Abstract P314: High Prevalence of Behavioural Cardiovascular Risk Factors in a Nepalese Community undergoing Socio-epidemiological Transition

Behavioral risk factors, particularly the major four- tobacco consumption, harmful use of alcohol, unhealthy diet and physical inactivity- are accountable for a major proportion of cardiovascular disease burden. Driven by urbanization and globalization, these risk factors are rising in many of the low- income countries including Nepal. Population-based surveys and hospital reports indicate cardiovascular diseases are rising in Nepal. The first countrywide WHO-STEPS risk factor survey conducted in 2007-2008 showed that 23.8% are daily smokers, 28.5% are current drinkers, and 61.9% consumed less than five servings of fruits and vegetables, while 5.5% had low physical activity. Risk factors are generally more prevalent in urban areas than rural. We aimed to study the status of these behavioural risk factors in a community that is rapidly undergoing transition from a village to an urban community. We conducted the study in Duwakot and Jhaukhel, two urbanizing villages 13 kilometers from the capital Kathmandu. Six clusters were randomly selected. Altogether 777 adults aged 25-59 years consented to participate. Questions were based on the WHO-STEPS survey. Current smokers included those who affirmed to the question ‘Do you smoke?’ Current drinkers had ‘consumed alcohol in the previous one month’. Inadequate fruit and vegetable intake indicated those who were taking less than five servings of combined fruit and vegetable in a day. Physical activity was subjectively measured by asking about the respondent’s activities at work, travel and leisure. Having low physical activity meant not meeting any of the GPAQ (Global Physical Activity Questionnaire) criteria of adequate physical activity. Prevalence of the four major behavioral risk factors was high in the community. Addictive behavioural risk factors were particularly higher in the males than in the females: current smoking (33.5% vs. 14.7%), current smokeless tobacco consumption (20.2% vs. 3.3%), and current alcohol drinking (34.5 % vs. 12.6%). Only 2% of the study population fulfilled the recommended minimum five servings of fruits and vegetables. Low physical activity was present in 43.3% of the respondents, with a higher prevalence seen among the females, elderly, more educated and those on non-agro works. In conclusion, there is particularly high physical inactivity and low fruit and vegetable intake in the community indicating negative effects of urbanization such as changing dietary habit and escalating sedentary lifestyle. Because the community is still beginning to urbanize, it is important that timely population-based health promotional strategies and favourable environmental modifications are implemented to counteract the effects of urbanization.

Background: Cancer has affected humans since pre-historic times, but in recent years its occurrence has sharply increased. The common causative factors are an individual’s diet, decreasing physical activity, consumption of tobacco and alcohol. Cancer deaths are increasingly popular as the world is ignorant of its harmful lifestyle habits. Interventions focusing on behavior change are key to curb diseases caused by behavioral risk factors. The aim of the study was to identify the association between multiple behavioral risk factors and cancer in rural women.Methods: A cross-sectional study was conducted in Bharat hospital and Institute of oncology for a period of 5 months (February 2021-June 2021). A total of 380 rural women diagnosed with cancer were included in the study. A structured proforma was used to collect data about behavioral risk factors. The data was coded and entered in MS Excel and analyzed using SPSS version 25. Descriptive statistical analysis was done. Crosstables were constructed, Chi square test was done to find the association of behavioral risk factors causing cancer.Results: Significant association was found between the frequency of dairy products intake/week, frequency of fruits intake/week, frequency of vegetable intake/week, frequency of sweets intake/week, the frequency of tea, coffee consumption/day, frequency of smoking/week, frequency of snuff, sun protection practices and various cancers.Conclusions: Multiple behavioral risk factors like diet, physical activity, tobacco consumption, alcohol use, sun exposure, etc., are can be modified and treated. Proper education and health promotion are the keys to decrease non-communicable diseases.

Negative health behaviors incorporated into lifestyle are considered the main risk factors for chronic non-communicable diseases (NCDs) in adults and the elderly. However, the relationship between the aggregation of these factors and the sociodemographic conditions of the elderly needs to be better elucidated. The aim of this study was to analyze the simultaneity of the five risk factors for NCDs in the elderly with low economic status living in a rural city in Brazil, and their association with sociodemographic variables. Cross-sectional study was conducted with elderly people from Family Health Units of the city of Ibicui-Bahia, Brazil, where 310 elderly were enrolled. Rates of physical inactivity in leisure (PIL), alcohol consumption, sedentary behavior, overweight/obesity and tobacco consumption were collected through a questionnaire in an individual interview. The average age among participants was 71.62 (± 8.16) years. The group presenting the five behaviors had high scores in both sexes (men O/E = 242.5; women O/E = 161.7). Among men and women, the highest scores found through clustering of simultaneous NCD risk factors were for the consumption of alcohol with smoking, and physical inactivity with smoking. When analyzing the association between groups and sociodemographic characteristics, men were more physically inactive than women (OR = 0.96, CI = 0.92-0.98) and concomitantly had unhealthy habits (smoking). In conclusion, the elderly are exposed to health risk factors for concurrent CNCDs and the accumulation of these risk factors was not associated with sociodemographic variables, it is suggested that further studies be carried out with the prevalent variables as well as to analyze why the elderly population presents these levels.

ObjectivesTo assess the prevalence, clustering and sociodemographic distribution of non-communicable disease (NCD) risk factors in adolescents in Nepal.DesignData originated from Global School Based Student Health Survey, Nepal conducted in 2015–2016.SettingThe study sites were the secondary schools in Nepal; 74 schools were selected based on the probability proportional to school enrolment size throughout Nepal.Participants5795 school-going children aged 13–17 years were included in the study.Primary outcomesNCD risk factors: smoking, alcohol consumption, insufficient fruit and vegetable intake, insufficient physical activity and overweight/obesity were the primary outcomes. Sociodemographic distributions of the combined and individual NCD risk factors were determined by Poisson regression analysis.ResultsFindings revealed the prevalence of smoking (6.04%; CI 4.62 to 7.88), alcohol consumption (5.29%; CI 4.03 to 6.92), insufficient fruit and vegetable intake (95.33%; CI 93.89 to 96.45), insufficiently physical activity (84.77%; CI 81.04 to 87.88) and overweight/obesity (6.66%; CI 4.65 to 9.45). One or more risk factors were present in 99.6%, ≥2 were in 83% and ≥3 were in 11.2%. Risk factors were more likely to cluster in male, 17 years of age and grade 7. Prevalence of smoking (adjusted prevalence ratio (aPR)=2.38; CI 1.6 to 3.51) and alcohol consumption (aPR=1.81; CI 1.29 to 2.53) was significantly high in male, and in 16 and 17 years of age. Prevalence of insufficient physical activity and overweight/obesity was significantly lower in higher grades.ConclusionInsufficient fruit and vegetable intake and insufficient physical activity were highly prevalent in the populations studied. Risk factors were disproportionately distributed and clustered in particular gender, age and grade. The study population requires an age and gender specific preventive public health intervention.

Purpose: The association between preference for and intake of fruits and vegetables was examined among Albertan children. Methods: Data used were collected as part of a provincial population-based survey among grade 5 children in Alberta. Intake of two fruits and five vegetables was assessed using the Harvard food frequency questionnaire, and preference for individual fruit and vegetable items was rated using a three-point Likert-type scale. Random effects models with children nested within schools were used to test for associations between fruit and vegetable preference and intake. Results: A total of 3398 children aged 10 to 11 years returned completed surveys. Children who reported a greater liking for fruits and vegetables also reported significantly (p<0.001) higher intake. On average, children who liked a food a lot ate 0.5 to 2.7 more weekly servings of the food than did children who did not like the food. Conclusions: These findings suggest that focusing on interventions designed to increase taste preference may lead to increased fruit and vegetable intake among children. Introducing children to unfamiliar fruits and vegetables through taste testing may be an effective and practical health promotion approach for improving dietary habits.

Background and aims: Behavioral risk factors of noncommunicable diseases (NCDs) are established during early age and continued into adulthood. In Bangladesh, the scenario of NCD risk factors among students was inadequately studied. The aim of this study was to assess and compare the prevalence of behavioral risk factors of NCDs between undergraduate medical and nonmedical students. Methods: This cross-sectional study was conducted among 280 undergraduate students (equal number of students from medical and nonmedical backgrounds). Respondents were selected purposively from 5 purposively-selected institutions. Modified WHO STEPS instrument was used for data collection. Respondents were asked about their behavioral risk factors (tobacco use, insufficient fruit and vegetable intake, inadequate physical activity, and alcohol consumption) by face-to-face interviews. Results: Men students were more in both groups. The mean age of the medical and nonmedical students was 22.1±2.0 and 21.8±1.9 years, respectively. Tobacco use was more among nonmedical students than that among the counterpart (22.1% vs. 15.7%). Medical students were more used to take insufficient fruits and vegetables compared to nonmedical students (97.9% vs. 93.6%). Equal proportions (71.4%) of students in both groups were used to perform inadequate physical activity. Alcohol consumption was observed more among nonmedical students (12.9% vs. 8.6%). Conclusion: Behavioral risk factors of NCDs were remarkable among students of both groups, mainly among nonmedical students.

Introduction: Many modifiable risk factors for chronic diseases are related to dietary patterns and physical activity. This study aims to establish the prevalence of adequate fruit and vegetable intake and physical activity among community-ambulant patients in Singapore. Methods: A cross-sectional study was conducted among community-ambulant patients using convenience sampling. Data were collected through a survey questionnaire, covering self-reported level of physical activity and daily fruit and vegetable intake. Demographic information, awareness and adherence to recommendations were summarised using descriptive statistics. To identify factors influencing awareness and behaviour, exploratory logistic regression analyses were performed. Results: Three hundred and eighty-five complete responses were analysed. For intake of fruits and vegetables, 29.1% and 21.3% of participants were aware of and adherent to the fruit and vegetable intake recommendation respectively. For physical activity, 21.3% and 26.8% of study participants were aware of and adherent to the recommendation respectively. Only 4.7% and 5.5% of participants were aware of and practising both recommendations respectively. Gender, age, ethnicity and body mass index were found to have significant influence on the awareness of the recommendations. However, there was no significant factor found to be contributing to the adherence to the recommendations. Conclusion: Awareness of and adherence to lifestyle recommendations among patients remain low despite ongoing publicity efforts. Many are not fully aware of and adherent to the dietary and physical activity recommendations publicised. Apart from intensifying public outreach on every possible occasion and delivering messages in manners understandable by our older patients, strategies targeting the knowledge–behaviour disconnect among working middle-aged patients also deserve continued attention in the near future.

Purpose. This paper investigated whether stage of change for health behaviors was associated with the presence of chronic conditions. Design. A stratified cross-sectional survey by mail with telephone follow-up. Settings. This study was conducted at a mixed-model HMO with 650,000 members based in Minnesota. Subjects. The sample consisted of a random sample of 8000 HMO members age 40 or over with systematic oversampling of members with hypertension, diabetes, dyslipidemia, or heart disease. Measures. In addition to demographics, readiness to change for physical activity, fat intake, fruit and vegetable intake, and smoking were assessed. Results. The adjusted response rate was 82.4%. In a logistic regression analysis, members with one or more than one chronic condition had greater readiness to change for three out of four risk factors compared to members with no chronic conditions. Conclusions. The stage-of-change distribution of HMO members with chronic conditions suggests that members at highest risk of adverse health outcomes have the greatest readiness to change behavioral risk factors. Based on these observations, targeted, stage-specific efforts to support behavior change are likely to be both acceptable and effective in HMO members with chronic conditions. Improving stage of change for behavioral risk factors for members with diabetes may present special problems and opportunities.

This study aimed to identify the prevalence of leisure-time physical activity (LTPA) and fruit and vegetable intake (FVI) behaviors and to examine relationships between biological, sociocultural, and psychological personal factors and these behaviors among young adult Millennials aged 20 years to 32 years. Descriptive and inferential statistics were performed on data from the 2013–2014 National Health and Nutrition Examination Survey. Of the 1,137 respondents, 47% met the recommendations for LTPA, and only 13% and 17% met the recommendations for intake of fruits and vegetables, respectively. Engagement in LTPA was associated with being male, race/ethnicity, and increased education and income. Obesity, measured by body mass index, and poor perceived health status were associated with not meeting the recommendations for LTPA. Increased education was associated with meeting the recommendations for vegetable intake. No other regression models were significant. These findings suggest contemporary young adults would benefit from health promotion efforts.

Abstract The article describes the relative frequency ok of risk and protective behaviors for chronic non-communicable diseases (NCDs) in adults residing in Viçosa, Brazil. A cross-section-al population-based study including 1,226 adults living in the municipality. We used a structured questionnaire containing questions sociodemographic and behavioral The risk and protection factors evaluated were: smoking, physical activity, excessive consumption of alcohol and food consumption. The proportion of risk and protection factors was calculated in the total population, according to gender, education and socioeconomic status. The studied population has a high frequency of risk factors for NCDs, such as excessive consumption of alcoholic beverages, habit of consuming whole milk, habit of eating meat with visible fat, regular consumption of soft drinks and 78.5% did not achieve the minimum recommendation for physical activity in leisure time. With regard to protective factors, 86.2% of the population reported regular consumption of fruits and vegetables, and 73%, of beans. It was found the highest frequency of risk factors in among males, in younger people and middle socioeconomic status. This population has an urgent need for public policy of municipal planning to change this current scenario.
