Summary. Today it is known that among maxillodental abnormalities crowded teeth have a leading position and often accompanied by periodontal diseases, lesions of the hard dental tissues, functional disorders of maxillodental system and psycho-emotional disorders of patients is a relevant problem in dentistry. The researchers ascertained the fact of increasing the activity of the course of periodontal disease in patients with maxillodental abnormalities. The aim of the study – to estimate the intensification of inflammatory processes in periodontal tissues of young individuals with crowding of the teeth using periodontal indices.Materials and Methods. The article presents the comparative estimation of the intensity of inflammatory processes in periodontal tissues from 104 people in the age of 18−35 years with chronic catarrhal gingivitis (CCG) on the background of crowding of teeth (main group) and 33 people with CCG without orthodontic pathology (comparison group). For the estimation of the state of periodontal tissues the indexes PMA, bleeding gums and СPI were used. The obtained data were worked out statistically. Results Discussion. It is set on the results of studies that in dependence on the degree of severity of CCG and with increase of the age of examined increases the intensity of inflammatory processes in tissues of teeth-restraint apparatus in both study groups, but in patients with CCG on the background of crowding of teeth this tendency was more pronounced. The average value of the PMA index by its criteria of index revealed to gingivitis of the average severity in the main and comparison groups of the study. With the increase of age of patients in both study groups the increase of the index of bleeding was set. In patients with crowding teeth the values of CPI index exceeded the values of the index in individuals without orthodontic pathology.Conclusions. In patients with crowded teeth on the background of chronic catarrhal gingivitis the intensification of inflammatory processes in periodontal tissues increased with age and depended on the severity of chronic catarrhal gingivitis and was higher than in patients without orthodontic pathology.

Background. Periodontal tissues inflammatory diseases are widespread among young people. Objective. This study was aimed at elaborating the method to assess risks of periodontal inflammatory diseases and determining its efficacy depending on the state of dental tissues, gum tissues and sex.          Methods. The study included 182 students (93 men, 89 women) aged 19-29: 22 individuals had no lesions of hard dental tissues and no signs of periodontal disease; 51 individuals were found to have DMF index <6; 52 individuals – DMF index ≥6; 57 individuals were diagnosed with chronic catarrhal gingivitis. Primary groups were formed in autumn; re-examination was carried in spring. The research participants were assessed for detection of risks of periodontal inflammatory disease by the method developes by the authors (Patent UA 54041). Results. The study revealed that the risk of development of preiodontitis increases in individuals with high caries and gingivitis intensity. In spring, more individuals suffer from microbial imbalance in in the composition of gingival sulcus fluid and decrease in the mean stability coefficient value that indicates an increased risk of inflammatory periodontal disease development. Women were less likely to experience seasonal dysbiotic changes in the gingival sulcus fluid composition compared with men. Conclusions. The method suggested for assessment of the risk of periodontal inflammatory diseases is of high informativeness. It allows clinicians detecting early pre-nosological signs of oral microbiocenosis imbalance that enhances the effectiveness of early diagnosis of inflammatory periodontal diseases.

Aim. Cathepsin C is the activator of the polymorphonuclear leukocyte-derived proteinase 3, which contributes to inflammatory processes. The aim of the present study was to investigate gingival crevicular fluid (GCF) proteinase 3 and cathepsin C levels in periodontal diseases.Design. Eighteen patients with chronic periodontitis (CP), 20 patients with generalized aggressive periodontitis (G-AgP), 20 patients with gingivitis, and 18 healthy subjects were included in the study. Periodontal parameters including probing depth, clinical attachment level, papilla bleeding index, and plaque index were assessed in all study subjects. GCF proteinase 3 and cathepsin C levels were analyzed by ELISA.Results. GCF proteinase 3 total amount was significantly higher in diseased groups compared to control group, after adjusting ageP<0.05. No differences were found in GCF cathepsin C levels among the study groupsP>0.05. Periodontal parameters of sampling sites were positively correlated with GCF proteinase 3 total amountsP<0.01but not with cathepsin C total amountsP>0.05.Conclusions. Elevated levels of GCF proteinase 3 in CP, G-AgP, and gingivitis might suggest that proteinase 3 plays a role during inflammatory periodontal events in host response. However, cathepsin C in GCF does not seem to have an effect on the pathogenesis of periodontal diseases.

Relevance. Before starting the therapeutic treatment of various periodontal diseases, it should be clearly understood that a high-quality treatment is possible only with complex therapy, excluding local irritating adverse factors in the oral cavity, elimination of inflammatory processes in the periodontal tissues, as well as violations of its function. One of the main tasks of the treatment of periodontal diseases is the preservation of the dentition with a single functional system.Purpose – to assess the use of the drug Kollapan in tooth extraction and treatment of periodontitis.Materials and methods. The clinical observation group consisted of 40 patients aged 19 to 65 years. The gender ratio was 3 : 2 (60% men, 40% women). The largest percentage of patients (65%) came to the clinic with complex tooth extraction. The remaining 35% – with generalized periodontitis of moderate and mild severity.Results. Clinical observation showed that in the first group of patients, whose therapy included the use of Collapan, after a complex tooth extraction, physiological healing of the hole was observed, the disappearance of inflammation within 24–36 hours after the surgery

Obesity prevalence has significantly increased especially in young adults, which is caused by a particular lifestyle, food quality and dietary behavior. Obesity leads to development of huge array of comorbid conditions such as arterial hypertonia, heart stroke, arthritis and other diseases. We conducted standard clinical examination of oral cavity of 154 young patients (18-21 years old) – all of them were students of Ukrainian Medical Stomatological Academy. It has been determined that the majority of patients with BMI >30 kg/cm2 have aggravated family heredity, in 66,2% cases one parent of obese students had obesity and 32,43% patients have both parents with obesity that is significantly higher compared with persons with normal BMI and overweight. There was a high prevalence of periodontal diseases about 74% and carious lesions of teeth - 97.4% among young people despite the age and contingent of examined patients - medical students. In patients with BMI >30 kg/cm2 prevalence of generalized forms of gingivitis and periodontitis was by three times higher compared with individuals with normal BMI. The values of oral hygienic indexes were poor in all examined groups, but their values were slightly lower in patients with normal BMI than in those with overweight and obesity. The prevalence of inflammatory changes in gums was higher in persons with obesity: all of them had a mild degree of lesions in periodontal tissues. Inflammatory processes in the gums were the most intense in patients with the second degree of obesity. According to the results of the study, the presence of the first and the second degree of obesity should be considered as a risk factor triggering periodontal tissues diseases. For persons with BMI >30 kg/cm2 with periodontal disease measures for the secondary prevention of inflammatory and inflammatory dystrophic periodontal diseases should be carried out and in persons without periodontal disease on the background of obesity measures primary prevention should be done.

The abnormalities of maxillodental system take one of the leading places among diseases of the maxillofacial area. Thus, they are diagnosed more than in 50% cases among the younger population and in 30% cases among the adult population.   Most often, there are prerequisites for the incorrect position of individual teeth: anomalies are formed at the genetic level because of heredity of pathologies of size, amount, shape of teeth and size of the jaw bones of the facial skeleton. If the position of the teeth is incorrect, the periodontium retains the occlusal load of the functional injury. Therefore, the aim of our research was to study the prevalence of periodontal tissue diseases in young individuals on the background of crowded teeth. The material and research methods. The study was conducted on the base of Dental Center of Danylo Galytsky Lviv National Medical University during 2015−2017 years. 1146 people at the age from 18 to 44 years were examined. Every participant of research underwent general dental examination, as well as a special examination aimed at clarification of orthodontic treatment. The diagnosis of periodontal tissue diseases was made according to the classification of N. F. Danilevskyi and was determined using paraclinical indicators. The obtained results were worked out statistically using functions of MS Excel and Statistica 6. Results of the research and their discussion. The total number of the examined with orthodontic pathology was found on average in 77.24% of patients (885 people). At the same time, abnormalities of dental jaw system were not detected in about 261 (22.76%) of the examined. As a result of the conducted studies we found that among the examined without dental jaw abnormalities 38.89±.,32% of patients had intact periodontium, that was 1.6 times more for data in individuals with crowding − 23.73±2.00, p<0.01. At the same time, in patients of the main group the prevalence of periodontal tissue diseases was 1.2 times higher than in the examined of the comparative group (76.27±2.00% against 61.11±3.32%, respectively, p<0.01). It should be noted, that with the increase of the age of patients the prevalence of periodontal tissue diseases increased in both study groups, but in patients with crowded teeth this tendency was more pronounced. So, in the younger age group in patients with crowding the prevalence of periodontal diseases was 1.5 times higher in relation to the data of their peers from the comparison group (71.37±3.00% vs. 46.99±5.48%, p<0.01). In 27−35-year-old patients the prevalence of periodontal tissue diseases amounted to 72.38±4.03% of the examined of the main group and 65.62±5.94% of the examined of the comparison group, but the obtained data were not statistically significant, p>0.05. At the same time, in the patients of the main group aged 36−44 years the prevalence of periodontal tissue diseases was maximal and exceeded the value of their peers in the comparison group by 1.2 times (92.08±2.69% vs. 73.91±5.29%, respectively, p<0.01). The analysis of the obtained data allowed to assert that on average crowding of teeth on the upper jaw was found in 237 patients (52.55%) and on the lower jaw in 214 patients (47.45%). Thus, the maximum frequency of crowded teeth on the upper jaw was observed in the examined in the age range of 27−35 years, 60.16±4.41% of patients, with the minimum prevalence of this pathology in the youngest group (18−26 years), 49.34±4, 72%. The analysis of diagnostic structure of periodontal tissue diseases showed that inflammatory diseases of periodontal tissues (gingivitis, localized periodontitis) were found in patients with crowded teeth, on average 1.2 times more often than in patients without orthodontic pathology (42.42±1.88% vs. 36.33±3.54%, respectively, p<0.05). Conclusions. Thus, as a result of the conducted studies, higher prevalence of periodontal tissue diseases was proved in patients with crowding of teeth, which made progress in increasing the age of the examined than in patients without orthodontic pathology.

Biomarkers of inflammation can be used as indicators for monitoring the treatment of periodontal diseases, as well as for finding the safest and most effective drugs with different mechanisms of action. The concentration of biomarkers in saliva was determined by enzyme immunoassay and chemiluminescent analysis. The obtained results reflect the degree of activity of inflammatory processes in periodontal tissues that occur in gingivitis and reveal an imbalance in the processes of free radical oxidation and antioxidant protection of the oral cavity.

One of the biomaterials used in veterinary dentistry is hydroxyapatite (HAp). It aids the biological process of bone reconstruction and provides the basis on which damaged tissues can be rebuilt. It is also exceptionally osteoconductive and bioactive towards bone and other tissues. The aim of the present study was to verify the usefulness of hydroxyapatite microporus ceramics for the treatment of periodontal diseases and post-extraction defects. The study was performed on 40 dogs. Dogs were qualified for the in vivo test: 2 study groups and 2 control groups (K1, K2) were created. Group G1 comprised 10 dogs diagnosed with periodontitis with 4-8 mm gingival pockets and mobility of mandibular/maxillary incisors. In order to avoid extraction, hydroxyapatite implantation into the bone pockets was performed. Group G2 comprised 10 dogs that required the extraction of maxillary canines, following which biomaterial was introduced into the post-extraction cavities. Control groups were performed without using of microporous hydroxyapatite. In group G1, animals displayed significant shallowing of gingival pockets. The mean depth of pockets was significantly reduced in those dogs and considerably better reconstruction of periodontal tissues was observed when compared to the control group K1. In group G2, significantly faster healing of bone cavities was stated when compared to the respective control group K2. The study confirmed the validity of using microporous hydroxyapatite granules and shaped blocks in reconstructive periodontal treatment as well as prevention of oronasal fistulas after canines extraction and facilitation of the post-extraction healing process..

Dental cement materials have been developed with the aim to replace hard dental tissues. The first material used for pulp capping, root canal obturation, bifurcation perforation and apexification is calcium hydroxide (in 1920). A half century later, glass-ionomer cements began to suppress it as dentine substitutes. Finally, in the 1990s, calcium silicate (CS) material appeared in the dental research community as the most promising dentine substitute capable to adequately meet all clinical requirements. The aim of this paper is to present an overview of literature related to studies about CS materials taking into account their physical, chemical and biological properties and clinical applications. This review aims to discuss beneficial and adverse characteristics of CSs concerning interactions to the hard dentine and soft pulp/periodontal tissues. This review article deals with the literature data about currently commercially available CS concerning laboratory and clinical findings. 109 scientific articles were analyzed of which 62 references reported in vitro and 26 in vivo investigations while 21 references comprised reports, reviews and books dealing with both, in vitro and in vivo investigations. Although further data collection is necessary, CSs are promising materials that represent a gold standard for numerous dental clinical procedures.
