This study aimed to evaluate the efficacy of passive ultrasonic irrigation (PUI) on dissolving the organic tissue inside simulated internal root resorption (IRR) using sodium hypochlorite (NaOCl) or chlorhexidine (CHX). A total of 40 human lower premolars were collected based on dimensional and morphological similarities. The roots were embedded in cylinders (3 cm diameter; 2.5 cm height) of self-cured acrylic resin, and then an IRR was simulated. The specimens were divided into 4 groups (n = 10) according to irrigation protocols: group 1: CHX + PUI; group 2: CHX; group 3: NaOCl + PUI; group 4: NaOCl. The total irrigation time was 150 s at a flow rate of 5 mL/min. A tissue mass of porcine palatine mucosa was used to simulate the organic tissue, it was weighed before and after the irrigation using an analytic balance, and the difference between both readings was calculated and transferred to percentage values. Data were submitted to statistical analysis using two-way ANOVA (factors: irrigant type and with/without PUI) and Tukey’s test for multiple comparisons among the experimental groups (α = 0.05). There was a significant difference in both factors (irrigant: p = 0.04 ; PUI: p ≤ 0.001 ). The groups that used PUI were more effective in dissolving the organic tissue of the IRR simulation than the groups without PUI. PUI is more effective than the syringe and needle irrigation in organic tissue dissolution.

The objective is to examine and compare the usability of two physiotherapy programmes, analyzing respiratory function in patients before and after cardiac surgery in hospital during seven postoperative days (POD). Quantitative randomized prospective study of 157 patients before and after the valve replacement surgery, coronary artery bypass graft and combined surgeries, who moved independently. Participants were randomized into two groups (1 and 2) with different physiotherapy programmes. The routine physiotherapy of breath-enhancing techniques, micro-circulation improvement were used for the first group, for the second group - modified physiotherapy – the improvement of inspiration muscles, mm. quadriceps. gluteus max strength. Anthropometric measurements were defined for both groups on the day before surgery and during spirography - dynamic indicators - forced vital capacity (FVC), forced expiratory volume in the 2nd second (FEV1), Tiffeneau index (FEV1 / VC (%)), peak expiratory flow (PEF), operation parameters. By comparing the postoperative respiratory parameters between the two groups and using independent samples t test, it was found out that the difference in FVC between groups is 1.71 [95% CI: −8.25 to 4.8] and it is not statistically significant (t (155) = −0.52; p = 0.60). Based on the Leuven test results FVC the distribution variance is not statistically notably different for group 1 and group 2 (F = 0.27, p=0.60). Using routine and modified physiotherapy and comparing postoperative dynamic respiratory performance of the two groups, there was no statistically significant difference, proving that the two models are equally effective.

Abstract Background This study aimed to evaluate the remaining amount of intra canal medicament in the root canals and their effect on microhardness of radicular dentine. In this study, ninety straight single-rooted teeth were used. Teeth were decoronated at the cemento-enamel junction to ensure a uniform root length of 14 mm (± 1 mm). Root canals were prepared using ProTaper rotary files till size F5. The teeth were then randomly assigned to 3 treatment groups (n = 30); Group 1: Ca(OH)2 pastes (Metapex), Group 2: Moringa oleifera leaf powder mixed with Moringa oleifera extract liquid and Group 3: Moringa oleifera root powder mixed with Moringa oleifera extract liquid. The access openings of all teeth were sealed with Cavit and all teeth were kept in the incubator at 37 °C and 100% humidity for 1 week. After 1 week, the intra canal medicament paste was removed from all teeth. For examination of the ease of removal of the intracanal medicaments, root canals were sectioned longitudinally into buccal and lingual halves and were examined under stereomicroscope at 25× magnification. The remaining amount of intracanal medicaments was calculated using image analysis software (Image j). For microhardness evaluation, forty-five specimens were longitudinally sectioned in a bucco-lingual direction then the roots were split. The root segments were then horizontally embedded in autopolymerizing acrylic resin leaving their dentin surface exposed. The dentin surface of the mounted specimens was ground flat and smooth and polished to obtain a smooth surface. Microhardness measurement was recorded for each sample at baseline before and after application of the three intracanal medications using Vickers Microhardness Tester at magnification of 100× using a 25 g load for 10 s. Results Regarding the evaluation of the ease of removal of the intracanal medicaments after irrigation: apically, the Ca(OH)2 recorded the highest mean value of the residual remnants with a statistically significant difference with both Moringa oleifera leaf and root. The microhardness evaluation results showed that all groups showed increase in the microhardness after application of the intracanal medicament for 1 week with no statistically significant difference between them. Conclusions Moringa oleifera could be removed easily from the root canals and showed increase in the microhardness of root canal dentin similar to the commonly used calcium hydroxide.

Aim. To evaluate the influence of combination of omega-3 polyunsaturated fatty acids and atorvastatin on the risk of atrial fibrillation (AF) after coronary bypass surgery (CBS). Material and methods. The study included 114 patients divided into 2 groups, one comprised of 59 ones (75,6% men of mean age 62,0±7,3 yr given conventional medication), the other including 55 patients (80,0% men of mean age 59,4±6,7 yr given omega-3 polyunsaturated fatty acids 5 days before (2 g/d) and during 3 weeks after CBS (1 g/d)). All patients were treated with atorvastatin at the outpatient stage. IL-6,8, 10 and C-reactive protein (CRP), fibrinogen, troponin, NT-proBNP, superoxide dismutase (SOD), and myeloperoxidase were measured at admittance and on day 3.7±1.4 after surgery. Results. AF developed on day 5.9±4.9 (mean) after surgery. Patients of group 2 tended to have fewer new episodes of arrhythmia although no significant difference between the groups was documented (9,1% vs 18,6%, р=0,12). Group 2 included more smokers (74,5% vs 45,8%, р=0,002) and patients with atherosclerosis of lower limb arteries (87,3% vs 71,2%, р=0,03) but fewer those consuming nitrates (39,0% vs 18,2%, р=0,01) and Ca antagonists (45,8% vs 21,8%, р=0,006).Mean dose of atorvastatin given to patients of groups 1 and 2 was 24.7±12.5 and 25.1±10.5 mg/d respectively (р=0,2), duration of its intake 14.6±12.7 and 21.5±19.3 months (р<0,001). There. was no significant difference between leukocyte count, leukogram, IL-8, IL-10, NT-proBNP, and troponin levels before and after CBS. Surgical myocardial revascularization caused a rise in leukocyte count, shifted the leukogram toward predominance of stab and segmented cells, increased IL-8, IL-10, fibrinogen, NT-proBNP, CRP, and troponin levels in both groups. IL-8 and IL-10 levels remained normal before and after surgery. Preoperative IL-6 level in group 1 was significantly higher than in group 2 (21,7±13,0 vs 2.5±2,2 pg/ml, р<0,001). Postoperatively, the difference was absent. The CRP level before surgery was high in both groups (3122.7±2175.8 vs 3670.8±2490.0 U/g) but decreased after CBS although remained higher in group 1 (1957.6±1660.3 vs 1069.8±630.2 U/g, р<0,001). Myeloperoxidase level increased postoperatively in both groups but the difference was insignificant. Fibrinogen and CRP in the postoperative period increased more significantly in group 2 than in group 1 (4,9±1,4 vs 4,4±1,1 g/l, р=0,02 and 8,6±2,2 vs 5,4±2,3 mg/l respectively, р<0,001). Conclusion. The study revealed an insignificant decrease in the number of AF episodes in the early post-CDS period in patients treated with omega-3 polyunsaturated fatty acids and atorvastatin compared with those given the latter medication alone. Also, fibrinogen and CRP levels as markers of inflammation increased while SOD antioxidative activity decreased.

The present study aimed to investigate the efficiency of passive ultrasonic irrigation (PUI), EndoActivator (EA), standard needle irrigation (SNI), and XP-endo Finisher files in removing pure calcium hydroxide (pCH) and injectable CH from in vitro root resorption cavities. Using a rotary system, the root canals of 116 extracted single rooted teeth were prepared. Imitated internal resorption cavities were then created in root halves. The specimens were divided into two groups according to the form of CH (n = 58): (I) pCH; (II) injectable CH. The teeth of each group were randomly divided into six subgroups: negative control (n = 5), positive control (n = 5), PUI (n = 12), XP-endo Finisher (n = 12), EA (n = 12), and SNI (n = 12). The root canals were irrigated using NaOCl and EDTA and split longitudinally, and both halves were evaluated with a stereomicroscope. Kruskal–Wallis and Mann–Whitney U tests were used to analyze data. The present results revealed that PUI completely removed pCH in 79% and injectable CH in 70.8% of the internal resorption cavities which was significantly higher than other methods (p<0.05). There was no statically significant difference between different forms of CH in terms of CH removal (p=0.918). The PUI technique was observed as the most efficient method of P-CH and injectable CH removal from a replicated internal resorption cavity. Finally, according to the findings, different forms of CH were comparable in terms of CH removal.

Abstract Background In this research the biomechanical properties of a bone model was examined. Porcine ribs are used as experimental model. The objective of this research was to investigate and compare the biomechanical properties of the bone model before and after implant placement. Methods The bone samples were divided in three groups, Group 1 where ALL-ON-FOUR protocol was used during pre-drilling and placing the implants, Group 2 where ALL-ON-FOUR protocol was used during pre-drilling, and implants were not placed, and Group 3 consisting of intact bones served as a control group. Static and dynamic loading was applied for examining the model samples. Kruskal–Wallis statistical test and as a post-hoc test Mann–Whitney U test was performed to analyze experimental results. Results According to the results of the static loading, there was no significant difference between the implanted and original ribs, however, the toughness values of the bones decreased largely on account of predrilling the bones. The analysis of dynamic fatigue measurements by Kruskal–Wallis test showed significant differences between the intact and predrilled bones. Conclusion The pre-drilled bone was much weaker in both static and dynamic tests than the natural or implanted specimens. According to the results of the dynamic tests and after a certain loading cycle the implanted samples behaved the same way as the control samples, which suggests that implantation have stabilized the skeletal bone structure.

The aim of the present study was to investigate the clinical outcomes of two different standardized endodontic irrigation protocols. It was assumed that the additional use of ethylenediaminetetraacetate (EDTA) and passive ultrasonic irrigation (PUI) would result in an increased rate of absence of symptoms and remission based on the periapical index (PAI) compared to passive irrigation using only sodium hypochlorite (NaOCl). Data and radiographs from 199 teeth retrieved from the institutional endodontic database were analyzed retrospectively. In 106 teeth irrigation was performed using only NaOCl (protocol 1). Ninety-three teeth were irrigated using NaOCl and EDTA (protocol 2). Chlorhexidine (CHX) was additionally used in revision treatments in both groups. All irrigants in group 2 were activated by PUI. Mean follow-up periods were: protocol 1 = 9.2 ± 4.4 and protocol 2 = 6.6 ± 2.5 months (p < 0.0001 (chi-square test). The frequencies of the PAImasterpoint and PAIfollow-up scores did not differ significantly between teeth, which received either protocol 1 or 2 (p = 0.555 and 0.138). Statistical analysis revealed no significant association between treatment success (absence of clinical symptoms and PAIfollow-up = I or PAImasterpoint > PAIfollow-up > I) and the applied protocol (success rates: protocol 1 = 72.6% vs. protocol 2 = 82.8%; p = 0.203). Furthermore, the frequency of extractions did not differ significantly between the two protocols (p = 0.102). No association was found between follow-up time and treatment success (p = 0.888). The hypothesis was not confirmed. Even though the obtained success rate was higher after supplementing the irrigation protocol with EDTA and PUI, no significance was recorded. Hence, protocol 2 was not superior to protocol 1 regarding therapy success, at least within the limited follow-up period. It may be cautiously concluded that sufficient mechanical debridement combined with passive NaOCl irrigation results in comparably high success rates compared to EDTA and PUI.

This study evaluated the influence of apical patency, root filling removal technique and cleaning of the apical foramen, concerning the amount of debris extruded during root canal retreatment. Forty mandibular incisors were randomly assigned to 4 groups - GIM (n=10), GIIM (n=10), GIPT (n=10) and GIIPT (n=10), which were named according to leaving (I) or not (II) apical patency during canal preparation and filling removal technique (manual - M or ProTaper system - PT). After filling material removal, each specimen of each group had the apical foramen cleaned by sizes 15, 20 and 25 instruments, generating 12 subgroups: GIM15, GIM20, GIM25, GIIM15, GIIM20, GIIM25, GIPT15, GIPT20, GIPT25, GIIPT15, GIIPT20 and GIIPT25. Extruded filling debris was collected by a Milipore filtration system, an HV-durapore, 0.45 µm pore filter with a 25 mm diameter. The filters were weighed before and after the collection on an analytical scale (10–5 g), and the difference was calculated. The mean weight of extruded filling debris was analyzed statistically by Kruskal-Wallis and Friedman ANOVA tests (α=0.05). The mean values found in the groups (in mg) were: GIM (0.95±0.94), GIIM (0.47±0.62), GIPT (0.30±0.31) and GIIPT (0.32±0.44). There was no statistically significant difference among any of the groups or subgroups (p>0.05). ProTaper provided the smallest amount of extruded filling material, regardless of presence or absence of apical patency, followed by manual technique, without and with apical patency. Additional amounts of debris were collected during cleaning of the apical foramen, regardless of the instrument, presence/absence of patency or root filling removal technique.
