Abstract Background It is well established that body mass index (BMI) and troponins are independently associated. However, whether the obesity could cause myocardial injury independent of coronary heart disease (CHD) remains unclear. This study focuses on the relationship between BMI and troponins, and whether this relationship is being attenuated when CHD is accounted for. Methods In populations without acute ischemic events, 383 patients with coronary artery stenosis less than 75% were included, that is, people who have not yet reached the indications for coronary intervention, and of them 70 patients being obese according to BMI ≥ 28 kg/m2. Continuous variables were represented as mean ± SD or median(inter quartile range[IQR]). Chi-square test was adopted for categorical data. Correlations between variables were evaluated by Spearman analysis, multiple regression or logistic regression. Results The circulating hs-cTnT level was higher in the obese group [8(6,11) ng/L vs. 6(4,9) ng/L; p < 0.001). In subgroup analysis based on the presence or absence of coronary heart disease(CHD), the adjusted β(95%CI) for circulating hs-cTnT exhibited a proportional relationship with BMI when the non-obesity were defined as the reference[β; 2.22(95%CI, 0.73 to 3.71) in non-CHD, 5.58(95%CI, 0.70 to 10.46) in CHD, p < 0.05]. Additionally, the degree of coronary stenosis has shown a positive correlation with circulating hs-cTnT (rho = 0.1162; p < 0.05). Conclusion When CHD is taken into account, obesity is independently associated to the elevation of circulating hs-cTnT, a biomarker of myocardial injury, potentially indicating the impact of obesity on non-ischemic subclinical myocardial injury.

Introduction: Heart disease is a major problem in public health lately. Delay in treatment is still a major problem in managing coronary heart disease. Objective: The purpose of this study was to analyze the pre-hospital care of a heart attack and its relationship with the delay to the hospital. Method: The study was conducted at Dr. Hardjono Ponorogo Regency, a representative sample of 108 respondents was taken purposively. The cross-sectional analytic design was conducted to measure the relationship between pre-hospital care and delay to the hospital. Analysis of the chi-square test with α = 0.05 was used to test the association between pre-hospital care and its relationship with the delay in the hospital for coronary heart disease patients. Result: The results of the study illustrate that there were 70.4% of families doing first aid in the wrong category, and there were 73.1% of families late in bringing CHD patients to the hospital. There was a significant relationship between behavior with patient delay to the hospital (p = 0.000). There was a significant relationship between behaviors with patient delay in the hospital. Discussion: The promotion of early signs of heart disease should be focused on the level of the family as well as integrated emergency care services immediately implemented to reduce the delay in coronary heart disease patients to the hospital.

Background: Metabolic syndrome (MS) is associated with higher CVD risk. High sensitivity troponin T (hsTnT) is a marker of myocardial injury and an emerging marker for heart failure (HF) risk prediction. We examined whether hsTnT is associated with increased HF risk in people with similar number of MS components present at baseline in 10316 ARIC participants without prevalent HF. Methods: We used Wald Chi-square test to assess the interaction between MS and hsTnT and Cox model for the association of incident HF hospitalization by hsTnT categories across groups created by the number of MS components after adjusting for risk factors and NT-proBNP (Table). Results: The mean age of the study population was 63 (SD, 6) years (56% women). Mean hsTnT levels were higher with increasing MS components (Table). There were 1353 HF hospitalizations over a median of 14 years. The interaction of MS with hsTnT for HF was borderline significant (p-interaction 0.059). Compared to individuals without MS and hsTnT<5 ng/L the HRs (95%CIs) were 1.7 (1.4-2.1) in those without MS and hsTnT≥5 ng/L; 1.7 (1.3-2.1) in MS and hsTnT<5 ng/L; and 3.6 (3.0-4.4) in MS and hsTnT≥5 ng/L. In groups with 1-5 MS components present, increasing hsTnT was significantly associated with higher hazards for HF in each group with the highest HR in those with all 5 MS components (Table). Conclusion: Presence of higher MS risk components was associated with increasing subclinical myocardial injury as assessed by higher hsTnT. The hazards for HF were numerically similar in individuals without MS but detectable hsTnT (>5 ng/L) as to those with MS but undetectable hsTnT. In people with similar number of MS components higher hsTnT levels were associated with increased HF hazards suggesting that in MS hsTnT could be a useful marker for identifying those at higher risk for incident HF.

ABSTRACT: RELATIONSHIP BETWEEN PHYSICAL ACTIVITY AND FAMILY HISTORY WITH CORONARY HEART DISEASE AT PUSKESMAS  BANJARSARI METRO Introduction: Based on data in coronary heart events Metro City 2018 total which is 1,309 new cases. For new diseases 190 men and 22 for women cases,  for old cases 369 men and 528 women in 2019 have a very drastic decrease, with total of new and old 229 patients with heart disease , for new diseases which is 32 men and 57 cases and women, for an old cases which is 60 cases for men and 80 cases for women.Purpose:  to known relations between physical activity and family history with coronary heart disease at Puskesmas Banjarsari Metro in 2020.Method: quantitative research type, analytical survey research plan with cross sectional approach, population and sample in research patients with heart failure are22 respondents. Univariate and bivariate data analysis used chi square test.Result : From 23 respondents, 13 respondents (56,5%) with low activity, families at risk which is 14 respondents (55,0%),  12 respondent (52,2%) with coronary heart, results of statistical tests using the chi square test obtained p-value = 0,022(< 0.05) , p-value = 0,006(< 0.05).Conclusion : Results of using the chi square test obtained p-value = 0,022(< 0.05) that means there is a relations between physical actiivity with occurrence coronary heart disease in Puskesmas Banjarsari Kota Metro 2020. Results of using the chi square test obtained p-value = 0,006(< 0.05) that means there is a relations between history of family with occurrence coronary heart disease in Puskesmas Banjarsari Kota Metro 2020. For peoples who risk of coronary heart disease (CHD) to behave healthy living by maintaining a diet by consuming foods that conform to diet coronary heart patients, avoid smoking, not consuming alcohol beverages, maintain weight stability and exercise regulary. Keywords : Knowledge, Self Care, Quality of life.       INTISARI:HUBUNGAN ANTARA AKTIVITAS FISIK DAN RIWAYAT KELUARGA DENGAN KEJADIAN PENYAKIT JANTUNG KORONER DI PUSKESMAS RAWAT INAP BANJARSARI KOTA METRO  Pendahuluan: Berdasarkan data kejadian jantung koroner di Kota Metro tahun 2018 pada berdasarkan jumlah kasus baru dan lama  sebanyak 1.309 dengan pembagian penyakit baru, laki-laki sebanyak 190 orang, perempuan sebanyak 22 orang, dan kasus lama, laki-laki sebanyak 369 orang, perempuan sebanyak 528 orang, pada tahun 2019 mengalami penurunan yang sangat drastis, yaitu total pasien jantung baru dan lama sebanyak 229 orang, dengan pembagian penyakit baru, laki-laki sebanyak 32 orang, perempuan sebanyak 57 orang, dan kasus lama, laki-laki sebanyak 60 orang, perempuan sebanyak 80 orang.Tujuan: Diketahui hubungan antara aktivitas fisik dan riwayat keluarga dengan kejadian penyakit jantung koroner di Puskesmas Banjarsari Kota Metro Tahun 2020.Metode: Jenis penelitian kuantitatif, rancangan penelitian survey analitik dengan pendekatan cross sectional, populasi dan sampel dalam penelitian ini adalah pasien gagal jantung sebanyak 22  responden, Analisa data menggunakan univariat dan bivariat menggunakan uji chi square.Hasil : Dari 23 responden dengan aktivitas kurang sebanyak 13 responden (56,5%), riwayat keluarga beresiko sebanyak 14 responden (55,0%),  jantung koroner sebanyak 12 responden (52,2%), Hasil uji statistik menggunakan uji chi square didapat nilai p-value = 0,022 (< 0.05) , p-value = 0,006 (< 0.05).Kesimpulan : Hasil uji statistik menggunakan uji chi square didapat nilai p-value = 0,022 (< 0.05) yang artinya terdapat hubungan antara aktivitas fisik dengan kejadian penyakit jantung koroner di Puskesmas Banjarsari Kota Metro Tahun 2020. Hasil uji statistik menggunakan uji chi square didapat nilai p-value = 0,006 (< 0.05) yang artinya terdapat hubungan antara riwayat keluarga dengan kejadian penyakit jantung koroner di Puskesmas Banjarsari Kota Metro  Tahun 2020Bagi seseorang yang mempunyai resiko terhadap terjadinya Penyakit Jantung Koroner (PJK) untuk berperilaku hidup sehat dengan jalan menjaga pola makan dengan jalan mengkonsumsi makanan yang sesuai dengan pola diit penderita PJK, menghindari merokok, tidak mengkonsumsi minuman beralkohol, menjaga kestabilan berat badan dan berolah raga secara teratur. Kata Kunci      : Pengetahuan, Self Care, Kualitas Hidup

Abstract: Sudden death could occur in anyone regardless of gender either male or female. This study was aimed to determine the relationship between gender and the incidence of sudden death at Prof. Dr. R. D. Kandou Hospital Manado. This was a descriptive analytical study with across sectional design using total sampling method. The results showed 345 cases of sudden deaths consisted of 209 males and 136 females. The age ranges were 0-1 years (7 cases), 1-18 years (7 cases), 18-40 years (38 cases), 40-60 years (138 cases), and >60 years (155 cases). Coronary heart disease was the leading cause of sudden deaths (40 cases). Data analyzed by using Chi-square test showed a P value of 0.838. Conclusion: There was no relationship between gender and the incidence of sudden death. Sudden deaths were most common among males, age >60 years, with coronary heart disease as the most common cause of sudden death.Keywords: gender, sudden death Abstrak: Kematian mendadak dapat terjadi pada siapa saja, tidak memandang jenis kelamin baik laki-laki maupun perempuan. Penelitian ini bertujuan untuk mengetahui hubungan antara jenis kelamin dan kejadian kematian mendadak di RSUP Prof. Dr. R. D Kandou Manado. Jenis penelitian ialah deskriptif analitik dengan desain potong lintang menggunakan metode total sampling. Hasil penelitian mendapatkan 345 kasus kematian mendadak terdiri dari 209 laki-laki dan 136 perempuan. Rentang usia 0-1 tahun berjumlah 7 kasus, usia 1-18 tahun berjumlah 7 kasus, usia 18-40 tahun berjumlah 38 kasus, usia 40-60 tahun berjumlah 138 kasus, dan usia >60 tahun berjumlah 155 kasus. Penyakit jantung koroner merupakan penyebab kematian mendadak terbanyak dengan jumlah 40 kasus. Analisis data menggunakan uji chi-kuadrat mendapatkan P=0,838. Simpulan: Tidak terdapat hubungan antara jenis kelamin dengan kejadian kematian mendadak. Sebagian besar kematian mendadak ditemukan pada laki-laki, kelompok usia >60 tahun, dengan penyebab penyakit jantung koroner.Kata kunci: jenis kelamin, kematian mendadak

Aim. Assessment of the dynamics of arterial hypertension (AH), coronary heart disease (CHD), postinfarction cardiosclerosis (PICS), stroke, cardiovascular risk factors prevalence by visit, during 6 years in the inhabitants of rural areas. Assessment of the impact of prevention events on the revealing rate of cardiovascular diseases.Material and methods. In the years 2015-17, a retrospective analysis was done, of the database on 2202 adults (≥18 y.o.) in Mokshinskaya rural outpatient facility. Of those 970 (44,1%) males and 1232 (55,9%) females. Study object — the reports for 2011-2016. For statistics, the IBM SPSS 21.0 was used, together with WinPEPI 10.49. Precise Fisher test applied and chi-square by Pearson. Statistics borderline set to 5%.Results. In 2016г the prevalence of AH — 9,54% (of all adult population), CHD — 2,00%, PICS — 0,59%, stroke — 0,27%. Prevalence increase by: AH — 32,5%, diabetes — 30,6%, obesity — 52,4%. There was tendency to decline of all CHD cases number: from 3039,8 by 100 thousand persons to 1998,2 by 100 thsd. PICS values did not change significantly. Cardiovascular mortality in the studied population was slightly higher — 7,3 promille in 2016, than cardiovascular mortality by Rosstat data — 6,2 promille and correlates strongly negatively with prevention events that have been performed.Conclusion. During a 6 year period there is significant increase of AH, diabetes, obesity prevalence that correlates significantly with prevention events. Nevertheless, the data on 2016 remains below mean statistics for entire country, except on obesity. Increase of the number of obesity persons by 52,4% is a serious medical and social problem of the studied population. It is a risk factor sreiously influencing cardiovascular morbidity and mortality.

Abstract BACKGROUND Higher growth differentiation factor 15 (GDF-15) concentrations are associated with cardiovascular (CV) and non-CV morbidity and mortality. However, information on associations between GDF-15 and the risk of specific CV and non-CV events in stable coronary heart disease (CHD) patients is limited. METHODS In 14 577 patients with stable CHD participating in the Stabilization of Atherosclerotic Plaque by Initiation of Darapladib Therapy Trial (STABILITY), GDF-15 and other prognostic biomarkers (N-terminal pro–B-type natriuretic peptide, high-sensitivity troponin T, cystatin C, and high-sensitivity C-reactive protein) were measured. In adjusted Cox regression models, the associations between GDF-15 and the composite CV end point [CV death, myocardial infarction (MI), and stroke], as well as other CV and non-CV events, were assessed. RESULTS The median concentration (interquartile range) of GDF-15 at baseline was 1253 (915–1827) ng/L. The hazard ratio for the composite end point for the highest compared to the lowest quartile of GDF-15 was 1.8 (95% CI, 1.5–2.2); for CV death, 2.63 (1.9–3.6); for sudden death, 3.06 (1.9–4.8); for heart failure (HF) death, 4.3 (1.3–14); for cancer death, 2.5 (1.3–4.7); for hospitalization for HF, 5.8 (3.2–10); for MI 1.4 (95% CI, 1.1–1.9); and for stroke, 1.8 (95% CI, 1.1–2.8). After adjustment for other prognostic biomarkers, GDF-15 remained significantly associated with all outcomes except for MI. CONCLUSIONS In stable CHD, GDF-15 was independently associated with CV, non-CV, and cancer mortality, as well as with MI and stroke. When also adjusting for other prognostic biomarkers, the associations to all fatal and nonfatal events were maintained except for MI. Information on GDF-15, therefore, might be helpful when assessing the risk of adverse outcomes in patients with stable CHD. ClinicalTrials.gov Identifier: NCT00799903

Abstract: Periodontitis is one of the factors causing systemic disease. It is often associated with increasing signs of inflammation and it is also an indicator of the risk factors of coronary heart disease (CHD). Infection of periodontal structures can accelerate the form of atherosclerosis that causes coronary heart disease due to systemic inflammation through the release of endotoxins, proteins, or acute phase reactors. This was a case control analytical study with a cross sectional design conducted at Prof. Dr. R. D. Kandou Hospital Manado from August to September 2016. There were 40 respondents (20 CHD patients and 20 non-CHD patients) obtained by using total sampling method. Periodontal disease indexes of the samples were evaluated by using periodontal disease index of Ramfjord 1959. The Chi-square test showed a p value of 0.01. Conclusion: There was a significant relationship between periodontitis and coronary heart disease in patients at Prof. Dr. R. D. Kandou Hospital Manado. Pada periodontitis sering didapatkan peningkatan tanda-tanda inflamasi yang juga merupakan salah satu indikator dari faktor risiko penyakit jantung koroner (PJK). Infeksi struktur periodontal dapat mempercepat pembentukan aterosklerosis yang menjadi penyebab PJK dengan cara menimbulkan inflamasi sistemik melalui pelepasan endotoksin, protein, atau reaktor fase akut. Jenis penelitian ialah analitik menggunakan case control dengan desain potong lintang. Penelitian dilakukan di RSUP Prof. Dr. R. D. Kandou Manado pada bulan Agustus sampai September 2016. Terdapat 40 responden (20 pasien PJK dan 20 pasien non PJK) diperoleh dengan menggunakan metode total sampling. Pada penelitian ini dilakukan pengukuran indeks penyakit periodontal menggunakan pengukuran PDI Ramfjord 1959. Analisis data menggunakan uji Chi-square. Hasil analisis bivariat menggunakan uji Chi-square menunjukkan nilai p=0,01 (0,01<0,05) yang menyatakan bahwa terdapat hubungan antara periodontitis dengan PJK pada pasien di RSUP Prof. Dr. R. D. Kandou Manado. Simpulan: Terdapat hubungan bermakna antara periodontitis dengan penyakit jantung koroner pada pasien di RSUP Prof. Dr. R. D. Abstract: Coronary heart disease (CHD) is a disease with a high mortality rate in both developed and developing countries. Blood type is a particular feature of human in an individual's blood, such as ABO and Rhesus groups. The purpose of this study to determine the relate of blood type and CHD. Methods Cross sectional study using a study by the number of respondents in this study were 42 patients CHD checks from November to December 2012 at poliklinik Jantung BLU RSUP Prof. Dr. R. D Kandou, Manado. Based on Chi Square test showed the value of Pearson Chi Square, which obtained significance value is 1.000 (significant value <0.05), so it was concluded that there is no significant relate between blood type and CHD. Keywords: CAD, Blood Group, Cross Sectional     Abstrak: Penyakit jantung koroner (PJK) merupakan penyakit dengan angka mortalitas yang tinggi baik di negara maju maupun berkembang. Golongan darah merupakan ciri khusus darah dari suatu individu, seperti golongan ABO maupun Rhesus. Tujuan penelitian ini untuk mengetahui hubungan golongan darah dan PJK. Metode penelitian menggunakan studi Cross Sectional dengan jumlah responden dalam penelitian ini adalah 42 pasien PJK yang melakukan pemeriksaan mulai bulan November hingga Desember 2012 di poliklinik Jantung BLU RSUP Prof. Dr. R. D Kandou, Manado. Berdasarkan uji Chi Square menunjukkan nilai Pearson Chi Square, yang didapatkan nilai signifikansinya adalah 1,000 (nilai signifikan <0,05), sehingga disimpulkan bahwa tidak terdapat hubungan bermakna antara golongan darah dan PJK.
