Summary Introduction Local therapy of denture stomatitis (DS) associated with Candida species fungi infection usually involves the application of nystatin and miconazole. Due to the fact that these drugs may be less efficient against biofilm and possible resistance development, a new approach in the treatment includes the use of antiseptic agents. The aim of the study was to compare clinical and microbiological therapeutic outcomes of antiseptic solution Listerine® and Daktanol® antifungal oral gel in the treatment of DS associated with Candida species fungi. Material and Methods The study included 30 patients affected by DS, divided into the two treatment groups, control group (n=15) treated by Daktanol® gel and experimental group (n=15) treated by the antiseptic solution Listerine®. Successful treatment was evaluated based on palatal mucosa inflammation reduction classified according to the Newton classification and the difference in the number of fungal colony- forming units (CFU) isolated by smears before and after the treatment that lasted 14 days. Results Reduction in inflammation intensity and fungal CFU number on palatal mucosa (p<0,01) as well as on denture base (p<0,01) were observed in both groups of subjects after the treatment. Conclusion Antiseptic solution Listerine® and Daktanol® antifungal gel both reduced palatal mucosal inflammation and CFU number of fungi in mouth without significant differences among them. CFU number of fungi isolated from denture base was significantly lower after the treatment with Listerine® (p<0.05).

Denture stomatitis is a multifactorial pathological condition of the oral mucosa that affects up to 72% of denture wearers. It is commonly seen on the palatal mucosa and characterized by erythema on the oral mucosa that are in contact with the denture surface. The aim of this study was to incorporate 2-methacryloyloxyethyl phosphorylcholine (MPC) and dimethylaminohexadecyl methacrylate (DMAHDM) into a high impact polymethylmethacrylate heat-cured denture base acrylic resin as a potential treatment for denture stomatitis. We used a comparative study design to examine the effect of incorporating MPC as a protein repellent agent and DMAHDM as an antifungal agent to prevent the adherence of Candida albicans to the denture base material. The dual incorporation of MPC and DMAHDM reduced C. albicans biofilm colony-forming unit by two orders of magnitude when compared to the control group devoid of the bioactive agents. Although the addition of MPC and DMAHDM alone or in combination significantly reduced the flexural strength of the material, they showed reduced roughness values when compared to control groups. This new denture acrylic resin provides the benefit of enhancing C. albicans biofilm elimination through dual mechanisms of action, which could potentially reduce the prevalence of denture stomatitis.

Importance. Exfoliative cheilitis is an inflammatory disease accompanied by damage to the red border of the lips. With dry exfoliative cheilitis, scales form on the lips, which are densely soldered to the underlying tissue in the center, and along the periphery, on the contrary, are raised. At the same time, patients indicate a feeling of dryness, burning. When the exudative form of cheilitis is observed puffiness, sore lips. Diagnosis of exfoliative cheilitis is reduced to the collection of complaints, the preparation of anamnesis of the disease, physical examination. For the treatment of exfoliative cheilitis, Bucca rays, moisturizing creams, acupuncture techniques, as well as agents that increase body resistance are used. As it turned out, in practice, all existing methods of treatment are ineffective, and the disease is manifested by frequent relapses. Pathology is more common in women. The dry form of the disease is mainly detected at the age of 16-40 years. Exudative cheilitis is diagnosed in patients aged 16-65 years. Objectives ― evaluate the effectiveness of lip balm based on lamb fat, and make a conclusion based on the clinical data and patient's subjective sensations. Methods. Twenty patients aged from 18 to 45 years with a diagnosis of exfoliative cheilitis were selected for work. All patients were divided into two groups: control (10 people) and main (10 people). Local therapy in both groups began according to the generally accepted scheme: sanation of the oral cavity, treatment of periodontal diseases according to indications, selective grinding off of the sharp edges of the teeth. In the main group, patients received treatment in the form of an application with a cool furatsilina solution, and applying the dosage form in the form of lip balm., Consisting of red-hot lamb fat and filtered through gauze, lanolin, pumpkin oil, ylang ylang oil, geranium oil, and also vitamins oil A and E and medical paraffin, this dosage form was prepared in paraffin and sterilized, after complete cooling it was applied to the lips with a spatula, and the patients easily used it at home as a balm for K in the morning and evening. Patients of the control group received the traditional treatment of cheilitis, anestezin applications, lip treatment with an antiseptic solution and keratoplasty applications - vitamin A and E oil. The results of the treatment were evaluated according to the clinical examination of patients, as well as taking into account the subjective feelings of the patients. Results. Based on the schemes of complex treatment of exfoliative cheilitis in patients of the main group, a positive result was obtained after the 3rd visit. A decrease in pain syndrome, a decrease in lesion elements (flakes), epithelization of cracks, a decrease in pain syndrome, a decrease in complaints of dryness of the red border of the lips were revealed. Conclusions. The development of this algorithm for treating ecfoliative cheilitis, including a mutton fat lip balm, has reduced the treatment time for this pathology.

Maintaining acrylic denture hygiene is a necessary to prohibit the growth of several oral fungi such as Candida albicans. Candida albicans are the main etiological microorganism of denture stomatitis. One of the ways to clean denture is by immersion on synthetic denture cleaner liquid such as oxygenizing denture cleaners. However, synthetic denture cleaner have several limitation due to its biocompatibility on human body especially oral tissue. Chitosan is a derivate of chitin, a natural compound of sea creature exosceleton. Chitosan have antifungi and antibacterial nature. The objective of this study is to evaluate the antifungi efectifity of chitosan 2% on acrylic heatcured and compare it with oxygenizing denture cleaner. Aquades is used as a control. The sample consists of 15 acrylic heatcured bars (10x10x2 mm) prepared by contamination on Candida albicans suspensions at temperature 37°C for 24 hours and immersed on 3 different treatments wich are chitosan 2%, oxygenizing denture cleanser, and aquades as control and then, the solutions are being vibrate and put into sabouraud dextrose agar medium. The colony of Candida albicans counted manually. Result shows, highest mean of Candida albicans colony on aquades group (365,66 CFU/plate), followed by oxygenizing denture cleanser treatment (178,33 CFU/plate). The smallest number colony of Candida albicans are found at chitosan 2% treatment (45,33 CFU/plate). Statistical analysis using ANOVA one way shows F=9.826 and p=0, this means, there is minimal one pair of groups with very significant differential (p<0,01) which are chitosan 2% and oxygenizing denture cleanser compared to aquades as control. The difference between those two treatments also significant. Conclusions of this study is that chitosan 2% solution have a sifnificant effect on prohibiting the growth of Candida albicans colony on acrylic heatcured denture base materials. The effectivity of this solution is significantly higher than oxygenizing denture cleanser treatment.

Candida albicans (C. albicans) biofilm is a common etiological factor in denture stomatitis. The purpose of this study was to investigate the effects of incorporating 2-methacryloyloxyethyl phosphorylcholine (MPC) as a protein repellent into a new high-impact denture acrylic (HIPA) resin on the surface roughness, solution pH, and C. albicans biofilm adhesion to the denture base. The new acrylic denture resin base was formulated by mixing MPC into HIPA resin at mass fractions of 1.5%, 3%, and 4.5%. Surface roughness was measured using a Mitutoyo surface roughness tester. C. albicans biofilm growth and viability were assessed via colony forming unit counts. The pH of the biofilm growth medium was measured using a digital pH meter. Adding MPC to the HIPA resin at percentages of 1.5% and 3% increased the roughness values significantly (p < 0.05), while adding 4.5% MPC resulted in no difference in roughness values to that of the control group (p > 0.05). All experimental groups demonstrated neutral pH values (pH ≅ 7) and were not significantly different from each other (p > 0.05). Incorporating 2-methacryloyloxyethyl phosphorylcholine at 4.5% resulted in a significant (≅1 log) colony-forming unit reduction compared with the control group with 0% MPC (p < 0.05). A fungal-retarding denture acrylic resin was developed through the incorporation of MPC for its protein-repelling properties. This newly developed denture acrylic material has the potential to prevent oral microbial infections, such as denture stomatitis.

When PMMA denture base acrylics are exposed to oral environments for prolonged periods, the denture base absorbs water, which has a negative influence on the denture material and the degree to which the denture base will be clinically effective. This study assessed the water sorption, desorption, and hygroscopic expansion processes within PMMA denture-base resins reinforced with nanoparticles or fibre in comparison to the non-reinforced PMMA. The surfaces of the fillers were modified using a silane coupling agent (y-MPS) before mixing with PMMA. Group C consisted of specimens of pure PMMA whereas groups Z, T, and E consisted of PMMA specimens reinforced with ZrO2, TiO2 nanoparticles, or E-glass fibre, respectively. The reinforced groups were subdivided into four subgroups according to the percentage filler added to the PMMA resin by weight (1.5%, 3.0%, 5.0%, or 7.0%). Five specimens in disc shape (25 ± 1 mm × 2.0 ± 0.2 mm) were tested for each group. To assess water sorption and hygroscopic expansion, specimens from each group were individually immersed in water at 37 ± 1 °C for 180 days. The samples were then desorbed for 28 days at 37 ± 1 °C, to measure solubility. Water sorption and solubility were calculated using an electronic balance in accordance with ISO Standard 20795-1, and hygroscopic expansion was measured using a laser micrometre. Statistical analysis was undertaken at a p ≤ 0.05 significance level using a one-way ANOVA followed by Tukey post-hoc tests. The results demonstrated that the values of sorption (Wsp), mass sorption (Ms%), and % expansion within the tested groups reached equilibrium within 180 days. A noticeable difference was observed in groups Z and E for (Wsp)/(Ms%) compared to the Group C, but this was not significant. However, the difference between Group C and Group T for these measurements was significant. Non-significant differences also existed between each respective reinforced group and the control group in terms of hygroscopic expansion % values. During the 28-day desorption period, there were no differences in the values of solubility (Wsl)/mass desorption (Md%) between Group C and each of the reinforced tested groups. The findings indicate that the inclusion of ZrO2 nanoparticles or E-glass fibres does not increase the water solubility/sorption of the PMMA. However, modifying the PMMA with TiO2 did significantly increase the water sorption level.

We studied the effect of experimenter expectations and different instructions in a balanced placebo design. 157 subjects were randomized into a 2 × 4 factorial design. Two experimenters were led to expect placebos either to produce physiological effects or not (pro- vs. antiplacebo). All subjects except a control group received a caffeine placebo. They were either made to expect coffee, no coffee, or were in a double-blind condition. Dependent measures were blood pressure, heart rate, well-being, and a cognitive task. There was one main effect on the instruction factor (p = 0.03) with the group “told no caffeine” reporting significantly better well-being. There was one main effect on the experimenter factor with subjects instructed by experimenter “proplacebo” having higher systolic blood pressure (p = 0.008). There was one interaction with subjects instructed by experimenter “proplacebo” to receive coffee doing worse in the cognitive task than the rest. Subjects instructed by experimenter “antiplacebo” were significantly less likely to believe the experimental instruction, and that mostly if they had been instructed to receive coffee. Contrary to the literature we could not show an effect of instruction, but there was an effect of experimenters. It is likely, however, that these experimenter effects were not due to experimental manipulations, but to the difference in personalities.

SummaryPrevious studies have shown that the fibrinolytic activity of peritoneum is depressed in local inflammation. We measured fibrinolytic parameters in peritoneal fluid and in plasma of 10 women with pelvic inflammatory disease (PID). Nine women, in whom laparoscopy for sterilisation was performed, served as a control group.In the peritoneal fluid of women with PID, PAI-Ag, t-PA-Ag and u-PA-Ag were many times higher than in the control group. In contrast to the antigens which may be present in inert complexes, the potentially active compounds, measured as t-PA activity and plasmin-activable scu-PA, were not significantly different in the two groups, and in none of the samples was the active enzyme tcu-PA detectable. Nevertheless, the mean peritoneal fluid TDP and FbDP concentrations were about twenty times higher in the PID group than in the control group. In plasma of PID patients, none of the parameters except u-PA-Ag differed from those in the control group. The difference between control and patient plasma u-PA-Ag was statistically significant, but too small to attach any relevance to the observation.Our data suggest that, in contrast to the classical concept of decreased fibrinolytic activity as a cause of adhesion formation, intraperitoneal fibrinolysis is enhanced in peritoneal inflammation through stimulation of the local production of t-PA and u-PA. Despite concomitant production of PAI, fibrinolysis occurs at a high rate, resulting in high levels of fibrin degradation products. Since this activated fibrinolysis does not meet the demand, therapeutic enhancement should be considered to prevent adhesions.

SummaryChanges in platelets in 48 patients with uterine myoma before and after hysterectomy with and without ovariectomy were examined. Bilateral ovariectomy in 25 cases (ovariec-tomized group) and unilateral or non-ovariectomy in 23 cases (control group) were performed at the hysterectomy. Platelet count and an appearance rate of secondary aggregation decreased at one day after and increased at one week after the operation, similarly in both the ovariectomized and the control group. The appearance rate of secondary aggregation was reflected in an intensity of aggregation at 5 min after the addition of reagent to PRP. At one month after the operation, the appearance rate of secondary aggregation induced by 3 μM ADP showed a statistically significant decrease in comparison with the preoperation value (P <0.05) and the enhancement of 5-min aggregation was still observed in the control group, while ceased in the ovariectomized group. The difference between the two groups was significant (P < 0.05). There was almost no change in the speed and intensity of primary and secondary aggregation during the observation period. No significant differences in collagen-induced aggregation were noted between the two groups. The results suggest that ovarian hormones, mainly estrogen, facilitate platelet activation which is mediated by the so-called secondary aggregation.

ABSTRACT The fractional determination of 17-ketosteroids in the daily urine was performed in nine cases of hyperemesis gravidarum and in four control cases, in the first trimester of pregnancy both before and after corticotrophin administration. The excretion of total 17-KS is similar in the two groups. Only in the hyperemesis group does the excretion of total 17-KS increase significantly after corticotrophin administration. The fractional determination reveals no difference between the two groups of patients with regard to the values of the fractions U (unidentified 17-KS), A (androsterone) and Rest (11-oxygenated 17-KS). The excretion of dehydroepiandrosterone is significantly higher in the hyperemesis group than in the control group. The excretion of androstanolone seems to be lower in the hyperemesis group than in the control group, but the difference is not statistically significant. The differences in the correlation between dehydroepiandrosterone and androstanolone in the two groups is significant. The high excretion of dehydroepiandrosterone and low excretion of androstanolone in cases of hyperemesis gravidarum is a sign of adrenal dysfunction.
