Turnover decisions of women accountants: using personal histories to understand the relative influence of domestic obligations

Studies of accountants' turnover intentions have been unable to demonstrate a clear link between conventional job-related antecedents and women's higher rates of turnover. This study utilizes narrative histories of three women, including the author herself, who have exhibited actual turnover behaviour to examine the factors underpinning their turnover decisions. The findings demonstrate that while the study participants did identify job related factors, their turnover decisions were more significantly influenced by a complex interaction of job and domestic factors. This article thus recommends that an understanding of women's turnover decisions must first utilize research methods such as narrative history that allow the complexity of factors influencing their decisions to be identified and second, must incorporate greater consideration of specific domestic drivers of the cumulative workload/stresses of both job and domestic factors.

A study was carried out to find out the retrospective incidence of uterine torsion in buffaloes among obstetrical cases in Amul milk shed area and factors influencing it. Data on 1,13,772 obstetrical cases attended from January 2017 to June 2018 by Amul Veterinarians were collected. Moreover, 50 buffaloes suffering from uterine torsion were taken up during July-December, 2018 to know the side, site, degree of uterine torsion, parity as well as sex and viability of the calf and the dam after detorsion/Caesarean section in the same area. In the retrospective study, a total of 2000 cases of uterine torsion were recorded among total 1,13,772 bovine obstetrical cases, which encompassed 1.76%. Of the 2000 cases, 92.20 (1844) percent torsions were found in buffaloes only. Among 57,111 obstetrical cases attended in buffaloes, the incidence of uterine torsion was 3.23%. Moreover, the region/center-wise incidence of torsion cases varied from 1.30–19.36%. The highest incidence of uterine torsion was found in Anand region (19.36 %) followed by Kathlal (11.23 %) and Mahemdabad (10.14 %) regions, while the lowest incidence was in Virpur (2.06%), Petlad (1.46%) and Piplata (1.30%). It was concluded that buffaloes mostly experience right side (100%), post-cervical (82%) uterine torsion of 270–360° (66%), at full term of gestation (70%)with lower survivability of the calves (30%), however, the survival rate of the dams post-treatment was 90%.

BACKGROUND: There is a growing interest in HR-analytics because of its ability to analyze employee behavior based on HR data. Predicting voluntary turnover of employees is an important topic of study, both in academia and industry. OBJECTIVE: The current study analyzes determinants for turnover, distinguishing between blue and white collar workers. The turnover analyses are based on a dataset from a payroll company, in contrary to previous turnover studies that used survey and interview data. METHODS: The studied dataset contains demographic and work specific factors for more than 380000 employees in 15692 Belgian corporations. Logistic regression is used to estimate individual turnover probabilities, the goodness of the model is tested with the AUC method. RESULTS: The study confirms turnover determinants and differences between blue and white collar workers that were described in previous work based on survey and interview data. Additionally, the study exposes so far unstudied turnover determinants and differences between blue and white collar workers. Confirmed determinants are among others age, seniority, pay and work distance. New determinants are company car, meal vouchers, night work and sickness. Different relationships to turnover are revealed for blue and white collar workers based on gender, number of children, nationality and pay. CONCLUSIONS: The presented dataset-based approach has its merit in analyzing turnover: it enables to study actual turnover instead of turnover intentions, and reveals new turnover determinants and differences between blue and white collar workers.

Introduction: Evidences suggest a significant decline in maternal health service uptake following the coronavirus disease 2019 (COVID-19) pandemic in Ethiopia. However, COVID-19 related factors impacting the service uptake are not sufficiently addressed. Hence, the current study was intended to explore COVID-19 related factors influencing antenatal care service uptake in rural Ethiopia. Methods: A community-based qualitative study was conducted from September 25/2020 to November 25/2020 among selected pregnant women residing in rural districts of Bench-Sheko Zone, and healthcare providers working in the local health care facilities. Six focus group discussions and 9 in-depth interviews were made between pregnant women, and health care providers, respectively. Data was collected by health education and behavioral science professionals who also have experience in qualitative data collection procedure. Data were transcribed, translated, and analyzed thematically using Open Code 4.0 software. Result: The study revealed several COVID-19 related factors influencing the uptake of antenatal care service during the pandemic. Health facility related barriers, perceived poor quality of care during the pandemic, government measures against COVID-19, anxiety related to the pandemic, and risk minimization were the identified factors possibly influencing the current antenatal care service uptake among women in rural Bench-Sheko Zone. Conclusion: COVID-19 preventive measures, and health facility related factors and individual factors were responsible for the current decline in antenatal care service uptake. Preserving essential health care service is critical to prevent avoidable losses of maternal and child lives during the pandemic period. Hence, programs and strategies designed to maintain maternal health services particularly, antenatal care service have better take the above determinants into consideration.

Objective: This study aimed to examine the psychological work environments and associated factors among clinical nurses in a provincial tertiary hospital in the North of Vietnam in 2020. Methods: A cross-sectional survey was conducted among all nurses of 22 clinical wards who directly provided care to patients. 261/318 eligible nurses returned the self-reported survey questionnaires that consisted of the Vietnamese version of the Job Content Questionnaire 22 items and originally developed questions on personal characteristics and work conditions. Descriptive analysis, t-test, ANOVA, and multivariate linear regressions were applied to describe the current job demand, job control, and related factors at the significant level p less than 0.05. Results: The response rate of the current study was 82.1%. Nurses reported moderate job demand (29.39±3.64) and job control (66.86±5.79). Increased job control was significantly associated with more frequency of receiving support from superiors (p<0.001), a self-perceived opportunity of promotion (p = 0.011), and more night shifts per week (p=0.004). Less support from co-workers and more night shifts were associated with a higher job demand score among study participants (p = 0.041 and 0.002, respectively). Conclusions: Several nurses in the provincial hospital work in unfavorable work environments with high job demand and low job control, which could adversely affect nurses’ well-being. It is essential to promote social support at work to improve the psychological working conditions and performance of study participants. Objective.To clarify the prevalence, risk factors, outcome, and outcome-related factors of hemophagocytic lymphohistiocytosis (HLH) in patients with dermatomyositis (DM), polymyositis (PM), or clinically amyopathic dermatomyositis (CADM).Methods.Data of patients with DM, PM, or CADM who were admitted to the First Affiliated Hospital of Zhejiang University from February 2011 to February 2019 were retrospectively collected. Patients diagnosed with HLH constituted the case group. A 1:4 case-control study was performed to identify risk factors for HLH in patients with DM, PM, or CADM through comparison, univariate, and multivariate logistic regression analysis. Intragroup comparison was made among patients with HLH to identify factors influencing unfavorable short-term outcome.Results.HLH was a rare (4.2%) but fatal (77.8%) complication in patients with DM, PM, or CADM. The retrospective case-control study revealed that higher on-admission disease activity (p = 0.008), acute exacerbation of interstitial lung disease (AE-ILD, p = 0.002), and infection (p = 0.002) were risk factors for complication of HLH in patients with DM, PM, or CADM. The following intragroup comparison showed that higher on-admission disease activity (p = 0.035) and diagnosis of CADM (p = 0.039) might influence the short-term outcome of patients with HLH. However, no risk factor was identified after false discovery rate correction.Conclusion.In this study, secondary HLH was a fatal complication, with higher on-admission disease activity, AE-ILD, and infection working as risk factors. The underlying role of infection and autoimmune abnormality in HLH in connective tissue disease was subsequently noted. Clinical factors influencing the short-term outcome of patients with secondary HLH require further study.

The aim of this study was to explore and describe the perceptions of registered nurses regarding factors influencing service delivery regarding expansion programmes in a primary healthcare setting, using a qualitative approach. The registered nurses, who have been working in the clinics for more than two years and have been exposed to the expansion programmes there, were purposively sampled. Two focus group interviews were conducted in a neutral place and the data collected by the researcher Nnoi A. Xaba (N.A.X.). Data were analysed by the researcher and an independent co-coder using the Tesch method. Categories, subcategories and themes were identified; those that formed the basis of discussion were disabling factors, enabling factors, client-related factors, service-related factors and solutions to problems. It is recommended that integration of programmes and coordination be done at a provincial level and planned together with the training centres in order to alleviate problems in service delivery. Training on expansion programmes in the form of in-service education should be carried out continually in the region.Die doel van die studie was om die persepsie van geregistreerde verpleegkundiges met betrekking tot die  faktore wat dienslewering van die uitbreidingsprogramme in ‘n primêre gesondheid opset beinvloed; te eksploreer en te beskryf. ‘n Kwalitatiewe benadering is gevolg in die iutvoering van die studie. ‘n Doelgerigte steekproef is uitgevoer vanuit geregistreerde verpleegkundiges wat vir langer as twee jaar in die klinieke werksaam was en blootgestel is aan die uitbreiding programme. Twee fokus groep onderhoude is deur die navorser Nnoi A. Xaba (N.A.X.) in ‘n neutrale opset uitgevoer. Data is deur die navorser en ʼn onafhanklike kodeerder ontleed volgens Tesch se metode van analise. Kategorieë, sub-kategorieë en temas was geidentifiseer. Die kategorieë fundamenteel tot die bespreking behels: remmende faktore, bydraende faktore, kliënt-verwante faktore, diens-verwante faktore, en oplossing van probleme. Daar word aanbeveel dat die integrasie en koordinasie van programme op provisiale vlak beplan word in samewerking met opleidings instansies om die dienslewerings probleem te verlig. In die streek behoort opleiding met betrekking tot die uitbreidingsprogramme deurlopend deur middel van indiensopleiding gedoen word.

AbstractIntroductionEarly diagnosis and prompt and effective treatment is one of the pillars of malaria control Malaria case management guidelines recommend diagnostic testing before treatment using malaria Rapid Diagnostic Test (mRDT) or microscopy and this was adopted in Nigeria in 2010. However, despite the deployment of mRDT, the use of mRDTs by health workers varies by settings. This study set out to assess factors influencing utilisation of mRDT among healthcare workers in Zamfara State, Nigeria.MethodsA cross-sectional study was carried out among 306 healthcare workers selected using multistage sampling from six Local Government Areas between January and February 2017. Mixed method was used for data collection. A pre-tested self-administered questionnaire was used to collect information on knowledge, use of mRDT and factors influencing utilization. An observational checklist was used to assess the availability of mRDT in the six months prior to this study. Data were analyzed using descriptive statistics such as means and proportions. Association between mRDT use and independent variables was tested using Chi square while multiple regression was used to determine predictors of use at 5% level of significance.ResultsMean age of respondents was 36.0 ± 9.4years. Overall, 198 (64.7%) of health workers had good knowledge of mRDT; malaria RDT was available in 33 (61.1%) facilities. Routine use of mRDT was reported by 253 (82.7%) healthcare workers. This comprised 89 (35.2%) laboratory scientists/technicians, 89 (35.2%) community health extension workers/community health officers; 59 (23.3%) nurses and 16 (6.3%) doctors. Predictors of mRDT utilisation were good knowledge of mRDT (adjusted OR (aOR):3.3, CI: 1.6-6.7), trust in mRDT results (aOR: 4.0, CI: 1.9 - 8.2), having being trained on mRDT (aOR: 2.7, CI: 1.2 - 6.6), and provision of free mRDT (aOR: 2.3, CI: 1.0 - 5.0).ConclusionThis study demonstrated that healthcare worker utilisation of mRDT was associated with health worker and health system-related factors that are potentially modifiable. There is need to sustain training of healthcare workers on benefits of using mRDT and provision of free mRDT in health facilities.

Objective: Sexual dysfunction is a side effect of methadone maintenance therapy (MMT). Opium Tincture (OT) has been used as a maintenance treatment. This study aimed to determine and compare the trend of sexual function and its related factors during treatment with both drugs. Method: An observational study was designed to measure the blood tests including free and total testosterone, prolactin, and sex hormone-binding globulin and a battery of questionnaires, including demographics and drug use history, in 42 and 53 patients entering MMT and OT treatment before and 1 and 3 months after the treatment. Results: Significant changes in testosterone levels were observed in the MMT but not the OT group. The difference between the two groups was not significant. Neither between nor within changes in the sexual function and premature ejaculation scores were significant (P =0.370& 0.698). Anxiety levels were significantly different (P= 0.001) within and between groups. There was a considerable difference in the trend of depression changes in the OT group, but not different in MMT group and between the two groups. Conclusion: No difference was found between MMT and OT effects on sexual function variables. The decrease in Testosterone during the three months of MMT, was not associated with diminished sexual function. In the MMT group, anxiety levels diminished during treatment. It seems that decreased testosterone in the MMT group was compensated by improved anxiety. Gonadotropin levels may not be the sole determinant in sexual activity, and complex interaction of mood and anxiety, agonist levels, and gonadotropins are involved.

Abstract Background: A study was carried out to identify the factors influencing the utilization of ultrasound scan services among pregnant mothers at Ndejje Health Centre IV, Wakiso District. Methodology: The study design was descriptive and cross-sectional and it employed both quantitative and qualitative data collection methods. A sample size of 30 respondents was selected using a simple random sampling procedure. An interview guide was used to collect data. Results: The study revealed various factors influencing the utilization of ultrasound scan services among pregnant mothers. For example, although all 30 (100%) had ever heard about ultrasound scan services, most 20 (66.7%) had ever used ultrasound scan services once 10 (50%) and 12 (60%) used the services in the 3rd trimester due to factors including 20 (66.7%) of ultrasound scan services, 21 (70%) having fears about using ultrasound scan services including 14 (66.7%) fear that the scan would identify bad conditions on the baby, 18 (60%) lacked partner support. The study results also revealed that respondents faced various health facility factors which influenced the utilization of ultrasound scan services. For example, most 20 (66.7%) respondents reported that Ndejje Health Centre IV was not equipped to provide ultrasound scan services due to 12 (60%) frequent breakdown and poor maintenance of equipment which led to 18 (60%) ultrasound scan services not being readily available, 12 (60%) long waiting time to receive services as most waited more than 2 hours to receive services. Conclusion and recommendations: Respondents faced various and health facility-related factors which influenced their utilization of ultrasound scan services. The key recommendations included ready availability of services through regular and timely maintenance of equipment, improving efficiency, and reducing waiting time as well as improved health education of mothers about the importance of using the services.
