Genotoxic risk in health-care professionals occupationally exposed to low doses of ionizing radiation

The purpose of this study was to evaluate the potential influence of occupational ionizing radiation (IR) exposure on health professionals, assessing DNA damage using the comet and micronucleus (MN) assays and analyzing relative risks, correlations, and associated factors between outcomes and socioeconomic and lifestyle factors. Blood and buccal samples were collected from 36 workers, who actively participated in an imaging sector of a hospital, who were either exposed to IR directly or indirectly (9 internal control and 27 exposed), and 27 individuals living in the same city but with no occupational exposure (external control, unexposed/healthy). All radiation dosages performed on the 36 workers were less than 20 mSv/y, not exceeding the effective dose limit for occupational exposure. A questionnaire identified socioeconomic and lifestyle factors associated with the outcomes. The results of the MN assay showed a significant difference between both internal control and the exposed group when compared to the external control. For the comet assay, there were significant differences between the percent of tail DNA of the exposed group and external controls, but no difference was found between the exposed group and internal controls. Relative risk associations were found in time of exposure, hours worked per week, and perceived stress. Correlations were found between the outcomes and age, consumption of alcohol, and frequencies of X-rays during life. Variables that showed to be significant in the adjusted analysis were skin color and recent exposure to radiation. Albeit limited, the findings of this study suggest genotoxicity in both blood and buccal mucosa cells of workers exposed directly or indirectly to IR and that lifestyle and socioeconomic factors are associated and correlated with the risk of developing these outcomes.

Abstract Background Gamma rays, x-rays, and high ultraviolet are classified as ionizing radiation as their photons have enough energy to ionize atoms, causing chemical reactions. People can be exposed to ionizing radiation under different circumstances, at home, in public places (public exposures), at their workplaces (occupational exposures), or in a medical setting (as are patients, caregivers, and volunteers). Radiation damage to tissue or organs depends on the dose of radiation received, or the absorbed dose which is expressed in a unit called the gray (Gy). The potential damage from an absorbed dose depends on the type of radiation and the sensitivity of different tissues and organs. Objective To investigate the long-term influence of the ionizing radiation on the human lens. Patients and Methods Type of Study: Cross-Sectional Study. Study Setting: Ain Shams University hospital, Ophthalmology Department. Study Period: 6 months. Results: A significant difference was found between groups regarding the presence of cataract.as 50% of exposed group had cataract compared to 26.9% of non-exposed group (P = 0.043). There was a significant positive correlation between exposure duration and cataract grade as it was longer in cases with cataract. Conclusion A significant difference was found between groups regarding the presence of cataract. As there is a risk that other ocular pathologies are related to occupational radiation exposure, further investigative studies are required to define these. It can be strongly recommended that all personnel exposed to occupational radiation have routine eye examinations.

Three groups of 30 12- to 15-yr.-old males, students of California junior high schools, matched for age and IQ, were tested on the Rotter I-E Scale. Group A (school basketball players) were considered to be group sports participants, Group B (gymnasts) individual sportsmen, and Group C non-participants in any school activity. A significant difference in the direction of internal control for group sports participants was noted. Individual sports participants and non-participants were alike in being significantly higher in external control than group players. Further research on the relationship between internal control and self-esteem seems warranted for group sports participants.

BACKGROUND: Current radiation protection standards are based on premise that any radiation dose may result in detrimental health effects. The aim of this study was to evaluate extent of the DNA damages (measured by induction of micronuclei) and interphase cell death in circulating lymphocytes of medical personnel exposed to ionizing radiation. METHODS: Baseline micronuclei were assessed using the cytokinesis-block micronucleus test. Cytotoxicity was analyzed by flow cytometry for human white blood cells to identify cells that displayed apoptosis-associated DNA condensation. Necrotic cells were analyzed simultaneously. All parameters were compared with corresponding controls. RESULTS: A statistically significant difference (t = 4.54, p = 0.002) was found between exposed and control group in the yield of baseline micronuclei. The level of baseline micronuclei correlated positively with necrosis of leucocytes (r=0.09, p=0.68 in exposed group, r=0.02, p=0.97 in controls). An inverse correlation between baseline micronuclei and apoptosis was noted in both groups of examinees (r = -0.26, p = 0.27 in exposed group, r = -0.09, p=0.80 in controls). The data obtained also suggested an inverse correlation between necrosis and apoptosis (r = -0.37, p = 0.11 in exposed group, r = -0.89, p = 0.001 in controls). CONCLUSION: Flow cytometry being a rapid, fast, and accurate method is strongly recommended in evaluation of radiation injuries. The integration of apoptosis and necrosis into micronucleus assay could be very important in the assessment of cumulative effects of ionizing radiation in occupationally exposed medical personnel.

Abstract Mitochondria are organelles that perform major roles in cellular operation. Thus, alterations in mitochondrial genome (mtGenome) may lead to mitochondrial dysfunction and cellular deregulation, influencing carcinogenesis. Gastric cancer (GC) is one of the most incident and mortal types of cancer in Brazil, particularly in the Amazon region. Here, we sequenced and compared the whole mtGenome extracted from FFPE tissue samples of GC patients (tumor and internal control – IC) and cancer-free individuals (external control – EC) from this region. We found 3-fold more variants and up to 9-fold more heteroplasmic regions in tumor when compared to paired IC samples. Moreover, tumor presented more heteroplasmic variants when compared to EC, while IC and EC showed no significant difference when compared to each other. Tumor also presented substantially more variants in the following regions: MT-RNR1, MT-ND5, MT-ND4, MT-ND2, MT-DLOOP1 and MT-CO1. In addition, our haplogroup results indicate an association of Native American ancestry (particularly haplogroup C) to gastric cancer development. To the best of our knowledge, this is the first study to sequence the whole mtGenome from FFPE samples and to apply mtGenome analysis in association to GC in Brazil.

Background: Panoramic imaging is one of the most common imaging methods in dentistry. Regarding the side-effects of ionizing radiation, it is necessary to survey different aspects and details of panoramic imaging. In this study, we compared the absorbed x-ray dose around two panoramic x-ray units: PM 2002 CC Proline (Planmeca, Helsinki, Finland) and Cranex Tome (Soredex, Helsinki, Finland).Materials and Methods: In order to estimate x-ray dose, 15 thermoluminescet dosemeters (TLD-100) were placed in 3 semi-circles of 40cm, 80cm and 120cm radii. Around each unit, the number of TLDs in each semi-circle was 5 with equal intervals. The center of semicircles accords with the patient’s position. Each TLD was exposed 40 times. These dosemeters were read out with a Harshaw Model 4000 TLD Reader (USA). The calibration processing and the reading of dosemeters were performed by the Atomic Energy Organization of Iran.Results: The mean absorbed dose in three lines of PM 2002 CC Proline was 123.2±15.1, 118.0±11.0 and 108.0±9.1 µSv, (p=0.013). The results were 140.4±15.2, 120.2±10.4 and 111.6±11.2 µSv in Cranex Tome (p=0.208), which reveals no significant difference between two systems.Conclusion: There are no significant differences between the mean absorbed dose of surveyed models in panoramic imaging by two units (PM 2002 CC Proline and Cranex Tome). These results were less than occupational exposure recommended by ICRP, even at the highest calculated doses.

Abstract Background: Waste anesthetic gases (WAGS) leaked from new-type halogenated inhalational anesthetics such as sevoflurane were reported to pose a risk for the health of operation room personnel. The effects of WAGS on peripheral blood lymphocytes remain yet controversial. Purpose: The present study was undertaken to examine whether occupational exposure to sevoflurane has detrimental effects on the peripheral blood lymphocytes of exposed medical personnel in vivo. Methods: A cohort of 56 medical residents were divided into exposed group (n=28) and control group (non-exposed group) (n=28). Gas chromatograph was used to measure the concentration of sevoflurane in the medical resident’s breathing zone during surgeries under inhalation anesthesia in exposure group. The collection time lasted for one hour. Peripheral blood lymphocytes were isolated from venous blood and then apoptosis and cell cycle were analyzed by flow cytometry. EDTA-anticoagulated whole blood was harvested to analyze the lymphocyte subsets by flow cytometry. Immunoglobulins (IgA, IgM, IgG) were quantified by immunoturbidimetry.Results: The average concentration of sevoflurane in exposed group was 1.03 ppm with a range from 0.03 ppm to 2.24 ppm. Sevoflurane had no significant effect on the apoptosis and cell cycle of peripheral blood lymphocytes in the exposed group relative to the control group (P>0.05). Similarly, there was no significant difference in the lymphocyte subsets and the levels of immunoglobulins (IgA, IgM, IgG) between the two groups (P>0.05).Conclusion: Occupational exposure to low-level sevoflurane has no significant effect on the peripheral blood lymphocytes of operating room staff, but this conclusion needs to be confirmed by multicenter and long-term follow-up studies with large samples.Trial registration number and date of registration:ChiCTR2000040772, December 9, 2020 (Retrospective registration)

Research into the norm of internality ( Beauvois & Dubois, 1988 ) has shown that the expression of internal causal explanations is socially valued in social judgment. However, the value attributed to different types of internal explanations (e.g., efforts vs. traits) is far from homogeneous. This study used the Weiner (1979 ) tridimensional model to clarify the factors explaining the social utility attached to internal versus external explanations. Three dimensions were manipulated: locus of causality, controllability, and stability. Participants (N = 180 students) read the explanations expressed by appliants during a job interview. They then described the applicants on the French version of the revised causal dimension scale and rated their future professional success. Results indicated that internal-controllable explanations were the most valued. In addition, perceived internal and external control of explanations were significant predictors of judgments.

The locus of control dimensions among senior Pharm.D. students electing post-Pharm.D. residencies (N=15) are compared with first (N = 60) and second year (N = 55) Pharm.D. students, as well as with senior Pharm.D. students not electing residencies (N=14). Volunteer clinical faculty [hospital (N=27) and community (N = 23) pharmacy preceptors] and full-time clinical faculty (N = 22) are also compared. The Reid-Ware three-factor internal-external scale was completed by the study population. The dimensions measured were: self-control, social systems control, and fatalism. This study was undertaken to: 1. compare the locus of control dimensions among senior pharmacy students electing post-Pharm.D. residencies with senior pharmacy students not electing post-Pharm.D. residencies (and with other pharmacy students) and 2. compare the locus of control dimensions among senior pharmacy students electing and not electing post-Pharm. D. residencies (and with other pharmacy students) with their clinical, hospital, and community faculty to identify personality congruence (i.e., similarities, differences) which might facilitate the development of patient oriented-interactive behaviors necessary for clinical practice. Self-control (SC) was the major factor that generated the significant difference in the seven groups. The clinical (full-time) faculty group is significantly different from the student groups with the exception of the P-3, residency students. Over 50 percent of the P-3 residency group responded more like the faculty groups than like other students, with more P-3 residency students responding like clinical faculty than any other student or faculty group. The implication is that a similar personality profile exists in terms of internal/external control for the P-3 residency and full-time clinical faculty groups. Professors may have influenced, stimulated, or motivated those students most like themselves to enter a residency program. Based on the data, it would appear desirable to profile pharmacy students and give those who have a strong internal self-control factor encouragement to pursue post-graduate academic or residency programs and to retain as many of those individuals as possible in the areas of pharmacy that directly influence patient care and the pharmacist's role on the health care team.

Background:Tuberculosis (TB) is no longer a disease limited to developing nations and is still a major cause of significant morbidity and mortality worldwide. It can affect the different parts of the spine.Objectives:The aim of this study was to determine the preferred spinal location of TB.Methods:We conduct a retrospective and descriptive study in a single rheumatology department. Data were collected from observations of patients hospitalized in the past 20 years (2000-2020) who have been diagnosed with tuberculous spondylodiscitis (TS).Results:Fifty-two patients were included (37F/15M). Their mean age was 55.21 years ± 17.79 [19-91]. TS was more frequently unifocal (75%) than multifocal (25%). Lumbar spine involvement was the most common (57.7%) and more frequent in women (63.3%) but with no statistically significant difference (p = 0.2). Other localizations were described such as: dorso-lumbar (21.2%), dorsal (15.4%), lumbosacral (3.8%) and cervical (1.9%). Lumbar pain was present in 34 patients (65.4%) and 29 patients (55.8%) suffered from segmental lumbar stiffness. Imaging was contributive by showing the vertebral location using standard X-rays, computed tomography and magnetic resonance imaging. Disc pinch, erosion of vertebral plateaus and vertebral collapse were the major signs (82.7%, 65.4% and 67.3%, respectively).Conclusion:TS is a rare but serious clinical condition which may lead to severe deformity and early or late neurological complications. Spinal involvement is often unifocal and mostly diagnosed with lumbar pain or stiffness. Multifocal forms, touching several parts of the spine, however remain rare. Our findings remain consistent with those of the literature.Disclosure of Interests:None declared
