Zinc phosphide has been used widely as a rodenticide. Upon ingestion, it gets converted to phosphine gas in the body, which is subsequently absorbed into the bloodstream through the stomach and the intestines and gets captured by the liver and the lungs. Phosphine gas produces various metabolic and nonmetabolic toxic effects. Clinical symptoms are circulatory collapse, hypotension, shock symptoms, myocarditis, pericarditis, acute pulmonary edema, and congestive heart failure. In this case presentation, we aim to present the intensive care process and treatment resistance of a patient who ingested zinc phosphide for suicide purposes.

 Background : Congestive Heart Failure (CHF) is a physiological condition where the heart is unable to pump enough blood to fullfill the metabolic needs of the body that cause some clinical symptoms felt by clients such as dyspnea, orthopnea, paroxysmal nocturnal dyspnea (PND) shortness of breath at night day. The most effective position for clients with congestive heart failure disease is 450 degree slope semi Fowler position, using gravity to aid lung compliance and reduce the pressure of the abdominal diaphragm.  Aim: This study is to determine the effect of 450 semi Fowler bed position to increase the value of oxygen saturation in patients with congestive heart failure in dr. Loekmono Hadi Kudus General Hospital. The type of this research is to draft Experimental Pre-Pre and Post Test One Group Design. Conducted on 16 respondents with total sampling technique that met the inclusion criteria. Methods: The research data were analyzed using dependent t-test.   Result : The result showed the difference in median 2 L / s average 2%, which use oxygen 3 L / s average 1%, and that does not use oxygen to experience average increase of 1%. 450 semi Fowler bed position has an effect  to increase the value of oxygen saturation in patients with congestive heart failure. Conclution : The study recommends to give 450 semi Fowler bed position to patients with congestive heart failure with decreased oxygen saturation. Keywords: 450 semi Fowler bed position; CHF; SpO2              Latar Belakang : Congestive Heart Failure (CHF) merupakan suatu kondisi fisiologis ketika jantung tidak mampu memompa darah yang cukup untuk memenuhi kebutuhan metabolik tubuh yang menimbulkan beberapa gejala klinis yang dirasakan klien beberapa diantaranya dispnea, ortopnea, paroxysmal nocturnal dispnea (PND) sesak nafas pada malam hari. Posisi yang paling efektif bagi klien dengan penyakit gagal jantung kongestif adalah posisi semi Fowler dengan derajat kemiringan 450, yaitu dengan menggunakan gaya gravitasi untuk membantu pengembangan paru dan mengurangi tekanan dari abdomen pada diafragma. Tujuan : Penelitian ini untuk mengetahui pengaruh posisi tidur semi Fowler 450 terhadap kenaikan nilai saturasi oksigen pada pasien gagal jantung kongestif di RSUD dr. Loekmono Hadi Kudus Metode : Jenis penelitian ini adalah Pra – Experimental dengan rancangan Pre  and Post Test One Group Design. Dilakukan pada 16 responden dengan tehnik total sampling yang memenuhi kriteria inklusi. Data penelitian di analisa menggunakan uji dependent t – test. Hasil : Hasil penelitian didapatkan selisih median2 L/m rata – rata mengalami kenaikan 2%, yang menggunakan oksigen 3 L/m rata – rata mengalami kenaikan 1% dan yang tidak menggunakan oksigen mengalami rata – rata kenaikan 1%. Kesimpulan : Ada pengaruh posisi tidur semi Fowler 450 terhadap kenaikan nilai saturasi oksigen pada pasien gagal jantung kongestif. Penelitian ini merekomendasikan agar pasien gagal jantung kongestif dengan penurunan saturasi oksigen diberikan posisi tidur semi Fowler 450. Kata kunci : CHF; posisi semi Fowler 450; SpO2

<p><strong>Purpose</strong>: New onset supraventricular arrhythmias (SVA) are commonly reported in mixed intensive care settings. We sought to determine the incidence, risk factors and outcomes of new onset SVA in African American (AA) patients with severe sepsis admitted to medical intensive care unit (MICU).</p><p><strong>Methods:</strong> Patients admitted to MICU between January 2012 through December 2012 were studied. Patients with a previous history of arrhythmia or with new onset of ventricular arrhythmia were excluded. Data on risk factors, critical care interventions and outcomes were obtained.</p><p><strong>Results:</strong> One hundred and thirty-one patients were identified. New onset SVA occurred in 34 (26%) patients. Of those 34, 20 (59%) had atrial fibrillation (AF), 6 (18%) had atrial flutter and 8 (24%) had other forms of SVA. Compared with patients without SVA, patients with new onset SVA were older (69 ± 12 yrs vs 59 ± 13 yrs, P=.003), had congestive heart failure (47% vs 24%, P=.015) and dyslipidemia (41% vs 15%, P=.002). Additionally, they had a higher mean mortality prediction model (MPM II) score (65 ± 25 vs 49 ± 26, P=.001) and an increased incidence of respiratory failure (85% vs 55%, P=.001). Hospital mortality in patients with new onset SVA was 18 (53%) vs 30 (31%); P=.024; however, in a multivariate analysis, new onset SVA was associated with nonsignificantly increased odds (OR 2.58, 95% CI 0.86-8.05) for in-hospital mortality.</p><p><strong>Conclusion:</strong> New onset SVA was prevalent in AA patients with severe sepsis and occurred more frequently with advanced age, increased severity of illness, congestive heart failure, and acute respiratory failure; it was associated with higher unadjusted in hospital mortality. Background: Beta blockers, mainly propranalol, are usually administered to control heart rate in patients with thyrotoxicosis, especially when congestive heart failure presents. However, when thyrotoxicosis is not controlled, heart rate may be difficult to control even with maximal doses of propranolol. This presentation alerts physicians to the possibility of using ivabradine, a selective inhibitor of the sinoatrial pacemaker, for the control of heart rate. Case presentation: We present a 37-year-old woman with thyrotoxicosis and congestive heart failure whose heart rate was not controlled with a maximal dose of beta blockers during a thyroid storm. The addition of ivabradine, a selective inhibitor of the sinoatrial pacemaker, controlled her heart rate within 48 hours. Conclusion: Ivabradine should be considered in patients with thyrotoxicosis, including those with heart failure, in whom beta blockers are insufficient to control heart rate

Abstract Background and Aims The advanced congestive heart failure (CHF), may provoke functional kidney disturbances with insufficient and resistant to conservative therapy water excretion. The treatment of the chronic renal failure (CRF) with active dialysis related to ultrafiltration (UF), may overcome the renal diuretics resistance with diuresis restoration and oedema elimination. The aim of the study was to define the indications for active, discontinued UF in setting of the CHF refractory to drugs, especially in the treatment of oedema. Method We investigated 12 patients, mean age 62,4+/-5,3 years, with incipient renal failure, but advanced congestive heart failure. Seven of them were males and 5 females. The indication for discontinued UF, was the severe expressed heart failure, reduced diuresis and initial renal insufficiency. In all investigated patients, before and after each UF procedure, the serum sodium and potassium, blood urea nitrogen (BUN), creatinin and osmolality were detected. The body weight, abdominal and crural parameters were noted before and after dialysis. Results The recovery was achieved in 10 patients with CHF, but 2 patients out of 12 have not demonstrated satisfactory response to UF. The biochemical features encountered to CHF patients suggest chronic hyponatriemia, hypokalemia and hypovolemia. Proteinuria range 1,2 to 3,6 g/l, was present in 6 patients. The clinical data were performed with oedema formation, reduced diuresis and dispnea. Mean UF rate achieved after several dialysis was 12,4+/- 7,6, lit. Conclusion Chronic heart failure in chronic renal patients, with severe oedemas is an indication for UF therapy, even if the values of BUN and creatinin are not increased. Reduction of the body weight and the extracellular volume, contribute for improved survival in these patients. However the risk of complications is high and not always with successful treatment.

People with congestive heart failure (CHF) need a comprehensive rehabilitative program to restore physical abilities post-attacks and prevent re-attacks. Physical exercise in congestive heart failure (CHF) patients is aimed at optimizing the physical capacity of the body, providing counseling to patients and families in preventing worsening and helping patients to return to physical activity as before congestive heart failure (CHF). This study aims to explore in depth the experience of nurses doing physical exercise exercises in congetive heart failure patients. The research design used is descriptive phenomenology. Participants in this study amounted to 6 participants selected by purposive sampling technique. The data collected were in-depth interviews completed with field notes and then made in verbatim and analyzed using Colaizzi's approach. The research results found four themes, namely: the implementation of physical exercise exercise congestive heart failure patient is not optimal; increased knowledge of nurses and patients; the need for support from the hospital; and expectations for the exercise of physical activity. The results showed that things that could affect physical exercise activity in congestive heart failure patients were standard operating procedures, nurse knowledge, hospital support; and expectations for the exercise of physical activity. This study provides an understanding of the nurse's understanding of physical activity exercises so that nurses are able to provide appropriate nursing care.Keywords: experience, physical activity exercise, congetive heart failure

Abstract Background: Large intracardiac bronchogenic cysts are rare mediastinal masses, however they must always be considered in the differential diagnosis of heart failure. Case Presentation: We present a 60-year-old female patient with de novo atrial fibrillation and heart failure, resulting from an incidental large intrapericardial mass. The patient underwent successful surgical resection, with pathological findings confirming a bronchogenic cyst.Conclusions: Large bronchogenic cysts located intrapericardially are very rare, however they should be included in the differential diagnosis of patients presenting with atrial fibrillation and heart failure.
