Association of pesticide exposure, vaccination response, and interleukin-1 gene polymorphisms

We performed a cross-sectional study involving workers from four European countries in which exposure to pesticides and immune parameters were evaluated over a short period of time. The total study population consisted of 238 workers occupationally exposed to pesticides and 198 nonoccupationally exposed workers. The study showed that pesticide exposure at levels encountered by workers under different conditions in Europe did not affect the ability of the immune system to respond to vaccination. We could, however, identify individuals within the group of pesticide exposed workers who were genetically characterized by the 2.2 IL-1α polymorphism and who showed a lower antibody response, pointing out the importance of the understanding of genetic variability and the interaction between genetic and environmental factors in the identification of high-risk individuals, which may eventually lead to preventive measures.

Occupational Asthma in Subjects Occupationally Exposed to Herbal and Fruit Tea DustWe performed a cross-sectional study to detect occupational asthma (OA) in 63 subjects occupationally exposed to herbal and fruit tea dust and in 63 corresponding controls. The evaluation included a questionnaire, skin prick tests to workplace and common inhalant allergens, spirometry, and histamine challenge test. The evaluation of the work-relatedness of asthma in the exposed workers was based on serial peak expiratory flow rate (PEFR) measurements and bronchoprovocation tests. We found a higher prevalence of respiratory symptoms in the exposed workers, whereas spirometric parameters were significantly lower. The prevalence of sensitisation to allergens and of bronchial hyperresponsivenss (BHR) did not differ significantly between the groups. The prevalence of asthma was also similar in both groups (8.0 % vs. 6.4 %; P=0.540). Work-relatedness of symptoms was reported by all asthmatic tea workers and by no control with asthma. Significant work-related changes in PEFR diurnal variations and in non-specific BHR, suggesting allergic OA, were found in one tea worker with asthma (1.6 %). No specific workplace agent causing OA in the affected subject was identified. None of the tea workers with asthma met the criteria for medical case definition of the reactive airway dysfunction syndrome (RADS). Our data confirm workplace exposure to herbal and fruit tea dust as a risk factor for OA.

Temporomandibular joint (TMJ) is commonly involved in various immune-mediated rheumatic disorders accounting for significant disability and impaired quality of life. The aim of our study was to assess inflammatory and immune parameters in patients with TMJ arthritis related to rheumatoid arthritis (RA), juvenile idiopathic arthritis (JIA), ankylosing spondylitis (AS) and psoriatic arthritis (PsA) and to identify potential relation with severity and dysfunction of TMJ pathology. We performed a cross-sectional study in a cohort of 433 consecutive RA, 32 JIA, 258 AS, and 103 PsA. Only patients presenting with clinically significant TMJ involvement (273) related to their rheumatic condition were included in the final analysis. TMJ involvement is traditionally described in chronic inflammatory rheumatic disorders, particularly in patients with higher levels of inflammation as detected in rheumatoid arthritis and psoriatic arthritis. Disease activity and severity, as well as biological and positive serological assessments (rheumatoid factor, anti-cyclic citrullinated peptide, IL-1) remain significant determinants of the severity of TMJ arthritis.

Background: Thyroid diseases occur more frequently in people exposed to ionizing radiation, but the relationship between occupational exposure to ionizing radiation and thyroid pathologies still remains unclear. Objective: To evaluate the prevalence of thyroid diseases in healthcare workers exposed to low-level ionizing radiation compared with a control group working at the University Hospital of Bari, Southern Italy, and living in the same geographical area, characterized by mild iodine deficiency. Methods: We ran a cross-sectional study to investigate whether healthcare workers exposed to ionizing radiation had a higher prevalence of thyroid diseases. Four hundred and forty-four exposed healthcare workers (241 more exposed, or “A Category”, and 203 less exposed, or “B Category”) and 614 nonexposed healthcare workers were enrolled during a routine examination at the Occupational Health Unit. They were asked to fill in an anamnestic questionnaire and undergo a physical examination, serum determination of fT3, fT4 and TSH, anti-TPO ab and anti-TG ab and ultrasound neck scan. Thyroid nodules were submitted to fine needle aspiration biopsy when indicated. Results: The prevalence of thyroid diseases was statistically higher in the exposed workers compared to controls (40% vs 29%, adPR 1.65; IC95% 1.34-2.07). In particular, the thyroid nodularity prevalence in the exposed group was approximately twice as high as that in the controls (29% vs 13%; adPR 2.83; IC95% 2.12-3.8). No statistically significant association was found between exposure to ionizing radiation and other thyroid diseases. Conclusion: In our study, mild ionizing radiation-exposed healthcare workers had a statistically higher prevalence of thyroid diseases than the control group. The results are likely due to a closer and more meticulous health surveillance programme carried out in the ionising radiation-exposed workers, allowing them to identify thyroid alterations earlier than non-exposed health staff.

BackgroundIn 2012, ortho-toluidine (OT) was listed as a Group 1 carcinogen by the International Agency for Research on Cancer, based on epidemiologic observations in workers co-exposed to OT and aromatic amines. Between 2014 and 2017, 10 cases of bladder cancer (BC) were identified in Japanese males working at two plants of the same company manufacturing intermediates of organic dyes and pigments.ObjectiveTo describe the features of the BC epidemic at the plants.MethodsWe conducted a cross-sectional study of 76 OT and/or aromatic amine-exposed workers including the 10 BC cases at the plants in 2017. The co-exposed aromatic amines were para-toluidine, ortho-anisidine, aniline, 2,4-dimethylaniline, and/or ortho-chloroaniline. Details of each worker’s job-site histories were obtained from the company records. Past medical symptoms and histories were checked in physician interviews. The subjects were divided into the BC group (n=10) and the non-BC group (n=66) and compared. No quantitative exposure data were available. The surrogate level of exposure to each aromatic amine was calculated as the summed job-weight-month in each process in each job-year.ResultsThe mean ages of the non-BC and BC groups were 50 and 56 years and the durations of employment were 23 and 20 years, respectively. The smoking rate in both groups was 80%. Significantly higher rates of gross hematuria (70%) and cystitis (70%) were identified in the BC group’s past medical histories. In the BC group, the surrogate levels of exposure to OT were higher than those of exposures to other aromatic amines. The surrogate levels were high in the job processes of filtering, rinsing, drying and packing products.ConclusionsThe subjects with BC were associated with a high surrogate level of exposure to OT. OT-exposed workers with past histories of gross hematuria and cystitis need a careful follow-up.

Introduction: Leishman stain has been used as the stain of choice for peripheral blood films since many decades .But it has a disadvantage of consuming 15 minutes for the procedure alone thereby increasing the turn around time of peripheral smear reporting. In this study modified Leishman stain was made by adding phenol to conventional Leishman to reduce the staining time to 3 minutes without interfering with the quality of stain. Aim: To study the quality of modified Leishman stain in comparison with conventional preparation on peripheral blood smears. Materials and Methods: The present cross-sectional study was carried out in Central Haematology laboratory of a tertiary health care centre in Southern India. A pilot study was done to determine the ideal ratio of phenol to Leishman powder in preparing modified stain without compromising the staining quality. After obtaining consent, the blood sample was collected in anticoagulated vials. Sample size was calculated to be 85.Two thin wedge smears were prepared from each sample and one was stained with conventional Leishman stain and other with modified Leishman stain. The staining characteristics were assessed and scored by an experienced pathologist in terms of 6 parameters- RBC pattern, nuclear pattern, neutrophil granules, eosinophil granules, platelets and background staining. The grades were 1, 2, 3 & 4 (poor, satisfied, good and excellent). Smears each with grades 2, 3 and 4 were selected for study. The results were analysed by Statistical Package for the Social Sciences (SPSS) software version 16.0. Result: The mean value for the staining quality using conventional method was 4.73 and for modified method was 5.39, and the total score possible is 6. Photomicrograph showed excellent results with modified Leishman stain. Thus from these values we can interpret that modified method gave much more acceptable results than that of conventional method. Conclusion: Unlike the conventional method which requires a total of 15 minutes, to complete the staining process, modified Leishman staining techniques takes only 3 minutes. Blood films can be stained within a short period of time thus aiding in rapid diagnosis and treatment of patients. Keywords: Leishman stain, Modified Leishman stain, Phenol.

Introduction: Various forms of malocclusion are a matter of serious concern in Nepalese population. This study was carried out to understand the prevalence of malocclusion among the school children of Biratnagar. The objective of this research is to find out the prevalence of malocclusion of children from different schools of Biratnagar visiting the Pedodontics and Orthodontics department of Nobel Medical College and Teaching Hospital, Biratnagar. Materials and Method: A descriptive cross sectional study method was used in this research. Data was collected by using direct observation of the subjects and occlusal assessment was done according to Angle’s classification and Dewey’s modification types of class I, class II and class III malocclusion. Result: Subjects with normal occlusion was found to be 39 % and with malocclusion was found to be 61%. Among them, class I malocclusion (60%) and angles class II div I subjects (88.33%) were in majority of the total study population. Conclusion: The present study helps to determine the prevalence of malocclusion and need of orthodontic treatment for the school children of Biratnagar

Effects of pesticide exposure on farmer’s health are conducted mostly in developed countries, which is rare in least developed countries like Bangladesh. Here farmers had less guidance and instruction about the preventive measures to protect themselves and environment. This work was studied to assess the impacts of pesticide exposure on the paddy farmer’s health in two different Upazilas of Manikganj, Bangladesh. The study was conducted as cross-sectional study with quantitative and qualitative components. Five hundred eighty two farmers (368 sprayers and 214 non-sprayers) were interviewed. Chi-squared tests and multinomial logistic regression analysis were performed for statistical analysis. Three hundred sixty eight farmers (64.3%) sprayed pesticides and were directly exposed to chemicals. Among them 64.22% and 9.06%, farmers used moderately and highly hazardous pesticides, respectively. 257 farmers (69.8%) were not taken any protective measures to handle pesticides. Excessive sweating, burning eyes and fatigue were reported by 26.3%, 24.4% and 18.8% of the farmers, respectively. These types of symptoms were significantly associated among male farmers. The study reveals that due to lack of awareness, occupational exposures of pesticides among farmers are common in Bangladesh and it also emphasize the importance of use of personal protective equipments. Abstract The progression of cardiometabolic diseases is determined by both genetic and environmental factors. Gene-diet interactions may therefore be important in modulating the risks of developing metabolic diseases. The objectives were to investigate the effect of the interaction between BDNF Val66Met polymorphisms and Dietary Insulin Index and Insulin Load (DII and DIL) on Cardiometabolic Markers among diabetic patients. In this cross-sectional study, blood samples were collected from 667 patients. DIL & DII were defined using a validated food frequency questionnaire (FFQ). Genotyping the BDNF Val66Met polymorphism was conducted by the PCR-RFLP method. Interactions between dietary indices and gene variants were evaluated using a Generalized Linear Model (GLM). PGF2a concentrations were significantly higher among Val homozygotes than Met alleles carrier. This study revealed that, compared with individuals with the Val/Val genotype, those with the Met/Val or Met/Met genotype had lower BMI (P-interaction =0.04), TG (P-interaction=0.04), leptin (P-interaction =0.01), LDL (P-interaction=0.04) and TC (P-interaction =0.01) when they consumed diets higher on the DIL index. Moreover, the highest quartile of the DIL, compared to the lowest, showed increased in WC (P-interaction =0.02) and LDL/HDL (P-interaction =0.04) for Val/Val homozygotes compared to Met-allele carriers. BDNF Val66Met variants may interact with DIL and DII, thus be involved in the development of cardiometabolic risk factors. If diabetic patients with Met alleles regulate dietary intakes, they have a protective opportunity to regulate their cardiometabolic markers.
