Summary. Prevention and treatment of periodontal diseases remains one of the most important tasks of modern dentistry in connection with their high prevalence among all age groups of the population. Recently, much attention has been paid to the study of gum recession, as one of the clinical forms of periodontal disease. The paper presents data on paraclinical indexes in patients with gum recession, depending on age and biotype of periodontal disease. The aim of the study – to evaluate the intensification of inflammatory processes in periodontal tissues in patients with gum recession, depending on age and periodontal biotype using paraclinical indices. Materials and Methods. The article presents a comparative assessment of the intensity of inflammatory processes in periodontal tissues, depending on age and periodontal biotype in 204 somatically healthy patients who had a recession of the gums caused by different etiological factors. Results and Discussion. As a result of our research, it was found that with increasing age, there was a clear tendency to increase the indexes of all paraclinical indexes analyzed by us. Also, attention was drawn to the fact that the carriers of biotype periodontal A1-Thin S flow of inflammatory processes proceeded more pronounced and more intense.Conclusions. In persons with recession gum, the intensification of inflammatory processes in periodontal tissues increased with age, but depended on the periodontal biotype.Conclusions. Obesity prevalence has significantly increased especially in young adults, which is caused by a particular lifestyle, food quality and dietary behavior. Obesity leads to development of huge array of comorbid conditions such as arterial hypertonia, heart stroke, arthritis and other diseases. We conducted standard clinical examination of oral cavity of 154 young patients (18-21 years old) – all of them were students of Ukrainian Medical Stomatological Academy. It has been determined that the majority of patients with BMI >30 kg/cm2 have aggravated family heredity, in 66,2% cases one parent of obese students had obesity and 32,43% patients have both parents with obesity that is significantly higher compared with persons with normal BMI and overweight. There was a high prevalence of periodontal diseases about 74% and carious lesions of teeth - 97.4% among young people despite the age and contingent of examined patients - medical students. In patients with BMI >30 kg/cm2 prevalence of generalized forms of gingivitis and periodontitis was by three times higher compared with individuals with normal BMI. The values of oral hygienic indexes were poor in all examined groups, but their values were slightly lower in patients with normal BMI than in those with overweight and obesity. The prevalence of inflammatory changes in gums was higher in persons with obesity: all of them had a mild degree of lesions in periodontal tissues. Inflammatory processes in the gums were the most intense in patients with the second degree of obesity. According to the results of the study, the presence of the first and the second degree of obesity should be considered as a risk factor triggering periodontal tissues diseases. For persons with BMI >30 kg/cm2 with periodontal disease measures for the secondary prevention of inflammatory and inflammatory dystrophic periodontal diseases should be carried out and in persons without periodontal disease on the background of obesity measures primary prevention should be done.

Subject. With increasing age of patients, in many cases, the severity of periodontal diseases also increases, and dystrophic ones join the inflammatory processes. Local predisposing factors in the oral cavity are aggravated by concomitant somatic diseases and a decrease in compensatory processes against the background of physiological aging. Literature data indicate that the manifestations of the physiological process of aging and pathological processes (inflammatory and dystrophic) can be clearly observed on the example of buccal epithelial cells. The aim is to identify the relationship between the age of patients, their periodontal status and the cytological characteristics of buccal epithelial cells. Methodology. All study participants were divided into two groups: the first included patients with a conditionally healthy periodontium (72 people), the second - with a chronic inflammatory process in the periodontium (57 people). In accordance with the WHO classification, a gradation of age was carried out: young (18―44 years old), mature (45―59 years old), elderly (60―74 years old), senile (75―90 years old). All patients underwent a comprehensive dental examination, sampling and cytological examination of the buccal epithelium, calculation of the integral indices of the buccal cytogram. Results. In healthy patients, a weak positive correlation was established between the cytogenetic index, the index of the accumulation of cytogenetic disorders and age, a weak negative correlation was found between the proliferative index and age. In patients with an inflammatory process in the periodontal tissues, a multidirectional change in the values of the buccal cytogram indices was observed, which may indicate an imbalance in the processes of regeneration, apoptosis, and cellular damage in inflammatory periodontal diseases in older age groups. Conclusions. In patients with healthy periodontal disease, a regular accumulation of cytogenetic disorders occurs with increasing age, while proliferative activity, on the contrary, decreases. Patients with inflammatory phenomena in the periodontal tissues are characterized by imbalance, impaired coordination of regeneration and apoptosis processes, combined with the most pronounced reactivity in the middle age period.

Relevance. Patients undergoing orthodontic treatment of dental anomalies using braces have a high risk of developing inflammatory periodontal diseases, the occurrence of which is caused by an insufficient level of hygienic oral care and orthodontic apparatus. The quality of preventive measures during the period of orthodontic treatment affects the structure and prevalence of inflammatory periodontal diseases due to the low awareness of patients, their non-observance of the doctor’s instructions for hygiene measures, lack of hygiene quality monitoring and its correction during treatment [1-5, 7, 9].Purpose. The purpose of the research was studying the prevalence of periodontal diseases and its structure among orthodontic patients, taking into account the fixed appliances used.Materials and methods. 79 patients undergoing orthodontic treatment were involved in the study, they were divided into 4 age groups. In all groups, an inspection was conducted to determine the prevalence and intensity of the periodontal disease. From the outpatient records of dental patients, form «043/y», data was extracted. The intensity of periodontal diseases was determined using the communal periodontal index (CPI), the papillary-marginal-alveolar index (PMA), the gingival sulcus bleeding index (SBI), complex periodontal index ICP were also determined [19-22].Results. As a result of the study, a high prevalence of periodontal disease was established in patients with bracket systems, which was 84,81±6.12%, in the first group it was 77.78%, in the second group this indicator was equal to 81.25%, and in the third and fourth groups – 85.72% and 91.67%, respectively.Conclusion. The results obtained revealed a high prevalence of periodontal disease in orthodontic patients, which necessitates an improvement in the quality of dental health in this groups by means of dental education and improved hygienic procedures.

Gout is a systemic disease characterized by an inflammatory reaction at the sites of deposition of sodium monourate crystals in individuals with hyperuricemia and is caused by external factors and (or) genetic factors. Patients with gout often have comorbidities such as cardiovascular disease (including hypertension), diabetes and chronic kidney disease, and dental disease. In the professional literature there are almost no studies of periodontal tissues in patients with gout. The aim of our study was to investigate the prevalence of periodontal disease in patients with gout. The participants were divided into 2 groups: the main one included 150 patients with gout, and the comparative one included 80 people without rheumatic pathology. The age of the subjects ranged from 30 to 50 years or more. The clinical examination was performed according to a standard scheme, which included history taking and analysis of patient’s complaints. According to the results obtained, the patients with gout showed a significantly higher prevalence of periodontal disease in all age groups, compared with individuals with rheumatic pathology. A high percentage of periodontal pathology was observed in the young participants of the main group. The larges share in the structure of periodontal diseases among the patients with gout was represented by advanced generalized periodontitis class I and II, and parodontosis, while the individuals without rheumatic pathology demonstrated a higher percentage of initial stages of periodontal disease. With age, both groups demonstrated the increase in the prevalence of periodontal disease, but in patients with gout, this process was more intense. It can be concluded that systemic disorders associated with gout, among which chronic autoimmune inflammation, oxidative stress, endothelial dysfunction and a high risk of atherosclerosis are very common, can be regarded as predictors of both rapid progression and aggravation of destructive inflammatory lesions of the periodontium.

AbstractAccording to some studies, the Entamoeba gingivalis colonizing the gingival tissue is an important agent in bringing about periodontitis. Other studies, however, deem it an opportunist that is able to survive in the medium induced by periodontal disease. The aim of this study was to investigate the prevalence of Entamoeba gingivalis infection in patients from the Department of Periodontology, and compare this population with that of healthy people, so as to analyze the relationships between infection and patient sex and age. The result of this work is that in both groups, a correlation (p = 0,19) has been noted between the occurrence of amoebae and other diseases in the oral cavity. Indeed, 81,4% of all patients with some periodontal disease showed the presence of amoeba. Among those who are not afflicted with oral diseases, the presence of amoeba was indicated in 62,5% of the total. In addition, a correlation between the person's age and the presence of protozoa (p = 0,15) was strongly marked among women (p = 0,19). In the three age groups of women in this study (40-49, 60-69, and above 80 years), we observed a 100% presence of protozoa.Our study leads us to the conclusion that infections with Entamoeba gingivalis should be regarded as an factor that is associated with the pathological changes occurring in patients with periodontal diseases.

Relevance. Before starting the therapeutic treatment of various periodontal diseases, it should be clearly understood that a high-quality treatment is possible only with complex therapy, excluding local irritating adverse factors in the oral cavity, elimination of inflammatory processes in the periodontal tissues, as well as violations of its function. One of the main tasks of the treatment of periodontal diseases is the preservation of the dentition with a single functional system.Purpose – to assess the use of the drug Kollapan in tooth extraction and treatment of periodontitis.Materials and methods. The clinical observation group consisted of 40 patients aged 19 to 65 years. The gender ratio was 3 : 2 (60% men, 40% women). The largest percentage of patients (65%) came to the clinic with complex tooth extraction. The remaining 35% – with generalized periodontitis of moderate and mild severity.Results. Clinical observation showed that in the first group of patients, whose therapy included the use of Collapan, after a complex tooth extraction, physiological healing of the hole was observed, the disappearance of inflammation within 24–36 hours after the surgery

With diabetes, there is a violation of the microvasculature, a violation of the immune status, an increase in destructive processes in the bone tissue, which leads to the development and / or intensification of pathological processes. In the oral cavity, the most frequently diagnosed inflammatory periodontal diseases, chronic injuries of the oral mucosa (COP), recurrent aphthous stomatitis, COP candidiasis. The aim of our study was to study the dental status of patients with type 2 diabetes mellitus. Materials and Methods: A descriptive evaluative epidemiologically controlled, non-randomized study was conducted to determine the effect of type 2 diabetes on dental status. The main group (“case”) consisted of 68 patients with type 2 diabetes in the compensation phase (IA) and 56 patients in the subcompensation phase (IB). The comparison group included 60 patients without somatic pathology. All patients underwent a comprehensive dental examination, bacteriological and microscopic examination to identify fungi of the genus Candida. Results: in patients with subcompensated form of type 2 diabetes, there was an increase in the frequency of complaints of plaque on the tongue, burning sensation in the COP, dryness and changes in taste. Periodontal indices indicate that the patients of the main group have a high prevalence of inflammatory and destructive processes in the periodontal tissues (indices of PMA, KPI, Mullemann indices) with poor oral hygiene (OHI-S = 3.5 ± 0.4), which confirms the Friedman criterion (χ2 = 116.27; p = 0.0000) based on the calculation of Kendall's concordance coefficient (0.743). The diagnosis of oral mucosa candidiasis was confirmed in 74.6% of patients in the main group by bacteriological examination.

Background. Periodontal tissues inflammatory diseases are widespread among young people. Objective. This study was aimed at elaborating the method to assess risks of periodontal inflammatory diseases and determining its efficacy depending on the state of dental tissues, gum tissues and sex.          Methods. The study included 182 students (93 men, 89 women) aged 19-29: 22 individuals had no lesions of hard dental tissues and no signs of periodontal disease; 51 individuals were found to have DMF index <6; 52 individuals – DMF index ≥6; 57 individuals were diagnosed with chronic catarrhal gingivitis. Primary groups were formed in autumn; re-examination was carried in spring. The research participants were assessed for detection of risks of periodontal inflammatory disease by the method developes by the authors (Patent UA 54041). Results. The study revealed that the risk of development of preiodontitis increases in individuals with high caries and gingivitis intensity. In spring, more individuals suffer from microbial imbalance in in the composition of gingival sulcus fluid and decrease in the mean stability coefficient value that indicates an increased risk of inflammatory periodontal disease development. Women were less likely to experience seasonal dysbiotic changes in the gingival sulcus fluid composition compared with men. Conclusions. The method suggested for assessment of the risk of periodontal inflammatory diseases is of high informativeness. It allows clinicians detecting early pre-nosological signs of oral microbiocenosis imbalance that enhances the effectiveness of early diagnosis of inflammatory periodontal diseases.

The high prevalence of periodontal diseases, up to 90%, is an important issue in medicine. The periodontal biotype is one of the most important parameters that determine the course and prognosis of periodontal diseases. Gingival thickness, keratinized tissue, and bone morphology are the main characteristics of the periodontal biotype. This article presents a review of the literature on the characteristics of periodontal biotypes, their response to treatment, and treatment prognosis. A search of electronic databases (Medline, EMBASE, CyberLeninka, Elibrary, and RINC) was performed for the literature review. The article also presents studies about relationships among periodontal biotype, recession, and malocclusion. The thin biotype is a predictive factor of gingival recession. Recessions are characterized by apical migration of marginal gingiva and defects in root coverage. Gingival recession may result from orthodontic movement of teeth, particularly among patients with the thin gingival biotype. The thin and thick biotypes respond differently to inflammation, restorative and surgical effects, and parafunctional habits. A gingival thickness greater than 2 mm can be categorized as the thick biotype and less than 1,5 mm as the thin biotype. Gingival thickness can affect the results of restorative therapy and root coverage procedures. It is necessary to determine the gingival biotype before initiating treatment. Special care must be taken when planning treatment for cases with a thin biotype.

The review of the scientific literature is devoted to the topical issues of studying the etiology of periodontal diseases, which are one of the most common and complex pathologies of the maxillofacial region. Analysis of recent studies proves a stable relationship between the development of periodontal diseases and disorders in the immune system, the neurohumoral system, metabolic disorders, genetic predisposition, and so on. The article presents the data obtained in the course of studying the literature on the role of disorders in the functioning of individual organs (gastrointestinal tract, liver, lungs, heart, and urinary system) in the development of chronic periodontal diseases. The article notes that the anatomical and physiological proximity of the periodontal and digestive tract tissues, the generality of innervation and humoral regulation create prerequisites for the involvement of periodontal disease in the pathological process in diseases of the gastrointestinal tract. One of the main etiological factors in the development of inflammatory diseases of the gastrointestinal tract and periodontium is Helicobacter pylori, which is found in the loci of the oral cavity: in the oral and gingival fluid, on the mucous membrane of the tongue and cheeks, and in the periodontal pockets. It is pointed out that the liver also occupies a special place in the development of periodontal diseases, which is explained by the performance of its significant functions for the human body: regulatory, metabolic, antitoxic and other. There is evidence that the pathology of periodontal disease plays a leading role in the structure of dental diseases in patients with chronic obstructive pulmonary diseases, which is clinically manifested by symptoms of generalized periodontitis of the І-ІІ degrees of development and its complications - partial or complete secondary adentia, and with tooth preservation - defects in dental series and violations of occlusion, function, aesthetics. Scientists suggest a general biological mechanism for the development of generalized periodontitis and cardiovascular diseases, linking the development of periodontal diseases in patients with cardiovascular pathology with microcirculatory disorders. The dependence of the severity of inflammatory changes in the periodontal tissues on the disturbances of salt metabolism in urolithiasis is proved. The data obtained indicate that diseases of the internal organs contribute to the structural damage of periodontal tissues and they are a risk factor for periodontal diseases, which necessitate the presence of not only theoretical knowledge and practical skills in dentistry, but also their awareness of the features and clinical manifestations of somatic pathology. An urgent and justified step in the treatment of periodontal diseases is also the involvement in the process of rendering complex dental care to internist doctors capable of quickly and qualitatively assessment the condition of the internal organs and the basic systems of the patient's body.
