ObjectiveDietary patterns and practices can predispose or protect against metabolic syndrome (MetS) in humans. Despite the growing problem of MetS in adults, the underpinning dietary behaviour is poorly understood. We determined the dietary patterns and practices relevant to MetS in adults with central obesity attending a mission hospital in Kenya.Study designDescriptive, cross-sectional.SettingOutpatient clinic of a mission-based hospital in Nairobi.ParticipantsAdults (N=404) aged 18–64 years diagnosed with central obesity as per the International Diabetes Federation definition for MetS.Primary outcomesAnthropometric measurements, clinical-biochemical markers and dietary components, quantity and frequency of food intake, as well as time-lapse between consumption of dinner and sleeping.ResultsA high (87.2%) prevalence of MetS was observed for respondents who reported consumption of large amount of carbohydrates (p<0.001), proteins (p<0.001), processed/fast foods (p<0.001) and sugar (p=0.009). Frequent consumption of legumes (p<0.001), nuts (p<0.001), fruits (p<0.001) and vegetables (p=0.021) was linked to reduced MetS. Additionally, longer interval between eating dinner and going to bed was associated with reduced MetS.ConclusionRegular consumption of fruits, vegetables, legumes and nuts, as well as observing sometime after eating dinner before sleeping, was the dietary pattern significantly associated with a lower risk of MetS. Whereas, consumption of a large quantity of carbohydrates, proteins, processed/fast foods and sugar is likely to predispose to MetS. The findings underscore the need to focus on specific dietary intake patterns including frequency, quantity, quality and variety for MetS prevention and management. The MetS-related interventions could be implemented during individual consultation, group and community health messaging sessions.

Although metabolic syndrome (MetS) could be handled by lifestyle interventions, its relationship with dietary patterns remains unclear in populations from Central Europe. Using data from the Kardiovize Brno cohort, the present study aims to identify the main dietary patterns and to evaluate their association with MetS risk in a random urban sample from Brno, Czech Republic. In a cross-sectional study of 1934 subjects aged 25–65 years (44.3% male), dietary patterns were derived by food frequency questionnaire (FFQ) administration and principal component analysis. Metabolic syndrome was defined according to the International Diabetes Federation statement. Logistic regression models were applied. High adherence to the prudent dietary pattern was associated with lower odds of abdominal obesity, abnormal glucose concentration, and MetS. By contrast, high adherence to the western dietary pattern was associated with higher odds of abnormal glucose, triglycerides and blood pressure levels. Whilst our results confirm the deleterious effect of a western dietary pattern on several metabolic risk factors, they also indicate that the consumption of a diet rich in cereals, fish, fruit and vegetables is associated with a healthier metabolic profile. However, further prospective research is warranted to develop and validate novel potential preventive strategies against MetS and its complications.

Background: Urbanization, surplus energy uptake, decreased physical activities are general risk factors of metabolic syndrome However, it’s status, and associated components remain unexplored in the Terai region of Nepal. This study evaluated the prevalence of metabolic syndrome and its components among adults with central obesity of Terai region of Nepal using International Diabetes Federation criteria.Methods: Community based cross-sectional study was conducted in three Terai districts of Janakpur Zone, Nepal. A total of 378 adults having central obesity were selected using cluster sampling by camp approach. Interview, physical and clinical examination, measurement of fasting blood sugar, and lipid profile were conducted for all participants. The prevalence of metabolic syndrome and its components with 95% CI were estimated.Results: The metabolic syndrome prevalence was 74.9% (95% CI:70.2-79.2%), with no significant differences between male (77.7%, 95% CI:71.0-83.5%) and female (72.2%, 95% CI: 65.2-78.3%). The most common factors observed were low high density lipoproteins with highly significant differences between male (77.7%, 95% CI:71.0-83.5%)) and female (90.2%, 95% CI: 85.094.0%-; p=0.001) and hypertriglyceridemia with significant differences between male (57.6%, 95% CI: 50.1-64.5%) and female (46.9%, 95% CI: 39.7-54.2%; p=0.037). Conclusions: Higher prevalence of metabolic syndrome and its risk factors in Janakpur of Nepal likely suggest lack of awareness and health promotion activities for metabolic syndrome and indicate an urgency for a public health program to maintain quality of life. Objective. There is limited data comparing prothrombogenic or fibrinolysis biomarkers (tissue plasminogen activator (tPA) and plasminogen activator inhibitor-1 (PAI-1)) simultaneously in subjects with Metabolic Syndrome (MS), simple central obesity without MS (COB) and normal controls (NC). We investigated the concentrations of fibrinolysis biomarkers in subjects with MS, COB and NC.Methods. A cross-sectional study involving 503 drug naive subjects (163 males, aged 30–65 years old (mean age ± SD = 47.4 ± 8.3 years)) divided into MS, COB and NC groups. COB was defined as central obesity (waist circumference (WC) males ≥90 cm, females ≥80 cm) in the absence of MS according to the International Diabetes Federation 2006. Fasting blood levels of tPA and PAI-1were analyzed.Results. MS and COB had significantly higher concentration of all biomarkers compared to NC. The MS group had significantly higher concentration of tPA and PAI-1 compared to COB. WC and HDL-c had significant correlation with all biomarkers (tPAp<0.001, PAI-1p<0.001). Fasting plasma glucose and diastolic blood pressure were independent predictors after correcting for confounding factors.Conclusion. Central obesity with or without MS both demonstrated enhanced prothrombogenesis. This suggests that simple obesity possibly increases the risk of coronary artery disease in part, via increased susceptibility to thrombogenesis.

Introduction: The global prevalence of metabolic syndrome (MS) among people living with HIV/AIDS varies from 20% to 33%. Objective: to estimate the prevalence of metabolic syndrome and associated factors in a group of HIV-infected patients on antiretroviral therapy. Methods: This is a cross-sectional study with HIV-infected patients from a reference center in Bahia, Brazil. We evaluated clinical, socio-demographic and anthropometric data. MS was defined according to the guidelines of International Diabetes Federation. Results: We evaluated 152 patients with mean age of 47.3±11.6 years, 59.2% male. The main comorbidities detected were diabetes (3.3%) hypertriglyceridemia (9.3%) and metabolic syndrome (MS,38.2%). Patients with MS were predominantly women (55.2% vs 31.9%; p=0.005), older [52.1 (10.4) vs 44.3 (11.3); p<0.001], and had overweight (74.1% vs 23.4%; p<0.001). After multivariate analysis MS remained associated with age (OR = 1.076; 95% CI: 1.030 – 1.125), female sex (OR = 2.452; 95% CI: 1.114 – 5.374) and family history of hypertension (OR = 3.678; 95% CI: 1.431 – 9.395). Conclusion: Almost half of the HIV-infected patients in Bahia presents with MS which seems to be driven by classical risk factors.

Introduction: Metabolic syndrome (MetS) is a multifaceted syndrome comprises a cluster of metabolic abnormalities including hypertension, central obesity, insulin resistance, dyslipidemia, and is strongly associated with developing diabetes mellitus and cardiovascular diseases. Prevalence of MetS differs in different ethnic population by using different definition. Objectives: To determine the prevalence of the MetS and its component risk factors among Border Guard Bangladesh (BGB) personnel working in Chattagram Hill Tract (CHT). Materials & Methods: This descriptive cross-sectional study was conducted from October 2016 to February 2018 among 1455 male BGB personnel aged between 40 to 59 years and working in CHT of Bangladesh. Study subjects were selected at BGB hospital, Guimara, Khagrachari. The new International Diabetes Federation (IDF) guidelines were used to identify MetS. Detail history, clinical examination, anthropometry and blood pressure (BP) were measured and fasting plasma glucose (FPG) and lipid profile were assessed. Results: Among the 1455 respondents’ BMI, waist circumference, systolic and diastolic BP were found more than normal in 34.1, 37.7, 16.4% and 12.3% respectively. Biochemical parameters including FPG, TC, TG, and LDL-C was raised in 28.7%, 49.8%, 52.2% and 37.7% of respondents respectively. HDL-C was found decreased in 26.9% respondents. Age wise prevalence of MetS for age group 40-44, 45-49, 50-54 and 55- 59 years were found 21.3%, 21.7%, 24.1% and 26.1% of the respondents respectively. The overall prevalence of MetS was found 22.4%. Conclusion: The prevalence of the MetS in BGB personnel was found 22.4%. Appropriate measure should be taken to lower the MetS cases. Abstract Objectives Evaluate the association of dietary patterns in bank employees with the presence of metabolic syndrome (MetS), considering sociodemographic and behavioral factors as well as laboratory tests. Methods This was a cross-sectional study of 515 bankers in Brazil. Sociodemographic, occupational, behavioral, and food consumption data were collected. Dietary patterns were determined by principal component analysis with orthogonal varimax rotation. Results The dietary pattern of vegetables, fruits, cereals, and tubers was correlated with the presence of MetS and with waist circumference (WC) measurements and triglyceride (TG) levels. Individuals who adhered less well this pattern had a higher WC (93 ± 17 cm in the first quintile vs 87 ± 19 cm in the fifth quintile, P = 0.001; and 93 ± 17 cm in the first quintile vs 84 ± 18 cm in the third quintile, P = 0.019), as well as a higher level of blood TG (121 ± 71 mg/dL in the first quintile vs 111 ± 72 mg/dL in the fifth quintile, P = 0.003). Individuals in the third and fifth quintiles of the pattern “vegetables, fruits, cereals, and tubers” presented with 3.28 and 2.24 times less chances of MetS when compared to individuals in the first quintile of this dietary pattern (OR 0.30, 95% CI 0.13–0.67, and OR 0.44, 95% CI 0.21–0.92, respectively). Subjects over 45 years of age were almost twice as likely to develop MetS (OR 1.95, 95% CI 1.01–3.77). Conclusions Healthy eating represented by the dietary pattern “vegetables, fruits, cereals, and tubers” was associated with better health among bank employees, especially when evaluating competing metabolic complications such as MetS. The prevalence of metabolic syndrome (MetS) and its associated risks remain unappreciated in Bamboutos Division, west region of Cameroon. This study aimed to evaluate the prevalence of MetS, its individual components, and associated risk factors among Bamboutos Division’s adults population using a Joint Interim Statement of the International Diabetes Federation (IDF) Task Force on Epidemiology and Prevention definitions parameters. A cross-sectional study was conducted from May 2016 to May 2018 in Mbouda ADLUCEM Hospital and Mbouda District Hospital, two reference hospitals in Bamboutos Division, west region of Cameroon. Interview, physical and clinical examinations, and lipid and fasting blood glucose measurements were conducted for 604 adults. The definition of MetS proposed by IDF was used. The prevalence of MetS was 32.45% with highly significant female predominance (46.11% for females and 14.01 % for males). In the entire participants, the most common abnormalities were low-HDL (82.78%) and hypertriglyceridemia (53.97%) [p<0.001]. Participants with obesity (OR: 16.34; 95% CI: 9.21-28.96), overweight (OR: 7.45; 95% CI: 4.17-13.30), and highest hs-CRP (hs-CRP >11 mg/l) had a higher risk of developing MetS. The most common MetS component was abdominal obesity (OR: 353.13; 95% CI: 136.16-915.81). MetS is prevalent among Bamboutos Division’s adults in west region of Cameroon and abdominal obesity is the most common MetS component. This study highlights the need for evidence-based prevention, diagnosis, and management of MetS and its associated factors among Bamboutos Division’s adults in Cameroon.

Purpose This paper aims to investigate whether the interaction between vitamin D receptor (VDR) FokI polymorphism and dietary patterns is associated with metabolic syndrome (MetS) and its components. Design/methodology/approach In total, 304 Iranians were enrolled in this cross-sectional study. Dietary patterns were identified using factor analysis. Fasting serum glucose and lipid profile were also assessed. FokI polymorphism of the VDR gene was genotyped using the restriction fragment length polymorphism method. Findings Individuals in third tertile of “Unhealthy Patterns had greater odds for MetS (odds ratio: 2.9; 95 per cent CI: 1.3, 6.1; P for trend = 0.03) compared to those in first tertile. Significant results disappeared after controlling for covariates (p = 0.09). There was no significant relationship between adherence to ‘Healthy Pattern’ and odds of MetS (p = 0.55). There were not any interactions between FokI polymorphism and major dietary patterns associated with MetS. Originality/value No evidence found for the interaction between polymorphism FokI and major dietary patterns associated with MetS and its components in Iranian subjects. Genome-wide association techniques are needed to assess the direct effect of this polymorphism on MetS.

Abstract Objective: Our objective was to assess a 10-yr change in the prevalence of the metabolic syndrome defined by the National Cholesterol Education Program (NCEP) and the International Diabetes Federation (IDF) among Finnish men and women. Design and Subjects: Two cross-sectional population surveys were performed in Finland in 1992 and 2002. A total of 3495 participants aged 45–64 yr were included in the analysis. Results: In both years the metabolic syndrome was more common among men than women. In men the prevalence of the metabolic syndrome tended to increase slightly between 1992 and 2002, from 48.8–52.6% (P = 0.139) based on the NCEP definition, and from 51.4–55.6% based on the IDF definition (P = 0.102). In women the prevalence of the metabolic syndrome increased significantly from 32.2–39.1% based on the NCEP definition (P = 0.003), and from 38.0–45.3% based on the IDF definition (P = 0.002). In both sexes the prevalence of high blood pressure decreased, but the abnormalities in glucose metabolism increased between 1992 and 2002. The prevalence of central obesity increased in women between 1992 and 2002. Conclusions: In Finland the prevalence of the metabolic syndrome, based both on the NCEP and IDF definitions, is higher in men than women. However, the increase in the prevalence of the metabolic syndrome, from 1992–2002, was significant only among women.

Abstract Metabolic syndrome is common in older adults and may be modified by the diet. The aim of this study was to examine associations between a posteriori dietary patterns and metabolic syndrome in an older New Zealand population. The REACH study (Researching Eating, Activity, and Cognitive Health) included 366 participants (65-74 years, 36% male) living independently in Auckland, New Zealand. Dietary data were collected using a 109-item food frequency questionnaire with demonstrated validity and reproducibility for assessing dietary patterns using principal component analysis. Metabolic syndrome was defined by the National Cholesterol Education Program Adult Treatment Panel III. Associations between dietary patterns and metabolic syndrome, adjusted for age, sex, index of multiple deprivation, physical activity, and energy intake were analysed using logistic regression analysis. Three dietary patterns explained 18% of dietary intake variation – ‘Mediterranean style’ (salad/leafy cruciferous/other vegetables, avocados/olives, alliums, nuts/seeds, shellfish and white/oily fish, berries), ‘prudent’ (dried/fresh/frozen legumes, soy-based foods, whole grains, carrots), and ‘Western’ (processed meat/fish, sauces/condiments, cakes/biscuits/puddings, meat pies/hot chips). No associations were seen between ‘Mediterranean style’ [OR=0.75 (95% CI 0.53, 1.06), P=0.11] or ‘prudent’ [OR=1.17 (95% CI 0.83, 1.59), P=0.35] patterns and metabolic syndrome after co-variate adjustment. The ‘Western’ pattern was positively associated with metabolic syndrome [OR=1.67 (95% CI 1.08, 2.63), P=0.02]. There was also a small association between an index of multiple deprivation [OR=1.04 (95% CI 1.02, 1.06), P<0.001] and metabolic syndrome. This cross-sectional study provides further support for a Western dietary pattern being a risk factor for metabolic syndrome in an older population.
