Hydrothorax as a Complication of Pediatric Peritoneal Dialysis

Hydrothorax is a known complication of peritoneal dialysis (PD) but there is very little in the pediatric literature concerning this complication. From 1982 to 1984 seven of 29 of our patients who underwent peritoneal dialysis, developed pleural effusions as a complication of PD. These patients varied in respect to age, technique and duration of PD. Four of the seven developed respiratory symptoms at the time when effusions were discovered, while the other three were recognized during evaluation of loss of ultrafiltration. Five of the seven had right-sided effusions, one had a left-sided effusion, and one had bilateral effusions. No technical factors could be identified as causative agents. We have concluded that pediatric patients may be particularly likely to develop hydrothorax as a complication of peritoneal dialysis. This may present as a pulmonary emergency or as a subtle loss of ultrafiltration ability. The possibility of congenital potential communicating pathways seems more likely than any other single explanation for this phenomena. Hydrothorax as a complication of peritoneal dialysis (PD) is reported chiefly in the adult PD literature. The first reports were associated with trauma but in most subsequent cases, there was no explanation. We are aware of only two accounts of this complication in children covering a total of eight cases. Several of these may have been related to surgical trauma (1). However, in our own patient population, we observed seven who developed hydrothorax while on various forms of PD. Review of individual cases did not demonstrate common factors which would explain the hydrothorax. Thus, hydrothorax developed during PD in a significant number of infants over a short period and under a variety of clinical circumstances. This suggests the existence of potential anatomical channels which may open under a variety of circumstances.

Four patients on continuous ambulatory peritoneal dialysis (CAPD) developed large, symptomatic pleural effusions after commencing peritoneal dialysis. Pleuroperitoneal fistula in each case was diagnosed by the presence of a high glucose content in pleural fluid, with a normal corresponding blood sugar, and was confirmed by isotope or contrast peritoneography. Two patients had their effusions drained percutaneously, and then underwent pleural sclerosis with intracavitary tetracycline. Two patients had a thoracotomy performed, of which no fistula was identified in one case, and the other patient underwent pleurectomy. All four patients successfully recommenced CAPD several weeks after therapy, without recurrence of effusions. We conclude that pleuroperitoneal connections associated with CAPD do not mandate cessation of peritoneal dialysis and conversion to maintenance haemodialysis. Definitive diagnosis requires aspiration of pleural effusions for glucose estimation. Contrast or isotopic peritoneography is helpful in localising the fistula, but in our experience did not alter management. Simple sclerotherapy is effective and avoids the need for a formal thoracotomy.

A multicenter study was conducted to determine the relationship between intraperitoneal volume and solute (e.g., urea, creatinine) transport as determined by the dialysate to plasma (D/P) ratio and mass transfer area coefficient (MTAC). Two 4-h peritoneal equilibration tests were conducted on each of 12 pediatric peritoneal dialysis patients (mean age, 10.8 yr; range, 0.2 to 19.2 yr). One test exchange volume was 900 mL/m2 body surface area (BSA), and the other was 1,100 mL/m2 BSA. Dialysate samples were collected at 0, 30, 60, 120, 180, and 240 min. Blood samples were drawn at 0 and 240 min. Solute equilibration was significantly more rapid with the 900 mL/m2 BSA exchange volume than with the 1,100 mL/m2 exchange volume when evaluated by the D/P ratio. In contrast, no differences in solute transport were noted with either exchange volume when assessed with the MTAC. In conclusion, solute D/P ratios vary with changes in the intraperitoneal volume, necessitating the use of a standardized exchange volume for the reliable interpretation of evaluations such as the peritoneal equilibration tests. In contrast, the use of the MTAC allows for an accurate assessment of solute transport over a wide range of exchange volumes.

Attempts have been made to eradicate the tsetse flies G. morsitans and G. swynnertoni from two blocks of savannah woodland situated in the Central Province of Tanganyika.The insecticides were applied from aircraft. Coarse aerosols were used, with mass median diameters of approximately 90 microns; droplet diameters varied from 4 microns to 250 microns approximately.Eight applications of insecticides were made at intervals of two weeks. Each application was carried out at a nominal dosage of 0·25 gallons per acre, which was equivalent to 0·20 1b. per acre of the p, p'isomer of DDT or 0·03 lb. per acre of the γ isomer of BHC.In the area treated with DDT it is possible that both species of flies were eradicated for a short period, but small populations were re-established there by immigrant flies. In the other block the reduction was not so great, but it is not considered that this was due to a lesser effectiveness of the BHC, but to a combination of circumstances that led to less effective applications.Some general observations are made upon the use of aircraft for this sort of work, particularly in connection with the effect of meteorological conditions.

The pandemic has led to an increase in the use of pulse oximetry to assess and manage patients with COVID-19 disease. Paul Silverston explains the principles of pulse oximetry and the factors that can affect the reliability and accuracy of readings Pulse oximetry is performed to detect and quantify the degree of hypoxia in patients with respiratory symptoms and illnesses, including patients with COVID-19 disease. Pulse oximeters are non-invasive, simple to use and inexpensive, but it is important to know how to interpret the readings in the context of the patient's symptoms and the other clinical findings. In COVID-19 disease, very small differences in the oxygen saturation reading result in significant differences in the way that the patient is managed, so it is important to be aware of the factors that can affect these readings. It is also important to appreciate that a low reading in a patient with suspected or confirmed COVID-19 disease may be the result of another disease process.

Covid-19 are one of the viruses that were widely spreaded in 2019 and are still separate until nowadays. Thailand is one of the countries that are highly infected. The majority of people who are infected with this virus tend to have mild to severe respiratory symptoms. Furthermore, anyone can get sick, and it can lead to death. However, the most common symptoms of this virus are fever, cough, tiredness, and loss of taste or smell, on the other hand, characteristics in a minority of people, such as diarrhea and headaches. Due to the impact of the Covid-19 virus, people have to change their lifestyle to the online form. These changes have impacted mostly on economics and education in particular countries, so this problem also affects anxiety among high school students; who must prepare to apply to the university during the COVID-19 outbreak. In addition, we have collected 151 answers from high school students by surveying in order to know the feelings for entrance to the university during the pandemic situation. We found out that 81.3% of the students are highly affected by covid, and only 0.7% of students are slightly affected. We also found out that 55% of the students are worried about university entrance, and only 2.6% of the students were not worried at all. According to the result, most of the students in Thailand are facing the problem about their education and their entrance for the university which are caused by Covid-19. Shared psychotic disorder, characterized by shared delusion among two or more subjects (termed “Folie à deux,” “trois,” etc.), is often associated with strong religious beliefs or social isolation, factors creating strong psychological sympathy. Recently, we treated a rare familial case of “Folie à quatre” in central Tokyo without such influences. The proband was a schizophrenia patient and younger brother within monozygotic twins. Positive symptoms were “transmitted” to remaining family members, his elder brother, mother, and father father, in a relatively short period of three months. Although the pathophysiology of these positive symptoms (delusions and hallucinations) remains unclear, the transmission pattern suggests the primacy of social and environmental factors (and/or their interaction), while genetics appeared less influential in this “Folie à famille.” Although undiagnosed psychoses in the whole family cannot be excluded, they did not share the other negative schizophrenia symptoms of the proband. A strong familial connection appeared to be the most important factor for the common delusion and hallucination.

Abstract This study examines cross-speaker repetition in triadic conversations in the Japanese language. For the analysis, three sets of triadic conversations between close friends taken from a TV talk show are used. The results reveal that repetition in triadic conversations performs distinctive functions that are not observed in dyadic conversations: repetition often takes place between only two participants of the triad and allows the two to team up and strengthen their bond exclusively (teaming repetition), or even playfully tease the third participant (teasing repetition). Repetition is also shared between the three participants. In such cases, it allows the participants to create an instant bond by joking or referring to shared circumstances (immediate threefold repetition), or to gradually establish rapport by connecting their utterances and co-constructing a story (repetition relay). All these types of repetition express the participants’ points of view and contribute efficiently to their relationships that shift from moment to moment. The study further demonstrates cases in which one participant makes good use of various repetition types in a short period of time and efficiently allocates thoughts and feelings to the other two participants. The study concludes that repetition plays a significant role in Japanese conversation, helping speakers to converse smoothly and manage the dynamic relationships efficiently.
