Factors Influencing the Breastfeeding Practices of Young Mothers Living in a Maternity Shelter: A Qualitative Study

Background: Young mothers have the lowest breastfeeding rates in Canada. Young mothers and their infants who access maternity shelters are especially at risk for poor outcomes, some of which breastfeeding may help to mitigate, yet little is known of the breastfeeding practices of this population. Research aim: The purpose of this study was to answer the research question, “What factors influence the breastfeeding practices of young mothers who live or have lived in a maternity shelter?” Methods: The study was conducted using interpretive description methodology and inductive content analysis. Data were collected by means of one-on-one interviews, with the participants recruited from a maternity shelter in Ontario, Canada. Results: Nine young mothers ages 17 to 24 years, who had initiated breastfeeding and resided at a maternity shelter, were interviewed. The five themes that emerged from the data were as follows: (a) choice, (b) special, (c) importance of early postpartum support, (d) being part of the “in crowd,” and (e) importance of ongoing supports. The participants in this study took ownership of their choice to breastfeed. Hospital postpartum nurses and lactation consultants had a critical role in the establishment of early breastfeeding, and ongoing, accessible, and nonjudgmental peer, family, and community support were important to breastfeeding duration. Conclusion: A combination of emotional and practical supports from multiple trusted sources, including professional and peer supports on an ongoing basis, enabled young mothers to reach their breastfeeding goals.

Abstract Background France has one of the lowest rates in the world regarding breastfeeding initiation and duration. Few studies have explored breastfeeding practices in France since the middle of the twentieth century, or following from initiation to cessation. The purpose of our study was to determine trends in breastfeeding over the past decades regarding public health recommendations, and to examine mothers’ perceptions about factors known to have an impact on breastfeeding support and cessation. Methods From the NutriNet-Santé cohort, 29,953 parous women (launched in 2009 to study relation between nutrition and health), were included in the present study. Using web-questionnaires, they were asked retrospectively if they had breastfed their youngest child or not, and if so, the duration of exclusive and total breastfeeding. For those who had breastfed, we investigated their perceptions about support at initiation and during the entire breastfeeding period and reasons for breastfeeding cessation. We also asked those who did not breastfeed about their perceptions and reasons for infant formula feeding their youngest child. Analyses were weighted according to the French census data. Results In the NutriNet-Santé cohort, 67.3% of mothers breastfed their youngest child. The proportion of breastfed children increased over the past few decades, from 55.0% (95% CI 54.3, 55.6) in the 1970s to 82.9% (82.4, 83.4) in the 2010s. Total and exclusive breastfeeding duration went from 3.3 months and 2.4 months respectively in the 1970s to 5.9 months and 3.2 months respectively in the 2010s. Most mothers felt supported at initiation and during the breastfeeding period. A reported desire to have breastfed longer than two months was 59.5%. Mothers who did not breastfeed did it by choice (64.3%). They did not feel guilty (78.2%) and did not perceive a problem not to breastfeed (58.8%), but almost half of them would have liked to have breastfed (45.9%). Conclusion Breastfeeding duration has increased in the past decades but did not reach the public health recommendations threshold. Targets other than mothers have to be considered for breastfeeding education, like the partner and her environment, to increase breastfeeding practices. Trial registration The study was registered at ClinicalTrials.gov (NCT03335644).

Abstract This article analyzes how institutions influence the process of identity formation within the Polish minority communities in Belarus and Lithuania. We focus on ways that the identities of people who consider themselves Poles in Belarus and Lithuania are targeted by institutions like the state, schools, and nongovernmental organizations. We aim to shed light on how these processes are shaped by institutional settings and broader political contexts. The authors take a bottom-up approach to institutions and look at how members of the Polish communities in the two neighboring countries conceptualize the role of various institutions—NGOs, schools, Karta Polaka (the Polish Card)—to shape their sense of ethnic belonging. The article is built on a cross-case analysis. Data for the Lithuanian and Belarusian cases, consisting of interviews and secondary sources, were collected independently and then reread in light of a common research question. Through our analysis, we show differences and similarities in how analogous institutions function on the two sides of the border and elaborate on the reasons why these differences occur and what role state policy and supranational regulations play in the process.

Intimate partner violence (IPV) is a complex and multifaceted problem gaining increasing attention within mental health research and practice. IPV explanations focus on both individual and systemic levels; however, it is increasingly acknowledged that a single level explanation may not be sufficient. The practices of clinical disciplines may, however, still privilege an explanation at one level over another, which will influence how they work with clients. It is likely that one such clinical group, psychologists, may play a critical role in helping victim-survivors to understand and explain their experiences of IPV. However, we were unable to find any studies focusing on women’s perceptions of psychologists’ role in this. Additionally, we know little about women victim-survivors’ perceptions of why their partners use IPV. To address these gaps, the research question for this study was: What explanations resonate during counseling for women in understanding their partner’s abuse? To explore this question, 20 women who had seen psychologists after experiencing IPV participated in semi-structured interviews. The interviews were analyzed using reflexive thematic analysis and three themes constructed from the data. The first two themes, narcissist description was helpful and not all bad all the time, showed that the women found it powerful in their healing processes when psychologists offered the opportunity to discuss their partners individual characteristics as explanations for their use of IPV. The third theme, structural explanations, showed that some of the women also reflected on wider structural contexts. Implications for clinical practice include the potential healing effect when practitioners can move along a continuum of explanation levels, covering both the inner and outer worlds. Implications for research into IPV perpetration are that women hold expertise and insight into individual perpetrators and could make valuable contributions to this field.

Background: Mothers are encouraged to exclusively breastfeed for the first 6 months. However, cesarean delivery rates have increased worldwide, which may affect breastfeeding. Research aim: This study aimed to determine the potential effects of cesarean delivery on breastfeeding practices and breastfeeding duration. Methods: This was a 6-month cohort study extracted from a 24-month prospective cohort study of mother–infant pairs in three communities in Hunan, China. Data about participants’ characteristics, delivery methods, breastfeeding initiation, use of formula in the hospital, exclusive breastfeeding, and any breastfeeding were collected at 1, 3, and 6 months following each infant’s birth. The chi-square test, logistic regression model, and Cox proportional hazard regression model were used to examine the relationship between breastfeeding practices and cesarean delivery. Results: The number of women who had a cesarean delivery was 387 (40.6%), and 567 (59.4%) women had a vaginal delivery. The exclusive breastfeeding rates at 1, 3, and 6 months were 80.2%, 67.4%, and 21.5%, respectively. Women who had a cesarean delivery showed a lower rate of exclusive breastfeeding and any breastfeeding than those who had a vaginal delivery ( p < .05). In addition, cesarean delivery was related with using formula in the hospital and delayed breastfeeding initiation. Cesarean delivery also shortened the breastfeeding duration (hazard ratio = 1.40, 95% confidence interval [1.06, 1.84]). Conclusion: Healthcare professionals should provide more breastfeeding skills to women who have a cesarean delivery and warn mothers about the dangers of elective cesarean section for breastfeeding practices.

Objectives: To assess associations between maternal depression and breastfeeding practices in mothers in the Maldives. Design: A prospective cohort study. The validated questionnaires contained information on demographics, breastfeeding initiation, breastfeeding duration, and the Edinburgh Postnatal Depression Scale. Logistic regression analyses were used to assess association between Edinburgh Postnatal Depression Scale score and breastfeeding practices while adjusting for lifestyle determinants. Cox regression was done to measure the association between maternal depression and breastfeeding duration. Settings: Antenatal clinic at Indira Gandhi Memorial and ADK Hospitals in Male’, Maldives. Subjects: A total of 458 mothers from the antennal clinics were interviewed at 36 weeks of gestation and again at 1, 3, and 6 months after birth. Results: Antenatal depression at 36 weeks of gestation was associated with late initiation of breastfeeding (adjusted odds ratio = 3.0, 95% confidence interval = 1.3-6.8). Postnatal depression was associated with shorter duration of exclusive, full, and any breastfeeding ( P < .001). Conclusion. Depression is negatively associated with breastfeeding practices. There is a need to manage antenatal postnatal depression in mothers in order to encourage them to initiate breastfeeding earlier and to breastfeed for longer.

COVID-19 has impacted education on all levels, with many institutions turning to online formats to deal with the global public health crisis. This study aims to carefully consider participatory risk management, given concerns about the specific impact of COVID-19 upon environmental and outdoor education. An environmental and outdoor education expedition-style university-based field course at the Laponia World Heritage Site provided the context for considering environmental and outdoor education’s response to COVID-19. Whether or how risk could be effectively managed in the unique setting during the COVID-19 pandemic was explored using action research methodology. A combination of systematic instructor observation, student–instructor communication, and surveys to student participants provided the data to consider the research question. Outcomes underscore the critical role of participatory risk management in environmental and outdoor education settings and highlight the concept of interdependence in environmental and outdoor education risk management. In addition, the research provides support for the action research idea of practitioners as researchers.

All pre-entrepreneurial development training programs collect data on the participating individuals as baseline data. This paper’s research question is whether the baseline data of youths can be analyzed to establish comparative gender characteristics. The analysis, data compared gender characteristics to identify customized gender training programs and policies; and as a mechanism for monitoring these characteristics as individuals develop into entrepreneurs. Data from youths being trained after an insurgency in a developing country were used and analysis was conducted with the help of SPSS ver. 21. Findings were that male youths were significantly more interested than female youth in the training. Significant correlations of assosiated chararctertisitcs among the youths were found to be similar across gender, although some were gender related. ANOVA revealed a significant difference in education, meals eaten per day and savings with varying levels of effect among male and female youths, with male youths scoring higher than the females. Education mediated monthly income and savings among female youths; however, among male youth its mediation was on the number of dependents they had. Finally, a binary logit regression showed that both genders were predictable using their education, quality of house they reside in, number of dependents and amount of savings but with varying direction of likelihood and extent. Implications of the study are discussed in the paper.

ABSTRACT Background Recent reviews have proposed a causal relationship between prolonged breastfeeding and early childhood caries (ECC), but the evidence to date is inconsistent, with few cohort studies and limited investigation of key confounders. Objective This study aimed to investigate the relationship between dietary practices and early childhood caries in a birth cohort of Australian preschoolers. Methods Participants underwent a standardized dental examination at 2–3 y of age to determine the prevalence of ECC (based on the presence of decayed, missing, or filled tooth surfaces). Breastfeeding practices were reported at 3, 6, 12, and 24 mo of age. Intakes of free sugars were assessed at 1 and 2 y of age. Multivariable regression models generated prevalence ratios (PR) for the association between ECC and breastfeeding duration, and between ECC and sleep feeding practices at 1 y, controlling for sociodemographic factors and free sugars intake. Results There was no independent association between breastfeeding beyond 1 y of age and ECC (PR 1.42, 95% CI: 0.85, 2.38), or between breastfeeding to sleep and ECC (PR 1.12, 95% CI: 0.67, 1.88), although the direction of effect was suggestive of an association. The only factors independently associated with ECC were high free sugars intakes (PR 1.97, 95% CI: 1.13, 3.44), and greater socioeconomic disadvantage (PR 2.15, 95% CI: 1.08, 4.28). Most participants who were breastfed at 1 y of age had ceased by 18 mo or 2 y. Conclusions Breastfeeding practices were not associated with ECC. Given the wide-ranging benefits of breastfeeding, and the low prevalence of sustained breastfeeding in this study and Australia in general, recommendations to limit breastfeeding are unwarranted, and breastfeeding should be promoted in line with global and national recommendations. To reduce the prevalence of early childhood caries, improved efforts are needed to limit foods high in free sugars.

The purpose of this study is to extend existing approaches to civic and English as a foreign language (EFL) literacy education to include critical intercultural citizenship. Students not only need to learn how to communicate, but should also develop their positions as citizens who exercise literacy skills to demonstrate connections to others in intercultural domains. The central research question explored how teacher pedagogy in the intercultural citizenship curriculum helped students to become more intercultural and caring citizens. A practitioner action study was conducted for two years with 10 fifth- and sixth-graders (11–12 years old) in Korea. All 40 one-hour video-taped class sessions containing eleven graphic novel discussions, as well as the artifacts students produced, were collected for analysis. Data analysis illustrates that critical citizenship literacy skills can be developed with teacher pedagogy that help students reflect on the cultural knowledge and practices that seem natural to them. The pedagogies of dialoguing, active consideration of missing perspectives and direct juxtaposition of students’ own and others’ social contexts are unpacked to suggest ways of incorporating intercultural citizenship education into literacy education. Educational implications are considered in terms of teacher engagement with learners, expanded position-taking of learners as intercultural citizens, and selection of discussion topics.
