Undecided Responsibility due to internal technical factors (35.13%), the 3rd rank is the technical project factor namely Contractor's Responsibility (10.8%), Rank 4 is external technical factor and external non-technical factor, the last risk factor is internal non-technical factor (2.7%) namely Joint Responsibility. The article indicates the goals and objectives of risk management at industrial enterprises. The basic definitions of hazards and risks are given. In addition, they reviewed risk assessment methods related to the implementation of OSH management systems.

This paper addresses the role of human fatigue in workplace safety and risk management. It is well known that fatigue can increase the likelihood of workplace injuries, but the systematic application of this knowledge in safety and risk management is less well known. This paper presents a risk-based method for addressing fatigue in safety and risk management processes. The method incorporates elements of a data-driven fatigue risk management system (FRMS). Specific issues include potential data sources for the FRMS and practical applications within existing safety management systems. Special attention is paid to the fatigue risk assessment, which mirrors a common safety risk assessment and affords systematic control of fatigue-related human error.

The article considers the problems of application of aerospace technologies in the management of environmental safety of the planned activity with the help of integrated automated systems. The purpose of the article. The aim of the article is to determine the features of the application of aerospace technologies in the management of environmental safety of planned activities using integrated automated systems and the formation of a methodology for assessing environmental risk in the ecosystem. The scientific novelty is that the proposed methodology for assessing environmental risk in the ecosystem and provided scientific and practical recommendations for the use of space technologies for Earth observation to study the state of the environment in the management of environmental safety of planned activities Conclusions. The methodology of ecological risk assessment in the ecosystem is considered. The risk analysis scheme includes a wide range of interrelated problems and different stages: identification of risk factors, risk assessment, risk management. Recommendations for ecosystem risk assessment are provided. Features of risk management in the ecosystem are considered. The risk management strategy defined in the environmental decision-making support system may be based on the choice of the level of risk in the range from the minimum (which is considered to be quite small) to the maximum allowable. The article considers several strategies for environmental safety management: prevention of the causes of catastrophes up to the abandonment of products of hazardous industries, closure of emergency facilities; prevention of emergencies in cases where it is impossible to avert the causes of disasters (construction of protective structures, dams, the creation of an underground economy, early evacuation of the population, etc.); disaster mitigation, implementation of stabilization and compensation measures. The most suitable, from the point of view of the main goal of environmental safety management, is risk minimization, ie the implementation of the first and second strategies. However, in practice this is not always possible. A combination of all three types of strategies is most likely. Therefore, the selection of space survey materials is an important step in the classification for environmental monitoring. Procedures of thematic decoding and creation of digital maps of the area with the use of comic images are considered. When creating a digital map of ecological monitoring of regions, it is advisable to use the procedure of creating a vector layer of terrain in the ArcCatalog environment, and build a mosaic image using the software Erdas Imagine. The article considers general problems of implementing the enterprise risk management procedure. One of the ways to solve the problems arising when meeting Russian state standard ISO 9001-2015 requirements concerning risk-oriented thinking is presented. A risk assessment control model aimed at coordinating all kinds of the enterprise departments activities, forming a clear algorithm of risk management procedure implementation and attracting a greater number of employees towards this activity is offered.
