Spinal tuberculosis or tuberculous spondylitis is one of the most common types of skeletal tuberculosis. Complications of the spine and spinal cord tuberculosis include destruction of the vertebrae, deformity, and paraplegia. Since in some patients, the clinical manifestations of tuberculosis are unusual and timely diagnosis and treatment of this disease can prevent its serious consequences, so in the present study, some cases of rare manifestations of tuberculosis were investigated. The expression of the NF-κB gene in these patients was also evaluated. In this regard, 36 patients with spinal tuberculosis and 30 healthy individuals (as a control group) were assessed. Clinical symptoms, imaging, laboratory tests, pathology, and response to treatment related to patients with spinal tuberculosis and spinal cord tuberculosis were evaluated. NF-κB expression was also evaluated using the PCR technique in peripheral white blood cell samples. The obtained results were analyzed using SPSS ver. 16, χ2 and T-test statistical methods. Mann-Whitney U test and Kruskal–Wallis non-parametric tests were used to analyze non-parametric data. The results showed that out of 36 cases of spinal tuberculosis, 29 cases had spinal tuberculosis, five cases had tuberculous radiculomyelitis, one case had spinal intramedullary tuberculoma, and one case had syringomyelia. 52.78% of patients were male, and 70% of cases were observed between the ages of 35 and 55 years. Fever and back pain were seen in more than 80% of cases. The study of NF-κB expression in the control and case groups showed that the NF-κB expression in the case group increased compared to the control group. This increase was statistically significant (P = 0.0071). In general, in the present study, the methods of clinical diagnosis of spinal tuberculosis were evaluated. Also, the amount of NF-κB transcription factor was evaluated as an effective genetic factor in the diagnosis of this disease.

The aim of the study was to examine an association of the rs602662 FUT2 genetic locus with the status of H. pylori infection and development of related diseases (chronic gastritis, gastric ulcer and 12 duodenal ulcer).Methods: The study included 91 patients, divided into two groups - "case" and "control". Criteria for the “case” group enrollment: diagnosis of gastric or duodenal ulcer, chronic non-atrophic gastritis; positive test for H. pylori.The “control” group included patients with episodic complaints of dyspepsia while undergoing a comprehensive examination, with negative test for H. pylori, as well as having no history of former therapy on H. pylori elimination.The study for the presence of the polymorphic locus rs602662 of the FUT2 gene was carried out by the standard TaqMan PCR method on a Real-Time CFX96 Touch amplifier. The follow-up period was 6 months.Results: The main group included 50 patients aged 21 to 50 years, the control group – 41 patients. Patients infected with H. pylori more often noticed symptoms of dyspepsia - in 36%, compared with the control group - 9.7%. A family history of associated diseases in the main group was significantly differed, χ2 = 4.97, p <0.05.To assess the contribution of the rs602662 locus genotype in FUT2 gene to the risk of clinically manifested H. pylori infection, the main group was divided into subgroups. In the distribution of alleles in these groups, significant differences were revealed. Allele "A" has a protective effect regarding the onset of clinical symptoms of dyspepsia. The odds ratio (OR) with the carriage of allele "A" (genotypes A / A and G / A versus G / G) to have clinical symptoms with a positive H. pylori status was 0.175 (CI = [0.049-0.625] chi2 = 7.79 p = 0.0053 ).Conclusion:1. No association of alleles and genotypes of the rs602662 locus of the FUT2 gene with the status of H. pylori infection was revealed.2. Carriage of allele "A" have a significant association with the absence of clinical symptoms in patients with a positive status of H. pylori infection, OR 0.175 (CI = [0.049-0.625] chi2 = 7.79 p = 0.0053).

Background: Pneumonia, as a fairly prevalent illness, is the main cause of hospital mortality. The major cause of mortality and morbidity of pneumonia is due to bacteria. The presence of multi-drug resistant pathogens and no response to treatment have aroused considerable interest in the use of probiotic components to prevent infections. Objectives: Given that few studies have evaluated the efficacy of probiotics in reducing bacterial pneumonia, the current aimed to evaluate the role of probiotics in decreasing pneumonia. Methods: This double-blind, randomized clinical trial study was conducted on 100 patients diagnosed with bacterial pneumonia in Shahid Beheshti Hospital, Kashan, Iran, during 2018. Patients were randomly classified into two groups (n = 50). One group (case) received two sachets of probiotic/daily for five days, and another group (control) received placebo. Moreover, patients in both groups received the same treatment protocol. All data were extracted from medical records. Chi-square test and independent t-test were used for analysis of data. P < 0.05 was considered statistically significant. Results: No significant difference was seen between case and control groups regarding age, gender, and duration of symptoms before hospitalization (P > 0.05), which implies a completely random classification of two groups. The mean duration of hospitalization, dyspnea, tachypnea, cough, fever, and crackles was significantly decreased in the case group compared to the control group (P < 0.05). Conclusion: The use of probiotics can be effective in reducing the duration of dyspnea, tachypnea, cough, fever, and length of hospitalization. Therefore, probiotics may be considered a promising treatment for the development of new anti-infectious therapy. In addition, the usage of probiotics along with antibiotics is suggested for decreasing pneumonia complications and improving the efficacy of therapy.

Based on the extant studies conducted by scholars, recognizing the best and practical treatment methods in patients with bone metastases is an important and conspicuous focus in improving and amelioration of the treatment and prognosis of patients. Ergo, this study aims to compare the effects of radiotherapy and hyperthermia with radiotherapy only to reduce pain in one of the oncology centers located in Tehran (the capital city of Iran). In this randomized clinical trial study, patients with bone metastases were randomly assigned to one of two group’s radiotherapy and hyperthermia or radiotherapy exclusively and the collected data were processed through Statistical Package of Social Science (SPSS). Thenceforth, findings were evaluated and compared between the two groups. A total of 60 patients inclusive of 34 women and 26 men were investigated. In the baseline, 20 patients in the case group (67%) and 21 patients in the control group (70%) used palliative drugs. At the end of the trial, the consumption of palliative drugs was decreased in the case group (12 patients, 43%) but, no change was observed in the control group. After treatment, complete response to treatment (27% vs. 13%), experienced a decrease in the clinical pain (57% vs. 83%) and did not respond to treatment in a stable mode (17% vs. 4%) of which were different between case and control groups, respectively. Regardless of the difference in the ratio of partial response in two groups, there was no significant difference between the two groups in terms of response to treatment. Although the effect of radiotherapy and hyperthermia in comparison with radiotherapy exclusively was more efficacious in some aspects of the intervention of pain with sleep. However, there was no remarkable and significant difference in other cases.

Objective. To investigate the correlation between serum immune factor levels and allergic constitution in children with infectious mononucleosis. Methods. A total of 120 children who visited our hospital from March, 2019, to December, 2020, were selected as the research objects, and 40 children who came to our hospital for physical examination were included in the control group (CG). 40 children with IM were classified into the IM group (IG), and 40 IM children with allergic rhinitis, allergic dermatitis, asthma, and other allergic diseases were classified into the IM allergy group (AG). On the second day of admission, 5 ml of fasting venous blood was collected from all children in the early morning to observe the serum IgE level, the level of lymphocyte subsets, and the level of immunoglobulin of the patient. Results. The serum CD3, CD4, and CD8 levels of children in AG were significantly higher than those in IG and CG ( P < 0.05 ). The serum IgE, IgA, IgM, and IgG levels of children in AG were significantly higher than those of IG and CG ( P < 0.05 ). The serum IgE levels of children in AG were positively correlated with the serum CD3, CD4, and CD8 levels ( P < 0.05 ). There was a positive correlation between the serum IgE level and serum IgA, IgM, and IgG levels in children with AG ( P < 0.05 ). Conclusion. The results of this study showed that there may be a certain relationship between allergic constitution and the incidence, clinical manifestations, and prognosis of infectious mononucleosis. IgE level can be used as a reference index for the early severity of IM clinical symptoms.

Abstract Background: Although listeriosis is a foodborne infection with a high mortality rate, studies of listeriosis from Asia are limited. We aimed to compare the characteristics of listeriosis and non-listeriosis (defined as bacteremia without Listeria monocytegenes infection) in Japan.Methods: We conducted a case–control study of adults with listeriosis or with bacteremia without L. monocytegenes infection from January 1, 2010, to December 31, 2019, at Tokyo Medical University Hospital in Japan. We recorded the incidence of admissions and the clinical characteristics, including age and sex; underlying disease; clinical symptoms; onset month; whether listeriosis was community-associated or health care–associated; medicine; blood count; biochemical findings; antibiotics use; admission to the intensive care unit; and outcome.Results: A total of 12 patients were included in the case group and 28 patients were included in the control group. Most cases of listeriosis were present in patients of an advanced age, with underlying disease, and/or on immunosuppressant therapy. Two patients died within 30 days and one developed a brain abscess. Listeriosis onset was associated with the warm season (90.9% vs. 53.8%; P = 0.033), steroid use (54.5% vs. 19.2%; P = 0.042), and a lower ratio of neutrophils to lymphocytes (9.46 vs. 18.44; P = 0.015).Conclusion: The incidence of listeriosis in this study was similar to that in other Asian countries and the 30-day mortality rate was 18.2%. We observed that listeriosis onset was associated with the warm season, steroid use, and a lower ratio of neutrophils to lymphocytes.

Background: This study aimed to compare the temperature increase produced by halogen bulb (HAL) and light-emitting diode (LED) light curing units (LCUs) by irradiating dentin discs (0.5 mm and 1 mm thickness), and to evaluate their cytotoxic effects on fibroblast culture in the presence of dentin discs due to the increasing demand on resin composite restorations and teeth bleaching for esthetic purposes. Methods: A total of 20 bovine incisors were used to obtain dentin discs and divided into four experimental groups (n=10): HAL0.5: irradiation with halogen-tungsten bulb Curing Light XL 3000 at an intensity of 470 mW/cm2 over a dentin disc of 0.5 mm; LED0.5: irradiation with LED Optilight Max (GNATUS- Ribeirão Preto, SP, Brazil) at an intensity of 1200 mW/cm2 over a dentin disc of 0.5 mm; HAL1: irradiation as in HAL0.5 but over a dentin disc of 1 mm; LED1: irradiation as in LED0.5 but over a dentin disc of 1 mm. The temperature increase was measured using a digital thermometer and the cytotoxicity was evaluated using an MTT assay with a mouse fibroblast cell line (L929). Parametric Data were analyzed by ANOVA and Tukey and non-parametric data were analyzed by Kruskal Wallis with Conover-Iman for non-parametric data (all with α=0.05). Results: A significant statistical difference was found between the groups HAL0.5 and HAL1 and both were different of LED0.5 and LED1 which presented higher temperature. All the experimental groups were different of the control group (without irradiation), and promoted reduction of cellular viability. Conclusions: HAL LCU promoted a lower temperature change in the dentin compared to LED, regardless of the dentin thickness (0.5-1 mm). Both HAL and LED LCUs decreased fibroblast viability; however, LED promoted more significant cytotoxic effects.

Objective. To investigate the association of single-nucleotide polymorphisms (SNPs) in SP110 gene and TNF-α gene among pulmonary TB (PTB) and spinal TB (STB) patients. Methods. In a total of 190 PTB patients, 183 STB patients were enrolled as the case group and 362 healthy individuals at the same geographical region as the control group. The SP110 SNPs (rs722555 and rs1135791) and the promoter -308G>A (rs1800629) and -238G>A (rs361525) polymorphisms in TNF-α were genotyped. Results. TNF-α -238G>A polymorphism was involved in susceptibility to STB, but not to PTB. The TNF-α -238 A allele was a protective factor against STB (A versus G: OR [95% CI] = 0.331 [0.113–0.972], P=0.044). Furthermore, the presence of the -238 A allele was considered a trend to decrease the risk of STB (AG versus GG: P=0.062, OR [95% CI] = 0.352 [0.118–1.053]; AA + AG versus GG: P=0.050, OR [95CI%] = 0.335 [0.113–0.999]). However, SP110 SNPs (rs722555 and rs1135791) and TNF-α -308G>A (rs1800629) showed no association with PTB and STB in all genetic models. Conclusion. The TNF-α -238 A allele appeared a protective effect against STB, whereas the SP110 SNPs (rs722555 and rs1135791) and TNF-α -308G>A (rs1800629) showed no association with susceptibility to PTB and STB patients in southern China.

The hormonally active form of vitamin D3, 1,25(OH)2D3 (calcitriol), exerts actions through VDR receptor, which acts as a transcriptional factor. Calcitriol is an immunomodulator that affects various immune cells, and several studies link it to many autoimmune diseases. BsmI polymorphism affects the level of VDR gene transcription, transcript stability, and posttranscriptional modifications. It seems to be related to the systemic lupus erythematosus (SLE). Our study examined the characteristics of VDR gene BsmI polymorphism in Polish SLE patients and their relationship with clinical manifestations of the disease. We genotyped 62 patients with SLE and 100 healthy controls using the real-time PCR. There were no differences observed in the frequency of BsmI genotypes in SLE patients and in the control group. There was no significant correlation between BsmI genotypes and clinical symptoms of SLE, but the AA genotype correlates with higher levels of antinuclear antibodies (ANA) in this group (r=0.438; P=0.002). A larger study examining BsmI and other VDR gene polymorphisms is needed. It may allow explaining differences in the clinical picture of the disease and choosing a personalized therapy.

Abstract. Purpose – study the effect of enterosorbent on the immunological parameters of blood and the productivity of young cattle. Methods. The object of the study was newborn calves from 2 to 6 days of age (n = 54). Studies on the enterosorbent based on colloidal silicon dioxide (CSD) for calves with alimentary dyspepsia have been carried out. During the experiment, the clinical condition of the animals, the increase in live body weight were evaluated, blood samples were taken for immunohematological studies. Results. In calves at 2–6 days of age, alimentary dyspepsia is accompanied by the development of endogenous intoxication of the body. Animals show leukocytosis – 16.60 ± 5.11×109/l; leukocyte shift to the left with an increase in the number of adolescent and stab neutrophils – 1.41 ± 0.23×109/l; monocytes up to 1.21 ± 0.13x×109/l; circulating immune complexes (CIC) level in blood serum up to 202.4 ± 8.5 cu Under conditions of endogenous intoxication, the immunological protection of the organism was characterized by an increase in the number of phagocytic cells to 71 % of the total number of granulocytes. Clinical symptoms of endogenous intoxication in 86 % of cases disappeared by the 5th day of the use of enterosorbent CSD in the treatment regimen of calves with alimentary dyspepsia. This is associated with blocking and weakening the inflammatory response in the gastrointestinal tract. The effect of enterosorbent CSD on the immunological parameters was expressed in a balanced stabilization of the processes of phagocytosis and immunogenesis. The trend towards normalization of immunological parameters was registered: the level of the CIC was significantly reduced to 97.5 ± 5.48 cu; the phagocytic activity (PA) of the neutrophilic cells was 50.1 ± 2.4 %. Treatment of animals according to the basic scheme approved by the farm turned out to be less effective. In calves that did not receive enterosorbent CSD, in 11 % of cases, clinical manifestations of endogenous intoxication were recorded up to and including 14 days. In these animals, the level of the CIC remained high – 143.6 ± 8.57 cu, the voltage of phagocytic function was noted – 64.3 ± 7.6 % and the imbalance in the ratio of T/B-lymphocytes – 1.13 (normal 1.5–2.0). The effect of the CSD enterosorbent on the calves productivity was reflected in the fact that the rate of weight gain during the experiment was higher in the experimental group. At 4 months, their weight was 149.17 ± 13.57 kg, while in the control group and the comparison group – 135.00 ± 5.00 and 130.00 ± 22.73 kg, respectively. Scientific novelty. Alimentary dyspepsia causes the development of endogenous intoxication in the body of newborn calves. The lack of therapeutic measures leads to a long recovery period and a decrease in productivity.

The aim of the pilot study of a group of adolescents with H. pylori infection was to study the preliminary data obtained on the rs602662 locus of the FUT2 gene and to establish its role in the realization of clinical manifestations of chronic gastritis, gastric ulcer and duodenal ulcer associated with H. pylori.Methods: The study included 91 patients. The study for the presence of the polymorphic locus rs602662 of the FUT2 gene was carried out by the standard TaqMan PCR method on a Real-Time CFX96 Touch amplifier. The duration of the study was 6 months.Results: The main group included 25 adolescents aged 16 to 17 years 11 months, the control group included 20 patients. Patients infected with H. pylori more often noticed symptoms of dyspepsia - in 36%, compared with the control group - 9.7%. The presence of a family history in the main group for associated diseases had a significant difference, χ2 = 4.97, p <0.05.To assess the contribution of the genotype of the rs602662 locus of the FUT2 gene to the risk of clinical manifestations in H. pylori infection, the main group was divided into subgroups. In the distribution of alleles in these groups, statistically significant differences were revealed.Allele “A” has a protective effect against the onset of clinical symptoms of dyspepsia. The odds ratio (OR) with the carriage of allele “A” (genotypes A / A and G / A versus G / G) to have clinical symptoms with a positive H. pylori status was 0.175 (CI = [0.049-0.625] chi2 = 7.79 p = 0.0053).Conclusion. At the same time, carriers of the A allele have a pronounced association with the absence of clinical symptoms in patients with a positive H. pylori infection status of 0.175 (C.I. = [0.049-0.625] chi2 = 7.79 p = 0.0053).
