Glenoid Labrum Lesion in an Elite Tennis Player: A Clinical Challenge in Diagnosis

Shoulder pain among overhead-sport athletes is common and often presents a challenge to clinicians in making an accurate diagnosis. A case report of a young college tennis player is presented, with emphasis on the clinical examination process leading to the diagnosis of a superior labrum anteroposterior lesion. The current literature regarding the clinical diagnosis of glenoid labrum lesion in the shoulder with respect to specific clinical tests was reviewed. It is recommended that clinicians consider glenoid labral lesions in the context of shoulder instability. Glenohumeral internal-rotation deficit should be routinely evaluated and corrected in high-performance tennis players.

Abstract High performance modern tennis is characterised by high dynamism, speed in thinking and action, precision and high technical and tactical skills. In this study, we used direct observation and statistical recording of nine matches during two competition years in the tennis players Roger Federer, Rafael Nadal and Andre Agassi. In these tennis players, we studied mainly the return of serve, one of the most important shots in tennis, together with serve, as first shots in a point. We have chosen the three tennis players because they are the best example of return of serve as shown by the matches recorded and interpreted. The study we have carried out shows that return of serve makes Agassi a winner in most matches. The high percentage in Federer’s serves makes his adversaries have a lower percentage in return of serve, which prevents them to win against his serve. High percentage in return of serve results in more points on the adversary’s serve and an opportunity to start the offensive point. After comparing the three tennis players mentioned above, we can see that the highest percentage of points won on return of serve belongs to Agassi, which ranks him among the best return of serve tennis players in the world. The tennis player with the highest percentage in return of service is the one who wins the match, which shows, once again, the importance of the return of serve. Return of serve can be a strong counter-attack weapon if used at its highest level.

A high proportion of shoulder injuries in table tennis players are common, which is both a diagnostic and therapeutic challenge. An understanding of the interaction between biomechanical function of the shoulder and mechanisms of shoulder injuries in table tennis players is necessary to prevent injury and to conduct clinical treatment of the shoulder as soon as possible. The purpose of this minireview was to select the available evidence on the biomechanical characteristics of shoulder movement and potential relationships with various shoulder injuries that are common in table tennis players. Five studies revealed interesting biomechanical characteristics of shoulder movement patterns in table tennis players: large internal rotation torque, an increased torsion-rotation movement, and a greater angular velocity of internal rotation were found. Two studies were noted that were related to specific shoulder injury: glenohumeral internal rotation deficit (GIRD) and impingement syndrome. Unfortunately, it is difficult to draw conclusions on the mechanisms of shoulder injury in table tennis players due to the little evidence available that has investigated shoulder injury mechanisms based on biomechanical characteristics. Future studies should focus on the potential relationship between the biomechanical characteristics of the shoulder and injury prevalence to provide valuable reference data for clinical treatment.

In the high-velocity tennis serve, the contributions that the upper limb segments' anatomical rotations make to racket head speed at impact depend on both their angular velocity and the instantaneous position of the racket with respect to the segments' axes of rotation. Eleven high-performance tennis players were filmed at a nominal rate of 200 Hz by three Photosonics cameras while hitting a high-velocity serve. The three-dimensional (3-D) displacement histories of 11 selected landmarks were then calculated using the direct linear transformation approach, and 3-D individual segment rotations for the upper limb were calculated using vector equations (Sprigings, Marshall, Elliott, & Jennings, 1994). The major contributors to the mean linear velocity of the center of the racket head of 31.0 m · s-1 at impact were internal rotation of the upper arm (54.2%), flexion of the hand (31.0%), horizontal flexion and abduction of the upper arm (12.9%), and racket shoulder linear velocity (9.7%). Forearm extension at the elbow joint played a negative role (-14.4%) and reduced the forward velocity of the center of the racket at impact.

The reaction speed of the tennis player and the speed of the ball after the impact to the tennis blade, they both are important elements of the performance evaluation of a tennis player. The determination of those parameters is possible by using specialized contact sensors. Permanent ceramic magnets can be used to process such sensors and the paper presents the experimental results concerning the processing of such magnets of barium hexaferritte powders

Background Glenohumeral internal rotation deficit, often diagnosed in players of overhead sports, has been associated with the development of secondary shoulder lesions. Hypothesis Asymptomatic players of different overhead sports will exhibit variable degrees of glenohumeral internal rotation deficit. Study Design Cross-sectional study; Level of evidence, 3. Methods Fifty-four asymptomatic male volunteers (108 shoulders) divided into 3 groups (tennis players, swimmers, control group) underwent measurements of glenohumeral internal and external rotation using clinical examination with scapular stabilization. Measurements of dominant and nondominant shoulders were compared within and between groups. Glenohumeral internal rotation deficit (GIRD) was defined as the difference in internal rotation between the nondominant and dominant shoulders. Results In tennis players, mean GIRD was 23.9° ± 8.4° (P < .001); in swimmers, 12° ± 6.8° (P < .001); and in the control group, 4.9° ± 7.4° (P = .035). Dominant shoulders showed significant difference between all groups, and the difference in internal rotation of the dominant shoulder between the group of tennis players in comparison with the control group (27.6°, P < .001) was greater than the difference in internal rotation of the dominant shoulder found in the group of swimmers compared with the control group (17.9°, P < .001). Between tennis players and swimmers, the difference in internal rotation of the dominant shoulder was 9.7° (P = .002). Conclusion Dominant limbs showed less glenohumeral internal rotation than the nondominant limbs in all groups, with the deficit in the group of tennis players about twice the deficit found for swimmers. Mean difference between limbs in the control group was less than 5°, which is within normal parameters according to most studies. There were statistically significant differences between all groups when dominant shoulders were compared with each other, differences that were not compensated by external rotation gain. Tennis players had the least range of motion, followed by swimmers.

The aim of this study was to examine the impact of a tennis player’s body language and clothing (general vs. sport-specific) on the impressions observers form of them. Forty male tennis players viewed videos of a target tennis player warming up. Each participant viewed the target player displaying one of four combinations of body language and clothing (positive body language/tennis-specific clothing; positive body language/general sportswear; negative body language/tennis-specific clothing; negative body language/general sportswear). After viewing the target player, participants rated their impressions of the model’s episodic states and dispositions and gave their perceptions of the likely outcome of a tennis match with the target player. Analyses of variance revealed that positive body language led to favorable episodic impressions and low outcome expectations. Analysis also indicated that clothing and body language had an interactive effect on dispositional judgments. The study supports the contention that nonverbal communication can influence sporting interactions.

Purpose:High-performance tennis environments aim to prepare athletes for competitive demands through simulated-match scenarios and drills. With a dearth of direct comparisons between training and tournament demands, the current investigation compared the perceptual and technical characteristics of training drills, simulated match play, and tournament matches.Methods:Data were collected from 18 high-performance junior tennis players (gender: 10 male, 8 female; age 16 ± 1.1 y) during 6 ± 2 drill-based training sessions, 5 ± 2 simulated match-play sessions, and 5 ± 3 tournament matches from each participant. Tournament matches were further distinguished by win or loss and against seeded or nonseeded opponents. Notational analysis of stroke and error rates, winners, and serves, along with rating of perceived physical exertion (RPE) and mental exertion was measured postsession.Results:Repeated-measures analyses of variance and effect-size analysis revealed that training sessions were significantly shorter in duration than tournament matches (P < .05, d = 1.18). RPEs during training and simulated matchplay sessions were lower than in tournaments (P > .05; d = 1.26, d = 1.05, respectively). Mental exertion in training was lower than in both simulated match play and tournaments (P > .05; d = 1.10, d = 0.86, respectively). Stroke rates during tournaments exceeded those observed in training (P < .05, d = 3.41) and simulated-match-play (P < .05, d = 1.22) sessions. Furthermore, the serve was used more during tournaments than simulated match play (P < .05, d = 4.28), while errors and winners were similar independent of setting (P > .05, d < 0.80).Conclusions:Training in the form of drills or simulated match play appeared to inadequately replicate tournament demands in this cohort of players. Coaches should be mindful of match demands to best prescribe sessions of relevant duration, as well as internal (RPE) and technical (stroke rate) load, to aid tournament preparation.

Introduction: The main symptoms of Parkinson are: tremors, movement resistance, postural instability and bradykinesia. However, other diseases such as Progressive Supranuclear Paralysis, Multiple System Atrophy and Corticobasal Degeneration have similar symptoms. This similarity generates a difficulty of diagnosis, for example, Corticobasal Degeneration is often diagnosed by autopsy. Objectives: To define the differentiating symptoms of Parkinson and the diseases mentioned and to find clinical tests that could aid in the diagnosis. Methodology: The integrative review utilized Scielo and Pubmed databases and the selected clinical examinations were obtained by the book Exame Clinico - 8° edition. Results: Multiple Systems Atrophy is distinguished from Parkinson by occurrence of cerebelar abnormalities, therefore Romberg Test can evidence modified coordination, which may be indicative of Multiple System Atrophy. Corticobasal Degeneration causes loss of ability to identify things by touch and impaired sensitivity on one side of the body, thereby the verification of stereognosia and the examination of superficial sensitivity are useful. Supranuclear Paralysis Progressive generates difficulty of performing vertical movements, thus the examination of ocular motility is necessary. Conclusion: Only clinical examinations aren’t sufficient to generate an accurate diagnosis and complementary exams are necessary for greater precision. However, knowledge about differentiating clinical examinations helps to generate a line of reasoning and examinations to be requested.

Unforced errors are a significant issue in producing high performance in tennis. Identifying the causes of these errors in important to guide interventions to reduce unforced errors. The purpose of this study was to examine the different causes of unforced errors (UE) of Women’s Collegiate tennis players from the perspectives of coaches and players. Specifically, and based on previous research (Hirata, Sato, Murakami, Sato, & Saijo, in press; Shibahara, Tamaki, Hirata, Sonobe, Morii, & Saijo, 2015), a measure was developed to collect data on UE. The factor structure was examined using data collected from 283 Collegiate women tennis players and 77 coaches of women tennis players. These participants were divided into High-Skilled (HS) and Low- Skilled (LS), where the criteria for such selection was based in the competition level played in different Competitions. In order to access the HS competition, players needed to be winners of the LS competition. Comparing and contrasting both groups are in the basis of the current research study.
