Plasma C-reactive protein (CRP) has been proposed to be a strong independent predictor for cardiovascular disease. This circulating form of CRP (native CRP or nCRP) is well described. Recently, the existence of a conformationally distinct isoform of CRP (modified CRP or mCRP) has been reported. The relevance of each CRP isoform to atherosclerotic disease is unknown. The purpose of this study was to examine the natural expression of CRP in undifferentiated, differentiated, and stimulated macrophages, cells known to contribute to atherogenesis in vivo, and to determine whether transcribed CRP was expressed as nCRP or mCRP. Macrophages were generated from U937 monocytes using phorbol 12-myristate 13-acetate. Differentiated macrophages were further stimulated with lipopolysaccharides (LPS). In undifferentiated, differentiated, and stimulated cells, CRP expression was assessed by reverse transcription–polymerase chain reaction, and CRP protein production was measured by fluorescence microscopy and flow cytometry (cellular CRP) or high-sensitivity enzyme-linked immunosorbent assay (secreted CRP). CRP transcript was minimally expressed in undifferentiated cells. Expression increased markedly in macrophages during differentiation and was not affected by LPS at 24 hrs. Cellular CRP protein increased in a time-dependent manner after LPS stimulation, and this induction was mediated via interleukin (IL)-6 and IL-1β. A small amount of secreted CRP was detected in the media of differentiated cells, but it was not significantly increased after LPS stimulation. Using specific monoclonal antibodies, our data indicate that cellular CRP is directly translated as the mCRP rather than the nCRP isomer. These results indicate that U937-derived macrophages are a good cell model to further study the production of mCRP under conditions relevant for the atherogenic process.

Latar belakang. Sepsis merupakan salah satu penyebab morbiditas dan mortalitas pada anak. Diagnosissepsis ditegakkan berdasarkan gejala Systemic Inflammatory Response Syndrome (SIRS) dan penemuan bakteripada kultur darah. Kultur bakteri darah memiliki sensitifitas yang rendah dan membutuhkan waktu yanglama sehingga sering menyebabkan terjadinya overdiagnosis dan overtreatment. C-reactive protein adalahreaktan fase akut yang kadarnya meningkat pada keadaan infeksi. High sensitivity C-reactive protein (hs-CRP) adalah metode yang lebih sensitif untuk mengukur kadar CRP dalam jumlah kecil.Tujuan. Mengetahui peran hs-CRP sebagai parameter diagnostik dan prediktor luaran sepsis pada anakyang menderita SIRS.Metode. Penelitian uji diagnostik dengan desain potong lintang terhadap 85 anak dengan gejala SIRS berusia1 bulan sampai dengan 15 tahun dan dirawat di bangsal anak RS.Dr.M.Djamil Padang sejak Juni sampaiNovember 2012. Pemeriksaan hs-CRP dilakukan dengan metode enzyme-linked immunosorbent assay (ELISA).Data dianalisis dengan SPSS serta dilakukan uji diagnostik. Baku emas sepsis adalah biakan darah.Hasil. Cut off point hs-CRP untuk menentukan sepsis adalah 15,55 ng/ml, (sensitivitas 90,9% dan spesivisitas53,8%). Kadar rata-rata hs-CRP meningkat sesuai dengan beratnya penyakit.Kesimpulan. High sensitivity C-reactive protein dapat dijadikan sebagai parameter diagnostik sepsis padapasien SIRS dengan cut off point 15,55 ng/ml, serta dapat dipakai sebagai prediktor luaran sepsis.

Objective The objective of this paper is to investigate the utility of serum procalcitonin (PCT) and C-reactive protein (CRP) as markers of infection in systemic lupus erythematosus (SLE) patients. Patients and methods Sixty-nine SLE patients with symptoms and signs of infection proved by culture and/or a favorable response to antibiotics and 69 SLE patients without infection were included. Serum PCT and plasma high-sensitivity CRP were assessed by an enzyme-linked immunosorbent assay. Results SLE patients with infection had a significantly higher level of CRP than those without infection ((median (IQR) 104.5 (25.5–100.9) and 10.3 (5.4–23.1) mg/l, respectively), p<0.001). Conclusion Serum PCT could not differentiate SLE patients with or without bacterial infection in this study, while the utility of CRP as a marker of infection has been confirmed.

Latar Belakang: Komposisi makanan tinggi lemak dapat menjadi faktor terjadinya obesitasyang menyebabkan oksidasi lemak. Oksidasi lemak dapat menyebabkan inflamasi yang dikarakterisasikan dengan tingginya kadar High Sensitivity C-Reactive Protein (hs-CRP). Ubi ungu kaya akan antioksidan terutama antosianin yang mungkin dapat menurunkan kadar hs-CRP. Penelitian ini bertujuan untuk mengetahui pengaruh sari ubi ungu terhadap kadar hs-CRP tikus sprague dawley dengan pakan tinggi lemak.Metode: Jenis penelitian ini adalah true experimental dengan  pre-post test control group design. 24 tikus sprague dawley jantan dibagi menjadi 4 kelompok yaitu kontrol negatif, kontrol positif, perlakuan 1 dan perlakuan 2. Perlakuan 1 dan 2 diberikan sari ubi ungu dengan dosis 2 gram/200grBB dan 3 gram/200grBB berturut-turut selama 6 hari. Sebelum dan sesudah perlakuan, kadar hs-CRP dianalisis dengan menggunakan metode ELISA (Enzyme Linked Immunosorbent Assay).Hasil: Selisih kadar  hs-CRP pada keolompok kontrol negatif, kontrol positif, perlakuan 1 dan perlakuan 2 adalah 0.4±0.20, -0.07±4.70, -4.3±0.79 dan -8.1±0.45. Terdapat perbedaan signifikan kadar  hs-CRP antar kelompok sesudah intervensi (p=<0.001). Pada kelompok perlakuan 2 terdapat penurunan yang paling tinggi (persen delta 33,33% dengan nilai p=<0.001).Simpulan: Sari ubi ungu dapat menurunkan kadar  High Sensitivity C-Reactive Protein (hs-CRP) secara signifikan.

Although several biomarkers are available to monitor the acute phase response, the short pentraxin C-reactive protein (CRP) is dominating in clinical practice. The long pentraxin 3 (PTX3) is structurally and functionally related to CRP, but not liver-derived. In addition, increased levels of PTX3 have been linked to preeclampsia. Reference intervals are usually based on healthy blood donors. Several physiological and immunological alterations occur during normal pregnancy with subsequent potential effects on blood analytes. Hence, this study aims to determine pregnancy-specific reference intervals for CRP and PTX3. Longitudinal clinical data and blood plasma samples from the 1st, 2nd and 3rd trimester of 100 healthy, non-medicating, females aged 18–40 at the time-point of conception were available to us. High‐sensitivity CRP measurements were performed by turbidimetry and enzyme-linked immunosorbent assay (ELISA) was used to quantify PTX3. CRP and PTX3 levels followed each other during the first two trimesters and both increased during the third trimester. CRP showed a median of 4.12 mg/L in the third trimester, and were significantly higher compared to the first (median 2.39 mg/L, p&lt;0.0001) and the second (median 2.44 mg/L, p=0.0006) trimesters. In the third trimester PTX3 levels reached a median of 7.70 µg/L, and were significantly higher compared to the first (median 3.33 µg/L, p&lt;0.0001) and the second (median 3.70 µg/L, p&lt;0.0001) trimesters. Plasma albumin was inversely correlated with CRP (rho=-0.27, p&lt;0.0001), but not with PTX3. In conclusion, it is important to consider pregnancy-specific reference values as elevations of CRP and PTX3 during the later phase may occur in absence of infection.

BACKGROUND: The mechanism of aging goes along with age, one of which is characterized by cellular senescent, which occurs mostly in adipose tissue. Adipose tissue is the site of accumulation of large cell senescent, in the regulation of obesity and aging. Proteins p53 is marker for cell senescent, which are also known to induce inflammation. This study was aimed to determine the relationship between circulating protein p53 and high sensitivity C-reactive protein (hsCRP) in central obese men with inflammaging.METHODS: The study design is an observational study with cross-sectional approach. The subjects were 75 central obese men (waist circumference/WC > 90 cm), aged ≥ 45 years old. Subjects were divided into 2 age groups, those are middle age group: 45-59 years old (50.7%) and elderly group: ≥ 60 years old (49.3%). Examination of circulating p53 was done using enzyme-linked immunosorbent assay (ELISA) method, and the hsCRP examination was done by chemiluminescent method.RESULTS: It was found that there was a correlation between circulating p53 and hsCRP in elderly (r=-0.414; p<0.05) but not in middle age (r=-0.127; p=0.449).CONCLUSION: From this study, it is assumed that more senescence cells in elderly are resulting in increased chronic inflammation.KEYWORDS: aging, senescent, inflammaging, protein p53, hsCRP

Objective: to study and analyze the links between endothelium-dependent vasodilation (EDV) indicators and the levels of fractalkine (FN/CX3CL1) and high sensitivity C-reactive protein (hs-CRP) in patients with acute coronary syndrome (ACS).Materials and methods: among the examined individuals with ACS, 63 patients had acute myocardial infarction (MI); 41 patients had unstable angina (UA), represented by first – time angina in 15 people and 26 people had progressive angina. Control group included 20 healthy control individuals. Pharmacological test with 5% acetylcholine (AcH) was used to assess the functional state of the vascular endothelium. Enzyme-linked immunosorbent assay was used to determine the levels of FN/CX3CL1 and hs-CRP.Results: all the examined patients with ACS showed statistically significant changes in EDV indicators compared to the control group, as well as an increase in the level of FN/CX3CL1 and hs-CRP. The most pronounced changes of the values of EDV indicators and the levels of FN/CX3CL1 and hs-CRP, from the examined patients, were detected among patients with acute MI. Th e presence of correlations between the studied indicators was revealed. Th e strength of the identified links was greater among patients with acute MI, compared to patients with UA. Th e strength of the links found in patients with both UA and acute MI was greater between EDV indicators and FN/CX3CL1 levels, than between EDV indicators and hs-CRP levels.Conclusions:all the examined patients with ACS had the EDV disorders associated with the severity of systemic infl ammation. In the group of patients with acute MI, the severity of EDV disorders was greater than in patients with UA, which was apparently due to the infl uence of the resorption-necrotic syndrome, which potentiates increased systemic infl ammation and damage to the endothelium of microvessels with EDV disorders, which was confirmed by the results of correlation analysis. A positive link was found between the levels of FN/CX3CL1 hs-CRP.
