Nosocomial infections or hospital-acquired infections are infections that potentially occur in the patients under medical care. These infections are often caused by multidrug-resistant pathogens acquired via improper antibiotic use, not following infection control and prevention procedures. The main objective of this study was to investigate the contribution of medical wards contamination to wound infection and antibiotics susceptibility patterns at Ruhengeri Referral Hospital, Musanze district, Rwanda. This was a cross-sectional study where a total of 61 samples including air sampling to evaluate the contamination by airborne bacteria, working surface, equipment, and patients’ surgical wounds swabs were collected in intensive care unit (ICU), pediatrics, and surgery departments. Culture, Gram stain, and biochemical tests were performed for microbiological isolation and identification. Antibiotic susceptibility testing was performed using the Kirby–Bauer disc diffusion method. Statistical Package for Social Science (SPSS) version 22 was used for data analysis. Gram-negative bacteria were frequently from surgery, pediatric, and ICU with 68.8%, 63.9%, and 31.1%, respectively, while Gram-positive isolates were 37.7% in surgery, 32.9% in pediatric, and 18.0% in ICU. There was a statistically significant association with E. coli and swabbed materials and surgical wound sites (x2 = 10.0253, P value = 0.018). All bacterial contaminants were sensitive to clindamycin and erythromycin. Pseudomonas aeruginosa, E. coli, and S. aureus were resistant to nitrofurantoin. Hospital environment could be a contributing factor to surgical wound site infections. Hospitals should apply preventive measures in the hospital environment surrounding wound surgery patients to prevent wound infections during hospital stay.

A cross-sectional study was conducted at National Avian Disease Investigation Laboratory, Chitwan to determine antibiogram of Escherichia coli isolated from avian colibacillosis cases of broilers and layers in Chitwan. One hundred and sixty (95 from broilers and 65 from layers) liver samples were collected aseptically during postmortem. Samples were taken purposively from dead birds showing lesions perihepatitis, pericarditis, air-saculitis, omphalitis and egg peritonitis. Isolation and identification were made by examination of cultural characteristics of E. coli in MacConkey’s agar, Eosin methylene blue (EMB) agar, Gram’s staining and biochemical tests. Antibiogram of identified E. coli isolate was evaluated against six antibiotics of six different groups by disk diffusion method following CLSI guidelines. One hundred and three E. coliisolates (73 from broilers and 30 from layers) were isolated from one hundred and sixty samples. Highest resistance was observed against Ampicillin (100%) followed by Co-trimoxazole (86.40%), Doxycycline (46.60%), Levofloxacin (45.63%), Nitrofurantoin (26.21%) and Amikacin (10.68%). Nearly about all (96.12%) isolates from 103 isolated E. coli isolates showed multidrugs resistance to two or more than two antimicrobials. All multidrug resistance isolates showed 16 different patterns with each isolate being resistance to at least two drugs. The multiple antibiotic resistance indexing ranged from 0.2 to 0.8 and proportion of isolates with MAR index greater than 0.2 was 96.12%. Int. J. Appl. Sci. Biotechnol. Vol 8(1): 52-60

In the present study, Fifty random samples of each small-scale (plain yoghurt and kariesh cheese) were collected randomly from different dairy shops and markets in Zagazig city for isolation and identification of Escherichia coli which is considered a good indicator of faecal contamination and a major cause of food poisoning. Identification was done microscopically and biochemically by different biochemical tests (IMVIC).The incidence of E. coli in yoghurt and kariesh cheese samples were 36% and 50%, respectively. Also, the serological identification of E. coli isolates revealed that O26 is the most predominant serogroup by percentagesof27.77% and 28% in both yoghurt and kariesh cheese samples, respectively. Because of its rising resistance to antibiotics, E.coli represents a real threat to human health. Antimicrobial susceptibility testing (AST) was done by disc diffusion method against 10 antimicrobials and the results revealed that E. coli isolates were highly resistant to amoxicillin-clavulanate, ampicillin, Cefotaxime and Ceftazidime, and were highly sensitive to chloramphenicol, Ciprofloxacin and tetracycline. In addition, 83.72%of E. coli isolates showed multi drug resistance (MDR).Bacterial adhesion to food surfaces and the formation of biofilmisa source of food contamination that has an impact on food safety and industry. Micro titer plate assay was used for testing biofilm formation and revealed that 69.77% of E. coli isolates were non-biofilm producers, 9.30% were weak biofilm producers, 20.93%were moderate biofilm producers and none of isolates was strong biofilm producers. Virulence genes of E. coli isolates were identified and characterized by a multiplex PCR assay. The results showed that among the target genes, stx1was most frequently detected by a percentage of81.82%

Milk contains many nutrients including carbohydrate, protein, fat, vitamins and minerals. Milk protein has high nutritional value because it contains all the essential amino acids. Considering the nutritive value, milk is widely consumed by the people as an ideal food. The biochemical changes in milk and milk products by microorganisms can be either desirable or undesirable. The safety of milk and milk products with respect to food borne diseases is of great concern around the world. Therefore, the present study was undertaken to determine the prevalence of Escherichia coli in milk and milk product with their antibiogram assay. A total of 150 milk and milk product (yogurt) samples were collected from Rajshahi Metropolitan area of Bangladesh and analyzed by cultural, staining and biochemical tests for the isolation and identification of E. coli. Antibiogram assay of all the isolates were done by disk diffusion method. The overall prevalence of E. coli was 20.0% in milk and milk product (yogurt) in Rajshahi Metropolitan area. The prevalence of E. coli was 26.0% and 34.0% in raw milk and in milk product (yogurt), respectively. E. coli was not detected in pasteurized milk in this study. In antibiogram assay, isolated E. coli showed 100.0%, 60.0%, 40.0%, 40.0%, 33.3%, 33.3%, 20.0%, and 10.0% resistance to penicillin, gentamycin, ampicillin, streptomycin, amoxycillin, sulfamethoxasole-trimethoprim, nalidixic acid, and ciprofloxacin, respectively. The isolates also showed 73.3%, 60.0%, 53.3%, 53.3%, 30.0%, 23.3%, and 20% sensitive to ciprofloxacin, nalidixic acid, sulfamethoxasole-trimethoprim, streptomycin, amoxycillin, ampicillin, and gentamycin, respectively. The findings of this experiment speculated that the use of ciprofloxacin and nalidixic acid may have the preference in the clinical control of milk borne E. coli infection in Bangladesh. J. Fecal contamination of source waters and its associated intestinal pathogens continues to pose risks to public health although the extent and effect of microbial contamination of source waters gets very little attention in designing treatment plants in most developing countries. Coliform counts give an indication of the overall bacterial contamination of water and thus its safety for human consumption. However, their presence fails to provide information about the source of fecal contamination which is vital to managing fecal contamination problems in surface waters. This study explored the use of multiple antibiotic resistance (MAR) indexing as means of differentiating E. coli isolates from different sources. A total of 322 E. coli isolates were obtained from municipal wastewater and from fecal samples from domestic and wild animals. Conventional culture methods and standard chemical and biochemical tests were used for isolation and identification of E. coli. Isolates were assayed against 10 antibiotics using the micro-dilution technique. The results obtained generated antibiotic resistance profiles which were used to statistically group the isolates into different subsets. Correct source classification was obtained for 60% of human-derived and 95% non-human-derived E. coli respectively. These results indicate the validity of the usefulness of MAR indexing as a method of bacterial source tracking.

Background: Many factors affect the incidence of surgical wound infection, in addition to the surgeon’s skill and the hospital environment. Host attributes, such as age over 60 years, diabetes mellitus, malignant disease, obesity, malnutrition, length of preoperative stay or pre-existing infection may influence risk, as may such operation characteristics as site, urgency, duration and time of skin shaving. Objective was to study the preoperative, intra-operative and postoperative factors responsible for postoperative wound infection.Methods: This descriptive study was designed to study the problem of postoperative wound infection at tertiary health care center at rural set up over a period of two years during 2014 to 2016. Initial assessment of intra operative findings divided these cases into clean, clean contaminated and contaminated cases.Results: As the length of pre-operative stay increased, the occurrence of SSIs increased. As the duration of post operative hospital stay increased, the occurrence of SSIs also increased. The occurrence of SSI increased as the quality of surgical wound deteriorated. As the duration of surgery increased, the occurrence of SSIs increased. It was found that the order of surgery was not related to occurrence of SSIs. It was found that the rate of SSI was more (21.55%) when the drain was used in comparison to only 8.04% when the drain was not used. The most common organism found to cause SSIs was staphylococcus aureus in 33.07% of cases.Conclusions: Slightly low incidence of SSIs in our study may be attributed to the better infection control practices though it must be concluded that more stringent aseptic measures including rational antibiotic policy will be contributory in lowering the SSI rate further.

Background. Salmonella has been recognized as a major cause of food-borne illness associated with the consumption of food of animal origin. The present cross-sectional study was conducted from December 2017 to May 2018 in Ambo and Holeta towns to assess the prevalence, risk factors, and antimicrobial susceptibility patterns of nontyphoidal Salmonella isolates from raw beef samples from abattoirs, butchers, and restaurants in Ambo and Holeta towns, Oromia region, Ethiopia. Methods. A total of 354 beef samples were collected from abattoirs, butchers, and restaurants. Salmonella isolation and identification were carried out using standard bacteriological methods recommended by the International Organization for Standardization. Antimicrobial susceptibility testing was performed using the disk diffusion method. Besides, a structured questionnaire was used to collect sociodemographic data and potential risk factors for contamination of meat. Chi-square tests and logistic regression were used for data analyses. Results. Of the total 354 meat samples examined, 20 (5.7%, 95% confidence interval (CI): 3.5–8.6) were positive for Salmonella. Two serotypes belonging to S. typhimurium (11 isolates) and I:4,5,12: i:- (9 isolates) were identified. The Salmonella detection rate in abattoirs, butchers, and restaurants was 4.2% (5/118), 8.5% (10/118), and 4.2% (5/118), respectively. The antimicrobial susceptibility test showed that 40%, 30%, and 20% of the Salmonella isolates were resistant to azithromycin, amoxicillin, and ceftriaxone, respectively. The odds of Salmonella isolation when meat handlers are illiterate were 7.8 times higher than those when they are educated to the level of secondary and above ( P = 0.032 ). Similarly, the likelihood of Salmonella isolation was 6.3 and 7.6 times higher among workers of butcher and restaurants, respectively, who had no training ( P = 0.003 ) and no knowledge ( P = 0.010 ) on food safety and hygiene. Conclusions. The study showed widespread multidrug-resistant Salmonella isolates in the study areas. Therefore, raw meat consumption and indiscriminate use of antimicrobial drugs should be discouraged. Provision of food safety education for meat handlers and further surveillance of antimicrobial-resistant isolates are suggested.

Background: Emergence of multi-drug resistant uropathogenic E. coli strains is an increasing problem to empirical treatment of urinary tract infections in many countries. This study investigated the magnitude of this problem in Jordan. Methods: A total of 262 E. coli isolates were recovered from urine samples of Jordanian patients which were suspected to have urinary tract infections (UTIs). All isolates were primarily identified by routine biochemical tests and tested for antimicrobial susceptibility by disc diffusion method. Fifty representative Multidrug Resistance (MDR) E. coli isolates to 3 or more antibiotic classes were tested for the presence of resistance genes of blaCTX-M- 1, 9 and 15, carbapenemase (blaIMP, blaVIM, blaNDM-1, blaOXA-48), fluoroquinolones mutated genes (parC and gyrA) and clone of ST131 type using PCR methods. Results: A total of 150/262 (57.3%) of E. coli isolates were MDR. Urine samples of hospitalized patients showed significantly more MDR isolates than outpatients. Fifty representative MDR E. coli isolates indicated the following molecular characteristics: All were positive for mutated parC gene and gyrA and for ST131 clone, and 78% were positive for genes of CTX-M-15, 76% for CTX-M-I and for 8% CTX-M-9, respectively. Additionally, all 50 MDR E. coli isolates were negative for carbapenemase genes (blaIMP, blaVIM, blaNDM-1, blaOXA-48), except of one isolate was positive for blaKPC-2 . Conclusion: This study indicates alarming high rates recovery of MDR uropathogenic E. coli from Jordanian patients associated with high rates of positive ST131 clone, fluoroquinolone resistant and important types of blaCTX-M.

Aim: The present study was carried out to determine the prevalence and molecular detection of fluoroquinolone-resistant Escherichia coli carrying qnrA and qnrS genes in healthy broiler chickens in Mymensingh, Bangladesh, and also to identify the genes responsible for such resistance. Materials and Methods: A total of 65 cloacal swabs were collected from apparently healthy chickens of 0-14 days (n=23) and 15-35 days (n=42) old. The samples were cultured onto Eosin Methylene Blue Agar, and the isolation and identification of the E. coli were performed based on morphology, cultural, staining, and biochemical properties followed by polymerase chain reaction (PCR) targeting E. coli 16S rRNA genes. The isolates were subjected to antimicrobial susceptibility test against five commonly used antibiotics under fluoroquinolone (quinolone) group, namely gatifloxacin, levofloxacin, moxifloxacin, ofloxacin, and pefloxacin by disk diffusion method. Detection of qnrA and qnrS genes was performed by PCR. Results: Among the 65 cloacal samples, 54 (83.08%) were found to be positive for E. coli. Antibiotic sensitivity test revealed that, of these 54 isolates, 18 (33.33%) were found to be resistant to at least one fluoroquinolone antibiotic. The highest resistance was observed against pefloxacin (61.11%). By PCR, of 18 E. coli resistant to fluoroquinolone, 13 (72.22%) were found to be positive for the presence of qnrS. None of the isolates were found positive for qnrA. Conclusion: Fluoroquinolone-resistant E. coli harboring qnrS genes is highly prevalent in apparently healthy broiler chickens and possesses a potential threat to human health.

Abstract Background: Worldwide, food borne illness is often associated with consumption of meat and poultry products sold at retail markets. A study on the bacteriological status of chicken carcass in Osogbo, Nigeria, was carried out to determine the prevalence of Arcobacter species, Escherichia coli and Staphylococcus aureus in chicken carcasses. Methodology: A total of 100 samples of chicken carcasses were collected from two major processing points in Osogbo, Nigeria. The samples were analysed for the presence of bacterial contaminants using standard microbiological isolation and identification procedures, with antimicrobial susceptibility test performed using the disk diffusion method. Result: Of hundred chicken carcasses sampled, 38% were positive for Arcobacter species and E. coli while 60% accounted for S. aureus isolates. Ninety percent of Arcobacter spp isolates were susceptible to ciprofloxacin, 85% to gentamicin, and pefloxacin, 70% to chloramphenicol and 90% were resistant to amoxicillin, 85% to augumentin and 80% to streptomycin. Hundred percent of E. coli isolates were susceptible to ciprofloxacin, pefloxacin, 95% to gentamicin and 100% were resistant to streptomycin, 85 % resistant to amoxicillin, augumentin, while 100% of S. aureus isolates were susceptible to trimethoprim sulphamethoxazole, 90% susceptible to gentamicin, 80% to streptomycin and 100% of the S.aureus isolates were resistant to ampliclox. Conclusion: The bacteriological status of chicken carcass revealed high contamination with Arcobacter, E coli and S. aureus with varying degree of antibiotic resistance therefore, improvement in meat processing procedures and strict hygiene measures towards reduction of these pathogens in food products should be encouraged.

Introduction: Patients with malignancies frequently develop infections as a result of surgical procedures and fungating wounds leading to pus formation. This cross-sectional study was conducted to explore the bacteriological spectra of infections of various cancer sites and their antibiotic sensitivity patterns among the patients visiting minor operation theatre (OT) of B.P. Koirala Memorial Cancer Hospital (BPKMCH), Chitwan, Nepal. Methods: Over a period of 3 months from September to November 2018, a total of 183 wound exudates and pus samples were collected and analyzed by standard microbiological procedures. Isolates were identified based on the colony characters, Gram staining and an array of biochemical tests. Antibiotic susceptibility testing was performed by Kirby-Bauer disc diffusion technique according to criteria set by CLSI, 2016. Methicillin resistance in Staphylococcus aureus was tested with the help of cefoxitin using disc diffusion method. Results: Out of the 183 samples, 149 (81.4%) were culture positive. Among 13 different isolates identified, S. aureus (43.0%) was predominant followed by E. coli (14.0%). Higher incidence of bacteria was seen among the males (52.3%), in the age group 51 to 60 years (26.8%) and among the patients undergoing surgical intervention to deal with cancer (34.2%). The prevalence of wound infection was significantly affected by gender, age, and treatment regimen ( P < .01). Out of the total 68 S. aureus isolates, 38 (44.1%) were deemed as Methicillin-resistant Staphylococcus aureus (MRSA). Among the 158 isolates, 85 (53.8%) were multi-drug resistant (MDR). Cefepime was the most effective antibiotic for Gram positive isolates whereas both imipenem and meropenem were found to be equally more effective for Gram negative isolates. Conclusion: This study suggests that patients with malignancies harbor pathogenic bacteria; therefore, prudent use of antibiotics is essential to prevent the emergence of MDR pathogens.
